Abstract

Pendahuluan: Kwarshiorkor merupakan kondisi malnutrisi berat yang disebabkan asupan protein tidak adekuat. Kwashiorkor seringkali tidak terdiagnosis karena kecurigaan klinis masih rendah, adanya edema menutupi malnutrisi, kurangnya pengetahuan mengenai manifestasi klinis tipikal kwarshiorkor. Tujuan: Meningkatkan pengetahuan klinis terhadap kasus kwashiorkor, khususnya yang datang dengan manifestasi kulit, sehingga tidak terjadi keterlambatan diagnosis. Kasus: Anak lelaki 3,5 tahun dengan lesi kulit luas seluruh tubuh dirujuk dengan kecurigaan lupus eritematosa sistemik. Pasien terkesan cengeng, murah marah, tampak edema, dan alopesia yang jelas. Status dermatologikus secara umum didapatkan lesi kulit hipopigmentasi dan hiperpigmentasi dengan eritema, erosi multipel dan skuama putih kecoklatan. Status antropometri berdasarkan kurva WHO 2006 menunjukkan berat badan (BB) menurut umur 1 SD kurang dari Z score kurang dari 2 SD, tinggi badan (TB) menurut umur -1 SD kurang dari Z score kurang dari 0 SD, dan BB menurut TB lebih dari 3 SD. Terdapat riwayat penurunan BB 12 kilogram (kg) dalam 6 bulan terakhir. Pemeriksaan laboratorium terdapat hipoalbuminemia dan defisiensi seng. Selama perawatan pasien mendapat formula khusus gizi buruk F100 dan F135, suplementasi mikronutrien, dan vaselin album. Pada hari perawatan kelima eritema, erosi, dan deskuamasi kulit mulai menghilang. Berat badan menurun seiring dengan berkurangnya edema. Kesimpulan: Setiap tenaga kesehatan harus memikirkan kemungkinan kwarshiokor sebagai diagnosis banding pada pasien dengan edema dan menifestasi kulit luas, yang memiliki riwayat penurunan berat badan bermakna sebelumnya.

Abstract

Pendahuluan: Berat badan lebih (overweight) dan obesitas merupakan kondisi pandemik global yang prevalensinya terus meningkat. Kondisi ini juga banyak ditemukan pada wanita usia subur. Keadaan berat badan lebih dan obesitas pada kehamilan merupakan salah satu kondisi obstetri berisiko tinggi. Kondisi berat badan lebih dan obesitas terbukti dari berbagai penelitian dapat meningkatkan risiko morbiditas dan mortalitas ibu dan janin, antara lain dapat meningkatkan risiko hipertensi dan diabetes gestasional pada masa antepartum, komplikasi intrapartum seperti distosia bahu, perdarahan postpartum, dan kegagalan induksi maupun persalinan pervaginam pasca bedah sesar. Selain itu, pada masa postpartum, obesitas terbukti meningkatkan risiko tromboemboli. Pada janin, obesitas dalam kehamilan meningkatkan risiko makrosomia dan kecacatan janin. Oleh karena itu, guideline-guideline dari Amerika, Kanada, Australia, serta Inggris menganjurkan tata laksana kolaboratif multidisiplin antara dokter umum, bidan, dokter spesialis obstetri dan ginekologi, ahli anestesi, ahli gizi, serta kedokteran olahraga dalam melakukan tata laksana pada ibu hamil dengan obesitas.

Abstract

Undang-Undang Dasar 1945 mengamanatkan setiap warga negara berhak mendapatkan pelayanan kesehatan secara berkeadilan tanpa memandang status sosial dan ekonomi seseorang. Hak untuk hidup sehat juga merupakan bagian dari Hak Asasi Manusia dan merupakan sebuah investasi jangka panjang bagi sebuah negara. Rendahnya rasio dokter per penduduk di Indonesia merupakan sebuah tantangan dalam mewujudkan hal di atas. Saat ini, Indonesia memiliki rasio dokter per penduduk sebesar 1:3333, terendah di Asia Tenggara yang memiliki rerata 1:769 dan masih dibawah rekomendasi World Health Organization (WHO) untuk memiliki rasio 1 dokter per 1000 penduduk.1

Abstract

Introduction: Rectosigmoid infiltration in posterior deep infiltrating en-dometriosis (DIE) requires radical and interdisciplinary surgery. Standard di-agnostic modalities such as ultrasound and laparoscopy are weak in assesingposterior DIE infiltration to inaccessible areas. MRI has the potential value tobe modality of choice because it is multiplanar and excellent in soft tissuedifferentiation. This study aimed to compare pelvic MRI with surgical findingsand histopathology to predict depth of rectosigmoid infiltration in posteriorDIE. Methods: Thirty-seven patients who underwent surgery and pelvic MRI ex-aminations for posterior DIE at the endometriosis referral center (2015-2018)were studied retrospectively. MRI images were analyzed by a gynecology ra-diologist independently then compared to the surgical report and histopatho-logical analysis. Statistical analysis was performed using the McNemar testand the Cohen’s Kappa coefficient. Results: Concordance values (McNemar p) and Kappa coefficients (p) forassesment of rectosigmoid extension in posterior DIE were 100% (1,000) and1 (0,000); infiltration into themuscular layer of rectosigmoid were 89.2% (0.617)and 0.78 (0.001); infiltration into the (sub)mucosal layer of rectosigmoid were81.1% (1,000) and 0.35 (0.146). Conclusion: There was good concordance between pelvic MRI and surgical-histopathologic findings to predict depth of rectosigmoid infiltration in poste-rior DIE.

Abstract

Introduction: Indonesia has the most dengue outbreaks in the ASEAN region. Lack of diagnostic modality and poor sanitation especially in rural area hinders proper and prompt management of Dengue Fever. Aim: Finding the early, simple, and reliable predictor of Dengue Shock Syndrome (DSS) in Indonesia. Materials and Methods: A retrospective study reviewing the medical record of pediatric patient with dengue infection from 1 January 2016 to 31 March 2016 were conducted. Age, gender, dengue signs, and simple laboratory result were collected. The data acquired were then analyzed using logistic regression for multivariate analysis (P less than 0,05) to find the severity predictor. Results: 80 cases of dengue infection were included, mean age was 7,7 ± 3.4 years old, the average length of hospital stay was 4.59 days. 28.75% of patients had DSS. From the multivariate analysis, it is found that lethargy (p= 0.00; OR 21.23), bleeding (p= 0.01; OR 0,04), cold extremities (p=0.00;OR=22.35), and increased haematocrit level (p=0.01;OR=4.72) has significant relationship with DSS while other indicator did not. Conclusion: Patients presenting with lethargy, cold extremities, and increase in haematocrit level should be treated promptly and intensively as they are more likely to develop DSS compared to patients who did not.

Abstract

Introduction: This study was aimed to described current anesthesia methods to facilitate no-scalpel vasectomy procedure. Methods: A systematic review based on PRISMA guideline was conducted which included all the studies discussing no-scalpel vasectomy and focusing on its anesthesia method. Outcomes searched were pain during anesthesia and procedure. Literature search was done through search engines (PubMed, EBSCO Host and Cochrane library) using (“no-scalpel vasectomy” OR “scalpel-free vasectomy” OR “vasectomy”) AND (“anesthesia”) as keywords and through manual search. Literature selection process was done by two reviewer and qualified studies were subjected to quality assessment. Results: A total of 446 literatures were found and five studies satisfying the eligibility criteria and discussed following anesthesia methods: local infiltration anesthesia (LIA), spinal cord block (SCB), no-needle anesthesia (NNA), mini-needle anesthesia (MNA) and eutectic mixture of local anesthetic (EMLA). This study showed that the average score of pain (VAS 1-10) during no-scalpel vasectomy procedure for LIA, combination of LIA and SCB, NNA, MNA and combination of LIA and EMLA were 1.86 – 2.7, 0.64, 0.66 – 2.13, 0.66 and 2.15, respectively. Conclusion: Combination of LIA and SCB, NNA and MNA showed promising results as anesthesia methods. However, these promising results should be further proven by more studies.

Abstract

Introduction: Classical music is a complex melody composition that couldactivate the Brodmann’s area 22, 9, 46, 17, 18, and 19, which are associated inspatial ability as well. Spatial ability is a cognitive function in devising, por-traying, and visualizing an object in space accurately. In the medical field,spatial ability greatly required in learning anatomy, radiograph interpretingand doing the surgical procedure. The aim of this study is to prevail the effectof classical music on spatial ability. Method: An experimental study conducted on the 182 subjects consists of 117female and 65 male medical students. Subjects were divided into two groups,classical music(treatment) group, and silence (control) group. The treatment group listened to Mozart sonata K448 for 10 minutes, while at the very sametime but in a different place, the control group just sat tight for10 minutes aswell. Soon after the 10 minutes, both groups performed the spatial ability test. The spatial test result then analyzed by independent t-test. Result: The spatial score in the treatment group is significantly higher (p less than 0,05)than the control group, both female (33,6379 vs. 31,6949) and male (36,8387vs. 33,9198). Conclusion: Listening to the Mozart sonata K448 could acutely improve thespatial ability

Abstract

Introduction: Multidrug-resistant TB (MDR-TB) is described as Mycobacterium tuberculosis infection resistant to rifampicin and isoniazide with or with-out resistance to other drugs. MDR-TB, if left untreated or undiagnosed inpregnancy, is associated with higher maternal morbidity, mortality, and increased risk of vertical transmission. There are increased risk of obstetrical complications like spontaneous abortions, fetal growth restriction, oligohydram-nios, preterm labor and increased neonatal mortality. Objective: This study was intended to present a case series of MDR-TB treat-ment among pregnancies and fetal outcome in our hospital. Methods: Evaluation of 3 pregnant woman who had been diagnosed with MDR-TB. Data were obtained by history taking, physical examinations, laboratory results and outcome of the neonates. Results: Two out of 3 pregnant woman were known resistant to rifampicin andwere given ethionamide and levofloxacin for more than a month. C-sectionwere performed in all pregnancies at 32 weeks, 36 weeks and 36 weeks withbaby weight 1450 gr, 2350 gr, and 2600 gr respectively. There was no congeni-tal abnormality found in all neonates. Conclusion: Case series of MDR-TB in pregnancy has been reported. Two outof 3 patients were received ethionamide and levofloxacin treatment during pregnancy, showed no deformity or congenital abnormality in the neonates. Low birth weight neonates were found in two patients.

Abstract

Introduction: Achilles tendon is the thickest and strongest tendon in the humanbody. Like any other tendon, rupture may also occur. Statistically, the incidence of Achilles tendon rupture is 18 in 100.000 every year. There are stilldebates whether to choose conservative or operative treatment to treat thisinjury. Some clinicians prefer operative treatment due to the lower recurrence rate over conservative treatment. Newer studies suggest that conservative treatment followed with early functional rehabilitation showed comparable recurrence rate compared with operative treatment, without the risk of concurrent infection. Operative treatment is preferred for younger, more active andathlete patient population that needs faster recovery. Newer studies also suggest that minimal invasive surgery followed by early functional rehabilitationis superior and has lower recurrence and infection rate compared to conven-tional open method.

Abstract

Pada dasarnya leptospirosis merupakan penyakit infeksi pada hewan (zoonotik) yang disebabkan oleh spesies patogenik Leptospira.1,2 Tetapi tidak jarang pula infeksi akut ini terjadi pada manusia, paling banyak disebabkan oleh Leptospira interogans.1-3 Penyakit ini dapat ditemukan baik pada daerah tropis maupun subtropis, terutama pada daerah dengan kondisi higienis yang buruk sehingga bakteri ini dapat bertahan hidup dan mnyebar ke penderita lain. Di banyak negara, termasuk di Indonesia, leptospirosis merupakan masalah yang kurang mendapat perhatian. Apakah di Indonesia sudah bebas dari penyakit ini? Tentunya belum. Berdasarkan data yang tercatat di kementrian kesehatan, pada tahun 2014 terdapat kejadian luar biasa leptospirosis di Kabupaten Kota Baru, Kalimantan Selatan. Data pada Kemenkes mencatat hingga November 2014 terdapat 435 kasus dengan 62 kematian akibat leptospirosis.
