Abstract

Introduction: Several datas studied of intractable epilepsy risk factors areavailable in childhood epilepsy. The objectve of this reasearch to investigatethe role of early or evolution several factors on childhood with generalizedepilepsy as risk factors of intractable epilepsy. Methods: Retrospective cohort study was conducted at neurology out patient pediatric and private outpatient clinic Cipto Mangunkusumo Hospital between January 2010 to December 2013. The inclusion criteria was childhood gener-alized epilepsy with age 1 month up to 18 years old which has been treated with anti epileptic drugs for at least 6 months. Risk factors were analyzed with bivariate and multivariate analysis. Results: Prevalence of intractable generalized epilepsy is 21% of 102 subject. Bivariate analysis showed that age onset of seizure, initial seizure frequency, non-responder of early treatment, unfavorable gross motor development evo-lution and unfavorable epilepti form EEG evolution were significantly associated with intractable epilepsy. Based on multivariate analysis only non-responder of early treatment (p less than 0.001) and age onset less than 1 year old (p less than 0.001). Conclusions. Non-responder early treatment and age onset of seizure less than 1year old are strongly associated with intractable in childhood generalized epilepsy.

Abstract

Introduction: The consensus within the literature show that obese individuals have reduced maximum quadriceps muscle strength relative to body mass ascompared to non-obese individuals. These effects were shown to increase therisk of developing osteoarthritis, balance impairment, and potentially cause functional limitations. Retrowalking training programs have beenfound to in-crease quadriceps strength. The aim of this study is to examinethe effect ofretrowalking on Quadriceps strength in obese individuals. Methods: This study recruited healthy subjects withgrade I obesity at Reha-bilitation Departemen, RSCM- Jakarta. The subjects performed retrowalking training for 4 weeks (2 sessions/week, 15 min/session). Quadriceps strengthwas examined by one single examiner with handheld dynamometer inseveraltime intervals until training is completed (pretest, 2th week, 4th week). To ana-lyze the data and test the hypotheses, paired t-test was used. Result: Seven, obese grade I individuals were recruited in thisstudy. Theirmean age was 29,57±2,94 years old and Body Mass Index was 26,47±0,8. The Quadriceps strength improved significantly at the end of 2th week (D = +3,04,p less than 0,001) and 4th week (D = +5,59, p less than 0,001) as compared with their pretreat-ment values. There is no adverse effect that was reported during the wholecourse of training. Conclusion: a 2 weeks and 4 weeks retrowalking training is proven to be aneffective quadriceps strenghtening for obese individuals. Simultaneously, retrowalking training is a safe exercise to be performed.

Abstract

Introduction: Vitamin D is a nutrient whose main source is obtained from sunexposure. It is estimated that 1 billion of the world’s population from variousethnicities and age groups experience vitamin D deficiency (vitamin D defi-ciency = VDD). Research on vitamin D in adolescents is still limited in Indo-nesia.Objective: Knowing the vitamin D status of healthy adolescent communities inDepok City aged 15-18 years, and related factors, including nutritional status,vitamin D intake, fat and sun exposure scores. Method: Cross-sectional study with 60 subjects. Examination of vitamin Dlevels using the Direct Competitive Chemiluminescence Immunoassay (CLIA)method. Subject nutritional status based on the CDC 2000 IMT / Age chart.Sun exposure scores using the Sun Exposure Scoring by Hanwell et al. Assess-ment of vitamin D and fat intake with semiquantitative and 24 hours food recallFFQ. Results: The average value of vitamin D and vitamin D intake was 17.29±6.77ng/ml and 5.47±3.96 μg/day respectively. Middle BMI 19.67(14.22-36.98)kg/m2.Sun exposure 11 (4-42) and fat intake 42.1 (8.75-1 -, 58)g/day. Conclusion: Vitamin D status of the subject, healthy adolescents, showed in-sufficiency, with normal nutritional status.

Abstract

Introduction: Diphtheria has been reported as an outbreak in some regions inIndonesia, most of the cases are from East Java.Objective: To describe the profile of toxigenic Corynebacterium diphtheriaeisolates in East Java. Methods. An descriptive study from August 20th -November 30th, 2018. Isolates was selected by cluster random sampling. The viability and toxigenicity testwere done in BBLK Surabaya. Viable and toxigenic isolates were done forcharacteristic analysis. Result: A total of 114 isolates, 6 were non-viable, 108 were viable and toxi-genic. The majority of the hosts were male (58,3%), median age 6,5-year-old(minimum 1, maximum 14-year-old), 50% from the 1-5-year-old age group. Highest origin of isolates from patients (78,7%) rather than carriers (21,3%).The most isolates from Madura (47,2%) and horseshoe area (20,4%). Most ofthem (74.1%) were taken from a pharyngeal swab rather than a nasal swab(25,4%). Mitis was 76,9% and gravis was 23,1%. Conclusion: In this study, the majority of toxigenic Corynebacterium diphtheriae were found in the 1-5-year-old age group and most of them domi-cile in Madura and horseshoe area. Most of the isolates were taken from apharyngeal swab of patients. Mitis was the major variant.

Abstract

Introduction: Hyperimmunoglobulin E syndrome is a rare multisystem syn-drome which affects musculoskeletal system, connective tissue, and immunesystem. Immune dysregulation in hyperimmunoglobulin E syndrome often ac-companied with autoimmune disease, such as systemic lupus erythematosus.While the role of IgG autoantibody is well known in the pathogenesis of sys-temic lupus erythematosus, other subtypes of autoantibodies, IgE in particu-lar, are now known to be involved in autoimmune process. We describe a caseof adult onset hyperimmunoglobulin E syndrome whom then diagnosed as sys-temic lupus erythematosus on follow-up.

Abstract

Introduction: Sexual abuses often occur in Indonesia. In 2010-2014, data fromNational Commission for Child Protection showed that 42-62% of all violencein children were sexual abuses. Child sexual abuse usually happens in placesthat should be the safest place for children like home or schools. Child sexualabuse can occur verbally, non-verbally, or physically. Child sexual abuse mayaffect victim physically and mentally. The role of physicians in child sexualabuse cases are to provide medical care and statement for justice and to pre-vent negative impact of sexual abuse in the future

Abstract

Various scientific articles which include scientific writing, books or journals have been known to provide evidence based scientific facts. Thus, in order to achieve accurate evidence based results, a clear outcome measurement is required. Besides, the measurement should be based on precise measurements, both in methods and tools utilized during the measurement. This is a very essential matter, as the accuracy of an outcome measurement would correlate to intervention, program and prognosis of an individual.

Abstract

Introduction : More than in any other joint, osteoarthritis (OA) of the knee is a major cause of pain and disability. Quality of the lower limb muscles, whichis made up of lower extremity muscle mass and muscle strength of the quadriceps, is a risk factor for knee OA. Lower extremity fat-free mass (LE-FFM) represents lower extremity muscle mass. Physical activity level is a factor that affects LE-FFM and quadriceps muscle strength. Quadriceps muscle strengthening exercises can improve LE-FFM and quadriceps muscle strength. Methods : Forty two women with knee OA participated in this study. Interviews were conducted to obtain data about subjectscharacteristics, physical activity level and energy intake by recall 2 x 24 hours method. Measurement of LE-FFM used segmental multifrequency Bio-Impedance Analyzer (BIA) Tanita MC180 and quadriceps muscle strength used anisometric dynamometer BASELINE®. Evaluation was conducted before and after quadriceps muscle strengthening exercises [three times a week] for 8 weeks. Results : Quadriceps muscle strengthening exercises do not increase LE-FFM, but increasequadriceps muscle strength significantly (p=0,00) at a low level of physical activity also at high level physical activity. The difference of change in LE-FFM between the low level and high level physical activity is not different, but signific antly different for quadriceps muscle strength of the left extremity (p = 0.01) or right (p extremity= 0.04). Conclusion : LE-FFM changes between high and low physical activityis not different, whereas quadriceps muscle strength changes is different.

Abstract

Introduction: The development of chronic kidney disease (CKD) and later proportion of decline in renal function are diverse among individuals. This reflects the multifactorial biological mechanisms that are involved in the underlying disease process. The risk factors and resources for care of CKD also vary from place to place. Identifying the risk factors of CKD is crucial and develop prevention strategies, including screening, might be beneficial to reduce burden cost, morbidity and mortality of CKD. We aim at identifying the factors associated with CKD in individuals who have undergone hemodialysis. Material and Methods: All adult patients (age greater than 18 years) admitted to the department of internal medicine either as in- or out-patient for undergoing hemodialysis at the Mardi Rahayu Hospital in Kudus, Middle Java, Indonesia from January to August 2016 were included in the study. Results: Eighty-two patients were evaluated during the study period. Most of the patients were in older age (greater than 50 years old). Sixty (73%) were male and 22 (27%) were female patients. Fifty (61%) patients had mild anemia, 23 (28%) patients had moderate anemia and 8 (10%) patients had severe anemia. Among 82 patients, 77 (94%) of the patients were hypertensive, 19 (23%) of the patients had DM, 23 (28%) of the patients had renal stone, 8 (10%) of the patients had renal cysts, and 7 (9%) of the patients suffered from glomerulonephritis. Conclusion: Older age, male gender, anemia, and hypertension were factors that associated with CKD among patients undergoing hemodialysis.

Abstract

Introduction: Febrile seizure is the most frequent neurological disorder in children below five years old. The direct cause is unknown. Some studies report association between anemia and febrile seizure. Even though anemia is highly prevalent in Indonesia, only one study assessed this association. Objective: To compare the hemoglobin level in children with febrile seizure and febrile children without seizure, and to assess the relationship between anemia and febrile seizure. Method: This retrospective case control study used medical record of febrile children aged 6 months to 3 years old hospitalized at Landak General Hospital between May 2016 to April Hemoglobin level from first laboratory examination then compared between fifty children with first-time febrile seizure (case group) and a hundred febrile children without seizure (control group). Results: Hemoglobin level was lower in case group compared to control group (p less than 0,001). Children with febrile seizure were more likely to be anemic compared to febrile children without seizure [OR 6,73; 95% CI 3,18 to 14,26]. Conclusion: There is a significantmean difference between hemoglobin level of children with febrile seizure compared to febrile children without seizure. Children with febrile seizure were six times more likely to have anemia.
