Abstract

The foundation of decision-making in the health care system is patient care and results. For patient care delivery to be successful overall, staff members must be courteous, cooperative, and respectful of one another. The delivery of healthcare is positively impacted by cohesive health care teams and efficient communication. However, encounters in the health care industry are not always courteous. Among 5,931 cardiologists (77% male; 23% female), 44% reported a hostile work culture, and 79% of those who worked in a hostile work environment reported negative impacts on professional activities with coworkers and patients. These findings are supported by the Global Prevalence and Impact of Hostility, Discrimination, and Harassment in the Cardiology Workplace survey.

Abstract

Introduction: Carbapenem-resistant Enterobacterales (CRE) represent a significant global health problem due to their high resistance to antibiotics. Traditional methods for identifying resistance genes, such as Real-Time PCR (RT-PCR), are accurate but time-consuming and involve complex techniques. This study aimed to compare the Cepheid Xpert® Carba-R test with RT-PCR, used as the gold standard, to determine its effectiveness in rapidly identifying carbapenem resistance genes in CRE. Method: The study was performed from January to June 2022, involving patients admitted to the Intensive Care Unit (ICU) at Dr. Cipto Mangunkusumo National Hospital. Rectal or perirectal swabs were collected upon ICU admission. If patients tested negative for CRE upon admission, repeat swabs were taken at discharge or death, with a maximum length of stay (LOS) of 14 days in the ICU. Genotypic identification of carbapenem resistance genes, including KPC, NDM, OXA-48, VIM, and IMP, was performed using the Cepheid Xpert® Carba-R and compared to RT-PCR. Additionally, phenotypic identification through bacterial culture was performed using the Vitek method. Result: Out of 102 ICU patients, 10.23% (22/215 isolates) were found to be positive for CRE, with the most common bacterial isolates being Escherichia coli (148/215, 68.83%) and Klebsiella pneumoniae (53/215, 24.65%). CRE was found in 1 of 148 E. coli isolates (0.67%) and in 21 of 53 K. pneumoniae isolates (39.62%). The most frequently detected carbapenem resistance gene was OXA-48 (16/22, 72.73%), followed by NDM (6/22, 27.27%). The Xpert® Carba-R test demonstrated a sensitivity and specificity of 100%, with positive predictive value (PPV) and negative predictive value (NPV) both equal to 1. A discrepancy was observed between the phenotypic and genotypic identification, with 27 phenotypically identified CRE isolates compared to 22 genotypically confirmed ones. Conclusion: The Cepheid Xpert® Carba-R test provides a reliable, rapid alternative for identifying carbapenem resistance genes in CRE, demonstrating perfect sensitivity and specificity in this study. This study indicates that Xpert® Carba-R can be an alternative in the rapid identification of CRE resistance genes.

Abstract

Premature infants have a higher risk of mortality and morbidity compared to full-term infants. Therefore, screening in the neonatal unit is necessary to detect these conditions, allowing for timely intervention to be implemented. This article discusses various screening procedures for premature infants, such as head ultrasounds for intraventricular hemorrhage, eye exams for retinopathy of prematurity, hearing tests, heart evaluations, and others. Early identification can reduce morbidity and improve long-term outcomes for premature infants.

Abstract

Introduction: Malnutrition is a worldwide problem that has yet to be resolved, and its prevalence in Indonesia has increased in tandem with changes in people’s lifestyles. Method: This study aims to determine nutritional status based on anthropometric values and investigate malnutrition in male and female aged 17–22 years. The research was conducted using a cross-sectional study. The subjects were male and female, aged 17–22 years, and healthy, with a total of 1,669 male (n=210) and female (n=1459). A questionnaire was used to obtain data. The data was analyzed using a chi-square test (p less than 0.05) and an independent sample t-test (p less than 0.05). Result: Malnutrition was found in subjects who were underweight (male=28.10% and female=26.39%), overweight (male=8.10% and female=9.32%), and obese (male=8.57% and female=6.85%). Total malnutrition in male and female was 27.25% underweight, 8.71% overweight, and 7.71% obese. Nutritional status was not related to gender (p greater than 0.05): underweight (p=0.74), normo-weight (p=0.48), overweight (p=0.34), and obesity (p=0.74). Conclusion: The double burden of malnutrition in males and females aged 17-22 years in Banda Aceh, Aceh, Indonesia is undernutrition and overnutrition, cases of undernutrition were more numerous than overweight and obesity, even though the number of malnutrition cases did not show any difference between men and women in all categories.

Abstract

Introduction: Myofascial pain in the piriformis muscle associated with functional leg length disparities (LLD) in patients with non-specific low back pain (NSLBP) is often undetected, resulting in inappropriate management. High-intensity laser therapy (HILT) is a noninvasive modality for treating piriformis syndrome. However, correction of leg length with the insertion of a shoe lift is also necessary as an adjunctive treatment. This study aims to assess the effectiveness of HILT with and without the use of shoe-lift inserts in reducing myofascial pain in NSLBP patients with LLD. Methods: This single-blind, randomized clinical trial was conducted at the Medical Rehabilitation Clinic of Pasar Minggu General Hospital, Jakarta, from December 2022 to August 2023. Inclusion criteria were adult age, who endured NSLBP for 3-6 months, have LLD, and have myofascial pain in the piriformis muscle with a numerical rating scale (NRS). The Pelvic tilt was measured using an inclinometer. NRS scores in the treatment and control groups were calculated from week 1 to week 4.Results: This study involved 26 subjects assigned to intervention and control groups. The average age was 49.8 years, and 69% of them were female. There was no significant difference in pelvic tilt between groups (p=0.39). The decreasing NRS scores were lower in the intervention than in the control group during the first and second weeks (p=0.015 and p less than 0.001). There was no significant difference in NRS scores between the second and third weeks in intervention and control groups (p=0.157 and p=0.06 ), however, the control group showed a tendency to increase NRS scores in the third week.Conclusion: HILT combined with shoe lift insertion is more effective in reducing the intensity of piriformis myofascial pain in patients with NSLBP and LLD.

Abstract

Introduction: Reused medical devices are one of the causes of nosocomial infections that occur in health care in Indonesia. Infection occurs when a pathogen spreads to the host such as an invasive procedure or a medical device that enters the body. Differences were found regarding the length of storage time on the level of sterility of medical devices, so it is necessary to determine the length of storage time to prevent nosocomial infections. Objective: To obtain recommendations for the shelf life of sterile medical devices from the results of microbiology test results for devices that stored in pouches.Method: The type of research used is descriptive research. This research was carried out with the aim of identifying the presence or absence of bacteria on instruments that had been sterilized and then stored in pouches at the Central Sterilization Unit of the Puskesmas Kecamatan Kebayoran Baru. Sampling and sample inspection will take place from September 2022 to December 2022. Results: Microbiological examination of Escherichia coli and Bacillus cereus bacteria using a total plate count examination to calculate the number of microbes in the research samples. The results of examinations carried out at 0 days, 1, 4, 7, 10 and 12 weeks on research samples showed that no Escherichia coli and Bacillus cereus bacteria were found.Conclusion: The recommended shelf life for sterile medical devices stored in pouches at the Central Sterilization Unit of the Puskesmas Kecamatan Kebayoran Baru is 12 weeks.

Abstract

Introduction: Radial artery pseudoaneurysm is a rare but potentially severe complication that may occur after coronary angiography. Although uncommon, promptly identifying and addressing the issue is crucial to avoid further complications. Case Report: Here, we describe the case of a 65-year-old female patient who developed a radial artery pseudoaneurysm after undergoing coronary angiography to evaluate her chest discomfort. The diagnosis of a pseudoaneurysm was confirmed by Doppler ultrasonography, which identified a swirling flow pattern. Persistent pain symptoms and ineffective conservative treatment necessitated surgery.Case Discussion: This case highlights the importance of vigilance in identifying rare complications such as radial artery pseudoaneurysm following coronary angiography. Conclusion: Prompt diagnosis and appropriate management strategies, including surgical intervention, when necessary, are crucial for optimal patient outcomes.

Abstract

Introduction: Surgery for meniscus injuries has the potential to trigger osteoarthritis and early cartilage degeneration. This evidence-based case report aims to evaluate the effectiveness of physical exercise therapy compared to surgery in restoring knee function in a woman with a lateral discoid meniscus tear.Methods: A systematic literature search was conducted using databases such as PubMed, ScienceDirect, ProQuest, and Scopus. The focus of the search was on studies comparing the outcomes of physical exercise therapy and surgery for meniscus injuries.Result: Out of 737 identified articles, three relevant studies met the criteria and addressed the research question of this case report. These studies concluded that there was no significant difference between physical exercise therapy and surgery in restoring knee function in patients with meniscus injuries.Conclusion: Physical exercise therapy has proven effective as a safe alternative with the potential to reduce the risk of operative complications in patients with meniscus injuries. This non-invasive approach offers a viable option for maintaining knee function without increasing the risk of osteoarthritis or early cartilage degeneration.

Abstract

Interstitial lung disease (ILD) is the most common extraglandular manifestation in primary Sjogren’s syndrome (pSS), the second most prevalent autoimmune rheumatic disease. This literature review aimed to discuss about ILD in pSS (pSS-ILD), beginning from aspect of epidemiology, classification criteria, monitoring of disease activity, pathogenesis, diagnostic approaches, to management. The 2016 American College of Rheumatology (ACR)/European League Against Rheumatism (EULAR) classification criteria for pSS are useful for establishing a diagnosis of pSS. The level of disease activity can be assessed by using the EULAR Sjogren’s Syndrome Disease Activity Index (ESSDAI). Development of autoimmunity to epithelial cells is the pathogenesis mechanism of pSS not only in exocrine glands but also extraglandular, including the lungs. The main diagnostic modalities recommended in the screening and monitoring of ILD in pSS are pulmonary function tests (PFTs) and high-resolution computed tomography (HRCT) of thorax. Systemic therapy options for pSS-ILD include glucocorticoids, immunosuppressive agents and biological drugs.

Abstract

In recent decades, the world has faced a dual challenge in nutritional health: the rising rates of maternal obesity and the high prevalence of stunting in children. Data from the World Health Organization in 2022 revealed that over 2.5 billion adults were overweight or obese, with 44% of them being women. This condition occurs not only in high-income countries but also in low- and middle-income nations. National surveys indicate that the prevalence of obesity among adult women (over 18 years old) has increased from 29.3% (Basic Health Survey 2018) to 31.2% (Indonesian Health Survey 2023).
