Abstract

Apnea is a manifestation of immature breathing and frequently occurs in preterm infants treated in the Neonatal Intensive Care Unit. Apnea of prematurity (AOP) is characterized by episodes of apnea (cessation of breathing) lasting 15–20 seconds or more, or shorter episodes accompanied by bradycardia and/or desaturation. The incidence of AOP is inversely related to gestational age (GA) and birth weight. Caffeine is one of the pharmacological treatments for managing AOP. Caffeine belongs to the methylxanthine class, like theophylline and aminophylline. Caffeine has been used as a therapy of choice for AOP in developed countries since the 1970s. Meanwhile, in developing countries, aminophylline remains the most commonly used drug for AOP management. Compared to aminophylline and theophylline, caffeine has better therapeutic effects and absorption, a longer half-life, and fewer side effects. Therefore, caffeine is the first-line therapy for AOP management.

Abstract

Allergic rhinitis (AR) is clinically characterized by two or more symptoms including itching, sneezing, nasal obstruction and rhinorrhoea. It is known to be caused by IgE-mediated hypersensitivity reaction in response to inhaled allergens. It has been shown to have high burden of disease, as patients with AR show significant reductions in the number of healthy days, interruption of daily activities due to symptoms, and overall reduction in quality of life. The International Study of Asthma and Allergies in Childhood (ISAAC) estimates the prevalence of AR across children and adults to be about 10-30% in the United States (US) and other developed countries. Allergic rhinitis negatively impacts quality of life directly through, or from the consequences of, its symptoms such as sleep disturbances, fatigue, impaired learning and cognitive functions, leading to reduction in productivity, worsened school performances and may also lead to embarrassment.

Abstract

Introduction: Typhoid fever is a systemic disease caused by bacterial infection, so rational use of antibiotics is important in the treatment of typhoid fever to support effectiveness and prevent antibiotic resistance. The aim of this study was to assess the rationality of antibiotic use in pediatric typhoid fever patients at the Cut Meutia Regional General Hospital, North Aceh using the Gyssens method based on IDAI and Ministry of Health guidelines.Method: This research is descriptive. Samples were taken using purposive sampling technique and time limited sampling method. The samples in this study were medical records of pediatric typhoid fever patients in the inpatient room of Cut Meutia Hospital, North Aceh 2022.Results: The results obtained ceftriaxone as the most commonly administered antibiotic, followed by cefixime, and quinolones. For rationality, there was ceftriaxone classified as category 0 as much as 4,4%, Category II A in ceftriaxone and cefixime as much as 17,5%, category II B in ceftriaxone as much as 0,6%, category III B in ceftriaxone and cefixime as much as 51,2%, category IV B in quinolones as much as 6,9%, and category VI in incomplete medical records as much as 19,4%.Conclusion: The conclusion of this study is that the most frequently used antibiotic is ceftriaxone, while the level of rationality indicates a significant proportion of irrational use.

Abstract

Introduction: Gut microbiota modification based on dysbiosis of gut microbiota dysbiosis (DOGMA) theory may provide a new therapy approach in polycystic ovarian syndrome (PCOS). Research of this new therapy needs a suitable animal model thus this study was aimed to investigate whether Wistar rats that were injected by testosterone propionate (TP) could induce both PCOS and gut microbiota dysbiosis condition.Methods: Design of this study was post-test only control group design randomized control trial. Wistar rats were divided into two groups: control and TP. Blood, faecal and ovarian tissue sampling also vaginal smear were obtained after 28 days of TP injection.Results: TP group had testosterone concentration, preantral follicle and fasting blood glucose concentration higher than control group (p=0.047, p=0.018, p=0.032). Fasting insulin, HOMA-IR value, serum zonulin level, TNF-α concentration and gut microbiota diversity were not significantly different.Conclusion: TP injection intramuscularly (10 mg/kgBW) for 28 days succeeded to induce PCOS and hyperglycaemia in Wistar rat but was failed to induce insulin resistance, low grade inflammation, impaired gut permeability, and gut microbiota dysbiosis thus it’s not suitable as animal model for gut microbiota dysbiosis research in PCOS.

Abstract

Introduction: Fear of falling after a stroke can be caused by a lack of balance, muscle weakness, or impaired motor control. This can affect a patient's ability to move around and their overall quality of life after a stroke. However, virtual reality (VR) technology has the potential to help with this issue by providing simulated physical exercises that are safe and controlled. By combining VR rehabilitation with conventional stroke care, it's possible to improve a patient's ability to move around safely and their quality of life after a stroke.Methods: An experimental study was conducted on 18 ischemic post-stroke patients who sought treatment at the Medical Rehabilitation Installation of Prof. Dr. R. D. Kandou Hospital Manado from June to July 2023. The study aimed to analyze the impact of VR game training using Xbox 360° and KinectTM devices on fear of falling and post-stroke quality of life. The assessment of fear of falling was done using the Fall Efficacy Scale (FES) and quality of life using the EQ-5D. The intervention included four types of game exercises scheduled three times per week, with each session lasting 30 minutes with a total of eight sessions for each subject.Results: Following VR game training, there was a significant reduction in fear of falling (t = 15.353, p less than 0.001) and an improvement in quality of life across five dimensions of health problems as well as EQ-5D (t = 23.777, p less than 0.001).Conclusion: VR game training combined with conventional stroke care can reduce the fear of falling and improve the quality of life of post-stroke patients.

Abstract

Introduction: The Hospital Ethics and Law Committee (HELC) holds the primary responsibility of ensuring adherence to ethical and legal regulations within the hospital. However, due to the absence of specific provisions regarding the HELC, its role has often not been fully optimized. This study aims to evaluate the role of the HELC in enhancing the quality of services, ensuring patient safety, and resolving medical disputes at Syarif Hidayatullah Hospital, Jakarta. Methods: This cross-sectional study combines quantitative and qualitative methods, involving employees of Syarif Hidayatullah Hospital as respondents. Data collection was conducted through interviews from January to February 2024. The data were analyzed using univariate and bivariate methods, followed by multivariate variance analysis. Results: A total of 53.1% of respondents assessed the role of the HELC as well-executed. An effectively executed HELC role is correlated with increased patient safety, enhanced service quality, and effective resolution of medical disputes (all p less than 0.001). Multivariate variance analysis revealed that the HELC's role has the most significant impact on the improvement of hospital service quality (adjusted R-squared = 0.885). Conclusion: The role of the HELC is associated with improvement of patient safety, service quality, and the resolution of medical disputes in Syarif Hidayatullah Hospital. Hospital management needs to empower the HELC and enhance coordination with other committees and units within the hospital.

Abstract

Introduction: This study aims to evaluate the acceptance among Posyandu cadres of the Littlears application, which is designed to identify hearing impairments in children early on. Method: The LittlEars Questionnaire, designed to identify hearing impairment in children, has evolved into the LittlEars application with the objective of early detection of hearing loss. The evaluation of system usability within the framework of the USE Questionnaire involves four key elements: usability, user-friendliness, learnability, and user satisfaction, and is assessed through 35 specific questions. Result: The usability assessment of the LittlEars application through the USE questionnaire revealed that its usefulness stands at 95.4%, its ease of use at 93.5%, its ease of learning at 92.8%, and satisfaction levels at 94.6%. The average for all dimensions combined is 9.07%. Conclusion: The LittlEars application can be accepted by users, especially Posyandu cadres, and is an application that is easy to use in detecting hearing loss in children.

Abstract

Introduction: The PaO2/FiO2 (P/F) ratio is the main choice for invasively assessing ARDS. The SpO2/FiO2 (S/F) ratio can be used as a noninvasive oxygenation parameter in ARDS patients. This study was conducted to determine the relationship between the SpO2/FiO2 ratio and the PaO2/FiO2 ratio. Methods: This study was observational with a cross-sectional research method to see the relationship between the SpO2/FiO2 ratio and the PaO2/FiO2 ratio in ARDS patients. The total sample was 29 intubated ICU patients diagnosed with ARDS. This study excluded patients with congestive heart disease, anemia, and systolic blood pressure less than 90 mmHg. Result: A linear regression analysis was obtained between the PaO2/FiO2 and the SpO2/FiO2 ratio with a relationship coefficient of 0,955 and p-value of 0,001. A relationship was found between the S/F ratio and the P/F ratio in assessing the severity of ARDS (p-value less than 0.05). Conclusion: Assessment analysis of the SpO2/FiO2 ratio and the PaO2/FiO2 ratio has a relationship so that it can be used as a predictor of ARDS.

Abstract

Introduction: Pain can sometimes trigger a stress response and stimulate adrenergic-sympathetic activity. Delirium, a serious disruption of consciousness, is characterized by fluctuating perceptions, confused thinking, and varying levels of attention. Undiagnosed pain and delirium can lead to increased infection rates, prolonged mechanical ventilation, hemodynamic instability, and delirium. This study utilizes the Critical-Care Pain Observation Tool (CPOT) to measure pain and the Confusion Assessment Method (CAM) ICU to evaluate delirium in intubated patients in the ICU of RSUP HAM to explore the connection between pain and delirium. Method: The research employed a quantitative, cross-sectional observational analytical method. It investigated the association between pain and delirium in patients who were intubated at H. Adam Malik General Hospital Medan between January and February 2023, using the CPOT and CAM ICU scales. The chi-square test was used to analyze the relationship between pain and delirium, as well as the relationship between confounding factors and both pain and delirium. Results: The association between pain and delirium had a P-value of 0.001. The association between fentanyl dosage and pain in intubated patients also had a P-value of 0.001. Confounding factors included the length of stay with pain (P-value less than 0.001) and delirium with age (P-value = 0.026), gender (P-value = 0.013), and length of stay (P-value = 0.001). However, the doses of fentanyl and midazolam showed no significant association between patients with and without delirium (P-value = 0.5). Conclusion: There is a significant associations between pain and delirium.

Abstract

Introduction: Critically ill patients are at risk of developing persistent organ dysfunction that can deteriorate the patient's clinical outcome. This leads to patients in life-threatening conditions that require intensive care. Patients in the Intensive Care Unit (ICU) mostly require mechanical ventilation. Prolonged mechanical ventilation can cause a decrease in the contractile ability of the diaphragm along with diaphragmatic muscle atrophy. This is known as Ventilator Induced Diaphragm Dysfunction (VIDD). Diaphragm dysfunction remained a major cause of weaning difficulty or failure. Ultrasonography measurement of diaphragm function can be used to predict the outcome of weaning from mechanical ventilation. Methods: This study used a prospective observational study with a cross-sectional method. The total sample consisted of 36 critical patients on mechanical ventilators in the ICU who were planned for weaning. Patients were measured for diaphragm wall thickness fraction using ultrasonography. Result: In the diaphragm thickness fraction less than 30%, the successful weaning group was 3 people (8.3%), and failed weaning as many as seven people (27.8%); in the diaphragm thickness fraction greater than 30%, the successful weaning group was 24 people (66.7%) and failed weaning as many as two people (5.6%). The mean diaphragm thickness fraction in the successful weaning group was 40.77 ± 10.12, while in the weaning failure group, it was 24.50 ± 6.89. There is a relationship between diaphragm wall thickness as a predictor of successful weaning in critical patients in the ICU with a p-value = 0.001. Conclusion: Measurement of diaphragm wall thickness using ultrasonography can be used to predict weaning success in critically ill patients on ventilators in the ICU.
