Abstract

Introduction: The most common cause of worry for health in modern culture is non-communicable diseases (NCDs). Effective management of risk factors is essential when dealing with NCDs. Addressing NCDs requires a multifaceted approach, involving a range of solutions from different perspectives and at various levels, encompassing both individual and national dimensions.Method: Five databases were searched: JBI, Embase, Emcare, Medline, and Cochrane Systematic Reviews. The papers included reviews of social marketing in any form or empirical information about how social marketing affects the prevention of non-communicable illnesses.Result: Five papers were chosen from a pool of 36 titles and abstracts. Among the papers, 11 (45.8%) were observational studies conducted in various settings such as communities, hospitals, and clinics, while the remaining articles were from different healthcare facilities. The collected study designs encompassed systematic reviews, narrative reviews, brief communications, scientific reviews, non-randomized control trials, and randomized control trials. Five publications supported their strategies with empirical evidence.Conclusion: Social marketing approaches intended to prevent and handle NCDs need to be based on a deep comprehension of the intended audience and customized to fit the particular circumstances. Furthermore, they should promptly involve the public and establish connections with stakeholders. The elements of the intervention mix should work together harmoniously and complement each other. Furthermore, it's vital to include health education and capacity building and ensure cultural relevance.

Abstract

Introduction: Long COVID is a persistent symptom among COVID-19 survivors. Fatigue and other symptoms related to respiratory or non-respiratory cause disturbance of patient functional status. Pulmonary telerehabilitation program after hospitalization was needed to optimize patients’ functional status. Assessment of functional status using 30-Second Sit-To-Stand Test (30s-STST) was easily to apply. The aim of this study was to know the changes of functional status in COVID-19 survivors after pulmonary telerehabilitation program.Method: This study was analytic experimental with one group pre-test and post-test design, conducted in Persahabatan Hospital from August to September 2020. The COVID-19 patient’s discharge records were recruited consecutively, 24 patients were eligible for this study to get pulmonary telerehabilitation program for four weeks. The patients were evaluated with 30s-STST twice, at discharge (baseline) and four weeks after discharge.Result: Participants consist of 19 men (79,2%) and 5 women (20,8%). The result of thirty second-STST before and after four weeks pulmonary telerehabilitation program result was 7,75 ± 2,19 dan 13,70 ± 4,23 times. There was a strong positive correlation (significant) between 30s STST result before and after program (r = 0,757, p less than 0,01).Conclusion: There was functional status improvement in COVID-19 survivors after pulmonary telerehabilitation for four weeks, evaluated with 30s-STST.

Abstract

Introduction: Pseudocyesis originates from the Greek language, consisting of 2 syllables, namely: Pseudo (false) and Kyesis (pregnancy). Pseudocyesis is a mental health condition where the affected individual believes they are pregnant, accompanied by signs and symptoms of pregnancy. The Kei society adheres to a patrilineal kinship system and has preserved customary laws handed down through generations known as “Larvul Ngabal”. These customary laws regulate several aspects of life including marriage and descent.Case Report: A 60-year-old woman came to the outpatient clinic to checked her pregnancy. The patient stated that for the past 2 months, her abdomen had been growing larger and she felt fetal movements inside. Obstetric examination and supporting tests did not reveal the presence of a fetus. The patient was educated that she was not pregnant, but she remained convinced that she was pregnant.Case Discussion: This patient has already entered post-menopausal age and has been married for the second time. The patient and her husband desire to have a male child. The Kei society adheres to a patrilineal system, where a male child is hoped to continue or strengthen the paternal lineage. However, the patient currently does not have any offspring. Therefore, this situation may have an impact on her condition.Conclusion: Pseudocyesis reflects somatization disorder in the patient due to the unfulfilled desire to have offspring. This condition can be influenced by several factors including socio-cultural ones.

Abstract

Female breast cancer is the most common diagnosed cancer worldwide in 2020, with an estimated 2.3 million new cases or 11.7% from total 18,1 million cases, followed by lung, colorectal, prostate, and stomach cancers. In 2040, the breast cancer cases are predicted will be increased into 3 million cases per year or grow 40% from 2020 data. It is also estimated the number of deaths due to breast cancer will be one million death per year in 2040. In Indonesia, the number of new breast cancers reached 68,858 cases (16.6%) of a total of 396,914 new cases of cancer in 2020. ​ The incidence and mortality rates of breast cancer are 44 and 15.3 per 100,000 population, respectively in Indonesia. The most common breast cancer treatments used currently are surgery, chemotherapy, and radiation. These three treatments involve a rather invasive approach and often causes side effects. Serious side effects can reduce a patient's quality of life, lead to non-compliance to chemotherapy, and lead to drug resistance.​ Some cancer patients use alternative therapy such as complementary medicine including herbal medicine to treat the cancer. Patients try to discover and utilize natural-derived anti-cancer drugs because it believes has fewer side effects and more affordable. One of Indonesia plant which is therapeutically beneficial is Ficus deltoidea. Ficus deltoidea leaves had been used in traditional medicine to treat diabetes, inflammation, diarrhoea, cancer and infections. F. deltoidea leaves contained flavonoids, saponins, tannins, steroids and terpenes which known for its anticancer effect.

Abstract

Introduction: Stunting is a form of chronic malnutrition which is still a nutritional problem in Indonesia. Stunting is characterized by a decrease in insulin-like growth factor-1, closely related to the release of excess reactive oxygen species that trigger oxidative stress. An increase in reactive oxygen species that bind to membrane lipids produces lipid peroxidase, which impacts cell damage. The research aims to obtain information regarding the role of malondialdehyde measurement as an early marker and a prognostic determinant of oxidative stress in protein-energy malnutrition children, such as stunted and marasmic.Method: Systematic literature was conducted with published works journal searches from 23rd – 30th November 2023, relevant keywords related to the research topic in ScienceDirect, PubMed, and Cochrane databases using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis 2020 method. The quality of selected studies was evaluated using The Joanna Briggs Institute Critical Appraisal Tools. The research protocol was registered at PROSPERO (CRD42024568540).Results: The data obtained were five articles from cross-sectional studies with the results had increased oxidative stress biomarkers, including malondialdehyde, which is significantly higher in protein-energy malnutrition children, such as stunted and marasmic, than in healthy children (p less than 0.05).Conclusion: The malondialdehyde levels as biomarkers of oxidative stress were found to be high in stunted children.

Abstract

Introduction: Monocytes are very sensitive to changes in the metabolic environment, including hyperglycemia, such as type 2 diabetes mellitus (T2DM). T2DM is characterized by chronic low-grade inflammation and becomes one COVID-19 comorbid. TNF-α and IFN-γ are cytokines often linked to inflammation, the severity of COVID-19, and long COVID-19. This study aims to analyze the relationship between COVID-19 severity and TNF-α and IFN-γ in T2DM one month post-infection.Methods: This research is an experimental study at the Integrated Laboratory Faculty of Medicine, Universitas Indonesia, for four months. The total samples are 44 cryotubes of PBMC (18 T2DM and 26 Non-T2DM) from the CARAMEL study (COVID-19, Aging, and Cardiometabolic Risk Factors). PBMCs were stimulated with inactivated whole virions SARS-CoV-2 and incubated for 24 hours. Monocyte subsets and intracellular cytokines are detected by flow cytometry.Results: Research showed that IFN-γ in the T2DM group was higher in all subsets. There was no significant difference between the T2DM and non-T2DM groups. Based on the history of the severity of COVID-19, MedFI IFN-γ classic and intermediate monocytes differed significantly between the COVID-19 severity groups (p = 0.049 and p = 0.022). Further research is needed to analyze the risk factors involved.Conclusion: MedFI of classic and intermediate monocyte IFN-γ differed significantly between the severity of COVID-19. There were no significant differences between the T2DM and non-T2DM groups. The history of the severity of COVID-19 is significantly related to long COVID-19.

Abstract

Wernicke-Korsakoff syndrome is a common complication of thiamine (vitamin B1) deficiency. The prevalence of Wernicke-Korsakoff syndrome (WKS) worldwide is 0-2%, especially in the homeless, elderly, and psychiatric patients. Individuals with malnutrition due to any cause are at risk for developing WKS. The most common social factor associated with WKS is chronic alcohol abuse. Ethanol has been shown to interfere with thiamine absorption in the gastrointestinal tract. The syndrome is classically described as a clinical triad consisting of altered mental status (confusion or dementia), nystagmus (or ophthalmoplegia), and ataxia. The diagnosis of WKS is based on the history and clinical findings supplemented by laboratory and radiological examinations. Treatment of thiamine deficiency in patients suspected of having Wernicke’s is intravenous injection of thiamine 500 mg infused over 30 minutes. The drug is given three times daily for two days. Thereafter, transition to 250 mg either intravenously or intramuscularly once daily for 5 days.

Abstract

Introduction: COVID-19 is a disease that can severely reduce life quality, particularly in the aged population. The research aim was to establish the correlation between health-related life quality and dyspnea severity within aged COVID-19 survivors.Method: The cross-sectional study was conducted on older COVID-19 survivors at Persahabatan Hospital, Jakarta. The use of WhatsApp® video call and Zoom® video conference applications for audio and video evaluation in data collection. The European Quality of Life-5Dimensions-5L (EQ-5D-5L) and the European Quality of Life-Visual Analog Scale (EQ-VAS) were used to assess health-related quality of life. Modified Borg and modified Medical Research Council (mMRC) dyspnea scale were used to assess dyspnea severity. The data were analyzed using correlation test. Results: Based on 44 data collected, it was found that seven (15.9%) of the participants experienced mild breathlessness, while three patients (6.8%) had severe level of dyspnea on the modified Borg dyspnea scale assessment. The mMRC dyspnea scale assessment indicated that over nine patients (20.4%) had mMRC value greater than one. The study found a significant correlation (p less than 0.01) between the EQ-5D-5L and the modified Borg Dyspnea Scale, particularly in the areas of anxiety or depression (r=0.52), self-care (r=0.51), mobility (r=0.42), and EQ-VAS (r=-0.53). The EQ-5D-5L and the mMRC dyspnea scale showed a significant correlation (p less than 0.01), particularly in relation to usual activities (r=0.69), self-care (r=0.62), and mobility (r=0.65), as well as EQ VAS (r=-0.58).Conclusion: Moderate positive correlations existed between life quality issues including self-care, anxiety, depression and dyspnea severity .

Abstract

Introduction: This study aims to determine the prevalence and survival rate of advanced cervical cancer patients with obstructive uropathy based on grade of hydronephrosis, type of hydronephrosis, deobstruction action, type of deobstruction, clinical stage, and cancer cell type.Method: This research is a descriptive analytical study with a cross-sectional retrospective approach. Samples were obtained from medical records of patients with stage IIIB, IVA and IVB cervical cancer who were treated at RSUD Dr. Saiful Anwar Malang in 2016–2023. Sampling was carried out using the total sampling method.Result: Of the 171 patients, the majority of patients had stage IIIB cervical cancer (71.9%), moderate (72.5%) and bilateral (90.1%) hydronephrosis were found in the majority of patients. SCC dominated the sample (82.5%) and was followed by ADC (14.6%). Most patients underwent deobstruction (64.3%) with DJ stent insertion being the most common type of deobstruction (84.5%). Patients with stage IVB have a lower survival rate than IIIB and IVA (p greater than 0.05). Patients with SCC had lower survival (3%) than ADC (5%). The average survival rate for the three grades of hydronephrosis was not significantly different (p greater than 0.05). Patients undergoing deobstruction showed a higher survival rate (5%) (p greater than 0.05). DJ stent, PCN, and UCS insertion showed similar survival rates in patients (p greater than 0.05).Conclusion: Stage IIIB cervical cancer, moderate hydronephrosis, bilateral hydronephrosis, and SCC dominated the patients in the study. Patients with stage IVB cervical cancer, SCC, unilateral or severe hydronephrosis have a worse prognosis. The presence of deobstruction indicates a better prognosis and can be performed with PCN, DJ stent insertion, or UCS.

Abstract

Introduction: Benign Prostate Hyperplasia (BPH) is an enlarged prostate gland that causes problems in micturition and results in disruption of urine flow. Factors that cause prostate enlargement from a combination of chronic and progressive urine retention, high bladder pressure and ureterohydronephrosis can cause progressive kidney injury. The obstructive process changes the cells and physiological conditions in the bladder muscle and collagen contributes to increasing bladder pressure and the inability to empty the bladder and ends up causing kidney failure. This study was to determine the prevalence of chronic kidney disease (CKD) in BPH patients undergoing transurethral resection of the prostate (TURP) surgery.Method: Using non-probability sampling method with consecutive sampling technique. Data were analyzed by univariate analysis using data frequency tables and bivariate analysis using the chi-square test.Result: Of the total 178 BPH patients, 15 of them suffered from CKD (8.4%). BPH prevalence was highest in the age category 61-70 (39.3%), the majority had prostate volume greater than 50 ml (59%), the most common symptoms were severe lower urinary tract symptoms (LUTS) (83.1%) and urine retention (74.7%), some of them were accompanied by comorbidities such as hypertension (33.7%). The most common indication for surgery in patients who underwent TURP was recurrent retention (79.2%). The chi-square test showed an association between BPH and the incidence of CKD.Conclusion: The prevalence of CKD at all stages in BPH patients who underwent TURP surgery was 8.4%. The most influential factors for the occurrence of CKD in BPH patients are prostate volume 30-40 ml, prostate volume 41-50 ml, and men aged 71-80 years.
