Abstract

According to the World Health Organization (WHO), Indonesia is ranked 7th among the 10 countries with the highest number of sufferers, namely 10.7 million people. Complications from diabetes mellitus with diabetic gangrene rate in Indonesia is around 15%, and the amputation rate is 30%. In diabetes mellitus patients there are many nursing problems, one of which is impaired skin/tissue integrity. Diabetes mellitus is a disease caused by metabolic disorders as a result of the pancreas being unable to produce enough insulin needed by the body. A complication that often occurs in diabetes mellitus sufferers is gangrenous wounds where tissue deformity occurs either partially (Partial Thickness) or completely (Full Thickness). This study used a qualitative research design with a case study approach. The sample was 2 (two) Diabetes Mellitus patients with Gangrene Wounds with nursing problems of Skin/Tissue Integrity Disorders. The case study was carried out at Karawang District Hospital. Data collection techniques were carried out by: interviews, observation, physical examination and documentation studies. The data was analyzed narratively based on a comparison of 2 (two) patients who were processed based on the stages of the nursing process. Based on the assessment, it was found that patient 1 and patient 2 had gangreen wounds which is one of the complications that occurs in diabetes mellitus sufferers. The nursing problem that occurred was disruption of skin/tissue integrity and the implementation carried out for these two patients was wound care. The nursing problem of skin/tissue integrity disorders in both patients showed that the problem was not resolved and the intervention was continued by the nurse who was on duty in the room. Further research with another intervention and larger sample size needed to find out the best intervention for gangrene. Keywords: Diabetes Mellitus, gangrene, skin integrity

Abstract

The integrated clinical practicum equips prospective nursing students with the essential skills required to operate effectively as healthcare professionals who will transition into physicians following licensure. The population of nursing students at STIKes Budi Luhur Cimahi is 632 students. The sample was 244 students with cluster random sampling techniques, 36 nurses, 148 nursing S1 students, and 60 nursing D3 students. Data analysis used chi square and kolmogorov Smirnov tests. Based on the results of P-Value 1.00 with the age of 18-20 years with a total frequency of 18 with skilled skills of eight twenty-two points two percent (22.2%), ages 21-23 years with a total frequency of 30 with skilled skills of twenty point four percent (20.4%) and ages 24-26 years with a total frequency of 5 with skilled skills of twenty-one point seven percent (21.7%). Regarding gender, based on the results of P-Value 0.244 with male gender with a total frequency of 73, sixteen point four percent (16.4) were obtained. And the female gender with a total of twenty-one point seven percent (27.1%) is skilled. In acquiring health assessment skills while in the undergraduate program, nurse educators must consider the variables of age profile, gender, and type of student. Hence, it is imperative for nurse educators to cultivate diverse pedagogical approaches in order to cater to the varying requirements of diverse student populations. Abstract

During the third trimester of pregnancy, women often experience various discomforts, including anxiety. One non-pharmacological treatment to alleviate this anxiety is the Five-Finger Hypnosis technique. At Clinic "A" in 2023, out of 143 pregnant women, 50 in the third trimester reported experiencing anxiety. This study aims to evaluate the effectiveness of midwifery care using the Five-Finger Hypnosis technique to reduce anxiety in third-trimester pregnant women. Data collection methods included interviews, observations, and documentation review. The subject of this study was Mrs. "L," a G1P0A0 patient at 35 weeks and 6 days of gestation, who reported anxiety during pregnancy. The results showed that comprehensive midwifery care was conducted normally without complications. Mrs. "L" received the Five-Finger Hypnosis intervention six times: on the first day of her clinic visit and again on the 3rd, 6th, 8th, 11th, and 14th days following her initial visit. After these sessions, her anxiety significantly decreased, and she reported feeling more comfortable and relaxed. This study concludes that comprehensive midwifery care, including the Five-Finger Hypnosis technique, was effective in reducing anxiety for Mrs. "L." Such care can be beneficial in minimizing potential complications during pregnancy, childbirth, the newborn period, postpartum, and contraceptive services. Abstract

In the third trimester, women usually experience discomfort such as low back pain, frequent urination, fetal movements, heartburn, leg cramps, fatigue, especially sleep disturbances or difficulty sleeping until the morning. Third trimester pregnant women often experience sleep disturbances, this causes a decrease in sleep duration in pregnant women. Based on the Ministry of Health of the Republic of Indonesia, in 2021, the number of pregnant women in Indonesia who experienced sleep disorders reached 64%. This study aims to determine midwifery care to overcome sleep disorders in pregnant women. The method in this research is a case study approach. Data collection in this research used interviews and observation. The subject of this research was Mrs "U" G4P1A2 38 weeks pregnant with complaints of discomfort and sleep disturbances. The causes of sleep disorders in Mrs "U" were an enlarged uterus, increased fetal weight, shortness of breath, restlessness, difficulty falling asleep and frequently waking up at night because she wanted to urinate. The research results showed that after intervention through the use of video 6 times in more than 1 week with a duration of ± 10 minutes, Mrs. “U” decreased and felt better, felt more relaxed, slept more comfortably, and woke up less frequently at night. It can be concluded that the case study of comprehensive midwifery care for Mrs. “U” runs normally and is effective in overcoming the discomfort experienced. The result of this study being limited to a single patient, restricts the generalizability of the findings.Future research with a larger sample size is needed to validate these results and explore the broader applicability of this midwifery intervention.
