Abstract

Psoriasis is a residual chronic inflammatory skin disease, characterized by a predominance of pustular eruptions accompanied by systemic symptoms such as fever lasting several days. . Psoriasis in pregnancy can lead to spontaneous abortion, stillbirth and preterm birth. Reported the case of Mrs. YS 38 years old, received from the genital skin section of RS M Djamil Padang with a diagnosis of G3P2A0H2 + pustular psoriasis. Fetomaternal ultrasound was performed, obtained biometrics: BPD: 80mm, FL: 61 mm, HL: 54 mm, AC 271 mm, AFI 10.7 cm, SDAU 2.79, estimated fetal weight 1700-1800 grams. Impression: gravid 31-32 weeks, single live fetus intra uterine. Patients are planned for administration of high doses of corticosteroids. Informed consent to the patient and family about the actions to be performed. Corticosteroid was administered as much as 30 mg long term with a dose of prednisone in tappering off every 2 weeks. The patient was discharged for control to the skin clinic and obstetrics, the patient came 12 days later and was consulted to the obstetrics department for termination of the pregnancy but from the obstetrics department there was no confirmation for termination of the pregnancy. The patient then came 13 days later with 18 hours PRM. Pregnancy termination by cesarean section was performed. a baby girl was born with LBW: 3000 gr, PB: 48 cm, A / S: 8/9, there were no congenital abnormalities such as growth disorders, cleft lip, cataracts, and polycystic kidney disease in infants. Abstract

Background : Pyometra is a rare condition with an incidence ranging from 0.01 - 0.5% among all gynecological patients.2 Pyometra is known to be rare in the general population and is more common in elderly women.3 In postmenopausal elderly patients, the incidence increases to 13.6% .4. The classic triometry of pyometra is postmenopausal bleeding, purulent vaginal discharge, and lower abdominal pain,Objective : Report the handling of pyometra casesCase Report : reportedly a 73-year-old woman came to the ER of RSUP Dr. M. Djamil on December 10, 2018 at 14.30, a referral from BMC Padang hospital with a diagnosis of abdominal pain susp. intraabdominal tumor. Ultrasound examination showed the uterus in normal size which was not in accordance with the picture of pyometra. Laparotomy with adhesionolysis and aspiration of pus in the patient was performed. This is appropriate for evacuating the pyometra mass in this case.Conclusion : In this case, to overcome the infection and evacuate the masses. The recommended antibiotics are broad-spectrum antibiotics, such as penicillin, piperacillin tazobactam, imipenem, meropenem, metronidazole, and vancomycin. Mass evacuation can be done by laparotomy or curettage accompanied by cervical dilatation.Keywords: Pyometra, pascamenoupose

Abstract

Until recently, the exact etiology and pathophysiology of preeclampsia have not been discovered yet, but based on the clinical symptoms and the defect that appeared, researchers submitted some ways as assumption or as early detection of preeclampsia and eclampsia. Some researchers have suggested the examination of calcium / creatinine excretion ratio in urine from preeclampsia patients as the result of kidney's function changes. This study has been performed with a pre and post test group design experimental method at Obstetric and Gynecology Polyclinic in RS. Dr. M. Djamil Padang and several midwife private practices in Padang since July 2013 until the samples reached the quantity up to 40 samples. Analysis has been done to describe the relationship between calcium intake and blood pressure. Furthermore, we analyzed the differences of systolic - diastolic and MAP before and after calcium intake. Data has been shown on the table and analyzed by Pearson correlation, Wilcoxon test and Paired T test. if p <0.05, it shows a significant result. The mean of systolic after calcium intake was the same for both of the groups (121.5 + 8.02: 121.5 + 6.71). Mean of diastolic after calcium intake in controls group was lower than trials group (75.9 + 4.32: 75.9 + 4.32). Mean of MAP after calcium intake for controls group was lower that trials group (91,088 + 4.47: 91,956 + 6.08). There is an influence of calcium intake on decreasing maternal blood pressure. Abstract

Objective: To determine the description of the HPV Test results with the PAP Test in patients with cervical precancerous lesions at Dr. M. Djamil Padang.Methods: This study used a descriptive method with a sample of all patients with cervical precancerous lesions who had the HPV test and PAP test results in Dr. M. Djamil Padang from January 1, 2015 - December 31, 2017. Data were taken from the patient's medical records, including diagnosis, PAP test results using the Hybrid Capture (HC) method, and HPV test. Results: During the study period, 80 patients had PAP test results leading to precancerous lesions and cervical cancer. Of the 80 patients, only 4 cases with positive HPV test results. Conclusion: In this study, there were many cervical precancerous lesions with negative HPV test results.Keywords: Cervical precancerous lesions, PAP test, the HPV test

Abstract

During normal pregnancy, glomerolus filtration rate (GFR) is increased so that the concentration of urea and creatinine decreased. With the onset of hypertension in pregnancy, renal perfusion and glomerular filtration decreases, the greater of decline showed more severe illness. This was an observational analytic study with Cohort design and performed in Obgyn Department of M. Djamil Hospital Padang, general district hospital in Batusangkar and Achmad Mukhtar, Private Practice Midwife in Batusangkar from June-December 2014. 100 samples of first trimester of pregnancy, each subject were examined ureum, creatinine, cystatin-c and glomerular filtration rate (GFR) based on CKD-EPI Cystatin and Creatinine 2012 Equation formula. Then divided into two groups, high glomerular filtration rate (GFR) high and low glomerular filtration rate (GFR) group. Each subject was evaluated blood pressure every 3 weeks and statistical analysis was done using the Independent samples test and chi square. There was significant association difference in the levels of urea, creatinine and cystatin-c between high GFR group and low GFR group (p <0.05). There was a statistically significant relationship between low GFR group of pregnant women with changes in systolic and diastolic blood pressure that persists or increases of 5-10 mmHg (p <0.05).Keywords: Preeclampsia, glomerular filtration rate (GFR), ureum, creatinine, cystatin-c, blood pressure

Abstract

The maternal mortality rate in Indonesia Demographic and Health Survey (IDHS) 2012, increased sig- nificantly to 359 per 100,000 births hidup.1 Obtained in Household Health Survey (Survey) 2001 mater- nal mortality rate can be lowered only by basic health services such as antenatal. Based on a UNICEF report, only about 45% were informed about the signs of pregnancy complications. This research was conducted to determine the cross sectional differences in the level of knowledge of pregnant women who had antenatal care at obstetricians and midwives in the city of Padang. Primigravida pregnant women who visit antenatal care at obstetricians and midwives in Padang City, amounting to 73.8% have a high level of knowledge. Abstract

It remains controversial whether antral follicle count is influenced by hormonal contraception.. This study quan-tified the effect of hormonal contraception on sonographic ovarian reserve markers in 32 users and 32 non-users of hormonal contraception. This was a prospective clinical trial study and has been performed in gynecology Department of M. Djamil Hospital Padang, primary health care March to July 2014. Samples then divided into two groups, group A was users of non hormonal contraception and group B was users of hormonal contraception. On day 2–4 of the menstrual cycle transvaginal sonography was performed and measured repeatedly in the same individual before and after the use of one cycle oral contraceptives (for group B), and on first month and second month (for group A). Statistical analysis to assess significance using the unpaired t test and chi square on SPSS 18.0 for windows. There was no significant association of the antral follicle count between users than non-users of hormonal contraception (p>0,05). There were no significant difference of antral follicle count between users and non- users of hormonal contraception. Abstract

Objective: Report a case of thanatophoric dysplasiaMethod: Case report Result: Case of a 25-year-old woman, with a diagnosis of gravid preterm G4P2A1H2 31-32 weeks + polyhydramnios + fetal hydrops, a single intrauterine live fetus with thanatophoric dysplasia. On ultrasound examination found fetal biometry; BPD: 7.78 cm, FL: 3.58 cm, HL: 3.11 cm, AC: 30.90 cm, HC: 28.48 cm AFI: 33.27 cm, a frontal bossing (+) picture appears, claver leaf skull (+) and micromelia (proximal, distal, phalanges). The ultrasound examination suggested Severe skeletal dysplasia (thanatophoric dysplasia), polyhydramnios, + single intrauterine live fetus + SC 1x scars. Then an amnioinfusion is performed and results are obtained. Chromosome analysis is carried out using the G-banding technique. Chromosomes have been studied from 20 cells from 3 different cell culture preparations and obtained the number of chromosomes in each cell studied is 46, XY which means the number of chromosomes 46 pieces with fetal sex chromosome XY. Mosaic chromosome abnormalities generally occur due to non-disjuntion in the mitotic phase after conception. At 33-34 weeks gestation, an infant was born by SC with birth weight: 1900 g, baby’s length: 31 cm, A / S 2/3.Conclusion : Thanatophoric dysplasia is a "lethal" skeletal dysplasia. A careful prenatal examination is needed in the diagnosis and termination of pregnancy.Keywords: Thanatophoric dysplasia, prenatal diagnosis

Abstract

Trauma to the pelvic floor during delivery is now recognized as a major etiological factor against PFM disorders such as urinary incontinence, pelvic organ prolapse and fecal incontinence. This study was conducted to analyze the differences in mean levels of differences of pelvic muscle strength before and after spontanous labor between stress urinary inconti-nence group and normal group. This research was done using analytic method with cross sectional design in 13 women with stress urinary incontinence, and 17 women with normal group. Subjects were collected in hospitals of Pariaman, Padang from May to December 2014. Examination of the pelvic floor muscle strength was performed with a perineometer. Differences between the mean difference in the strength of the pelvic floor muscles before and after spontaneous delivery between the two groups were analyzed using independent t test. The mean difference between the strength of the pelvic floor muscles before and after spontaneous labor in stress urinary incontinence group was larger than normal group (3.85 + 1.281 cmH2O vs 2,00 + 1.173 cmH2O, p = 0.000). The mean difference between the strength of the pelvic floor muscles before and after spontaneous labor in stress urinary incontinence group was significantly greater than the normal group.Keywords: Pelvic Floor Muscle, Urinary Incontinence, Spontaneous Labor

Abstract

Tujuan?:?Untuk mengetahui hubungan lamanya pemberiaan MgS04 pada pasien PEB dan eklampsia dengan kejadian hipermagnesium.Metode?:?Penelitian ini merupakan observasional komparatif menggunakan desain cross sectional study. Penelitian dilakukan di rekam medis RSUP Dr.M.Djamil Padang selama periode 1 Januari 2016 sampai 31 Agustus 2017 terhadap ibu hamil dengan preeklampsia berat ( PEB) dan eklampsia, didapatkan 30 subyek penelitian yang memenuhi kriteria inklusi. Selanjutnya data dicatat dalam suatu formulir penelitian yang telah disediakan, kemudian dilakukan pengolahan data penelitian. Analisis statistik untuk menilai kemaknaan menggunakan uji chi-squere .Hasil:. Terjadi peningkatan kadar Magnesium pada ibu hamil dengan preeklampsia berat (PEB) dan eklampsia sebanyak 21 koresponden. Peningkatan kadar Magnesium terhadap pasien preeklampsia berat (PEB) dan eklampsia yang mendapatkan tatalaksana MgS04 selama 48 jam sebanyak 16 koresponden ( 84,2%) dan 5 koresponden dengan pemberian MgS04 selama 24 jam.Kesimpulan:?Terdapat hubungan yang bermakna lamanya pemberian Magnesium Sulfat dengan kejadian hipermagnesium pada pasien preeklamsia berat dan eklamsia.Kata kunci?:?PEB, eklampsia, magnesium sulfat, kadar Magnesium
