Abstract

Sistem penomoran adalah pemberian nomorrekam medis bagi pasien yang datangberobat demi memudahkan pencaharianberkas rekam medis. Pelaksanaan penomoranrekam medis di Puskesmas Ambunten tidakberjalan dengan baik sehingga menyebabkanduplikasi nomor rekam medis. Tujuan daripenelitian ini bertujuan untuk meninjau sistempenomoran rekam medis pasien diPuskesmas Ambunten. Jenis penelitian yangyang digunakan deskriptif kualitatif. Carapengumpulan data dengan dokumentasidan wawancara. Subjek penelitian yaituinforman utama merupakan petugaspendaftran dan informan triangulasi yaitukepala rekam medis dan petugas poli. Metode penentuan akar permasalahanmenggunakan analisis diagram tulang ikan(fishbone) dengan metode 5M (Man, Money, Method, Materials, Machine).‟ Faktorpenyebab duplikasi dapat diketahui dalam 5Myaitu Man, Money, Method, Materials, danMachine. Prioritas masalah yang didapat yaitupada faktor Man dan Method. Pada faktorMan, petugas tidak teliti dalam menuliskannomor rekam medis pada KIB. Pada faktorMethod, penomoran masih dilakukan secaramanual, SOP belum berjalan dengan baik. Berdasarkan hasil penelitian adapun saranyang diusulkan yaitu, petugas lebih telitidalam proses pelaksanaan penomoranrekam medis, petugas poli lebih disiplindalam pengembalian dokumen rekam medis, melakukan pelatihan atau workshop terkait pelaksanaan penomoran, menambah Wi-Fidan membuat KIB dari bahan plastik untukmeminimalisir kerusakan.

Abstract

Luka kaki diabetes yang sulit sembuh mejadi penyumbang terbesar dalam proses amputasi. Beberapa metode dalam penanganan luka diabetes antara lain dengan menggunakan Negative Pressure Wound Therapy (NPWT).Tujuan penulisan ini untuk mengetahui efektivitas NPWT terhadap penyembuhan luka kaki diabetes. Metode penulisan menggunakan penelusuran online data base diantaranya google scholar, proquest, dan ebsco dengan kata kunci NPWT or VAC, Diabetic Food Ulcer or DFU. Hasil Penelitian ditemukan delapan jurnal RCT dan 1 sistemativ review. Kesimpulan terdapat pengaruh NPWT terhadap penyembuhan luka kaki diabetes dan dapat menjadi rujukan dalam penatalaksanaan pasien dengan luka kaki diabetes.

Abstract

Prematur babies are one of the causes of infant mortality, especially in the first month of life. The incidence of prematurity in pregnancy is caused by multifactors, namely maternal factors, fetal factors, and other factors. The purpose of this study was to determine the effectiveness of Early CPAP on Down Score Improvement for Prematur Babies at 32 Weeks Gestational Age in the Neonate Room who were born at RSUD with referral births at RSUD Dr. HaryotoLumajang. In this study, a pre-experimental research design was used with a one group pretest posttest design approach. The study population was 32 weeks' gestational prematur infants who were born in RSUD with referral births in the neonatal room at RSUD Dr. HaryotoLumajang as many as 22 samples. The sampling technique used purposive sampling. Data collection used Observation Sheets and patient medical record data, for data analysis using the T-Test. This research was conducted from March 12 2023 to April 12 2023. The results showed that those who received CPAP treatment earlier had a better mean score of 4.36 compared to those who received CPAP late, which was 6.18. Early administration of CPAP is effective in improving the Down Score of prematur babies born at 32 weeks' gestation at RSUD dr. Haryoto Lumajang with a value of p = 0.00 <0.05. From this research it is suggested that health services become a source of information for health agencies and recommendations for policy makers to take strategic steps to deal with prematur babies.

Abstract

Type II DM is a glucose metabolism disorder caused by insulin resistance, insulin secretion which can cause various chronic complications. Diabetes Self Management Education (DSME) facilitates knowledge of self-care skills so that patient can solve the problem. The research aims to determine the effect of Diabetes Self Management Education (DSME) on Life Satisfaction of Diabetics Type II Melitus at the Kedopok Health Center, Kedopok, Probolinggo. The study is pre-experimental, with a one-group pre-post design. Purposive sampling was used to select a population of 69 respondents and a sample of 59 respondents who met the research inclusion requirements. An assessment questionnaire sheet was used as the instrument. The data for the Satisfaction with Life Scale (SWLS) is gathered by editing, coding, scoring, and tabulating. The Wilcoxon signed rank test was used to analyze the obtained data.The results before the DSME was carried was satisfied with 22 respondents (38.6%), after The most DSME carried out were very satisfied 27 respondents (47.4%). The analysis test is found There is The Influence of DSME on Life Satisfaction of Type II Diabetes Mellitus Patients in Kedopok Public Health Center Kedopok Probolinggo with -value 0.000 0.05. Abstract

Diabetes mellitus type 2 is one of the non-communicable diseases that can be caused by unhealthy living habits and diet that cause hyperglycemia, this is due to the body's lack of insulin both absolute and relative so that blood viscosity increases and impacts ineffective peripheral perfusion.Type 2 diabetes mellitus is one of the non-communicable diseases that can be caused by unhealthy living habits and diet that cause hyperglycemia, this is due to the body's lack of insulin both absolute and relative so that blood viscosity increases and impacts ineffective peripheral perfusion. Foot spa therapy is one of the nursing interventions that can be done to prevent ineffective peripheral perfusion and other complications in the feet. Case study objective is to describe foot spa therapy in patients with type 2 diabetes mellitus with ineffective peripheral perfusion nursing problems. The design of this scientific paper is descriptive with a case study research approach by applying nursing interventions based on evidence of practice. This case study data collection method uses participatory observation, structured interviews, documentation, literature study. The results of the case study show that the application of foot spa therapy for five days can improve peripheral perfusion so that blood flow is smooth and prevent the occurrence of ineffective peripheral perfusion. The conclusion obtained is that the application of foot spa therapy in type 2 Diabetes Mellitus patients with ineffective peripheral perfusion problems can be prevented so that based on the article it is proven that the application of foot spa therapy is effective to be applied as a prevention of ineffective peripheral perfusion.

Abstract

Hemodialysis is the most common kidney replacement therapy performed by patients with chronic kidney failure. Patients undergoing hemodialysis must first have a vascular access installed to drain blood from the patient’s body to the machine. Complications that can occur in vascular access is inflammation at the insertion site. Nursing efforts to reduce inflammation are by using cold compresses. The purpose of this research was to analyze the effect of cold compresses on the incidence of vascular access inflammation. The research design was pre experimental with a one group pretest posttest research design. The population in this research were all patients who had recently undergone hemodialysis who experienced inflammation of the vascular access. The samples were taken using a purposive sampling technique so that 19 samples were obtained. Data collection tool to determine the incidence of inflammation using an observation sheet. The result of the Paired T-test before and after the cold compress intervention proved that there was a decrease in the incidence of inflammation vascular access with a value p = 0,000 and  < 0,05. It can be conclude there is an effect of cold compresses on the incidence of inflammation of the vascular access in patients who have just undergone hemodialysis. Based on the results of this study, it is hoped that it nurses are advised to apply cold compress therapy as an intervention to reduce inflammation of the vascular access of hemodialysis patients.

Abstract

Hemodialysis is a medical treatment to replace kidney function when is in trouble. Hemodialysis which is performed regularly, should provide comfort to the patient Comfort can improve the quality of life for patients who routinely undergo hemodialysis, including the selection of vascular access for hemodialysis patients. The aim of this study was to determine the relationship between the choice of vascular access and quality of life for patients undergoing hemodialysis. This research is a quantitative study using cross sectional correlation method design. The population of this study were all regular CKD HD patients in the hemodialysis room at Dr. Haryoto Lumajang with a total of 144 patients. The samples were 132 respondets taken using total sampling technique who met the inclusion criteria. The data collection the WHOQOL-BREF (The World Health Organization Quality of Life) questionnaire. The Data analyzed using the Spearman Rank Test. The results obtained showed that most of the respondents used vascular AV shunt access of 63.6% and most of the quality of life of hemodialysis patients was good, 52.3% with a significance value of 0.000 (p <0.05) meaning that there is a relationship between the selection of vascular access using the AV shunt method and the patient's quality of life. The Spearman correlation value indicates that vascular access has a sufficient correlation (0.350) and indicates a positive correlation direction. where the AV shunt vascular access shows the patient's quality of life to be good. Based on the results of this study, it is hoped that it can add to and increase knowledge about the quality of life of patients undergoing hemodialysis therapy, especially for hemodialysis patients based on vascular access

Abstract

Background: Nowadays, the incidence of cardiac arrest affects about 135 million people with the survival rate ranges from 2% to 11%. Seeing the trend of the incidence of cardiac arrest that occurred at this time, the necessary steps for appropriate treatment increased the survival rate of cardiac arrest patients performing well in CPR. Increasing the high quality of CPR gives a great impact on patient safety in both cardiac arrest outside and inside the hospital. The aim of this study was to identify the strategies to improve high quality of CPR. Methods: This literature review was done by collecting and analyzing articles concerned on high quality CPR. Articles were collected through electronic databases: EBSCOhost, ProQuest and Science Direct used keywords about CPR, high quality CPR, and cardiac arrest. A number of research articles were reviewed and the criteria of articles was full text and published between 2009 until 2021. Results: The important strategies in achieving indicators of high quality CPR are: correct patient position, using hand dominant position, turn over the rescuer position, body mass index and body posture, using video simulation, and using music to maintain the correct rhythm during CPR. Conclusion: High quality CPR is essential to apply both intra or out of hospital to maintain the survival rate of cardiac arrest patients. Therefore, using a strategy is expected to improve skill and outcome of high quality CPR.

Abstract

Schizophrenia is a mental disorder in which a person loses his mind and is unable to interact with other people. Therefore, family support is very important in the healing process of schizophrenic patients. Family support is an important factor in medication adherence in schizophrenic patients and therefore can have a positive impact on mental health. The purpose of this study was to see the relationship between family support and factors that influence the recurrence of schizophrenic patients. The type of research method used is correlational using a cross sectional design. sampling using the Incidental Sampling technique, namely as many as 90 respondents. The results of the study showed that the majority of schizophrenia patients relapsed frequently, namely 50 respondents (55.5%), family support based on low majority ratings, namely 46 respondents (52.3%), family support based on informational majority, low, namely 60 respondents (66.7%), family support based on emotional majority is high, namely 52 respondents (57.8%), family support based on instrumental is high, namely 51 respondents (56.7%), the results of cross-tabulation between family support and the incidence of relapse in schizophrenia patients, the results obtained are p -value of 0.001 with a significant level of p <0.05, the family support variable that influences relapse in schizophrenia patients is the category of emotional support with the highest Old Ratio (OR) value of 1.734. Abstract

Low preparedness can lead to more serious disaster impacts (floods), namely the number of fatalities, serious injuries, the number of refugees and the occurrence of diseases due to poor environmental conditions. (Kusno F., 2019). The research aims to Improve Disaster Mitigation through Disaster Risk Reduction (DRR) Strategies for Flood Disaster Response. Mitigation Activities through Disaster Risk Reduction (DRR) include Disaster Risk Assessment Methods, Seasonal Calendar Analysis, and Table Top Applications. By knowing the degree of vulnerability and hazard and the amount of capacity they have, the community is able to plan anticipatory actions to be taken. Strategies that can be carried out to reduce disaster risk including several activities aimed at creating resilience in society against the consequences of natural, technological and environmental disasters and improving the disaster defense model into disaster impact management by incorporating risk prevention strategies into activities sustainable development.
