Abstract

Background. The insertion of an Endotracheal Tube (ETT) can trigger the occurrence of Ventilator Associated Pneumonia (VAP) due to bacterial colonization of secretions in the oral or tracheal area of the patient. One of the prevention of VAP can be in the form of providing oral hygiene in intubated patients. The purpose of the implementation is to find out the application of the use of oral hygiene guidelines from the Beck Oral Assessment Scale (BOAS) in intubated patients in the ICU Room of Prof. Hospital. Dr. Margono Soekarjo Method. Implementation was carried out using a case study method with a total of 10 patients divided into 2 groups. Both groups were assessed for BOAS at the time the patients were intubated and extubated. Evaluation was carried out using the Mucosal-Plaque Score (MPS). Results. Characteristics of respondents have an age range of 37-66 years. It is known that the average difference in the decrease in BOAS scores is the intervention group. While the MPS Score in the two groups did not have a difference. Conclusion. Oral care hygiene with the guidance of the Beck Oral Assessment Scale (BOAS) can be used to reduce the risk of Ventilator Associated Pneumonia (VAP) in intubated patients.

Abstract

Background: COVID-19 is caused by the Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) virus. The spread of this virus is very fast, so patients come to the Emergency Department (ED) for examination. The initial examination was carried out by triage the patient using the RT-PCR method. Then, there are recent studies that Computed Tomography (CT) can be used as a triage tool and help in the early diagnosis of patients with suspected COVID-19. Objectives: To determine chest CT for triage in the Emergency Department (ED) in patients with suspected COVID-19. Method:Search for articles was conducted electronically using Google Scholar, Sience Direct, and PubMed databases. Article used from March 2020 to January 2021. Keywords in this article search used the keywords "chest CT" for "triage" and "emergency department" for "COVID-19" and "chest x-ray" for "triage" and "emergency department" for "COVID-19", thus getting 5 research articles reviewed. Results: A chest CT that can be used as a triage tool based on the speed at which the chest CT results can be released depicting a confirmed COVID-19 patient in 60 minutes. Second, based on the sensitivity of chest CT there was 90.2% and according to chest CT instructions there was 89%. Conclusion: Chest CT examination is very effective as the first triage in diagnosing patients with suspected COVID-19.

Abstract

INTRODUCTION : Acquired immunodeficiency syndrome (AIDS) is a collection of disease symptoms that attack human body after HIV (Human Immunodeficiency Virus) damage the immune system. HIV-AIDS is a disease that cannot be cured now, but there are therapies to help improve the quality of life for people with HIV, and also to increase the antibodies for people with HIV and the therapy is called ARV. The main goal of ARV treatment is to reduce the number of HIV viruses to delay and stop the growth of the virus. METHOD : Determine the latest intervention regarding evidence based on HIV-AIDS and ARV pharmacological therapy. After determining the topic, the next step is to search for literatures that will be reviewed, the literatures are in the form of scientific journals obtained from databases such as: Science Direct, ProQuest, and Google Scholar with keyword: “ARV for HIV/AIDS” for international journals and for the national journals using Google search engine from 2014-2019. RESULT : Based on journals as a literature, the conclusion is that ARV therapy can improve the quality of life for people with HIV-AIDS, the characteristic of the therapy is to form the antibodies and produce proinflamation agents to reduce the growth of the viruses, beside that the therapy is also effective if it given directly to HIV-AIDS patients who early diagnosed as a HIV-AIDS patient, and also the ARV therapy can improve the quality life of children and reduce the risk of HIV-AIDS in children who have mothers with HIV-AIDS. CONCLUSION : ARV therapy can improve the quality of life and prevent the transmission from mothers with HIV to their children.

Abstract

Introduction: Difficulty in assessing pain in someone who cannot express the pain they feel, for example in children with cognitive impairment, provides a significant obstacle in pain management. The purpose of this study is to review the literature containing pain assessment instruments in children with cognitive impairment and to discuss instruments that can be applied effectively and have clinical use. Method: Search for English articles in the Medline, Cinahl, and PubMed databases, and is limited from 1990 to 2013. Through keywords according to topic and obtained 5 articles that were reviewed. Result : Pain assessment in children with cognitive impairment can be done through rFLACC, NCCPC-PV, NCCPC-R and NAPI. rFLACC is the most effective instrument and has great clinical utility with ICC : 0.73-0.9). Analysis: rFLACC assesses 5 indicators (face expression, leg, activity, cry & consoability) that are simpler and easier to apply to all children with cognitive impairment at all levels. Discussion: Elective postoperative pain assessment in clinical settings in children with cognitive impairment can be performed with rFLACC. In an effort to increase the reliability of the instrument, the improvement of leg items and activities can be the basis for further research.

Abstract

Gouty arthritis is a disease that continues to cause pain and tenderness due to deposits of monosodium crystals that accumulate in the joints. Regular consumption of apple juice can decrease pain response by decreasing pain intensity and impulses. The purpose of this study was to analyze the relationship between consumption of apple juice and a decrease in pain response in patients with gout arthritis in the Kebonsari Public Health Center, Pasuruan City. This research is a non-experimental quantitative study using a descriptive study with a cross sectional approach. The population of this study were all patients with gout who went to the health center as many as 60 patients. The sampling technique in this study was consecutiv sampling, totaling 60 respondents. The instrument used to examine pain response and juice consumption was an observation sheet. From the data collected, then testing using sperm rank. The results obtained indicate that based on the test value of 0.000 (ρ <0.05) in H1 is accepted. This means that there is a relationship between consumption of apple juice with a decrease in pain response in patients with gout. Lifestyle changes for gout sufferers are needed to reverse uric acid levels in the blood.
