Abstract

The aspect of Occupational Health and Safety (OHS) is a crucial factor that must be implemented across all industrial sectors, especially in production processes involving heavy machinery and high-risk physical activities. This study was conducted at PT. Putra Agri Pas with the aim of identifying potential hazards and assessing the level of occupational accident risks using the Hazard and Operability Study (HAZOP) approach. This method is applied to detect process deviations and to formulate appropriate preventive strategies. Data were collected through field observations and employee interviews, then analyzed using HAZOP worksheets and the Root Cause Analysis (RCA) method. The research findings indicate that most hazards fall into medium to high risk categories, primarily due to a lack of discipline in using personal protective equipment (PPE), insufficient safety training, and weak operational supervision. The study recommends enhancing control measures, conducting regular training sessions, and organizing the work environment to ensure a safer workplace.

Abstract

The construction of the Patung Lembuswana–Sebulu STA 31+975 road section in East Kalimantan faces slope stability challenges due to hilly topography and high rainfall, which trigger landslides. This study aims to analyze slope stability using the Fellenius method, evaluate the safety of retaining walls with Rankine and Coulomb approaches, and design pile foundations based on site geotechnical data. The analysis shows a slope safety factor value of less than 1.5, indicating instability. The retaining wall is safe against overturning (safety factor > 2), but unsafe against sliding and bearing capacity failure. The pile foundation design provides a bearing capacity of 823.593 tons, exceeding the planned load of 482.743 tons, and is therefore considered safe. A combination of slope reinforcement, retaining wall design, and pile foundations is recommended to improve overall stability and reduce landslide risk at the project sit.

Abstract

Waste management issues in Indonesia’s rapidly urbanizing areas have driven the emergence of community-based initiatives such as waste banks. This study examines the role of Pinang Lestari Waste Bank, located in Gunung Panjang Subdistrict, Samarinda Seberang, in reducing landfill-bound waste and empowering the local economy. Using a mixed-methods approach, data were collected through interviews with managers and residents, alongside quantitative records of waste volumes and recycled products. The findings show a monthly waste reduction of approximately 7 tons and annual revenue of around IDR 40,000,000 from compost, eco-enzymes, and plastic-glass crafts. Community participation in waste sorting increased, reflecting the effective application of circular economy principles. These results offer a foundation for replicating this waste bank model in other regions as a sustainable waste management strategy

Abstract

Inefficient material distribution at PT XYZ causes production delays in tactical vehicles and increases operational costs. This study aims to identify waste in the material distribution process using the Lean Logistics approach with the help of Value Stream Mapping (VSM) and Process Activity Mapping (PAM) tools. The analysis shows that out of a total distribution time of 6,835 minutes, only 273 minutes (4%) are value-added (VA) activities, while the remainder consists of non-value added (NVA) and necessary non-value added (NNVA) activities. These findings highlight the dominance of wasted time due to delays, transportation, and inefficient storage. The study recommends continuous improvement through lean tools and the implementation of digital technologies to accelerate distribution flow and reduce waste. This approach can enhance overall logistics efficiency and company productivity.

Abstract

Quality is one of the important factors in the production process. Damaged or defective products cause significant losses to the company. In this study, defective products are products that do not meet the quality standards that have been set during the manufacturing process. When customers feel that the product cannot be used according to their needs, the product will be estimated as a defective product. This research uses the fishbone method, which is more comprehensive than other methods, because it can cover the causal factors of consequences clearly and in detail. Failure Mode and Effects Analysis (FMEA) method to evaluate the risk of failure at the production stage with a quantitative approach. With the application of these two methods, it is expected to analyze the problem quickly, precisely and focus on the cause effectively. Data is collected through direct observation (primary data) and interviews with employees (second data). The results of this study are expected to enable the company to identify and improve the quality of products to be produced in the future.

Abstract

This study aims to analyze ergonomic risk in manual lifting activities at the food storage warehouse of PT XYZ, located in Cikarang Barat, using the NIOSH Lifting Equation method. A total of 20 workers were observed, with ergonomic variables measured such as load weight, lifting frequency, lifting distance, and body posture. The results showed that the average initial Lifting Index (LI) was 2.328, significantly exceeding the safe threshold (LI ≤ 1), indicating a high risk of musculoskeletal disorders. After an intervention using a team lifting system, the average LI decreased to 1.207. Although not yet fully within the safe zone, this method effectively reduced ergonomic risk. Recommendations include ergonomic training, use of lifting aids, and broader implementation of team lifting. This study employs a descriptive quantitative approach.

Abstract

PT. X is a manufacturing company that produces fire extinguishers (APAR), where cylindrical plate welding is a critical process. However, the process is often inefficient due to several non-value-added steps. This study aims to improve welding productivity using Lean Six Sigma through the DMAIC (Define, Measure, Analyze, Improve, Control) approach and by measuring Overall Equipment Effectiveness (OEE). The initial OEE value was 67.49%, far below the world-class standard of 85%. Pareto analysis showed the highest downtime was due to cycle time issues, especially during setup and cutting of additional plates. Improvements were made by eliminating the cutting process and adding a stopper to ensure accurate welding positions. These changes reduced the cycle time from 180 seconds to 120 seconds and increased the OEE to 76.12%.

Abstract

Production planning and control are essential aspects in ensuring smooth production processes, creating profitable products, and making sure products are available on time. The home industry for tempeh production located in Link. Jombang Kali, Jombang District, has been operating since 1993 and continues to this day. However, this tempeh factory often experiences fluctuations in production, including both overproduction and underproduction. Therefore, a study was conducted to evaluate and forecast the production levels for upcoming periods. Forecasting errors were evaluated using Mean Absolute Deviation (MAD), Mean Squared Error (MSE), and Mean Absolute Percent Error (MAPE). The purpose of this study is to compare the Moving Average and Exponential Smoothing methods. The results of the analysis show that the Exponential Smoothing method produces lower error values, with MAD of 387.163, MSE of 261,914.1, and MAPE of 0.055349, compared to the Moving Average method.
