Abstract

SARS CoV-2 is a new type of coronavirus and infects respiratory tract. Covid-19 may occur secondary bacterial infection, notably in severe degrees. Secondary bacterial infections are generally associated with viral respiratory infections concurrently after the primary infection. Sputum examination is essential to diagnose the etiology of the disease. So far, there has not been much research on secondary bacterial infections in Covid-19 patients. This research is a descriptive retrospective study. The study was conducted on moderate and severe Covid-19 patients treated in the PINERE Room by looking at the results of bacterial sputum cultures in confirmed Covid-19 patients from the Clinical Pathology Laboratory of Arifin Achmad Hospital, Riau Province starting from August 2020 to July 2021. This study is for the results of bacterial sputum culture with secondary infection in moderate and severe Covid-19 patients. The purpose of this research to analyze the relationship between sputum culture results and severity and outcome in Covid-19 patients. Of the 204 samples of bacterial sputum culture, the results obtained were normal flora 42.16%, Klebsiella pneumonia 22.55%, no growth of bacteria 10.78%, Acinetobacter baumannii 6.37%, Staphylococcus aureus 5.39%, Pseudomonas aureginosa 3, 43%, Staphylococcus maltophilia, Escheria colli, Enterobacter clocae 2.45% each. Based on the morphology of Gram-negative bacteria, 41.1% more than Gram-positive 6.37%.

Abstract

The assessment of learning in the community in the Kampung Pelita Medika (KPM) program of the Medical Faculty of Universitas Riau has been carried out as a physical portfolio. Physical portfolio causes problems due to accumulating working papers, manually inputting data, and the non-integration of data. One of the things that can be done is to use a Learning management system (LMS). The study aims to design an LMS-based assessment system suitable for the KPM program and assess its ability to be implemented according to student perspectives. The survey was conducted on 30 students using quantitative and qualitative approaches. Data was taken by filling out the LMS proficiency satisfaction questionnaire and open-ended questions. Research showed that 37% uploaded on time, 50% were late, and 13% did not upload. Most (85 %) stated the LMS was easy to use, 15 % said it was quite difficult, and 0% stated it was difficult. Some obstacles were uploading large documents, unstable internet networks, less informative task commands, and adaptation to the application. Some proposals for improvement were a simpler document upload system, an online checklist feature, and an unsubmit component. This study concluded the LMS-based KPM assessment system was quite feasible to use. However, qualitative data indicate the need for improvements to the upload system and more informative task commands. Further research needs to be done on a larger sample to assess the effectiveness of using the LMS for the assessment system by reviewing applications to lecturers, administrative personnel, and perceived benefits

Abstract

Cervical cancer is the fourth most common cancer among women globally that is caused by high-risk Human Papillomavirus (hrHPV). One of the most common HPV genotypes causing cervical cancer is HPV 18. However, there is a lack of data regarding hrHPV type 18 prevalence in Indonesia, especially Riau Province. This study aimed to describe the number of high-risk Human Papilloma Virus type 18 cervical cancer patients in Arifin Achmad General Hospital Riau Province. The method used in the study was descriptive with consecutive sampling of HPV 18 of the cervical cancer patients in Arifin Achmad General Hospital from January – July 2022. HPV DNA isolate was identified with a polymerase chain reaction (PCR) test. We recruited 63 women to participate and 32 women (50.79%) were tested for HPV 18. Among the 32 women, the result showed that the most common type of cervical cancer based on the anatomical pathology test was non-keratinizing squamous cell carcinoma (65.625%), keratinizing squamous cell carcinoma (9.375%), and non-keratinizing squamous cell carcinoma invading lymphovascular nodes cervix (6.250%). This study provides descriptive information about the frequency and type of anatomical pathology of cervical cancer caused by HPV 18 among female patients in Riau province. Therefore, it can be used as data in the diagnosis and prevention of HPV in Indonesia.

Abstract

Vitamin D is a potent steroid hormone calcitriol precursor which is known to have important roles during pregnancy, especially for fetal growth. Previous research proved vitamin D supplementation during pregnancy can decrease the risk for women to give birth to small for gestational age babies. Rupat Utara District is a remote area and located far from secondary health care facilities or hospitals, therefore iexat became one of the most concerning areas in Bengkalis Regency due to the high number of stunting cases. This is why complete examination in pregnant women with vitamin D deficiency risks living in Rupat Regency is important to lower the number of stunting cases. This study is aimed to describe the prevalence and understand the factors that can contribute to vitamin D deficiency conditions in pregnant women. This study used the descriptive method. The sample recruited was pregnant women living in Rupat Utara District and 35 pregnant women matched the inclusive and exclusive criteria for the study. The result showed that the number of vitamin D deficiencies was 60%. Subjects with vitamin D deficiency mostly were in the third semester gestational age (42.9%), Melayu ethnicity (71.4%), not working (81.0%), being outside for less than 1 hour/day (71.4%), and not regularly taking vitamin D supplementation before the study (57.1%). This high number of vitamin D sufficiency in Rupat Utara District reflected how vitamin D supplementation during pregnancy is important.
Correlation Between C-Reactive Protein (CRP) and Lactate Dehydrogenase (LDH) Levels with Severity and Predicting Outcome of Confirmed Covid-19 at Arifin Achmad Hospital, Riau Province

Abstract

Coronavirus Disease 19 (COVID-19) triggers a series of inflammatory processes. The inflammatory process damages the hematopoiesis system, especially the bone marrow, releasing white blood cells and stimulating the liver to secrete acute phase proteins such as ferritin, C-Reactive Protein (CRP), and Lactate Dehydrogenase (LDH). Laboratory tests including CRP and LDH can determine the severity of COVID-19. This study has the purpose to investigate the correlation between LDH and CRP with the severity of confirmed COVID-19 patients and predicting the outcome. This study used a cross-sectional design to determine whether CRP and LDH were able to predict mortality in confirmed COVID-19 patients about illness severity who were hospitalized from April 2020 - July 2021 at Arifin Achmad Hospital Pekanbaru. This study included 522 patients, of which 73 patients (14.00%) died and 449 patients (86.00%) experienced improvement during treatment. The mean LDH and CRP levels in this study were 293.47 ± 192.64 u/L and 55.48 ± 79.03 u/L, respectively. The area under the curve (AUC) ROC for patient outcomes on LDH and CRP examinations was 0.74 and 0.80 with cut-off levels of LDH and CRP to predict mortality in COVID-19 patients of 297 (sensitivity, specificity, PPV, NPV = 60.2%, 53.9%, 17.5%, and 89.3%) and 52 (sensitivity, specificity, PPV, NPV= 63.0%, 78.1%, respectively, 31.9%, and 92.8%) with p-values for all results <0.001. CRP and LDH testing can be used to predict death, but in this study, CRP has an advantage in predicting death.

Abstract

Choosing the right education for their children is the most crucial factor that parents must do especially in Indonesia. The popular choice for study must be an S1 university degree while studying in vocational education probably becoming the least choice despite the raising popularity of vocational education recently. One of the private vocational education institutions in East Java is Politeknik Ubaya. It has several study programs and one of the study programs is the Business English study program. Although it has been established for about twenty years the study program also struggles to attract more students. Therefore, in the middle of 2022, they decided to do a rebranding and change the curriculum becoming an English for Business and PR study program. The purpose of this study is to find the need for the rebranding of the Business English study program as well as the impact, whether the rebranding attracts more students or not. The research method used for this research is quantitative research by using questionnaires and non-formal interviews as a tool. The target respondents were the new students or semester one students of the study program. The writer has conducted preliminary observation for this review, and so far the study program attract more students to register, but the further in-depth study is needed to reveal the issue. This research will be beneficial for vocational education, especially to prove that rebranding can be an important factor to attract students. The suggestion for further research is emotional branding on vocational education to attract students.
Analysis of Antihypertensive Drug Use in Pregnancy

Abstract

The management of hypertension in pregnacy therapy aims to control the patient's blood pressure to the extent that it does not interfere with or damage other organ or physiological functions, reducing the risk of morbidity and mortality. The drugs used must be safe, effective and rational to achieve the desired therapy based on the high blood pressure experienced. The aim of this study was to analyze the use of antihypertensives in inpatient pregnancy at the Arifin Achmad Hospital, Riau Province, whether it was in accordance with the guidelines. The research method used a cross sectional study, data were taken using medical records and samples were taken by total sampling. Data taken from January 2019 to December 2021. The analysis was carried out descriptively. The results showed that out of 176 hypertensive patients in pregnancy, most were sin the age group of 21-35 years. The most common diagnosis was preeclampsia 90.9%. The most widely used antihypertensive by patients was a combination of Nifedipine and Methyldopa (64%). Most of the use of antihypertensive is in accordance with the guidelines (89.8%). In general, the use of antihypertensives in inpatient pregnancy at Arifin Achmad Hospital is in accordance with the guidelines. Proper management of hypertension will reduce morbidity and mortality.

Abstract

Coronavirus disease (COVID-19) is an infectious disease that has been designated by the World Health Organization (WHO) as a global pandemic. There are different types of COVID-19 vaccines that have been developed. Reactogenicity is a reaction that occurs immediately after vaccination. Differences in individual immune responses against antigens can trigger local and systemic reactions. Medical events due to immunization are known as Adverse Event Following Immunization (AEFI). The purpose of this study was to determine the reactogenicity profile of health workers receiving the heterologous mRNA (Moderna) booster vaccine at Arifin Achmad Hospital. This study was a cross-sectional study to determine the Adverse Event Following Immunization (AEFI) of health workers receiving heterologous mRNA booster vaccines (Moderna) at Arifin Achmad Hospital. Respondents in this study were mostly women (63.2%), aged 30-39 years (42.5%), overweight (50.6%), had no comorbid disease (74.7%) and were not survivors of COVID -19 (86.2%). From a total of 87 respondents, there were KIPI in the form of general complaints of fever/chills in 70 respondents (80.5%), swelling in the injection area in 44 respondents (50.6%), muscle pain in 72 respondents (82.8%), decreased/increased appetite 17 respondents (19.5%), often sleepy/sleep longer 14 respondents (16.1%), numbness in hands and/or feet 8 respondents (9.2%), nasal congestion 11 respondents (12.6%), and cough 6 respondents (6.9%).

Abstract

Purwokerto Islamic Hospital is an Advanced Referral Health Facility that receives referrals from first-level health facilities so the hospital management system will become a very important requirement for managers, clinicians, and patients and their families. Hospital Management Information System is a system used to collect management data from a hospital, in which many installations, departments, and units are incorporated into a complete system. The Ministry of Health seven years ago has launched 7 Health Development Reforms, namely 1) revitalization of health services, 2) availability, distribution, retention, and quality of human resources, 3) seeking availability, distribution, safety, quality, effectiveness, affordability of drugs, vaccines and medical equipment, 4) Health insurance, 5) partiality to disadvantaged areas, borders and islands (DTPK) and health problem areas (DBK), 6) bureaucratic reform and 7) world-class health care. Currently, the hospital management information system in the hospital unit needs an operator to operate it. So that managerial and clinicians in accessing information systems in hospitals are also still limited. So the researchers tried to assess the application of a special hospital management information system in intensive care to explore further the advantages and disadvantages or the benefits and obstacles as well as the quality of this hospital management information system. Especially since the implementation until now SIMRS has never been evaluated. From the search results, researchers found many studies on SIMRS. After trying to sort it out, seven studies on SIMRS were taken, starting from the 2005 study to the 2018 study. Most of them used qualitative methods, there was one study in Uganda that used Mixed Methods and two studies that used quantitative (Greece and Kenya). Most of the data collection was in the form of observation and interviews, except for Greece and Kenya with questionnaires and scales, and Uganda which used both questionnaires and interviews. The results of these seven studies show that SIMRS is beneficial from various points of view. General Purpose, To analyze the application of the hospital management information system application program in the intensive care unit of RSI Purwokerto. The specific objective is to find out the benefits or improvement of service quality, the obstacles and the quality of the application program of the hospital management information system of RSI Purwokerto as well as the flow of systems and service procedures in the unit.

Abstract

Stress Urinary Incontinence (SUI) significantly affects women’s quality of life through physical and psychosocial limitations and sexual dysfunction. This condition may cause a huge individual and family burden, leading to a global burden. The coastal area is geographically located in islands confined by the sea, thus becoming the basis of health problems due to its limited access to healthcare facilities. No study has been conducted to identify the SUI prevalence rate. This study aims to help determine problems caused by SUI, eventually conceiving effective prevention and treatment strategies to be proposed to health policymakers. This research is a quantitative study with women of reproductive age (20 – 44 years old) and >44 years old in 4 coastal areas (Bengkalis, Dumai, Siak, and Rumbai Pesisir) as the key population. Of 320 patients, 178 (53,80%) were screened for urinary incontinence. Among 178 screened subjects, 152 (85%) were classified as stress urinary incontinence, with the highest incidence rate found at Dumai, with 59 (39%) cases. Bengkalis, Siak, and Pekanbaru had an incidence rate of 51 (33%), 33 (22%), and 9 (6%), consequently. This study found most women had urinary incontinence, with the most common type being stress urinary incontinence. Based on this study, due to the high prevalence of SUI, the authors agreed that effective prevention and treatment strategies are needed to prevent SUI's severity and further development.
