Abstract

Based on research at RSUPN Dr.  Cipto Mangunkusumo, Central Jakarta, there was a delay in the distribution of medical record documents to arrive at the surgical clinic more than 30 minutes. The purpose of this research was to identify the causes of the delayed in the distribution of medical records documents to arrive at the surgical clinic based on 5 management elements, such as man factors, materials factors, machine factors, methods factors, motivation factors and the factors of arrange the recommendations effort for improvements in the supply of medical record documents at RSUPN Dr. Cipto Mangunkusumo. This type of the research uses qualitative analysis. Based on the results and discussion, in the man factor there are filing staffs who still lack discipline such as, often come late, the one of the filing staff often absen. The material factor there is still a medical record document which folder / cover is torn, and therefore the medical record number and the patient's name are not visible or unclear, so that the staffs has trouble when searching for the medical record documents. The machine factor, which is EHR, usually occurs loading or could not be a accessed so the tracer not coming out and when the EHR can be accessed, the tracer will come out with a lot. Ther methods factor is KPI percentage of the speed of providing medical records of patients without agreement arrived at the  polyclinic in 2019 still has not reached the target of 90%. After an evaluation, in 2020 the time of the document arrived at the polyclinic would be less than 30 minutes by reducing the target to 75%. Motivation factor there is no external motivation provided by the head of the medical record .The conclusion is the filing staffs did not come on time and did not come, the materials of the cover on medical record documents are easily torn, EHR is often difficult to access, the achievement level has not yet reached the target of providing medical record documents, and there is still no motivation from the head of medical record.

Abstract

Outpatient coding of RSUP Dr. Kariadi has an average of 240 file targets to be coded for each officer, coding activity can trigger work risks such as stress, loss of concentration and illness due to work fatigue. The purpose of this study was to Analyze the occupational risks of outpatient coding officers in RSUP Dr. Kariadi Semarang. This type of research is a descriptive study with an interview and observation approach. The object of this study was outpatient coding officer in RSUP Dr. Kariadi. The results showed that the risks in outpatient coding officers there are 5 risks that can occur. Risks regarding workplace accidents as well as work system risks can be found in outpatient coding. The solution is to create an SOP on the safety of outpatient coding officers.

Abstract

One indicator in measuring the quality of medical records is the punctuality. The punctuality means managing the medical record file from the patient registering until the medical record file with the time limit specified in the standard operating procedure. At Haji Hospital Surabaya, the delay in providing the  inpatient medical records is still a problem. It can cause obstruction of the patient care process so that it will also affect service satisfaction. This research was aimed to analyze the activities of providing inpatient medical records at the Hajj General Hospital in Surabaya with the type of qualitative research. Data collection techniques used are observation, interviews, and documentation based on the variables Ability, Motivation, and Opportunities. The results of this study indicate that there are no SOPs related to the time the provision of inpatient medical records is the main factor being addressed. The Efforts can be made by rearranging SPOs related to providing inpatient medical record time.

Abstract

Based on the results of preliminary studies conducted at the RSUP dr. Soeradji Tirtonegoro is known thehighest delay count is the Anggrek of 95% room and 4th Dahlia room of 80%. The delay has an impactwhich daily census returned is not timely, and be it can increase the work of officers for the next day andsending the recapitulation results to the reporting department is delayed. This study aims to observe theimplementation of the daily census and the factors causing the inaccuracy in returning the daily census ofhospitalization, the determination of the main causes of the problem are using a fishbone diagram. Thistype of research uses qualitative and are collected a data with observation, interview and documentation.The results of the main problem are Man (the work of the recapitulation officer is delayed and ineffective,the employee's indiscipline). Method (there is not written policies of Standard Operating Procedure, theofficer does not know the Standard Operating Procedure). Minutes (The deadline for retuning the dailycensus of hospitalization are not consistent). Machine (there are no features that support the daily censusof hospitalization in SINERGIS, daily inpatient census is manually). Money (there is no rewards).

Abstract

Patient satisfaction is the result of the patient's assessment of the service received based on truth and reality that is then compared to his expectations. The results of observations made by researchers at Surabaya Haji Hospital based on medical record data on the number of outpatients visiting Surabaya Haji Hospital in January to March 2020 there is a decrease in the number of outpatient visits each month. The purpose of this study was to determine the level of patient satisfaction in the outpatient registration at RSU Haji Surabaya. This study uses a literature review method of 10 journals using measurement techniques of 5 dimensions of health service quality, namely tangibles, reliability, responsiveness, assurance, and empathy. The results of the literature review indicate that the satisfaction of the quality dimension is considered the most satisfied is the dimension of assurance and reliability, while the highest dimension of dissatisfaction is the dimension of tangibles and empathy.

Abstract

Filling room has an important role in maintaining medical records, especially in terms of storage and protection of medical records. The creation of officers ease in taking and storing medical records in supported by the existence of an ergonomic storage space that is adapted to the anthropometric aspects. The design of the storage rack in accordance with the anthropometric data of the officer can facilitate the work of the officers. Storage racks at Airlangga University Hospital are not in accordance with the anthropometry of the officers, resulting in work discomfort and fatigue. The purpose of this study was to analyze the ergonomics aspects of the storage rack based on the anthropometry of the filling officer. This type of research is descriptive and data collection is done by observation. The population of this study was 5 filing officers. The results of this study indicate that the condition of the storage rack in the filling room of the Airlangga University Hospital is not in accordance of the filling officer. The anthropometry of the filling officer used in designing the rack, namely the reach of the hand up to 202 cm, is used to measure the height of the medical record file storage rack, data fathom length 162 cm is used to reference the length of the medical records file storage rack and shoulder width data used to reference the distance between medical record file storage rack.

Abstract

There were 482 medical file missfile incidents in 2019, with the highest number occurring in June 2019 with 61 medical record misfile, this could cause officers to experience difficulties in searching for medical record files and increasing the work of officers to more busy because they have to do a search on other number shelves that allow it to be misplaced, so the service process tends longer. This research aims to evaluate the event of medical record misfile using the FOCUS PDCA method at Sanglah Hospital Denpasar. Data collection techniques used in this research were interviews and observation. The data analysis stage in this research includes the identification of stage of find, organized, clarify, understand, select, plan, do, check, and action. The solution chosen to solve the problem is by designing the media information about the range of medical record storage on each shelf, carrying out the process of attached numbering information media to each storage shelf, and conducting information dissemination related to the attached media. The results of this evaluation are storing medical record files that are arranged according to the number on the shelf and re-monitoring at the end of every month all medical records in the place.

Abstract

The hospital is a health care institution that conducts complete individual health services and provide inpatient, outpatient and emergency services. Medical record is a unit to collect data patient, processing data, and reporting data that will be an information about performance of the hospital for making decision. In the implementation, competent officers are needed, so that patient services can be carried out optimally and not be hampered by workload in the medical record unit (Zebua, 2016). Pertamina Central Hospital is a plenary hospital managed by PT Pertamina Bina Medika. In 2014-2016 the visitor of outpatient and inpatient was 1157662 visite. In 2017 to 2018, patient visits increased by 21109 visits or 9%. Based on a preliminary survey, the schedule of filling officer is divided into 3 shifts (morning, afternoon, and night). The filling officer has overload working time of 1.5-2.5 hours. The overload working time can be a cause of work accidents. According to Eko (2016), service activities in medical record unit will run smoothly if human resources are sufficient by human needs, workload determined, and based on the standard. In this research is using WISN (Workload Indicator Staffing Needs) method. WISN is a method used to calculate labor needs based on workload. In this method there are 5 stages, selecting the HR category, calculating available work time, calculating workload standards, calculating leeway factors, and calculating labor requirements.

Abstract

Puskesmas Patrang merupakan salah satu puskesmas yang sudah menggunakan aplikasi Primary Care (P-Care). Aplikasi ini digunakan untuk mengentrikan data sosial, data pemeriksaan pasien dan pembuatan surat rujukan untuk pasien BPJS. Dalam pelaksanaannya masih ditemui berbagai kendala yang dialami oleh pengguna,yaitu  seperti aplikasi p-care yang mengalami error,  pembuatan surat rujukan hanya dilakukan oleh 1 petugas serta pengguna harus melakukan double entry data guna untuk kebutuhan laporan yang dibutuhkan oleh puskesmas. Tujuan dari penelitian ini adalah mengevaluasi implementasi aplikasi  Primary   Care dengan   menggunakan    metode   TTF   (Tasks  Technology  Fit) . Jenis penelitian ini adalah penelitian kualitatif dengan pendekatan deskriptif. Hasil evaluasi implementasi aplikasi Primary care   di Puskesmas Patrang menunjukkan bahwa dari 5 responden menyatakan karakteristik  tugas sudah sangat baik, semua tugas yang dilakukan sudah terfasilitasi disetiap menu aplikasi. Kriteria karakteristik dalam penerapan teknologi yang digunakan sudah baik, meskipun ketersediaannya belum merata. Kriteria kesesuaian tugas yang dirasakan oleh pengguna sudah sesuai dengan teknologi yang digunakan. Kriteria kebermanfaatan diperoleh bahwa pengguna merasakan butuh dengan aplikasi ini. Serta kriteria dampak kinerja dalam penerapan aplikasi ini yaitu ini sudah dirasakan dengan baik. Upaya penambahan kebutuhan pendukung seperti komputer dan scanner, penggunaan SIMPUS serta pembuatan SOP perlu diterapkan agar implementasi aplikasi primary care bisa berjalan dengan baik.

Abstract

National Center General Hospital Dr. Cipto Mangunkusumo functions as an educational hospital, so the medical record file is often used for research materials. It can be seen from the hospital data in 2019 that there were 53,963 requests for medical record files, but only 42,138 medical record files could be fulfilled by research officers, meaning that 22% of files could not be provided by research officers. If this is done continuously, it will cause a decrease in the quality of the research officer. Based on observations that have been made on 2 people from the Medical Records Research Center Dr. Cipto Mangunkusumo is known that the officer concurrently had several jobs. This causes the workload that is not in accordance with the number of officers. The purpose of this study was to analyze the workforce needs of the Central Medical Records research staff based on WISN theory at Dr. RSUPN. Cipto Mangunkusumo. This type of research uses qualitative research with 2 informants. Data collection methods are observation, interviews, and documentation. Based on the workload calculation using the WISN method, it is known that the number of workers needed in the research unit is 8 people. It is expected that with the results of the calculation of labor needs based on WISN theory can be used as an effort to plan the addition of officers to improve the quality of hospital services and in accordance with the existing workload.
