Abstract

Stroke is a serious health problem. Micronutrients are nutrients that the body needs in small amounts, such as vitamins and minerals. Neurological deficits due to acute stroke can cause malnutrition during hospital treatment. To determine the role of micronutrients in gastrointestinal function and protection in stroke patients. A systematic-narrative literature review following the PRISMA 2020 guidelines. Data sources included SINTA, Scopus, ClinicalKey, as well as proceedings and dissertations published between 2020 and 2025. Articles were assessed based on methodological quality and relevance to the management of stroke patients in Indonesia. A thematic analysis was conducted to synthesize the latest evidence. Several studies showed that micronutrients such as vitamins A, C, D, E, B9 (folate), B12, zinc, magnesium, and selenium play an important role in maintaining intestinal mucosal integrity, reducing oxidative stress and inflammation, and supporting intestinal microbiota balance in stroke patients. Adequate micronutrient intake helps prevent gastrointestinal complications and accelerates post-stroke motor and cognitive recovery. Micronutrient optimization is important for maintaining gastrointestinal function in stroke patients, supporting recovery, preventing complications, and improving quality of life.

Abstract

Adolescent mental health issues require early detection to prevent worsening conditions. This study aim developed a rule-based expert system for automating mental health screening instrument interpretation using Forward Chaining inference and Certainty Factor for uncertainty handling. The system encodes interpretation guidelines from two validated instruments: Mini MindHEAR Youth Scale V.1 for ages 10-18 years and Self-Reporting Questionnaire-29 for ages 19-24 years. From 710 survey respondents, 494 representative samples were selected using Stratified Random Sampling for validation. The knowledge base consists of 17 rules for Mini MindHEAR Youth Scale V.1 and 8 rules for Self-Reporting Questionnaire -29 with Certainty Factor values ranging from 0.5 to 0.95 based on symptom severity. Validation results showed the system achieved an overall guideline-alignment accuracy of 89.68% (443 matching interpretations out of 494 samples), measuring the system's ability to faithfully reproduce instrument interpretation guidelines rather than clinical diagnostic accuracy. The system demonstrated high explainability through transparent reasoning traces. This expert system can assist healthcare workers in automating screening instrument interpretation, particularly in resource-limited settings

Abstract

Many adolescent girls suffer from menstrual pain (dysmenorrhea), which can interfere with daily functioning, academic performance, and overall quality of life. While pharmacological treatments are commonly used, complementary therapies using natural ingredients such as turmeric and tamarind (known locally as kusam) have gained attention due to their safety and fewer side effects . The aim of this study was to assess how dullness can function as a supportive therapy to reduce the level of menstrual pain experienced by female adolescents at SMA Negeri 1 Batu Engau. A quantitative method was applied in this study, with a quasi-experimental design and pretest–posttest with a control group. Fifty eligible participants were divided into two groups: the intervention group received kusam (a turmeric and tamarind herbal drink) for three consecutive days at the beginning of menstruation, while the control group received no intervention. Pain intensity was measured using a Visual Analogue Scale (VAS), and the Wilcoxon test and the Mann–Whitney U test were applied to analyze the data. This study revealed a significant reduction in menstrual pain levels among the intervention group compared to the control group. The Wilcoxon test showed that the intervention group experienced significant improvement after consuming kusam (p < 0.05). Similarly, the Mann–Whitney U test showed a significant difference between the groups at post-test, with the intervention group achieving a higher mean score. Turmeric has been shown to be effective as a complementary treatment for reducing menstrual pain. intensity in adolescent girls.

Abstract

Fluor albus or vaginal discharge is a common reproductive health problem among adolescent girls. When abnormal, it can interfere with daily activities, cause discomfort, and potentially lead to more serious health issues if left untreated. Despite this, many adolescents are reluctant to report symptoms to parents or health professionals because the issue is considered sensitive. This situation highlights the need for safe, acceptable, and effective complementary therapies to manage fluor albus in adolescents. This study aimed to examine the effectiveness of the traditional herbal medicine KUSIR, made from turmeric and betel leaf, in reducing the severity of fluor albus among adolescent girls.The study employed a quantitative approach using a quasi-experimental pretest posttest control group design. A total of 64 adolescent girls participated and were divided into an intervention group receiving KUSIR herbal treatment and a control group without intervention. Data were analyzed using the Wilcoxon test to examine changes before and after treatment and the Mann Whitney U test to compare outcomes between groups.The findings showed a clear reduction in the severity of fluor albus in the intervention group compared to the control group. Mann Whitney U analysis indicated a significant difference in posttest scores, with a higher mean rank in the intervention group than in the control group. The Wilcoxon test confirmed that all participants in the intervention group experienced a decrease in symptoms. KUSIR herbal treatment is effective as a complementary therapy for reducing fluor albus severity in adolescent girls. Turmeric and betel leaf offer a natural, affordable, and acceptable alternative for adolescent reproductive health care.

Abstract

Hypertension in children is a serious threat due to a sedentary modern lifestyle. Low physical activity and high screen time are strongly suspected to be risk factors for increased blood pressure. Objective: To analyze the relationship between physical activity levels and television viewing/lying down duration with blood pressure in school-aged children in Mluweh Village. Methods: This observational study involved 105 students (6-17 years old) selected by total sampling. Blood pressure was classified based on the 2017 AAP Clinical Practice Guideline standards. Physical activity and television viewing/lying down duration were measured using a modified GPAQ questionnaire. Data were analyzed using the Chi-Square test (α = 0.05). Results: The majority of respondents had light physical activity (64.7%) and high television viewing/lying down duration (94.3%). Statistical analysis showed no significant relationship between physical activity levels and blood pressure (p = 0.814). However, a statistically significant relationship was found between television viewing/lying down duration and blood pressure (p = 0.027). Conclusion: High television viewing/lying down time exceeding 2 hours per day is a strong predictor of increased blood pressure in children. Limiting screen time from an early age is necessary to minimize the risk of long-term cardiovascular disorders.

Abstract

This study examined the association between Body Mass Index (BMI) and visceral fat with the risk of Metabolic Syndrome (MetS) among obese adolescents in Surabaya and Sidoarjo, Indonesia. The research addressed the increasing burden of cardiometabolic disorders in adolescents, focusing on the importance of early screening for central adiposity. A cross- sectional design was employed involving 72 obese adolescents from junior and senior high schools. BMI was measured using standard anthropometry, while visceral fat was assessed through Waist Circumference (WC)and Bioelectrical Impedance Analysis (BIA). MetS was identified based on established clinical criteria. The findings revealed that both BMI and visceral fat were significantly associated with MetS, with BIA-derived visceral fat demonstrating the strongest predictive value. Adolescents with high visceral fat exhibited substantially higher risks of dyslipidemia, hypertension, and overall metabolic abnormalities. These results highlight the limitations of BMI as a standalone indicator and reinforce the need for integrating visceral fat assessment in school-based screening programs. Early identification and targeted interventions are essential to mitigate the progression of MetS during adolescence.

Abstract

This study analyzes the implementation of the National Health Insurance (JKN) policy administered by the Badan Penyelenggara Jaminan Sosial in enhancing service quality at Royal Prima Marelan General Hospital. The primary issues identified include insufficient dissemination of policy guidelines to medical personnel and patients, complex administrative procedures, and limited service facilities available to JKN participants. Using a descriptive-analytic approach, the research applies the SERVQUAL (Service Quality) model and the Expectancy–Disconfirmation Theory (EDT) to evaluate the gap between patient expectations and perceived service quality. Data were obtained through observations, interviews, and questionnaires involving medical staff, administrative officers, and JKN patient participants. The findings reveal that the Tangibles and Reliability dimensions are relatively satisfactory; however, Responsiveness and Empathy remain low due to limited human resources and heavy workloads. The implementation of JKN policies by the Badan Penyelenggara Jaminan Sosial has not been fully optimized, primarily due to inadequate training, the absence of integrated digital information systems, and limited managerial support for systematic service quality monitoring and evaluation. Based on the EDT assessment, a notable mismatch exists between patient expectations and actual service experiences, resulting in dissatisfaction. Integrating the JKN policy framework with SERVQUAL and EDT highlights the need to strengthen policy dissemination, enhance human resource capacity, and reform management systems through digitalization and a human-centered approach to ensure equitable, efficient, and sustainable health services at Royal Prima Marelan General Hospital.

Abstract

Ischemic stroke remains a leading cause of morbidity and mortality globally and in Indonesia, with multifactorial risk factors involving vascular, metabolic, and lipid disorders. Understanding patient characteristics at the regional healthcare level is essential to support targeted clinical interventions. Bangli Regional General Hospital, as a referral hospital in Bali, has not yet systematically published data on ischemic stroke patient profiles, making this study important for local clinical planning. This retrospective descriptive observational study analyzed medical records of 160 ischemic stroke patients treated at Bangli Regional General Hospital throughout 2024. Variables included age, gender, hypertension, type 2 diabetes mellitus, history of heart disease, onset of arrival to the emergency department (<4.5 hours), and laboratory parameters comprising total cholesterol, LDL, HDL, triglycerides, and uric acid levels. Data were analyzed descriptively using frequencies, percentages, means, and standard deviations. Most patients were male (64.4%), with the largest proportion aged 60–75 years (38.75%), followed by middle-aged patients (32.5%). Hypertension was present in 67.5% of patients, type 2 diabetes mellitus in 15.6%, and a history of heart disease in 34.4%. Only 18.1% of patients arrived at the emergency department within 4.5 hours of symptom onset. Mean lipid values were 178.62 mg/dL for total cholesterol, 116.69 mg/dL for LDL, 45.31 mg/dL for HDL, and 123.35 mg/dL for triglycerides. Hyperuricemia was identified in 28.16% of men and 24.56% of women. Overall, ischemic stroke patients at Bangli Regional Hospital were predominantly elderly men with a high prevalence of hypertension and cardiometabolic abnormalities, reflecting national epidemiological patterns.

Abstract

The Ocular Surface Disease Index (OSDI) questionnaire and Dry Eye Questionnare-5 (DEQ-5) are widely used to evaluate subjective symptoms of dry eye disease (DED) as a primary diagnostic criterion. This study aims to compare the validity, reliability, and accuracy of the Indonesian version of OSDI and DEQ-5 questionnaires. Hospital-based cross-sectional study. A total of 100 participants were enrolled in the study. The Indonesian version of the OSDI and DEQ-5 questionnaires were analyzed to evaluate the validity and reliability of both questionnaires. Then, the accuracy of both questionnaires was compared with the diagnostic test and the receiver operating characteristic curve (AUC) analysis. All the question items on both questionnaires are valid and reliable. The validity test using Pearson’s product-moment correlation coefficient, the value exceeds 0.23 (P<0.01) and the reliability test using Cronbach’s alpha coefficient test showed good and excellent internal consistency for OSDI and DEQ-5 (Cronbach’s alpha of OSDI 0.78 and DEQ-5 0.81). The accuracy of both questionnaires was good and there was no significant difference with AUC OSDI=0.78 and AUC DEQ-5=0.79. The Indonesian version of OSDI and DEQ-5 questionnaires met the validity and reliability requirements so that they were declared valid and reliable as effective instruments for DED assessment in the Indonesian population.
