Abstract

The Maternal Mortality Rate (MMR) in Padang City has increased during the 2018-2020 period. One of the efforts to reduce MMR is to provide integrated pregnancy services. The coverage of K1, K4, and K6 pregnancy visits is an important indicator in assessing the quality of these services, but the achievements of the three in 2019-2022 in Padang City have not reached the target. This study aims to find out the factors related to the visit of pregnant women to the Health Center. The method used is a mixed method with a cross sectional and sequential explanatory design. A total of 142 pregnant women in the third trimester became respondents in quantitative research, and 11 informants in qualitative research. The research was conducted in three health centers in Padang City. The results of the quantitative analysis showed that of the 10 variables studied, the level of knowledge had a significant relationship with pregnant women's visits (p = 0.009), and was the most dominant factor based on multivariate tests. The qualitative results showed that most of the respondents obtained information from digital platforms, but the Mother And Child Health book has not been used to the fullest, the classes of pregnant women are not evenly distributed, and there is still an assumption that pregnancy is a natural process that does not need to be studied. Efforts are needed to improve health promotion and education for pregnant women to increase knowledge and awareness about the importance of pregnancy visits, so that the target of pregnancy service coverage can be achieved.

Abstract

Pharmaceutical personnel are a very important part of health services in hospitals. Based on a preliminary study at three private hospitals in Padang City, it was found that the turnover of pharmaceutical personnel reached 14.8-19.6% in the last three years. Severe burnout, low job satisfaction and increased turnover intention among pharmaceutical personnel which still receive little attention in the literature. The purpose of this study is to determine the effect of burnout on turnover intention mediated by job satisfaction in pharmaceutical personnel in five private hospital pharmaceutical installations in Padang City. The design of this study was cross-sectional by sampling using a questionnaire involving 118 respondents consisting of pharmacists and pharmaceutical technical personnel. The research data was analyzed using Structural Equation Modelling Partial Least Squares (SEM-PLS) and the results of the study were obtained. Job satisfaction significantly mediates the effect between burnout and turnover intention. Burnout and job satisfaction have a significant effect on turnover intention. Burnout has a significant effect on job satisfaction. Demographic factors do not have a significant effect on burnout and job satisfaction. Job satisfaction significantly mediates the influence between demographic factors and turnover intention. Burnout did not significantly mediate the influence between demographic factors and turnover intention. Based on the results of the study, burnout and job satisfaction affect turnover intention. Therefore, the hospital can determine a burnout management strategy and maintain good job satisfaction to prevent an increase in actual turnover.

Resveratrol, a naturally occurring polyphenolic compound, has been widely investigated for its potential antidiabetic effects in preclinical studies, although findings across animal models remain inconsistent and require systematic synthesis. This study aimed to review and synthesize evidence on the effects of resveratrol on glycemic regulation, insulin signaling, lipid metabolism, and oxidative stress in animal models of T2DM. The review followed PRISMA 2020 guidelines. Electronic searches were conducted in PubMed, Scopus, Sinta, and Google Scholar for articles published between 2019 and 2024. Eligible studies were original experimental investigations employing animal models of T2DM with resveratrol as the primary intervention. Owing to heterogeneity in study designs and outcome measures, data were synthesized using a qualitative narrative approach. Seven studies met the inclusion criteria. Overall, resveratrol administration was associated with reductions in fasting blood glucose, improvements in insulin sensitivity, and modulation of key insulin signaling pathways, particularly the PI3K Akt FOXO1 axis. Most studies also reported favorable effects on lipid profiles and reductions in oxidative stress markers. Although one study reported no significant improvement in glycemic indices, the overall pattern indicated consistent positive metabolic effects. This systematic review suggests that resveratrol exerts antidiabetic effects in animal models of T2DM by improving glycemic control, insulin signaling, lipid metabolism, and oxidative stress regulation. However, further well designed studies are needed to clarify dose response relationships and facilitate clinical translation.

Abstract

Colorectal cancer is one of the leading causes of cancer-related morbidity and mortality worldwide, with increasing incidence particularly in developing countries. Conventional treatments face challenges such as resistance and toxicity. Resveratrol, a natural polyphenol found in grapes, peanuts, and berries, has been reported to exert antioxidant, anti-inflammatory, and anticancer effects. To systematically evaluate the effects of resveratrol on parameters related to colorectal cancer in experimental animal models. Method: A qualitative systematic review was conducted by searching PubMed, Scopus, Sinta, and Google Scholar for studies published between 2020 and 2025. Inclusion criteria were in vivo studies using animal models of colorectal cancer, with resveratrol as the sole intervention, and reporting at least one parameter such as apoptosis, cell proliferation, oxidative stress, or inflammatory markers. Studies in vitro, in humans, or with combined interventions were excluded. A total of 512 articles were initially identified. After screening and applying eligibility criteria, seven studies were included. The findings consistently demonstrated that resveratrol suppressed tumor initiation and progression via multiple mechanisms: inhibition of Wnt/β-catenin signaling, induction of apoptosis and autophagy through FOXQ1 inhibition, reduction of oxidative stress and DNA damage, suppression of NF-κB signaling, and inhibition of epithelial mesenchymal transition and angiogenesis. Furthermore, resveratrol reduced tumor burden, metastasis, and inflammation in various colorectal cancer models. Resveratrol exhibits significant chemopreventive and therapeutic potential against colorectal cancer through pleiotropic mechanisms targeting proliferation, apoptosis, oxidative stress, and inflammation. These findings support resveratrol as a promising candidate for adjunctive therapy, although further clinical studies are required to confirm efficacy and safety in humans.

Abstract

Anemia remains a major public health problem among women of reproductive age worldwide, particularly in developing countries such as Indonesia. Limitations of conventional anemia management, including adherence issues and side effects of iron supplementation, have encouraged interest in complementary therapies such as wet cupping therapy (WCT). This study aimed to systematically map existing empirical evidence on the effects of wet cupping therapy on hemoglobin levels and hematological parameters in women through a scoping review approach. The review was conducted following the PRISMA-ScR guidelines. Literature searches were performed across multiple databases, including PubMed, Scopus, Google Scholar, and institutional repositories, resulting in 312 identified records. After screening and eligibility assessment, six studies published between 2016 and 2023 were included. The selected studies employed diverse designs, including experimental clinical trials, quasi-experimental, pre–post, cross-sectional, prospective cohort, and retrospective observational studies, with sample sizes ranging from 17 to 364 female participants. Across all studies, wet cupping therapy was associated with either maintenance or improvement of hemoglobin levels, as well as favorable changes in iron, total iron-binding capacity, and other biochemical parameters. No evidence indicated that WCT caused anemia when administered appropriately. Overall, the findings suggest that wet cupping therapy may serve as a safe and potentially beneficial complementary intervention for improving hematological status in women. However, further research using randomized controlled trials and standardized intervention protocols is required to establish its clinical effectiveness and inform evidence-based practice.

Abstract

Dysmenorrhea is one of the most common menstrual disorders among women of reproductive age, characterized by lower abdominal pain due to increased prostaglandin levels that trigger excessive uterine contractions. Although nonsteroidal anti-inflammatory drugs (NSAIDs) are widely used, their long-term use may cause adverse effects. Therefore, natural and safe alternatives are needed, such as Ajwa dates (Phoenix dactylifera L.), which contain bioactive compounds like flavonoids, phenols, and phytoestrogens that may reduce menstrual pain through hormonal regulation and anti-inflammatory mechanisms. This study aimed to review the effect of Ajwa date consumption on the intensity of menstrual pain in women of reproductive age. This research employed a literature review design with a narrative approach. Data were obtained from five relevant national and international journals that discussed the effects of Ajwa dates on estrogen, progesterone, and inflammatory mechanisms. The collected data were analyzed qualitatively to identify the relationship between variables and summarize the main findings of each study. The review showed that Ajwa dates can increase estrogen and progesterone levels, which help suppress prostaglandin production, relax uterine muscles, and inhibit cyclooxygenase enzymes (COX-1 and COX-2). Additionally, their nutritional content, such as vitamins and minerals, provides a natural analgesic effect that supports physiological balance during menstruation. Consumption of Ajwa dates has the potential to serve as a safe and effective non-pharmacological therapy to reduce menstrual pain intensity through synergistic hormonal, anti-inflammatory, and nutritional mechanisms

Abstract

The growth of the world's population is increasing day by day, the increase in the number of people also has an impact on the maternal mortality rate (AKI), The factor that causes the increase in population is the lack of people in the use of contraception, especially in the use of long-term contraception, one of which is IUD. The purpose of this study is to find out "Factors Related to the Use of IUD Contraceptives in the Working Area of the Andalas Padang Health Center This type of research is analytical with a Cross Sectional design, which will be carried out from June 2024 to January 2025. A sample of 68 PUS people using IUD contraceptives was taken using propotional random sampling. Data collection was conducted using questionnaires and analyzed using Chi Square and Logistic Regression tests. The results of the study found that the level of knowledge was high (69.1%), positive attitude (79.4%), support for husbands who did not support (60.3%), and support for health workers (75.0%). Based on multivariate results, it was found that the most dominant factor related to the use of IUD contraceptives was the husband's support with the highest OR value of 4,398. The conclusion of this study found that there was a relationship between the level of knowledge, attitude, husband support and health worker support. The most dominant factor is the husband's support. It is hoped that there will be cross-sectoral cooperation and education in the role of husbands to increase the use of IUD contraceptives in PUS.

Abstract

The utilization of health services after dental and oral health screening among elementary school children is an issue that requires serious attention, particularly in relation to efforts for the early prevention and control of dental diseases. This study aimed to analyze the factors influencing the utilization of health services for follow-up examinations after dental and oral health screening among elementary school children in Karo District. The study employed an analytic survey with a cross-sectional design. The sample size was determined using the Isaac and Michael table, resulting in 232 mothers whose children were diagnosed with dental caries, selected through multistage sampling. Data were collected using a validated and reliable questionnaire and analyzed with multiple logistic regression. The findings showed that the utilization of health services for follow-up examinations after dental and oral health screening among elementary school children in Karo District was 13.4%, far below the target (100%). The analysis indicated that predisposing, enabling, and need factors had a significant influence on the utilization of health services for follow-up examinations after screening, with a p-value < 0.005 and Exp (β) of 1.086. These factors were shown to significantly affect health service utilization. Efforts are needed to improve oral health education, empower families, and optimize the role of community health centers and schools in supporting the utilization of health services after screening.

Abstract

Waiting time is a critical aspect of healthcare service quality that influences patient satisfaction and overall service evaluation. This study aims to explore patients’ perceptions of waiting time in healthcare services by examining experiential factors that shape how waiting is interpreted and tolerated. A qualitative research design was employed, using in-depth interviews with four patients who had direct experience with healthcare service delivery. Data were analyzed thematically to identify recurring patterns in participants’ accounts. The findings reveal that perceptions of waiting time were influenced not only by its duration but also by procedural clarity, staff responsiveness, communication and information transparency, service environment, and the presence of personalization and emotional support. Clear service procedures and timely, understandable information reduced uncertainty and moderated negative perceptions of waiting. Additionally, clean and comfortable environments, along with empathetic and attentive staff interactions, enhanced patients’ ability to tolerate waiting and contributed to more positive service experiences. The study concludes that waiting time should be understood as a multidimensional experience shaped by both operational and interpersonal factors. These findings highlight the importance of adopting patient-centered strategies that integrate effective communication, environmental comfort, and emotional support in managing waiting time within healthcare services.

Abstract

Ureteral trauma is an uncommon but serious iatrogenic complication of hysterectomy, with risk influenced by surgical indication, patient factors, and intraoperative conditions. A 50-year-old woman presented with oliguria six days after hysterectomy for uterine myoma. She had abdominal distension and mild tenderness, with laboratory findings showing impaired renal function. Ultrasonography revealed bilateral hydroureteronephrosis and intraperitoneal ascites. Emergency exploratory laparotomy identified approximately 1.5 liters of intraperitoneal urine, bilateral ureteral dilatation, and bilateral distal ureteral ligation with urine leakage. Surgical management included intraperitoneal drainage, bilateral ureteroneocystostomy, ureteral stenting, peritoneal lavage, and bladder drainage. Eight days later, the patient developed abdominal wound dehiscence and underwent bilateral ureterocutaneostomy in collaboration with a urology surgeon. At follow-up, the patient showed gradual improvement and was able to resume daily activities under regular outpatient monitoring. Management of ureteral trauma depends on the timing of diagnosis, extent of injury, and associated complications. Early recognition and appropriate surgical intervention are essential to preserve renal function and reduce morbidity. Bilateral ureteral ligation is a rare but severe complication of hysterectomy, often presenting with postoperative anuria or oliguria and requiring urgent urological management. Careful intraoperative identification of the ureters and close monitoring of urine output are critical to prevent and promptly detect ureteral injury.
