Abstract

AbstractBackground: Perineal rupture is a condition in which the perineum is torn as a result of vaginal delivery either by action or not, which affects the health status of women. Age, parity, and birth weight are influencing factors for perineal rupture. The aim of this study was to analyze the relationship between age, parity, and birth weight with the degree of perineal rupture. Methods: Observational analytic research with a case control approach. The sample used in this study were the patients who get diagnostic of perineal rupture during April – December 2019 in the RSUD Jayapura as many as 654 patients. The sampling technique used is total sampling. The independent variables used were age, parity, and birth weight, while the dependent variable was perineal rupture. Bivariate analysis test using Spearman rank correlation. Multivariate analysis test using multiple logistic regression with backward method. Results: The results of the bivariate analysis in this study was showing that the younger mother's age, lower paratity values and higher birth weight related with the degree of perineal rupture, then the multivariate results obtained birth weight had a dominant effect of degree perineal rupture. This is because the heavy of birth weight make the perineum more strecthcing, so the perineum gets more thinner and easier to rupture. Conclusion: There is a relationship between age, parity, and birth weight with the degree of perineal rupture. Birth weight is the most dominant independent factor on the degree of perineal rupture. So, it is necessary to give education about childbirth preparation and preconception counseling to mothers.

Abstract

Abstract Background: Preeclampsia is one of the biggest causes of maternal-fetal morbidity and mortality. Based on the prognosis, the classification of Preeclampsia is early onset (<34 weeks) and late onset (> 34 weeks). Purpose: to investigate the relationship between the onset of severe Preeclampsia and perinatal complications. Method: This research is a quantitative study with a retrospective observational analytic study type and collected medical record data. The study population was severe Preeclampsia  patients who gave birth at RUMKITAL Dr. Ramelan Surabaya for the period January 2018 - June 2020 and has no previous history of chronic hypertension. The research sample was 79 subjects with 44 subjects early onset, and 35 subjects late onset. Perinatal complications  examined are preterm delivery, asphyxia, LBW, IUGR, stillbirth. The chi-square test or Fisher’s Exact Test was used to analyze relationships. Result: From the results of the study, the comparison of the percentage from early onset and late onset that experienced complications was 93.2% vs 48.6%, p-value = 0.000, OR = 14.5, CI = 3,764–55,635.  At preterm delivery, it was found that 75% vs 28.6%, p-value = 0.000,  OR = 7.5, CI = 2,754-20,422. . In asphyxia, it was found 41.7% vs 31.4%, p-value = 0.46. At LBW, it was found 72.7% vs 17.1%, p-value = 0,000, OR = 12.9, CI = 4,285-38,771. In IUGR, it was found that 15.9% vs 2.9%, p-value = 0.000. In stillbirth, it was found 18.2% vs 0% and p-value = 0.008. Conclusion: the onset of severe Preeclampsia is related with perinatal complications. Complications associated with the onset severe Preeclampsia are preterm, LBW, stillbirth. Meanwhile, complications that are not related with the onset severe Preeclampsia are asphyxia and IUGR

Abstract

AbstractBackground : Trained woean who experiencing menstrual disoders are found with high blood cholesterol level. This situation is contrary to who don’t experience menstrual disorder. Mentrual disorder condition has a low estrogen level. The low estrogen level is associated with endothelial disfunction which causes blood cholesterol increase. The benefit of omega 3 supplementation have been clinically proven to prevent atherosclerosis, decrease blood cholesterol levels, and playong an important role in modulating inflamation. Omega 3 supplementation is expected to decrease blood cholesterol levels in trained women. Method: This research is using field experimental with case control method. The samples are 30 trained womens contained by 15 are controlled and 15 are given treatment. The sampling technique is randomized allocation. The independent variabel is omega 3 300mg supplementation and the dependent variabel is blood cholesterol levels. Data are analized using parametric statistic paired T-test. Result: The bivariate analysis showed there is a meaningful difference on blood cholesterol levels in the treatment group p=0,0027 and no change in control group p=0,191 during treatment for 8 weeks. Conclusion: Omega 3 supplementation decrease a blood cholesterol levels in trained women.

Mothers can prepare starting from knowledge of exclusive breastfeeding. In order to be able to carry out mature exclusive breastfeeding, preparation for lactation since pregnancy is an important factor. This study aims to analyze the effect of mother's readiness on breastfeeding success. Methods: This research uses analytical observational with cross sectional approach. The sample used was 100 babies according to the specified criteria. The data collection technique was done by filling out online questionnaires. The data were then analyzed using the Chi Square test. Results:  This study shows the effect of mothers readiness with breastfeeding success which obtained a p value of 0.045. the results of the chi square test showed that mother’s readiness = 0.045 (α < 0,05), mother’s knowledge = 0.005 (α < 0,05) and lactation preparation = 0.19 (α < 0,05). Conclusion: These data indicate that there is an influence of mother's readiness on breastfeeding success.

Data analysis uses univariate analysis. Results: The most valued statements both from each dimension namely; tangible: Midwife performance reliability: recording in the MCH handbook, responsiveness: responses to complaints, assurance: the nature of the Midwife, empathy of communication between midwives and patients. The most valued statements are enough from each dimension namely; tangible: examination room, reliability: service procedures, responsieness: midwife response to patients waiting for long queues, assurance: guarantee to service, empathy: suitability of waiting time and duration of service.
