Abstract

Multidrug-resistant tuberculosis (MDR-TB) poses a significant challenge to global health. Current gold-standard diagnostic methods, such as culture, GeneXpert, and line probe assays, facing limitations including high costs, long processing times, and reliance on sputum samples. These challenges have driven the search for serum biomarkers for diagnostic approach. Serum biomarkers offer the potential for rapid and scalable diagnosis, especially for patients unable to produce sputum. This review synthesizes findings on key serum biomarkers, categorizing them into cytokines, oxidative stress markers, metabolic indicators, lipids, and genetic elements such as long non-coding RNAs and miRNAs. These biomarkers highlight the complex immune, metabolic, and molecular adaptations in MDR-TB, offering enhanced diagnostic precision and treatment monitoring capabilities. Future prospects include the development of cost-effective assays, large-scale validation studies, and the integration of multi-biomarker panels with advanced computational tools

Abstract

The purpose of this study is to examine prescribing practices at the Raya Public Health Center in Pematangsiantar City and the extent to which antibiotics are used rationally in toddlers. The 126 antibiotic prescriptions reported during the study period were included in a descriptive quantitative study using a cross-sectional technique. This study shows that, according to WHO and Indonesian Ministry of Health criteria, the majority of antibiotic prescriptions (90.7%) are rational. Acute respiratory infections (ARI) and acute diarrhea are the most common illnesses requiring antibiotics. Rationality of antibiotic use and prescribing patterns are significantly correlated (p < 0.05) according to the Chi-Square test, although there is no significant correlation with age or gender.

Abstract

Drug information services are very necessary, especially since many patients have not received adequate drug information about the drugs they are using, because incorrect use of drugs and non-compliance with taking drugs can harm patients. Patient compliance influences the success of treatment. Without patient awareness, therapy results will not be optimal, and this can even lead to treatment failure.Hypertension patients need to regularly check themselves at the hospital or independently at home periodically to monitor their condition and adjust their treatment in a timely manner for optimal healing effectiveness. Hypertension treatment aims to prevent and avoid dangerous complications. This study was descriptive with a prospective approach.In this study, the sampling technique used was nonprobability sampling with purposive sampling technique.The research instruments used were checklist sheets and questionnaires. The most respondents were in the 60-69 year age group, namely 44 respondents (44%), the most common gender classification for hypertension sufferers was female, namely 53 people (53%), the most common level of education was high school with a total of 55 people (55%), and the most common type of work was farmer with a total of 57 people (57%). Drug information services that are frequently provided are Drug Name, Dosage, How to Use and Indications, implemented 99% - 100%, Dosage Form (55%), Side effects (66%), Interactions 43%, and storage only 8%, while. Information that has not been provided is drug contraindications (0%) and drug stability (0%). The level of compliance is low with 58 respondents (58%), the level of compliance is high with 8 respondents (8%). Based on the results of the statistical test, there is a relationship using correlation, obtaining a significant value of 0.000, indicating that there is a relationship between drug information services and the level of compliance of hypertension patients

Abstract

Hospital is one of the health facilities where health efforts are carried out. Health efforts are every activity to maintain and improve health that aims to realize optimal health status for the community. Indonesian society currently requires quality health services based on five dimensions, namely tingible (physical evidence), reliability (reliability), responsiveness (responsiveness), assurance (guarantee) and empathy (attention). The aim is to determine the effect of the quality of Hospital Pharmacy Installation services on the level of patient satisfaction at Hospital TK IV 01.07.01 Pematangsiantar from February - April 2025. This research method was conducted using a cross-sectional approach observation. The number of samples was 78 people with sampling techniques. The results of the study were analyzed with SPSS using the chi-square test. Abstract

Background: Hypertension is an asymptomatic condition in which abnormally high pressure in the arteries causes an increased risk of stroke, aneurysm, heart failure, heart attack and others. One of the causes of hypertension is smoking, and smoking has become the consumption of many people including the teenagers or people in productive age. Aim: To determine the correlation between smoking behavior and hypertension of people in usia ≤ 45 years old ini Efarina Hospital. Method: This study employed correlational analytic design with cross-sectional approach. There were 183 respondents recruited as the sample through probability sampling with random sampling technique. The data were collected using observation sheets, sphygmomanometer tools and questionnaire. Further, the data used was bivariate data analysis with Spearman’s Rho test. Finding: The study found that based on statistical analysis test, the p-value (sig) was 0.000 (<0.05), which meant that there was a correlation between smoking behavior and hypertension of productive age people at Efarina Hospital. The significant correlation value = -.397. Conclusion: There is a correlation between smoking behavior and the incidence  of hypertension with a significant correlation value of -.397 which means that the higher the smoking behavior, the better the incidence of hypertension.

Abstract

Healthcare Associated Infections (HAIs) are infections that patients get while undergoing treatment in a hospital or other health care facility. According to the World Health Organization (WHO) in 2022, it was reported that there were 8.9 million incidents of Healthcare Associated Infections (HAIs) in health care facilities and 1 in every 10 percent had died from nosocomial infections. This study aims to determine the relationship between nurses' knowledge and the prevention of Healthcare Associated Infections (HAIs) in the inpatient room of Hospital Efarina. The methodology of this study uses a quantitative research design with a Cross Sectional approach. Sampling used Saturated Sampling with a total of 124 respondents. The results of the study based on the Spearman p value test 0.00 <0.05 showed that there was a significant relationship between the nurses' knowledge variable and the HAIs prevention variable. Nurses are expected to be able to improve their personal knowledge and maximize their performance, especially regarding the prevention of Healthcare Associated Infections (HAIs) by participating in training and monitoring nursing staff at the Hospital Efarina.

Abstract

Knee osteoarthritis is a degenerative joint disease that commonly causes chronic knee pain, limited mobility, and reduced quality of life in older adults. Conventional radiography (X-ray) remains the primary modality for initial evaluation due to its wide availability, cost-effectiveness, and ability to demonstrate characteristic structural changes such as joint space narrowing, osteophyte formation, subchondral sclerosis and cysts, and articular deformities. This literature review analyzed 20 national and international articles published between 2020 and 2025, retrieved from PubMed, ScienceDirect, Google Scholar, and Garuda using the PRISMA selection method. The findings indicate that appropriate examination techniques, particularly anteroposterior weight-bearing, lateral, and skyline projections, significantly influence the accuracy of radiological interpretation, while the Kellgren–Lawrence system remains the standard for grading disease severity. Radiography plays an essential role in early intervention planning, emergency department triage, and orthopedic referral, with additional modalities such as MRI and ultrasonography used to detect soft tissue abnormalities or early changes not yet visible on X-ray; moreover, emerging applications of deep learning and radiomics show promise in improving assessment consistency and predicting disease progression.

Abstract

Diabetes can cause various chronic complications, one of the most feared being diabetic nephropathy. Diabetic nephropathy is characterized by proteinuria and/or decreased glomerular filtration rate. Diabetic nephropathy is a major cause of chronic kidney disease, which can lead to end-stage renal disease requiring hemodialysis. Candesartan is an angiotensin receptor blocker antihypertensive drug. In addition to lowering blood pressure, candesartan is also thought to decrease renal vascular resistance, resulting in a decrease in microalbuminuria or proteinuria in patients with diabetes mellitus. Diabetic nephropathy increases with age, and geriatric patients have a lower response to therapy than adults. To determine the effect of candesartan on creatinine and albuminuria in geriatric patients with diabetic nephropathy. This was a cross-sectional study of 25 geriatric patients diagnosed with diabetic nephropathy who had been given 16 mg of candesartan as an antihypertensive at the Puri Raharja Hospital Denpasar outpatient clinic. Subjects who met the inclusion and exclusion criteria were recruited using consecutive sampling. The diabetic nephropathy subjects were evaluated for creatinine and proteinuria levels at the time of diagnosis and 3 months after receiving 16 mg of candesartan. All data were obtained from medical records. The study involved 25 patients consisting of 13 men (52%) and 12 women (485%). Subjects aged between 60-82 years with initial creatinine levels of 0.80-2.60 mg/dL, final creatinine levels of 1.395 ± 0.50 SD, initial positive proteinuria levels of 0-2, and final positive creatinine levels of 0-1. Based on the T-test, there was no significant difference between initial and final creatinine levels with a p value = 0.892 (95% CI -0.045-0.051). Based on the Wilcoxon test, there was a significant difference between proteinuria levels before and after candesartan administration with a p value p <0.05. This study did not find a significant decrease in creatinine after candesartan administration. However, candesartan significantly reduced proteinuria in geriatric patients with diabetic nephropathy.

Abstract

Introduction: Quorum sensing (QS), a bacterial cell communication system, has emerged as a key regulator of community level biofilm dynamics, yet its role in mediating this ecological transition has not been systematically synthesized. This scoping review aimed to map and synthesize evidence published over the last decade on the role of quorum sensing in driving the transition from symbiosis to dysbiosis in periodontal biofilms. Methods: A scoping review was conducted in accordance with the PRISMA-ScR guidelines. Searches were performed in PubMed, Scopus, and Web of Science for studies published between 2016 and 2025 that investigated quorum sensing mechanisms in periodontal or subgingival biofilms using in vitro, ex vivo, or in vivo models. Data were charted and analyzed descriptively. Results: Sixty one studies met the inclusion criteria. Autoinducer-2 mediated signaling emerged as the most consistently reported quorum sensing pathway in periodontal biofilms. Quorum sensing was shown to regulate interspecies communication, biofilm maturation, metabolic cooperation, and coordinated virulence expression. Evidence indicated that quorum sensing primarily drives dysbiosis through functional reprogramming of microbial communities rather than through numerical dominance of specific pathogens. Several studies also linked quorum sensing activity to enhanced host inflammatory responses and alveolar bone loss in experimental models. Conclusion: Quorum sensing as a central ecological mechanism orchestrating the transition from symbiosis to dysbiosis in periodontal biofilms. By coordinating community wide microbial behavior, host and microbe interactions, quorum sensing contributes to sustained dysbiosis and periodontal inflammation.

Abstract

Recurrent aphthous stomatitis (RAS) is a common inflammatory condition of the oral mucosa characterized by recurrent, painful ulcerations that significantly impair quality of life. Although its etiology is multifactorial, nutritional factors, particularly vitamin deficiencies, have been proposed as potential contributors through their roles in immune regulation, epithelial integrity, and wound healing. Evidence regarding vitamin status in adults with RAS, however, remains inconsistent. This systematic review aimed to synthesize and critically evaluate available evidence on vitamin levels in adult patients with recurrent aphthous stomatitis. A comprehensive literature search was conducted in PubMed and Scopus for studies published between January 2015 and December 2025, following PRISMA 2020 guidelines. Eligible studies included adult participants (≥18 years) diagnosed with RAS and reported serum or salivary vitamin levels. Observational and interventional studies were considered. Data were extracted on study characteristics, vitamin assessment methods, and key findings, and were analyzed descriptively. Seven studies met the inclusion criteria. Vitamin D was the most frequently investigated micronutrient, followed by vitamin B12 and other hematinic parameters. Several studies reported lower vitamin D levels in patients with RAS compared with controls, while others found similarly high rates of deficiency in both groups. Associations between vitamin levels and clinical severity of RAS were generally weak, although vitamin D deficiency was linked to increased ulcer multiplicity in one study. Vitamin B12 deficiency was primarily observed in populations with concomitant iron deficiency anemia rather than in otherwise healthy adults with RAS. Current evidence suggests that vitamin deficiencies, particularly vitamin D, may function as modifying or aggravating factors in adult RAS rather than direct causal agents. Further high-quality studies are required to clarify their clinical relevance and therapeutic implications.
