Abstract

Introduction: Infertility is a condition which is described by WHO as inability for having children after one year of relationship without birth control. this condition can be caused both from inside and outside the body. One of the major outside factors is exposure to chemical substances, which in this case nicotine substance that came from cigarettes. The CYP2A6 gene is a coding gene for the xenobiotic metabolic enzyme, in this case nicotine which can cause infertility in men. Mutations in this gene can lead to nicotine buildup, increasing nicotine effect thus increasing the risk of infertility in men. The aim of this study was to determine whether there were variations in the CYP2A6 gene in male infertile patients.Methods: The method used was the isolation of infertile and fertile sperm DNA samples (five samples each) which were then amplified at the target DNA (CYP2A6) by PCR method and then sequenced to see the nucleutide sequence.Result: The results of this study showed that there were variations in the CYP2A6 gene that occurred only in infertile patients, heterogeneous mutations in the 7788th(C-A), 8040th (C-T), 7661st (G­-C) nucleutide sequence of the CYP2A6 gene and SNP mutations in the 8250th(T>C), 8409th (T>G/C), and 8428th(C>T) nucleutide sequence of the CYP2A6 gene.Conclusion: The conclusion of this study is that there are CYP2A6 variations that found only in infertile male patients.

Abstract

Background: Asthma, eczema and allergic rhinitis are influenced by both genetic factor and environtment factors. Based on family history of allergy, allergic trace cards can identify the level of allergic risk in children. The relationship between family history of allergy and manifestation of allergic disease as a single allergy disease and allergic multimorbidity still need to be explored. The objective of this study was to identify the associations between family history of allergic disease and manifestation of allergic disease in school-age children.Methods: We conducted a cross sectional study on school age children with 6-13 years old at eight elementary schools in Yogyakarta. Family history of allergy was defined as reported asthma, eczema and allergic rhinitis in a parent or sibling and classified the level of allergic risk based on allergic trace cards. Subjects were questioned by standardized ISAAC questionnaire. The relationship between the two was analyzed by the Chi-squared analysis to assess the prevalence ratio (PR).Results: A total of 272 children with an average age of 8.4 years, allergic manifestations occurred more as multimorbidity (53%) with the highest incidence of RA+asthma (9%) compared with one allergic disease (47%) with the highest incidence of RA (11%). There was a significant relationship between family history of allergy and all manifestations of allergic disease with an increased risk of developing allergies by 3.3 (PR 3.3; 95% CI 2.3-4.8) and 2.8 times (PR 2.8; IK95% 1.8-4.3) in the moderate and high-risk score group. Asthma, RA and DA have a risk for coexistence with 2 other diseases of 2.5 (RP 2.5 IK95%; 1.9-3), 2.25 (PR 2.25 IK95%; 1.7 -3) and 1.9 times (PR 2.5 IK95%; 1.45-2.4).Conclusion: Family history of allergic disease is a risk factors for the development of allergic manifestation both a single allergic disease and multimobidity.

Abstract

Background: The Indonesian government has begub to administer vaccines en mass and is planned in stages as an effort to prevent and control Covid-19 by immunity. To reduce the negative impact of immunization on individual health and the implementation of vaccination itself, it is best to know Covid-19 vaccines in Indonesia and their AEFI.Objective: The purpose of this article is to discuss Covid-19 vaccines in Indonesia and their AEFIMethod: Systematic review of studies about Covid-19 vaccines that have been and will be used in Indonesia along with an overview of AEFI.Results: There are five Covid-19 vaccines manufacturer that has been and will be used in Indonesia. The types of antigens used in the Covid-19 vaccine include a whole virus, protein subunits, and nucleic acid. Conclusion: Covid-19 vaccines are relatively safe and can rarely cause AEFI.

Abstract

Background. Osteoarthritis (OA) is a chronic, progressive, degenerative disease which incidence increase with age dan occured majority in female. Pain in OA occurs due to accumulative process of degradation, repairment, and inflammation that causes symptoms of pain accompany with decrease in joint activity and function. These symptoms in elderly people, count about 9,8% of Indonesian population in 2020 which is muslim in majority, often causing complaints of difficulties to perform certain movements in shalat. Vitamin D is a nutritional factor which is known from several research wouldinhibit progressive joint destruction and relief of painful symptoms. And may have an important role to prevent the progressive process of the disease, thus promising to help the elderly to perform the shalat without knee joint problems. Objectives. The research’s objective was to determine the association between 25(OH)D serum level and the degree of WOMAC index of pain, stiffness, and functionality of knee OA in elderly female patients at Public Health Clinics (KPKM) Faculty of Medicine, Syarif Hidayatulloh State Islamic University of Jakarta (FK UIN Jakarta). Methods. The research used analytic Cross-Sectional design, with 106 elderly female as respondents, whose pain degree, stiffness, and functionality were measured using the WOMAC questionnaire of Knee OA and 25(OH)D serum level wasexamine. Results. Chi Square Fisher statistical analysis showed that there is no significant statistical association found (p 0,689) between 25(OH)D serum level and the degree of WOMAC index of knee OA in elderly female patients at KPKM FK UIN Jakarta. Conclusion. There is no significant statistical association found between serum vitamin D level and degree of WOMAC index of knee OA in elderly female patients at KPKM FK UIN Jakarta

Abstract

Background. The average achievement of hemodialysis adequacy which was carried out twice a week for 4 hours at Fatmawati Hospital was 1.49. As many as 82% had not reached adequate dialysis (Kt/V<1.8). Based on the calculation of the dialysate proportioning system, hemodialysis for 4 hours with a dialysate flow rate/Quick Dialysate Flow (QD) of 500 ml/minute will leave dialysate. In the 1 month observation, there were 1.755.6 liters residual dialysate parts Acid (A) and 6,283.2 liters parts Bicarbonate (B). This residual puts a burden on the hospital in treating liquid waste. But with QD 650 ml/minute, both part A and part B, dialysate is still sufficient. On the other hand, QD can affect the achievement of HD compliance.Aim. Knowing the difference in hemodialysis adequacy with QD 500 ml/minute and 650 ml / minute.Method. This study used a case-control analysis for the control group (QD 500 ml/minute) and the intervention group (QD 650 ml/minute) with a Quasy Experiment design through a comparative analysis approach with matching/cross over techniques. Hemodialysis adequacy (Kt/V) is measured 2 times using the QX calculate application (Daugirdas formula), namely at QD 500 ml/minute and QD 650 ml/minute for further analysis using the Wilcoxon Signed Rank test.Result. The study was conducted on 115 respondents with a total of 230 HD sessions. The mean Kt/V in the intervention group was 1.69 and in the control group 1.49 (p-value = 0.006) and it was seen that there was a shift in the achievement of HD adequacy to a better category; The very inadequate and inadequate adequacy category decreased from QD 500 ml/minute to QD 650 ml/minute, and conversely, the adequate and very adequate adequate category experienced an increase in number.Conclusion. Hemodialysis with QD 650 ml/minute resulted in better hemodialysis adequacy than QD 500 ml/minute.

Abstract

Objective: To assess the treatment outcome of interstitial cystitis with sodium hyaluronate (cystistat) installation by O’Leary-Sant interstitial cystitis symptom index (ICSI) and interstitial cystitis problem index (ICPI).Material and methods: From January 2019 to October 2020, 11 patients diagnosed with interstitial cystitis were treated with sodium hyaluronate (cystistat) installation. Each patient was assessed using the O’Leary-Sant interstitial cystitis symptom index (ICSI) and interstitial cystitis problem index (ICPI) before and after therapy.Results: The number of interstitial cystitis patients who completed the installation program in this study was 11 patients. In the assessment of interstitial cystitis symptom index (ICSI), there was a significant clinical improvement in each of the symptoms, which are urgency, frequency, nocturia, and bladder pain, with statistical test results obtained p-value (0.002, 0.003, 0.003, and 0.003) consecutively. In the interstitial cystitis problem index (ICPI) study, clinical improvements were obtained from each symptom in the form of urgency, frequency, nocturia, and bladder pain with statistical test results obtained p-value (0.002, 0.003, 0.003, and 0.003) consecutively.Conclusion: In all interstitial cystitis patients who were treated with sodium hyaluronate installation in this study, there was a significant difference in the values of O’Leary-Sant interstitial cystitis symptom index (ICSI) and interstitial cystitis problem index (ICPI) before and after therapy. This shows that sodium hyaluronate (cystistat) installation therapy is very effective for interstitial cystitis patients.
