Jurnal Klinik dan Riset Kesehatan (JK-RISK) – RSUD dr. Saiful Anwar Malang adalah jurnal open-access resmi yang dikelola oleh tim Penelitian dan Pendidikan Rumah Sakit Umum Daerah dr. Saiful Anwar Malang, Jawa Timur, Indonesia. JK-RISK menerbitkan satu volume yang terdiri atas tiga nomor setiap tahunnya yang diterbitkan pada bulan Oktober, Februari, dan Juni. JK-RISK hanya akan menerbitkan jurnal berbahasa Indonesia. Jurnal Klinik dan Riset Kesehatan – RSUD dr. Saiful Anwar Malang menerbitkan tulisan-tulisan ilmiah hasil penelitian, laporan kasus, tinjauan pustaka dalam bidang kedokteran dan kesehatan yang belum pernah diterbikan di jurnal manapun. Pernyataan kode etik publikasi JK-RISK merupakan pernyataan kode etik yang melibatkan pengelola jurnal, editor, mitra bestari serta penulis dalam proses publikasi jurnal ilmiah. Abstract

The clinical and hemodynamic features of Constrictive Pericarditis (CP) and Restrictive Cardiomyopathy (RCM) often overlap, making it difficult to differentiate and establish an accurate diagnosis. Both conditions are characterized by diastolic dysfunction, leading to symptoms such as dyspnea, fatigue, and peripheral edema. However, the underlying pathophysiology and management strategies of CP and RCM differ significantly, requiring an accurate diagnosis for appropriate management. Understanding the differences between CP and RCM is essential for effective patient management. Correct diagnosis influences treatment decisions. Constrictive Pericarditis is a reversible cause of heart failure, benefiting greatly from surgical pericardiectomy, while Restrictive Cardiomyopathy, rooted in permanent myocardial dysfunction, is managed primarily with supportive therapy. Echocardiography is established as the first-line imaging modality that can be used to treat these conditions. With the advancement of technology, the accuracy of diagnostic assessments of the features available from echocardiography has increased. Recent studies have validated conventional echocardiographic parameters that allow high sensitivity and specificity to differentiate CP and RCM. This literature review will combine several echocardiographic parameters that are important points in finding and assessing abnormalities arising from both diseases which will later facilitate analysis. The goal is to increase understanding of this condition among physicians and other health professionals in the field of imaging, which can ultimately improve diagnostic accuracy and more appropriate patient management. Continuous advances in echocardiographic techniques promise to improve outcomes and quality of life.

Abstract

Background: Saccular Abdominal Aortic Aneurysm with intramural hematoma carries a high risk of rupture, often presenting with atypical clinical manifestations that lead to delayed diagnosis. Case Illustration: A 68-year-old male presented with progressive abdominal pain over 4 months, diagnosed as gastritis. After 3 months without improvement, subsequently duplex ultrasonography and CT-Angiography confirmed a saccular AAA with intramural hematoma. The patient underwent EVAR with Stent Graft implantation. Case Discussion: This case highlights the diagnostic challenges of atypical AAA presentation, the importance of appropriate vascular imaging, and the significance of saccular morphology and intramural hematoma as high-risk factors for rupture. EVAR was selected for its lower perioperative mortality and morbidity compared to open surgery. Conclusion: Clinical vigilance for atypical AAA presentations, appropriate diagnostic imaging, and early endovascular intervention enable optimal management of this potentially fatal vascular condition.

Abstract

Hyponatremia is a decrease in blood sodium levels below 135 mEq/L. Sodium is essential for proper nerve and muscle function. Sodium disorders have traditionally been considered a marker of the severity of underlying disease, but increasing evidence suggests that these have detrimental effects on physiological function. Hyponatremia can occur due to various diseases, including lung diseases, which are lung cancer, chronic obstructive pulmonary disease (COPD), tuberculosis, pneumonia, cystic fibrosis (CF), and coronavirus disease 2019 (COVID-19). The most common causes associated with hyponatremia with it are SIDH, volume depletion (e.g., vomiting, diarrhea), anticancer therapy, diuretics, and decreased nutritional intake. Early detection and correction of hyponatremia can significantly improve patient outcomes, reduce morbidity and mortality, and enhance the quality of care.

Abstract

Background: Endometrial carcinoma holds the second highest incidence among gynecologic malignancies after cervical cancer, with increasing incidence and mortality globally. While it predominantly affects postmenopausal women, its incidence is steadily rising among premenopausal women. As the most prevalent subtype, endometrioid endometrial carcinoma (EEC) is closely linked to extended periods of unopposed estrogen stimulation. Obesity, type 2 diabetes, and nulliparity are also recognized as contributing risk factors. Aim: To describe the clinicopathological profile of EEC at Dr. Saiful Anwar General Hospital. Methods: This study employed a descriptive retrospective design based on secondary data obtained from medical records and histopathology reports. Inclusion criteria were patients diagnosed with EEC through hysterectomy specimens. Results: EEC cases increased over the past three years, with a 48% surge in 2024. Most patients were aged 51–60 years (mean age: 56.5) and showed deep myometrial invasion (≥½ thickness, 71.43%). The most common histological findings were FIGO grade 1 (45.23%) and stage IB (33.33%). Reproductive risk factors included postmenopausal status (63.3%), premenopausal status (36.67%), and nulliparity (23.3%). Among the patients, 18.33% had a history of diabetes mellitus, and 48.33% were obese (mean BMI: 27.83 kg/m²). Conclusion: There is a consistent upward trend in EEC incidence, with most patients diagnosed at an early stage and grade. An increasing proportion of cases is observed among obese and premenopausal women, with the youngest patient aged 29. Early detection and risk-based preventive strategies are essential to reduce EEC incidence and mortality.

Abstract

Cataract remains the leading cause of visual impairment and blindness worldwide. Cataract surgery utilising phacoemulsification technique with intraocular lens implantation may induce alterations in anterior chamber depth. This study aimed to compare anterior chamber depth before and after phacoemulsification cataract surgery with intraocular lens implantation. The research methodology employed a retrospective analytical design based on medical records of cataract during the period January–December 2019. Data collected included age, sex, operative eye side, and pre- and postoperative anterior chamber depth, which were subsequently analysed using SPSS software. The sampling technique utilised was consecutive sampling, yielding 125 subjects who fulfilled the inclusion criteria. Results demonstrated that the majority of patients were aged >60 years (64.8%), with females comprising the largest group (51.2%), and the left eye representing the most frequent operative side (51.2%). Paired sample t-test analysis revealed a significant difference in anterior chamber depth before and after surgery (p=0.000). Pearson correlation analysis also demonstrated a significant negative relationship between preoperative anterior chamber depth and postoperative changes (p<0.05). The conclusion of this study is that there was a significant increase in anterior chamber depth following phacoemulsification cataract surgery with intraocular lens implantation, as well as a significant relationship between preoperative anterior chamber depth and postoperative changes.

Abstract

Introduction: Haemoptysis is one of the most common clinical symptoms in tuberculosis (TB) patients, especially in chronic pulmonary TB cases. In Indonesia, as one of the countries with the highest prevalence of tuberculosis in the world, the incidence of haemoptysis in tuberculosis patients remains a challenge in clinical management and health care systems. Imaging of pulmonary arteries with CT angiography of the thorax compared to CT scan without contrast is an effective tool for evaluation and treatment of idiopathic and tuberculosis-induced haemoptysis. Case illustration: Case series of three patients with clinical symptoms of haemoptysis who underwent CT Angiography examination of the thorax at Saiful Anwar Hospital. Discussion: Case series of three patients with a history of haemoptysis found the source of bleeding from the bronchial arteries, where the underlying cause was Tuberculosis infection. Conclusion: three case series of patients with haemoptysis that gave a dilated and tortuous picture of the bronchial arteries. The importance of using CT angiography of the thorax in determining the location of the artery causing haemoptysis, may help the management of haemoptysis. Different arteries have different manifestations and management approaches.

Abstract

Background: Tuberculosis (TB) in children remains challenging to diagnose due to its paucibacillary nature and non-specific clinical presentation. Aim: This study aimed to describe the chest X-ray profile of pediatric TB patients confirmed by GeneXpert examination at Dr Saiful Anwar Regional General Hospital, Malang. Methods: A descriptive observational study was conducted using retrospective data of children aged 0–18 years who underwent both GeneXpert MTB/RIF testing and chest radiography between January 2022 and February 2025. Results: A total of 73 patients were included, consisting of 47 GeneXpert-positive (64%) and 26 GeneXpert-negative (36%) cases. Among GeneXpert-positive patients, 25 (53.2%) had infiltrates, 6 (12.8%) had combined effusion and infiltrates, 1 (2.1%) had effusion + infiltrate + lymphadenopathy, 3 (6.4%) showed miliary patterns, 1 (2.1%) had bilateral effusion, 1 (2.1%) had cavitation, 1 (2.1%) had mediastinal mass with metastatic-type pneumonia, and 9 (19.1%) had normal chest radiographs. In the GeneXpert-negative group, 10 (38.5%) had clinical TB, 7 (26.9%) were non-TB, 2 (7.7%) had latent TB, 1 (3.8%) had TB relapse, 3 (11.5%) had extrapulmonary TB, 2 (7.7%) had pulmonary TB with comorbidity, and 1 (3.8%) had congenital TB. Notably, 24 GeneXpert-negative patients still received clinical TB therapy despite negative molecular results. Conclusion: Infiltrates were the most common radiological finding among GeneXpert-positive patients, while a substantial proportion had normal or nonspecific radiographs. These findings emphasize that TB diagnosis in children requires an integrated approach that combines clinical, radiological, and molecular assessments.

Abstract

Stenosis vena sentral (SVS) merupakan komplikasi signifikan pada pasien hemodialisis (HD), yang memengaruhi panjang umur akses vaskular dan hasil luaran klinis pasien. Tinjauan ini memberikan analisis komprehensif tentang aspek anatomi, epidemiologi, patofisiologi, klinis, diagnostik, dan terapeutik SVS. Patogenesis SVS melibatkan penebalan dinding vena, obstruksi endoluminal, dan kompresi ekstrinsik, dengan faktor-faktor yang berkontribusi seperti kateterisasi vena sentral, laju aliran darah yang tinggi, dan cedera endotel. Diagnosis dipastikan melalui modalitas pencitraan, dengan venografi sebagai standar emas. Strategi manajemen dari Percutaneous transluminal angioplasty (PTA) hingga pemasangan stent (stent logam polos dan stent yang dapat mengeluarkan obat), intervensi bedah, dan strategi dialisis alternatif untuk pasien dengan SVS yang parah. Pendekatan endovaskular, khususnya angioplasti dengan pemasangan stent, tetap menjadi intervensi lini pertama, meskipun tingkat restenosis memerlukan prosedur yang berulang. Teknologi yang sedang berkembang, termasuk balon berlapis obat dan HeRO grafts, menawarkan alternatif yang menjanjikan. Kolaborasi multidisiplin antara ahli nefrologi, ahli bedah vaskular, ahli radiologi intervensional, dan intervensi vaskular sangat penting untuk mengoptimalkan hasil. Tinjauan ini menyoroti perspektif dunia saat ini tentang manajemen SVS dan arah masa depan untuk meningkatkan kemajuan pengobatan dan perawatan akses pada pasien HD.
