Jurnal Klinik dan Riset Kesehatan (JK-RISK) – RSUD dr. Saiful Anwar Malang adalah jurnal open-access resmi yang dikelola oleh tim Penelitian dan Pendidikan Rumah Sakit Umum Daerah dr. Saiful Anwar Malang, Jawa Timur, Indonesia. JK-RISK menerbitkan satu volume yang terdiri atas tiga nomor setiap tahunnya yang diterbitkan pada bulan Oktober, Februari, dan Juni. JK-RISK hanya akan menerbitkan jurnal berbahasa Indonesia. Jurnal Klinik dan Riset Kesehatan – RSUD dr. Saiful Anwar Malang menerbitkan tulisan-tulisan ilmiah hasil penelitian, laporan kasus, tinjauan pustaka dalam bidang kedokteran dan kesehatan yang belum pernah diterbikan di jurnal manapun. Pernyataan kode etik publikasi JK-RISK merupakan pernyataan kode etik yang melibatkan pengelola jurnal, editor, mitra bestari serta penulis dalam proses publikasi jurnal ilmiah. Abstract

Coronary artery disease (CAD) remains the most common cause of death globally despite advancements in treatment. Uric acid (UA) has been associated with the development of cardiovascular events in patients with Chronic Coronary Syndrome (CCS). Several hypotheses suggest that UA exerts a negative impact on coronary arteries and/or increases the risk of major adverse cardiovascular events (MACE).

Abstract

Background: Epiretinal Membranes (ERMs), also widely mentioned as macular puckers, are fibrocellular membranes which avascular and semi-translucent that cover the retinal surface. Complaints caused by ERM include visual impairments such as micropsia, macropsia, metamorphosis, monocular diplopia, and even blindness. Internal limiting membrane (ILM) peeling recently introduced as acceptable option for treating ERM, but the effectiveness of the procedure is still debatable. Aim: To compare the clinical outcomes of vitrectomy procedure with ILM peeling or without it for idiopathic ERM treatment. Methods: Study recorded relevant prospective or retrospective studies published in 2014-2024 by searching PubMed, Cochrane, Scopus, and ScienceDirect database. The mean change in BCVA and CMT was analyzed as a continuous variable, reported using Weighted Mean Difference (WMD) with 95% CI and heterogeneity was assessed using the chi-square test and I2 statistics. Result: There were 4 relevant studies with adequate data published in the last 10 years. 163 eyes consisted of 82 eyes in the No-ILM peeling group and 81 eyes in the ILM peeling group. Postoperative BCVA did not differ significantly between the two groups (MD -0,01; 95% CI: -0,07 to 0,06; P = 0,85) and no significant difference was found between the two groups in postoperative CMT (MD 7.70; 95% CI: -20.11 to 35.52; P = 0.59). Conclusion: There was no significant difference in BCVA and CMT between vitrectomy with and without ILM peeling in patients with idiopathic epiretinal membrane.

Abstract

Systemic Lupus Erythematosus (SLE) is a chronic autoimmune disease widely prevalent worldwide. Hematological abnormalities, one of them being AIHA, are commonly observed among SLE patients and brings about high morbidity rates. We present a 27-year-old woman with complaints of fatigue, dizziness, nausea, vomiting, body pain, and shortness of breath who came to the emergency department with 13 prior hospitalizations. She was present with SLE-related symptoms such as facial rash upon sun exposure, diffuse body pain mainly pronounced in her fingers, reddish-brown urine, hair loss, and fatigue. Physical examination observed stable vital signs yet copious purpura on her extremities. Facial examination revealed mild edema with rashes. Lab results show severe anemia with hemoglobin level of 5.2g/dL, hematocrit of 14.5%, a markedly reduced erythrocyte count of 1.24 x 10⁶/µL, and an elevated bilirubin level. She has been diagnosed with SLE with a positive ANA test and a homogenous speckled pattern consistent with SLE. The patient received intravenous methylprednisolone as first-line treatment as well as leukodepleted packed red blood cells, which she responded positively. The absence of key diagnostic tools in Indonesian hospitals remain as a challenge, particularly in cases where early recognition and timely treatment initiation is crucial. Abstract

Hypothyroidism was associated with bradyarrhythmia, conduction abnormalities, impaired myocardial function, and ventricular arrhythmia. The prevalence of ventricular tachycardia in hypothyroid was 2.6%. The patient was a 52-year-old woman who was experiencing palpitations, blurred vision, and syncope. ECG showed ventricular tachycardia (VT). Laboratory tests showed increased TSH and decreased T3 and FT4. Echocardiography showed global normokinetic, normal LV systolic function (EF 57%), normal RV systolic function, and normal heart chamber dimensions. DCA evaluation showed normal coronary artery. The patient underwent ablation of frequent PVCs originating from the anteroseptal RVOT. After ablation and hypothyroid treatment, there were no more VT episodes. The pathophysiological mechanism underlying the relationship between hypothyroidism and cardiac arrhythmias was the regulatory effect of thyroid hormones on cardiovascular hemodynamics and cardiac ion channels. Thyroid hormones directly regulate the function of ion channels such as sodium, calcium, and potassium channels. Ion channel dysfunction can prolong ventricular repolarization, which can trigger VT. This case report demonstrates the potential danger of thyroid dysfunction, especially hypothyroidism, in causing VT. Timely identification and treatment of hypothyroidism can prevent recurrence of arrhythmia and improve cardiac function.

Abstract

Background: Nocturia is the most common lower urinary tract disease that characterized by waking up to urinate more than once during the main sleep period and its prevalence increases with age. Aim: This study aimed to investigate frequency, bothersome and relation of nocturia with sleep quality during COVID-19 pandemic in medical students Methods: This study used a cross-sectional approach and an analytical observation method by distributing questionnaires. Questions include demographic characteristics, frequencies of nocturia, and bother score related to nocturia Results: Data on 90 individuals eligible for statistical analysis. The average age was 19.24 ± 1.08 years. Overall nocturia prevalence was 38.8% medical students had nocturia. The nocturia rate was 42.2% and 35.56% in men and women, respectively (p = 0.642). The frequency of nocturia per night was 24.4%, 13.3%, and 0.01% (1, 2, and 4 times, respectively). Conclusion: Nocturia is common among medical students, especially men and it is not related with sleep quality.

Abstract

Background: Pseudomonas aeruginosa is a prominent pathogen responsible for hospital-acquired infections. Its pathogenic potential is primarily driven by antimicrobial resistance and the ability to form biofilms. Aim: To examine the correlation between antimicrobial resistance patterns and the biofilm-forming ability of Pseudomonas aeruginosa isolated from endotracheal tube (ETT) sputum specimens. Methods: This analytic cross-sectional study used sputum samples from ICU patients with ETT. Biofilm detection was done using the microtiter plate method, and antimicrobial susceptibility was tested using Vitek. Results: Of the 23 multidrug-resistant (MDR) isolates identified, 45.5% demonstrated biofilm-forming ability, while 54.5% did not. Statistical analysis using the Chi-square test revealed no significant association between antimicrobial resistance and biofilm formation. Conclusion: There was no significant correlation between multidrug resistance and biofilm production in Pseudomonas aeruginosa isolates from ETT sputum samples.

Abstract

Pendahuluan: Infeksi saluran kemih (ISK) merupakan salah satu infeksi bakteri tersering pada populasi pediatrik dan memerlukan diagnosis yang cepat serta akurat untuk mencegah komplikasi. Pemeriksaan point-of-care testing (POCT) memungkinkan diagnosis secara cepat di fasilitas pelayanan kesehatan, namun akurasi diagnostiknya pada anak masih perlu dievaluasi secara sistematis. Tujuan: Menilai akurasi diagnostik pemeriksaan point-of-care pada infeksi saluran kemih anak berdasarkan publikasi ilmiah periode tahun 2015–2025. Methode: Pencarian literatur dilakukan secara sistematis melalui basis data PubMed, EuropePMC, OpenAlex, dan ScienceDirect. Studi yang diikutsertakan adalah penelitian yang mengevaluasi pemeriksaan diagnostik point-of-care (seperti uji dipstick dan pemeriksaan diagnostik cepat lainnya) pada populasi pediatrik usia 0–18 tahun serta melaporkan luaran akurasi diagnostik. Hasil: Dari 115 artikel yang teridentifikasi pada tahap awal, empat studi memenuhi kriteria inklusi. Uji dipstick menunjukkan variasi sensitivitas dan spesifisitas antar penelitian, sedangkan pemeriksaan uNGAL serta sampel urin yang diperoleh melalui kateter menunjukkan akurasi diagnostik tertinggi. Secara umum, POCT memberikan hasil yang cepat dan cukup andal, meskipun memiliki keterbatasan akurasi dalam mendeteksi keton. Kesimpulan: Pemeriksaan point-of-care memberikan diagnosis yang cepat dan relatif andal pada infeksi saluran kemih anak, sehingga dapat mendukung pengambilan keputusan klinis. Namun, diperlukan penelitian lebih lanjut untuk meningkatkan akurasi dan validitas penggunaannya pada populasi pediatrik.

Abstract

Vascular calcification (VC) is a pathological process marked by the abnormal deposition of calcium phosphate in the vascular wall, leading to increased arterial stiffness and the development of cardiovascular disease. Previously considered as a passive degenerative condition, vascular calcification is now understood as an actively regulated process involving vascular smooth muscle cells (VSMCs) and is influenced by metabolic disorders, especially metabolic syndrome (MetS). Metabolic syndrome, defined by insulin resistance, hypertension, dyslipidaemia, and central obesity, promotes vascular calcification via various molecular mechanisms, such as chronic inflammation, oxidative stress, and lipid dysregulation. Bone morphogenetic proteins (BMPs), particularly BMP-2, are essential for the osteogenic differentiation of vascular smooth muscle cells, facilitating vascular mineralisation and influencing cardiovascular morbidity. Increased BMP signalling in metabolic syndrome is associated with heightened vascular stiffness, endothelial dysfunction, and greater arterial calcification. This review examines the intricate relationship between vascular complications and metabolic syndrome, highlighting the role of bone morphogenetic proteins in vascular pathology. An enhanced comprehension of these molecular mechanisms may facilitate the development of novel treatment strategies designed to mitigate cardiovascular risk and enhance patient outcomes.
