Abstract

Type 2 diabetes mellitus (T2DM) is a chronic disease often accompanied by serious complications, such as diabetic ulcers. Poorly controlled blood glucose levels can worsen patient outcomes. This study aims to describe blood glucose levels, diet, and antihyperglycemic therapy in T2DM patients with diabetic ulcers in Tanjungpinang City. This descriptive observational study was conducted on 56 diabetic ulcer patients hospitalized in two hospitals in Tanjungpinang City. Data were collected from medical records and HbA1c laboratory tests. Variables analyzed included blood glucose levels, diet, and antihyperglycemic therapy. Univariate analysis was performed using percentages. The majority of patients (94.6%) had uncontrolled blood glucose levels (HbA1c >7%), with 60.7% having HbA1c ≥10%. Most patients (83.9%) were on a diabetic diet, although a few patients were not on a diabetic diet. Insulin was the most common antihyperglycemic therapy (58.9%). Patient characteristics showed a predominance of females (69.6%), normal nutritional status (62.5%), and diabetic ulcer severity at grade 4 (55.4%). Most diabetic ulcer patients in Tanjungpinang City have poorly controlled blood glucose levels despite receiving insulin therapy and a diabetic diet. This highlights the need for a more comprehensive approach to blood glucose management. Comprehensive interventions are required to improve blood glucose control, expedite diabetic ulcer healing, and enhance patient quality of life.

Abstract

ARI is an acute infection found in the upper respiratory tract, which includes the nose, sinuses, and throat, and the lower respiratory tract, which includes the bronchi and lungs. Bacteria, viruses, and fungi can cause it. This study used a cross-sectional design with an analytic approach and used secondary data from the SKI in 2023. Analysis was carried out univariate, bivariate, and multivariate. The prevalence of ARI among children under five was 11,95%. Factors associated with the incidence of ARI among children under five in Jambi Province were smoking (p-value = 0,000, PR= 1,89 95% CI: 1,38 – 2,58). Factors not associated with ARI were age, sex, birth weight, immunization status, nutritional status, floor type cooking fuel, maternal education, mother's occupation, and economic status. The dominant factor with ARI incidence was immunization status after controlling for the variables of nutritional status, maternal occupation and the presence of smokers. Therefore, more effective prevention efforts are needed, such as educating parents about the importance of adequate basic immunization and implementing a no-smoking policy in the residential environment.

Abstract

Riau Islands Province (Riau Islands) has great potential in developing the health tourism sector thanks to its strategic location on the Strait of Malacca, rich local culture, and stunning natural beauty. This paper reviews the opportunities and challenges of developing health tourism in the Riau Islands by highlighting medical and wellness tourism elements. Opportunities include geographical advantages, cultural heritage of traditional medicine, and economic potential from increased health tourist arrivals. However, Riau Islands also faces various challenges, such as the need for international standard health facilities, regulations that support investment, global promotion, and environmental sustainability issues. Collaboration between the central and local governments, industry players, and the community is needed to optimise this potential. With the right strategy, the Riau Islands has the opportunity to become a leading health tourism destination in the Asian region.

Abstract

Dharmasraya Regency recorded the second highest Case Fatality Rate (CFR) of Dengue Hemorrhagic Fever (DHF) in West Sumatra in 2024, alongside an Incidence Rate (IR) more than three times greater than the previous year with cases clustered in specific subdistricts. This study aimed to map the spatial distribution of DHF cases according to regional characteristics to inform targeted, area-based control policies. A quantitative descriptive design was employed using secondary data from 11 subdistricts, analyzed through univariate and spatial methods. Findings revealed that the DHF cases were highly concentrated in Sitiung, Pulau Punjung, and Koto Baru. Spatial analysis showed higher case counts in areas with higher population density, higher migration, more health facilities, and larger settlement areas. Consequently, dengue control efforts should prioritize these high-risk areas by strengthening surveillance, community education, health service capacity, integration of migration data into early warning systems, and cross-sectoral collaboration on settlement management and risk-based spatial planning.

Abstract

The high cases of DHF highlights the importance of identifying factors that contribute to the breeding of mosquito larvae. This study aims to examine the relationship between environmental conditions and human behavior related to the presence of Aedes spp larvae. A quantitative research method with an analytical approach and a cross-sectional design was used. The sample consisted of 150 households in the Baru Ladang Bambu Subdistrict, Medan Tuntungan District, Medan City. The results indicate that the habit of hanging clothes is significantly associated with the presence of Aedes larvae (p-value = 0.010). Other variables such as house type, occupancy density, ventilation, wire screens, opening ventilation, clutter, closing and draining landfills, burying used items, using abate powder, keeping larva-eating fish, and receiving fogging showed no significant relationship. In conclusion, the main risk factor identified is the indoor practice of hanging clothes. Based on these findings, it is recommended that mosquito control programs be enhanced through educational efforts, focusing on changing community behavior particularly discouraging the habit of hanging clothes indoors—as a preventive measure to reduce the presence of Aedes spp larvae.
