Abstract

ABSTRAKLuka kanker secara psikologis menunjukkan dampak negatif bagi pasien dan juga sulit untuk dirawat, apalagi jika kanker menginfiltrasi ke kulit. Hal tersebut seringkali dikaitkan dengan stadium yang lanjut, jumlah eksudat yang berlebihan dan mudah berdarah. Tujuan utama dari perawatan luka adalah peningkatan kualitas hidup, termasuk didalamnya tidak meninggalkan pengobatan seperti radiasi, khemotherapi, pembedahan dan tentunya perawatan luka.ABSTRACTMalignant cutaneous wounds are emotionally traumatic and difficult to manage lesion which occour secondary to infiltration of cancer into the skin. This symptom occour in patients with end stage disease and are highly exudative, malodorous, pain, pruritus and bleed easily. As malignant wounds very rarely heal, the aim of management is not wound closure, but rather to promote comfort, prevent social isolation and maintain or improve quality of life. The main goals of treatment are the elimination or control symptoms, which includes radiation, chemotherapy surgery and local wound care.

Abstract

Introduction: Salivary gland cancer is an uncommon malignancy in the head and neck. The most common histopathologic type in salivary gland malignancies is mucoepidermoid carcinoma (MEC). Radiotherapy has a role in salivary gland malignancy, especially in inoperable cases and postoperative settings. Definitive or postoperative radiotherapy with or without chemotherapy can improve locoregional control (LRC) in patients with parotid mucoepidermoid carcinoma. Case Presentation: We report a case of a 77-year-old male with inoperable MEC of the right parotid, who received definitive radiotherapy. From the three-month evaluation after radiation therapy, we found a significant reduction in the tumor mass. Conclusions: Surgery remains the treatment of choice for patients with salivary gland malignancies. Definitive radiotherapy can be a treatment modality in inoperable cases or patients who refuse surgery. Although the result is not satisfactory, radiotherapy can still give clinical benefits to patients.

Abstract

Depression is psychological distress often occurred on cancer patient. Depression can increase perception about pain, reducing drug efficacy and longer length of stay at hospital. The objective of this study was to describe the condition of cancer patients depression who undergoing treatment at RSUP Dr. Sardjito Yogyakarta and RSUD Prof. Dr. Margono Soekarjo Purwokerto. This was a descriptive study using a cross sectional design and a pilot study. Subject were taken using non probability with purposive sampling technique. Beck Depression Inventory (BDI) was employed for selection purposes. From this study, 25,71% are experienced with mild depression, 45,71% had moderate depression and 28,58% suffered severe depression. We propose that medicine has been rated helpful by cancer patient. Psycotherapy like music therapy is part of Complementery and Alternative Medicine (CAM) can also be considered useful in lowering negative emotions beside of routine medicine like psychopharmaceutical. Doctor and Nurse can safely recommend any of these interventions for depression.ABSTRAK Depresi merupakan masalah psikologi yang sering terjadi pada pasien kanker. Depresi dapat meningkatkan persepsi pasien akan rasa sakit, menurunkan sensitivitas khasiat pengobatan, dan memperpanjang waktu rawat di rumah sakit. Tujuan dari penelitian ini adalah untuk mengetahui gambaran kondisi depresi pada pasien kanker yang dirawat di RSUP Dr. Sardjito, Yogyakarta, dan RSUD Prof. Dr. Margono Soekarjo, Purwokerto. Ini adalah penelitian deskriptif dengan desain cross sectional dan merupakan pilot study . Total jumlah sampel adalah 70 responden. Pengambilan sampel dilakukan dengan cara non probability sampling dengan teknik purposive sampling . Alat ukur yang digunakan adalah Beck Depression Inventory (BDI). Hasil penelitian menunjukkan sebanyak 25,71% pasien kanker mengalami depresi ringan; 45,71% mengalami depresi sedang; dan 28,58% mengalami depresi berat. Saran dalam penelitian ini adalah bahwa diperlukan intervensi yang tepat untuk mengatasi gejala depresi. Selain pemberian psikofarmaka, tenaga kesehatan, khususnya dokter dan perawat, dapat memberikan psikoterapi lain, misalnya terapi musik yang merupakan bagian dari terapi komplementer

Abstract

Background: Current options for management of bladder cancer (BCa) still depend on traditional clinical determinants of stage and histological grade which often do not reflect the biological entity of the tumors. Therefore, new biomarkers are required to better determine suitable treatments for different types of bladder cancers. Recent research has shown osteopontin (OPN) expressions correlate with clinic-pathological variables and outcomes of bladder cancer. This study aimed to evaluate the expression of OPN in the Indonesian population, and it’s potential role as an immune-targeting therapy of BCa. Methods: Total RNAs from formalin-fixed paraffin-embedded tissues were extracted from 49 patients with bladder cancer consisting of normal histopathology (n = 4), chronic cystitis (n = 15), non-muscle-invasive bladder cancer (NMIBC, n = 15), and muscle-invasive breast cancer (MIBC, n = 15). The expression of OPN was measured using reverse transcription-polymerase chain reaction. Results: The baseline clinical and histo-pathological characteristics were not statistically different. The expression of OPN was statistically higher in bladder cancer compared to normal histology tissues (P < .001). The expression of OPN was statistically higher in MIBC compared to NMIBC (P < .001). Conclusions: The expression of OPN was significantly higher in bladder cancer and compared to NIMBC, the OPN expression in MIBC was significantly higher rendering the potential role of OPN expression as a surrogate biomarker marker to determine suitable treatment options for patients with bladder cancer.

Abstract

Introduction:  Locally advanced breast cancer (LABC) is a large breast tumor with infiltration to surrounding muscle, skin, and lymph nodes, and sometimes followed by inflammatory breast cancer, a rapidly growing cancer that shows a swollen red lump on the skin. Transcatheter intraarterial chemo infusion (TACI) is considered a palliative therapy for inoperable cancer. This case report shows the qualitative results of TACI on LABC in three patients. Case Presentation: Three female patients with LABC were diagnosed and treated at Dr. Sardjito General Hospital Yogyakarta over three years (2014–2016). The age ranged from 48 to 62 years, with a mean age of 56 years. Clinical features include lumps in breasts, soft tissue swelling, ulcers, dimpling of breast skin, nipple discharge, and pain sensation in the breast area. The three patients underwent the TACI procedure. Therapeutic response was evaluated by observing the clinical morphology of the lesions. Conclusions: TACI showed potency as an efficient and effective alternative palliative therapy in the three LABC cases presented. However, further study is needed to inspect TACI’s efficacy and adverse effects in breast cancer cases to be used as evidence-based medicine.

Abstract

Leukemia adalah penyakit neoplastik yang umumnya terjadi pada anak berusia 2-6 tahun dan ditandai dengan akumulasi sel darah putih dalam sumsum tulang. Pengobatan utama pasien leukemia adalah kemoterapi dalam waktu lama sehingga sangat mempengaruhi kualitas hidup anak dan keluarganya sebagai caregiver. Tujuan dari penelitian ini adalah untuk mendapatkan gambaran mengenai kualitas hidup keluarga dalam merawat anak dengan leukemia yang menjalani kemoterapi di RSUP Dr. Hasan Sadikin, Bandung.Penelitian ini menggunakan metode deskriptif kuantitatif dengan modifikasi instrumen yang dikembangkan oleh Munoz et al (2008) tentang kualitas hidup caregiver penderita kanker. Teknik sampling menggunakan accidental sampling selama 1 bulan sehingga diperoleh sampel sebanyak 33 responden. Hasil penelitian dianalisis menggunakan rumus Skor-T, kemudian diinterpretasikan menggunakan persentase dan distribusi frekuensi.Lebih dari setengah responden yang merawat anak leukemia yang menjalani kemoterapi di RSUP Dr. Hasan Sadikin Bandung memiliki kualitas hidup tinggi (54,5%) dan hampir setengah responden memiliki kualitas hidup rendah (45,5%). Tingkat kualitas hidup yang dipersepsikan oleh sebagian besar keluarga pada masing-masing dimensi adalahrendah (60,6%) pada dimensi fisik, tinggi (51,5%) pada dimensi psikologis, rendah (54,5%) pada dimensi sosial, dan tinggi (51,5%) pada dimensi spiritual.Kualitas hidup keluarga tinggi karena keluarga telah beradaptasi dengan tanggung jawab perawatan sejak anak mengalami sakit. Saran yang dapat diberikan adalah keluarga dapat mempertahankan kualitas hidup dengan cara meningkatkan peran hubungan personal dan sosial dengan tenaga kesehatan profesional dalam usaha memperoleh informasi, dukungan, serta pengalaman dalam merawat anak dengan penyakit kronis.Kata Kunci: leukemia anak, kemoterapi, kualitas hidup keluarga

Abstract

ABSTRACTGray scale ultrasonography (US) is the best choice modality for invasive breast carcinoma. Invasive breast carcinoma is the commonest breast carcinoma (65-80%) and grouped into malignant breast tumor that invade adjacent tissue with distant metastses. The important parameters in evaluating invasive breast carcinoma in US are inhomogenous hypoechoic texture, irregular, spiculated border and posterior accoustic shadowing. Gray scale US can also showed architectural distorsion. The sensitivity and spesificity of US in evaluating invasif breast carcinoma are 89,1 % ang 79,1%. Objective: to evaluating the diagnostic value of US in detection of invasive breast carcinoma which is confirmed by histopathology examination at Hasan Sadikin Hospital, Bandung. Methods: this research is a diagnostic research that evaluate sensitivity, spesificity, negative predictive value and positive predictive value of US in detecting invasive breast carcinoma, which is confirmed by histological examination. Samples taken by consecutive admission sampling on 23 subjects. Diagnostic evaluation result of gray scale US and histopathology in invasive breast carcinoma at RSUP Dr Hasan Sadikin Bandung shows characteristics including irregular margin and hipoechoic lesion (sensitivity 94,12%, specificity0,00%, PPV 72,7%, NPV 0,00%), inhomogenous texture (sensitivity 100%, specificity 16,67%, PPV 88,24%, NPV 100%), spiculated border (sensitivity 88,24%, specificity 66,67%, PPV 88,24%, NPV 66,67%), posterior acoustic shadow (sensitivity 100%, specificity 100%, PPV 100%, NPV 100%), and architectural distorsion (sensitivity 70,59%, specificity 66,67%, PPV 85,71%, NPV 44,44%). Gray scale US have a high sensitivity in diagnose invasive breast carcinoma that is confirmed with histopathology. Its specificity have a lower value compared with its sensitivity. ABSTRAKUltrasonografi (USG) gray scale merupakan modalitas pilihan yang tepat untuk karsinoma payudara tipe invasif. Karsinoma payudara tipe invasif merupakan karsinoma payudara yang paling sering (65%-80%) dan termasuk kelompok tumor ganas payudara yang menginvasi jaringan sekitar serta cenderung bermetastasis ke organ jauh. Parameter penting dalam menilai karsinoma payudara tipe invasif pada USG gray scale adalah gambaran hipoekhoik dengantekstur inhomogen, tepi ireguler, spikula, dan terdapat posterior accoustic shadowing. USG gray scale juga menunjukkan adanya distorsi jaringan. Nilai sensitivitas dan spesifisitas USG gray scale dalam menilai karsinoma payudara invasif sebesar 89,1% dan 79,1%. Tujuan penelitian ini adalah untuk mengetahui hasil uji diagnostik USG gray scale dalam mendeteksi karsinoma payudara tipe invasif yang dikonfirmasi oleh pemeriksaan histopatologi di Rumah Sakit Dr. Hasan Sadikin, Bandung. Metode penelitian yang digunakan adalah uji diagnostik yang menilai sensitivitas, spesifisitas, nilai prediksi negatif, dan nilai prediksi positif USG gray scale dalam mendeteksi karsinoma payudara invasif yang dikonfirmasi dengan hasil pemeriksaan histopatologi. Pengambilan sampel dilakukan secara consecutive admission sampling pada 23 subjek penelitian. Hasil uji diagnostik USG gray scale dengan histopatologi pada karsinomapayudara invasif di RSUP Dr. Hasan Sadikin Bandung menunjukkan karakteristik USG gray scale berupa batas/tepi ireguler dan lesi hipoekhoik (sensitivitas 94,12%; spesifisitas 0,00%; PPV 72,73%; NPV 0,00%), tekstur inhomogen (sensitivitas 100%, spesifisitas 16,67%; PPV 77,27%; NPV 100%), spikula (sensitivitas 88,24%; sensitifitas 66,67%; PPV 88,24% NPV66,67%), posterior accoustic shadow (sensitivitas 100%, sensitifitas 100%; PPV 100%; NPV 100%), dan distorsi jaringan (sensitivitas 70,59%; spesifisitas 66,67%; PPV 85,71%; NPV 44,44). Penelitian ini menyimpulkan bahwa USG gray scale memiliki nilai sensitivitas yang tinggi dalam mendiagnosis karsinoma payudara invasif yang dikonfirmasi dengan histopatologi. Nilai spesifisitas USG gray scale lebih rendah dibandingakan nilai sensitivitasnya.

Abstract

Astrocytoma is the most common primary central nervous system tumor with difficult management as it requires acombination of surgery, chemotherapy and radiotherapy. This multimodalities approach increases patients survivalrate significantly, however chemotherapy resistance is now commonly seen. One of the potential causes of chemotherapyresistance is the epigenetic factors from O6 methylguanine-DNA-methyltransferase (MGMT) gene. MGMT gene has arole in DNA repair and also have a protective effect against exogenous and endogenous alkylation agents. The methylationof MGMT gene promoter leads to the decrease of MGMT protein, attenuating its function. Therefore, the methylationstatus of MGMT gene promoter can act as an indicator for astrocytomas progressivity and treatment progressivity.The aim of this study to determine the frequency of MGMT gene promoter methylation among patients withastrocytomas using methylation specific polymerase chain reaction (MS-PCR) and methylation sensitive highresolution melting (MS-HRM).Clinical data, imaging and parafin blocks from astrocytoma patients were collected in RSCM from 2008-2012. Themethylation status of MGMT gene promoter was confirmed using MS-PCR and MS-HRM. This is cross-sectional study.The total of 13 samples collected including 7 low-grade and 6 high-grade astrocytomas. The MGMT gene promoter wasmethylated in 1/13 cases using MS-PCR and 4/13 cases using MS-HRM. All methylated cases were low-gradeastrocytoma. There was significant association between the methylation status of MGMT gene promoter with thedegree of malignancy which is 4/7 samples hypermethylated in low-grade with no hypermethylation in high-gradeastrocytomas (p=0.049). While other factors like age, sex, karnofsky performance scale (KPS) and astrocytomas locationhave no significant association (p= 1,000; p= 0,657; p= 0,354; p= 0,538).The present study showed difference of methylation of MGMT gene promoter in astrocytomas with others studies whichis hypermethylated MGMT only found in low grade astrocytomas. Our study was the first to report the frequency ofMGMT promoter methylation among Indonesian astrocytoma patients.

Abstract

Polyp hyperplasia is a neoplastic lession, abnormal gen hidding beyond the cell and developed to be malignant. Most of lession founded in rectum and sigmoid, and almost multiple, the smaller size 2 mm, appearance like a normal mucous colour and a little pale. Proportion in male and female is 13 : 1, most often in boy under 10 year-old (about four year-old), in adult around 18 year-old. Endoscopicaly incidens non adenomatous 15% - 86% under five mm in size, but 97% is adenoma if the size more than one cm. Familial Adenomatous Polyposis (FAP) 100% is became a malignancy.

Abstract

Penderita histiocytosis dengan keterlibatan multiorgan dan disertai gangguan fungsi organ mempunyai prognosis buruk dan tingkat mortalitas tinggi. Dilaporkan seorang penderita histiocytosis, usia 1 tahun 1 bulan yang datang dengan kelainan di kulit seperti dermatitis seboroik dan gizi buruk. Dalam pemeriksaan ditemukan keterlibatan sistem limfe, paru, hepar, tulang, dan kuku, selain kelainan kulit yang terlihat. Selain itu, juga didapatkan gangguan pada fungsi sumsum tulang yang ditunjukkan dengan adanya anemia dan trombositosis. Saat ini anak masih menjalani pengobatan sesuai protokol histiocytosis dan memasuki minggu ke-12 (fase rumatan) dengan kondisi kelainan kulit, organ, dan gangguan fungsi sumsum tulang yang membaik.Kata kunci: Langerhans cell histiocytosis, keterlibatan multiorgan, gangguan fungsi organ, prognosis
