Abstract

Background: Pelvic chondrosarcoma may be difficult to manage due to its proximities with vital structures. The study aimed to explain an alternative surgical technique for acetabular reconstruction.Case Presentation: We present a case of a 48-year-old female with large chondrosarcoma of the superior and inferior pubic rami with medial acetabular wall involvement. Pelvic type 3 resection was performed. There was a defect at medial acetabulum after resection. The defect was covered by autograft from iliac and fixation using screws. Rotational pelvic stability was maintained using a reconstruction plate. The functional outcome was assessed 6 months after operation using MSTS and the score was 30, which was painless, full weight bearing, normal gait, and no pain. Conclusions: Reconstruction of the pelvis after tumor resection requires a careful preoperative patient evaluation and extensive bone and soft tissue resection to achieve negative tumor margins and stable reconstruction of the osseous and soft tissue defects.

Abstract

Background: Surgical interventions such as intralesional curettage until the complete excision and reconstruction require distal radius GCT treatment. However, to maintain the wrist function and reduce recurrent tendency, a more comprehensive understanding is needed to decide on the best therapeutic option as the standard strategy to achieve complete resection and functional outcomes. Therefore, this study aims to find the current treatment trends based on a literature study on distal radius GCT, emphasizing their potential outcomes. Methods: The search strategy using the PubMed search engine (https://pubmed.ncbi.nlm.nih. gov/) was conducted on September 21, 2020, with the keywords: “distal radius giant cell tumor”. The search resulted in 261 articles, and they were filtered by “published in the last five years,” which yielded 67 articles. These articles then underwent further screening, resulting in 40 articles for the analysis. Results: For article types, there were 27% (11/40) prospective studies, 32% (13/40) retrospective studies, 5% (2/40) systematic reviews, 27% (11/40) case reports, and 5% (2/40) case series. For the case series, more than ten cases were reported in both articles. There were 47 treatments in those total articles, and there were two types of GCT excision: intralesional curettage (extended and non-extended) (74.4%; 35/47) and en bloc (wide) resection (25.5%; 12/47). Conclusions: The total number of articles published in the last five years on the distal radius increased in 2020. Most types of articles were retrospective studies, followed by case reports and prospective studies. The most common treatment published in the last five years is intralesional curettage than en bloc resection, with excellent results in intralesional curettage’s functional outcome.

Abstract

Breast cancer in Indonesia the second most common cancer in women after cervical cancer in 2008. FAC and TAC are chemotherapy regiments for breast cancer that give good therapy response but affect patient haematology. Study about hematologic toxicity of TAC and FAC in breast cancer patient has not been conducted in Dharmais Cancer Hospital. The purpose of this study was to determine hematologic toxicity of 6 cycles TAC (docetaxel 75 mg/m2, doxorubicin 50 mg/m2 and cyclophosphamide 500 mg/m2, day 1, 21) and FAC (fluorouracil 500 mg/m2, doxorubicin 50 mg/m2 and cyclophosphamide 500 mg/m2, day 1, 21) in breast cancer patients in Dharmais Cancer Hospital.This study used cross sectional design in 11 and 179 patient medical records that were indentified treated with TAC and FAC. Patients received first cycles both regiments in 2007-2008. All stages of breast cancer were included in this study. Differences between TAC and FAC in proportion of haematology count after chemotherapy and average decrease of hematology count were analyzed by chi square and t test statistic methods.From 66 hematology data in patients treated with 6 cycles TAC grade 1-4 of hematologic toxicity occurred 84,8% in haemoglobin with average decrease 0,71 g/dl; 97% in leucocyte with average decrease 7,51 x 103 cell/?l; 22,7% in trombocyte with average decrease 147,77 x 103 cell/mm3. From 1079 hematology data in patients treated with 6 cycles FAC grade 1-4 of hematologic toxicity occurred 72,6% in haemoglobin with average decrease 0,66 g/dl; 85% in leucocyte with average decrease 2,73 x 103 cell/?l; and 10,9% in thrombocyte with average decrease 104,25 x 103 cell/mm3.

Abstract

Tujuan: mendapatkan data tentang proporsi kejadian depresi serta faktor yang berhubungan pada pasien kanker di ruang rawat singkat Rumah Sakit Kanker Dharmais.Metode: studi potong lintang dilakukan terhadap 26 pasien dewasa ruang rawat singkat Rumah Sakit Kanker Dharmais (RSKD) untuk menentukan proporsi kejadian depresi dan faktor yang mungkin berhubungan. Data demografik meliputi nama, usia, jenis kelamin, tempat tinggal, pekerjaan, pendidikan, status pernikahan, dan pembiayaan. Data yang berhubungan dengan kanker meliputi jenis kanker, lama terdiagnosis kanker, kemoterapi, dan terapi lainnya (pembedahan dan radioterapi). Depresi dievaluasi menggunakan Beck Depression Inventory (BDI).Hasil: diperoleh 26 pasien kanker. Proporsi pasien yang mengalami depresi ambang, sedang, dan berat sebesar 13,1%; dan tidak ada pasien yang mengalami depresi sangat berat. Proporsi kejadian depresi lebih tinggi pada pasien dengan pembiayaan jaminan, sudah pensiun bekerja, dan menjalani pembedahan.Kesimpulan: proporsi kejadian depresi di Rumah Sakit Kanker Dharmais sebanding dengan berbagai studi. Perlu dilakukan penapisan gejala depresi pada pasien kanker sebagai bagian penatalaksanaan kanker secara holistik serta rujukan untuk pasien dengan risiko tinggi menderita depresi dalam usaha menurunkan morbiditas, perbaikan luaran penyakit, penghematan biaya kesehatan, dan peningkatan kualitas hidup pasien.Kata Kunci: Beck Depression Index (BDI), depresi, kanker

Abstract

Prostate cancer is a malignancy disease which originates in prostate organ that only can be found in men. This cancer is the second most common in men after lung cancer and the fifth in the entire world. The incidence will be increasing in the fifth decade of life with the peak in the eight decade. Several factors which are thought to be the etiology are: genetic, hormonal, diet, environment, and infection. Diet factor has great influence for both, the development and the prevention of this disease. Man who consumes high fat diet will have not only the risk to get that disease but also will increase the progresiveness of the disease. Several nutrients that are thought for decreasing the incidence of the prostate cancer are cruciferous vegetables and fruits which contain carotenoid. Furthermore, soya which contains much of isoflavon or phytoestrogen can also acts as the prevention of development of cancer cell. Green tea, lycopene ( carotenoid group that can be found in tomato products) can also act as the source of the antioxidant. This paper will discuss the role of nutrition factors of lycopene and soya in prostate cancer.

Abstract

Farmakogenetik dalam terapi kanker adalah pemberian terapi kepada pasien berdasarkan status atau profil genetik dari sel kanker. Dengan demikian, pemberian kemoterapi diharapkan bisa lebih efektif dan meningkatkan kualitas hidup pasien dengan menghindari terapi yang diketahui secara genetik tidak memberikan keuntungan klinis. Pemeriksaan DNA gen p53 sebagai salah satu tumor suppresor gene yang termutasi pada hampir sebagian besar tumor diharapkan mampu menjadi faktor prediktif ketika dilakukan secara rutin terhadap jaringan tumor dari pasien sebelum menentukan strategi kemoterapi. Status mutasi gen p53 pada tingkat DNA di sel kanker payudara cenderung berasosiasi dengan respon klinis yang baik untuk kemoterapi golongan taxane, yang menganggu proses polimerasi mikrotubula. Sedangkan kemoterapi yang bersifat genotoksik (merusak DNA) seperti anthracycline dan alkylating agents, mutasi pada gen p53 memiliki pengaruh yang lebih kompleks terhadap respon pasien.Kata-kata kunci: p53, kanker payudara, kemoterapi

Abstract

Background: Autoclave autograft is a promising alternative procedure in surgical treatment of bone tumors. A minimum effective autoclaving time of 15 minutes at 134C to devitalise tumour cells. This study aims to know the effect of autoclave to physical properties and histopathology of bone.Methods: We performed experimental study in 28 femoral bone of goat (Capra Aergagus circus) at age 1?2 year old. We divide into two groups with simple random sampling to be 14 autoclave and 14 control group. Autoclaving was performed to the bone at 134C for 15 minute. We examined physical properties each 7 autoclave group to axial and bending force compared with physical properties of the control group. We also examined histopatology of the bone both groups. Independent t-test statistical analysis to evaluate the difference of axial and bending force between autoclave and control group.Results: There were significant difference between autoclave and control group in axial force (p = 0.000) and bending force (p = 0.03). Physical properties of autoclave bone decreased 58.58 % in axial force and 20.33% bending force to control group. Histopathology examination revealed that all osteocytes were death in autoclave group but without matrix changes.Conclusion: Autoclave at 134C for 15 minute caused physical properties of bone decrease and statistically significant difference. It also made all osteocytes died but without matrix disturbance.

Abstract

Radioterapi masih merupakan standar baku untuk pengobatan paliatif pada metastasis tulang. Pada artikel ini kami akan menguraikan kembali tentang patofisiologi, diagnosis dan pengobatan metastasis tulang khususnya dari sudut pandang radioterapi. Kami juga menyajikan data dari instalasi radioterapi RS. Kanker "Dharmais" yang merupakan penelitian retrospektif terapi radiasi pada metastasis tulang dalam kurun waktu 2005-20C6.Kata kunci : metastasis tulang, patofisologi, diagnosis, terapi radiasi.

Abstract

Background: Endometrial cancer is the sixth most common malignancy incidence in women worldwide. The surgical staging in endometrial cancer management such as bilateral salpingooophorectomy and lymphadenectomy is not a routine procedure for all endometrial cancer patients. Therefore, a biomarker to estimate the presence of endometrial cancer metastases to adnexa and lymph nodes is needed. This study aims to find out the level of cancer antigen 125 (CA-125) in endometrial cancer endometrioid type and the relationship between the level of CA-125 and the incidence of adnexal metastases, lymph node metastases, clinical stage, and myometrial invasion.Methods: This observational analytic study used a direct observational design. The in-vitro quantitative examination was performed to measure CA-125 levels using CA-125 Advia Centaur CA-125 Kit. Fischer exact test used to analyzed CA-125 levels with the subject characteristics. All the statistical analyses were performed using SPSS v. 21.0.Results: Of the 30 research subjects with the characteristics of the most age groups of 51–60 years (60%), with the most multiparous parity (73.3%), grade III (43.3%), myometrial invasion > 50% by 66.7%, early-stage (63.3%), the incidence of adnexal metastases (30%), and the incidence of lymph node (LN) metastases (10%), a significant relationship between CA-125 levels and lymph node (LN) metastases was found with a p = 0.041 and the phi association of 0.016 and between CA-125 levels and adnexal metastases with a p = 0.035 and the phi association of 0.026. There was also a significant relationship between CA-125 levels and the clinical stage (advanced stage) with p = 0.004 and the phi association of 0.002. There is no significant relationship with a p = 0.702 and a phi association value of -0.098.Conclusions: CA-125 can be used as a tumor marker to predict the presence of extrauterine metastases (adnexa & lymph nodes).

Abstract

Background: Breast cancer patients often experience decreased the immune system such as a decreased of lymphocytes, which can increase mortality. Acupuncture is expected to be a therapeutic option to increase lymphocyte because current pharmacological therapy is very expensive with the risk of adverse event. This study aims to prove the action of Electroacupuncture (EA) as an immunomodulator for cellular immunity by increasing the percentage of lymphocytes of Peripheral Blood Smear (PBS) in C3H mice of breast adenocarcinoma model. Methods:Laboratory experimental study with post test only controls group design involving twenty PBS preparations taken from 20 C3H mice of breast adenocarcinoma models divided into 4 groups. Group K with no EA treatment, group P1 was given 1 time EA treatment, group P2 was given 2 times EA treatment and P3 was given 3 times EA treatments (with seven days interval). EA stimulation was performed at ST36 Zusanli, BL18 Ganshu, BL20 Pishu with frequency of 2 Hz for 15 minutes. Result:The mean weight of mice was 23.90 ± 3.03 gr in control group, 25.40 ± 1.54 gr in P1 group, 27.00 ± 3.87 gr in P2 group and 25.90 ± 5.83 gr in P3 group, with most of them were female mice. The mean volume tumor in mice of control group was 14.49 ± 2.75 mm3, 16.72 ± 6,40 mm3 in P1 group, 20.18 ± 5.94 mm3 in P2 group and 22.07 ± 6.51 mm3 in P3 group. The mean percentage of lymphocyte was 44.20 ± 15.72 in control group, 43.00 ± 11.70 in P1 group, 49.60 ± 7.02 in P2 group and 43.20 ± 13.10 in P3 group. The correlation between the number of electro­acupuncture treatments on the number of lymphocytes in the treatment group were compared to the control group with p = 0.811. Conclusions: Research acupuncture on cancer management has been done and provided good result. Acupuncture in particular EA has benefits in improving immunity by affecting lymphocyte enhancement. Acupuncture can be considered as adjuvant therapy in cancer patients breast. ABSTRAKLatar Belakang: Pada pasien kanker payudara sering terjadi penurunan sistemimun antara lain menurunnya limfosit, yang dapatm eningkatkan angka kematian. Akupunktur diharapkan dapat menjadi pilihan terapi untuk meningkatkan limfosit karena terapi farmakologi saat ini harganya sangat mahal dengan resiko efek samping. Penelitian ini bertujuan untuk membuktikan bahwa tindakan elektroakupunktur (EA) dapat berefek sebagaii mmunomodulator terhadap imunitas seluler dengan meningkatkan persentase limfosit sediaan apus darah tepi (SADT) pada mencit C3H model adenokarsinoma payudara. Metode: Penelitian eksperimental ini dilakukan dengan post test only control group design terhadap dua puluh sediaan apus darah tepi yang diambil dari 20 mencit C3H model adenokarsinoma payudara. Kelompok studi dibagi menjadi 4 kelompok yaitu Kelompok K tidak diberi perlakuan EA, kelompok P1 dilakukan EA 1 kali, kelompok P2 dilakukan EA 2 kali dan P3 dilakukan EA 3 kali.
