Abstract

Introduction: Percutaneous Ethanol Ablation (PEA) has been recommended as the first-line treatment option for symptomatic benign cystic or predominantly cystic thyroid nodules, and it has been shown to be more effective and safer than other techniques in previous studies. Here, we present a case of a 44-year-old man with thyroid nodules who underwent PEA.Case Presentation: We report the case of a 44-year-old man with a painless mass that is getting bigger on his right neck. The physical examination measured 3.0 × 3.0 × 3.0 cm, mobile on swallowing, no pain on palpation. Thyroid function tests show a euthyroid state (TSH 2.4 mIU/L, FT4 1.2 ng/dL). The ultrasound examination in January 2020 showed a colloid cyst on the right thyroid, measured 3.03 × 2.82 × 3.56 cm (TIRADS 1). Cytological examination showed the results of non-infectious cystic fluid. The patient underwent US-guided PEA using 7 ml of ethanol by the moving-shot technique. The procedure was well tolerated by the patient and no adverse events were noted. A week after the procedure, there was a visible and progressive reduction of the nodule with no complaint from the patient. A month after the procedure, the patient came back with no recurrences, complaints, or other complications.Conclusions: PEA for thyroid lesions is an effective and safe method. PEA should be the first option for the treatment of pure cystic and predominantly cystic nodules. PEA is a safe procedure that is easy to repeat and to be performed on an outpatient basis.

Abstract

Karsinoma serviks uteri (KSU) merupakan kanker kedua terbanyak pada wanita di seluruh dunia. Sampai saat ini, terapi pilihan utama KSU adalah operasi, radiasi, dan kemoterapi. Penelitian menunjukkan bahwa pemberian imunomodulator seperti vaksin BCG dapat meningkatkan respons terapi kanker, tetapi penggunaan vaksin BCG sebagai terapi KSU belum pernah diteliti. Penelitian ini bertujuan untuk menilai respons histopatologi dan respons klinik penderita KSU yang mendapat kemoradiasi dan BCG dibandingkan dengan yang hanya mendapat kemoradiasi.Metode: Penelitian uji eksperimental acak buta ganda dilakukan di RSUP Dr. Kariadi, Semarang, dengan kriteria inklusi adalah KSU stadium IIB-IIIB dengan gambaran skuamus sel karsinoma. Subjek dilakukan biopsi, pemeriksaan histopatologi, imunohistokimia (IL2), dan secara random dibagi menjadi kelompok BCG serta kontrol. Respons histopatologis dinilai dua minggu setelah kemoradiasi, sedangkan respons klinik dinilai 4 bulan setelah kemoradiasi. Perbedaan rerata antar 2 kelompok diperiksa dengan tes t tidak berpasangan, sedangkan respons sebelum dansesudah pemberian BCG serta kemoradiasi dengan test berpasangan.Hasil: dari 87 subjek, 77 di antaranya menyelesaikan protokol penelitian (BCG 35; plasebo 42). Setelah kemoradiasi, IL-2 kelompok BCG (20,46%) lebih tinggi secara bermakna dibandingkan kontrol (13,48%). Tidak didapatkan perbedaan respons histopatologi (p=1,000) dan klinik (p=0,437). Respons histopatologik yang baik didapatkan pada 52 subjek, tetapi tidak didapatkan perbedaan parameter imun seluler antara kelompok respons histoptologi baik dan jelek.Kesimpulan: vaksin BCG yang diberikan pada penderita KSU stadium IIBIIIB sebelum dilakukan kemoradiasi meningkatkan IL-2 dan cenderung meningkatkan imunitas seluler, tetapi tidak meningkatkan respons histopatologi.Kata kunci: kanker serviks, BCG, IL-2

Abstract

Dengan mengucapkan puji syukur ke hadirat Allah SWT, sudah sepantasnya kami, seluruh staf redaksi Indonesian Journal of Cancer, berbahagia dengan diperolehnya Akreditasi B dari Lembaga Ilmu Pengetahuan Indonesia (LIPI) untuk Indonesian Journal of Cancer yang usianya kini memasuki tahun ke-3. Pencapaian tersebut tentu tak lepas dari kerja keras semua pihak dari Direktur Utama Rumah Sakit Kanker Dharmais sebagai Pelindung hingga seluruh Staf Redaksi, yang dengan semangat tinggi selalu meningkatkan kualitas dan menjaga konsistensi waktu terbit. Abstract

Background: The variation of sizes, shapes, and location of kidney cancer complicates the choices of surgical treatment.To determine which technique to use, R.E.N.A.L. nephrometry scoring systems were established. This study was conducted to evaluate R.E.N.A.L.-NS profile in kidney cancer patients at CMHMethod: The data were collected retrospectively from patients that underwent both open and laparoscopic Radical (RN) and Partial Nephrectomy (PN) procedure from 2014-2017. R.E.N.A.L.-NS was calculated based on (R)adius, (E)xophytic/ Endophytic properties, (N)earness to the collecting system, (A)nterior or Posterior position of the tumor, and (L)ocation of the tumor. It was categorized into three complexity: low (4-6 points), medium (7-9 points), and high (10-12 points). Subjects then divided based on the procedure given. Profile of R.E.N.A.L.-NS was shown based on each procedure.Result: In this study, 63 patients were included. 52 underwent RN and 11 underwent PN. In low complexity tumors, all patients received PN. In medium complexity tumors, 22 (78.5%) patients received RN and 6 (21.5%) received PN. All high complexity tumors received RN. Mean renal score in all patient 9.03 (+1.72), RN 9.59 (+1.11), PN 6.36 (+1.6). Higher (R), (N), and (L) scores mean a higher prevalence of RN.Conclusion: Higher complexity tumors were more likely to be treated with RN. Furthermore, (R), (N), and (L) score can be useful to determine RN or PN as a treatment of choice. This study could be used as a reference to another study regarding R.E.N.A.L.-NS in Indonesia.

Abstract

Background: In the community of Surabaya, the 2nd largest city of Indonesia, women living with cancer are mostly the long-term survivors of advanced-stage cervical cancer, but the number of women living with breast cancer cannot be ignored. There were various physical and psychological problems related to the total suffering and worsening phenomena. The limited resources available in the community have led to various levels of unmet needs in their perspective. This study aimed to analyze and describe the unmet needs of female cancer survivors in the community of Surabaya.  Methods: This descriptive study involved five Public Health Centers (PHCs) among 63 PHCs in Surabaya (7.94%), Indonesia, which were selected by one-stage cluster random sampling. There were 41 and 40 women living with breast and cervical cancers, respectively, participating in this study (n = 81). The Supportive Care Needs Survey – Short Form 34 (SCNS-SF34) was used to collect the data of unmet needs (r = 0.234 – 0.821; Chronbach Alpha = 0.939). Descriptive statistic was used in data analysis (frequency, Mean, and Standard Deviation). Ethical clearance was issued. Results: Most respondents were late adult married women with low socioeconomic status. They were mostly short-term survivors and already had surgery. Most of them reported a moderate level of unmet needs (Mean ± SD = 109.30 ± 27.66). Health system and information needs became their top priorities (45.75), followed by patient care and support needs (37.89), physical and daily function needs (32.20), psychological needs (31.11), and sexuality needs (22.00). There were 9.88% of respondents who reported that there were no unmet needs. Conclusions: Female cancer survivors in the community reported a moderate level of unmet needs. Health system and information needs were the most reported unmet needs

Abstract

The Breast Unit in the University Malaya Medical Centre (UMMC) was set up in 1993 to cater for the increasing number of women presenting with breast complaints. The surgeon plays a central role in the coordination of the multidisciplinary team which consists of the breast surgeon, pathologist, radiologist, oncologist, plastic surgeon, physiotherapist, psychooncologist, as well as the specialist breast care nurse. The unit provides a comprehensive patient care yrogramme, with diagnostic services (triple assessment comprising clinical evaluation, radiological assessment and biopsy), breast screening services (clinical breast examination ana mammography / adjunct ultrasound), oncological services and palliative care services. Throughout the course of the treatment and post-treatment, emphasis is placed on supportive and emotional care of the patient and her relatives. Teaching and training of undergraduate and postgraduate students form part of the functions of this unit, and clinical and basic research is actively carried out. It has been shown that patients treated in a multidisciplinary breast unit have a better overall outcome: however in low and middle income countries, the development of such units are hampered but lack of manpower and resources. In UMMC, with the institution of treatment protocols and guidelines, it is hoped that survival of patients treated in this multidisciplinary unit will progressively improve.Key words : breast cancer, multidisciplinary breast unit, comprehensive breast care

Abstract

Penelitian ini bertujuan untuk mendapatkan data metastasis KGB retrofaring pada penderita KNF dengan pemeriksaan CT nasofaring di Rumah Sakit Kanker Dharmais.Penelitian ini menggunakan metode deskriptif analitik dari data sekunder CT nasofaring penderita Karsinoma Nasofaring (KNF) yang belum mendapatkan terapi radiasi dan kemoterapi. Penilaian metastasis Kelenjar Getah Bening (KGB) retrofaring dengan diameter aksial minimal ? 5 mm yang berada di level atlas dekat arteri karotis interna. Penilaian massa tumor menurut TNM AJCC edisi ke-7 tahun 2010. Dilakukan uji statistik untuk mengetahui adanya hubungan metastasis KGB retrofaring dengan massa tumor, tipe histopatologi, invasi lateral, dan massa tumor melewati midline.Hasilnya menunjukkan bahwa sebanyak 85 penderita KNF dengan subjek terbanyak laki-laki, umur rerata 43,2 tahun, metastasis KGB retrofaring sebanyak 81 subjek, dan metastasis KGB servikal level II merupakan metastasis KGB terbanyak.Kesimpulannya, metastasis KGB retrofaring adalah metastasis KGB terbanyak kedua setelah KGB servikal level II. Kedua metastasis KGB ini merupakan drainase pertama metastasis KGB pada KNF.Kata Kunci: Metastasis KGB retrofaring, Karsinoma nasofaring, Pemeriksaan CT nasofaring.

Abstract

Reporting two cases of mixed adenocarcinoma and neuroendocrine carcinoma of the ovary. A case report two cases of an ovarian cyst suspect malignancy after complete surgical staging in woman aged 39 year old and 72 year old. By this case report, we want to know prognosis of the malignancy. Mixed adenocarcinoma and neuroendocrine carcinoma of the ovary of the first case arised from mature cystic teratoma, and second case as metastatic process from gastrointestinal tract. Mixed adenocarcinoma and neuroendocrine carcinoma is rare hystologic type of ovarian cancer. Need further exploration to know the survival of this hystologic type.ABSTRAKArtikel ini melaporkan dua kasus, yaitu mixed adenocarcinoma dan neuroendocrine carcinoma ovarium, dua kasus kista ovarium curiga ganas pada pasien berusia 39 tahun dan 72 tahun. Terhadap kedua pasien dilakukan komplit surgical staging. Melalui laporan kasus ini, diharapkan bisa diketahui prognosis kasus. Mixed adenocarcinoma dan neuroendocrine carcinoma ovarium pada kasus pertama timbul dari teratoma matur kistik, sedangkan kasus kedua merupakan penyebaran dari traktus digestivus. Mixed adenocarcinoma dan neuroendocrine carcinoma merupakan tipe histologi kanker ovarium yang jarang. Ia membutuhkan eksplorasi lebih lanjut untuk mengetahui survival dari tipe histologi ini.

Abstract

Background: Renal cell carcinoma (RCC) is one of the most common malignancies in the kidney and causes a mortality rate of more than 100,000 each year globally. The neutrophil-lymphocyte ratio (NLR) is one of the markers in the inflammatory response that also correlate with the alteration of any cancer cells. We investigated the correlation between the NLR on tumor stage, Fuhrman nuclear grade, length of stay (LOS), mortality, and recurrence rate in renal cell carcinoma.Methods: We investigated 52 patients with renal cell carcinoma retrospectively from the databases of the patients who underwent radical and partial nephrectomy in Sardjito Hospital Yogyakarta between 2012 and 2017. Patients were classified as lower stage (T1-T2) and higher stage (T3-T4). We also classified it as a group of Fuhrman nuclear grade (G1, G2, G3, and G4). Two sample t-test or one-way ANOVA was used for the continuous variables and a chi-square test or Fisher’s exact test for the categorical variables. Pearson test was used to do the correlation test.Results: Among a total of 52 patients analyzed in our study, there were 36 males (69%) and 16 females (31%). A normal distribution of datasets was verified. The mean of preoperative NLR in these 52 patients was 6.35 (+4.01), with a range of 1.48 - 21.0. The value of NLR was positively correlated with tumor pathologic stage (p = 0.020), length of stay of the patients (p = 0.00), and the mortality rate (p = 0.012). Neutrophil lymphocyte ratio was not significantly correlated with Fuhrman tumor grade (p = 0.357) and tumor recurrence (p = 0.670).Conclusions: We compared the relationship of preoperative NLR with renal cell carcinoma tumor stage (T), Fuhrman grade, length of stay, mortality, and tumor recurrence. NLR was found to have a statistically significant higher T stage, mortality, and length of stay of the patients. Further studies with more patients are needed to confirm our study.

Abstract

ABSTRAKPenelitian ini bertujuan untuk mengetahui karakteristik dan faktor-faktor prediktif tumor testis dewasa di JakartaData dikumpulkan dari status khusus di departemen urologi RSCM dan RS kanker Dharmais. Data yang dikumpulkan adalah usia, tumor marker yaitu AFP, LDH, HCG, jenis operasi, jenis tumor, jenis terapi, dan stadium. Dilakukan analisa terhadap usia dengan stadium, Undescended Testicle (UDT) dengan stadium, tumor marker dengan stadium pada semua kasus, dan hubungan AFP dengan jenis nonseminoma dengan stadium menggunakan ANOVA pada SPSS 11,5 .Selama 10 tahun terdapat 149 kasus tumor testis, 129 kasus diantaranya adalah dewasa. Rentang usia yaitu 18-72 tahun, dengan usia rata-rata 33,03 tahun. Jenis tumor diantaranya seminoma 67 (51,9 %) pasien, nonseminoma 50 (38,8 %) pasien, non germinal 6 (4,7 %) pasien, tidak diketahui jenis tumornya 6 (4,7 %) pasien. Delapan puluh tujuh pasien (67,4 %) dilakukan orkhidektomi ligasi tinggi, 23 (17,8 %) pasien dilakukan orkhidektomi transscrotal. Insidens UDT terdapat pada 13 (10,1 %) pasien. Seminoma paling banyak ditemukan pada stadium 2c (36,9 %), dan non -seminoma pada stadium 3c (48,9 %). Kemoterapi dilakukan terhadap 51 (39,5 %) pasien, radioterapi pada 24 (18,6 %) pasien, dan 27 (20,9 %) pasien menolak dilakukan tindakan, dan 18 (14,0 %) pasien meninggal karena keadaan umum yang buruk. Pada penelitian ini tidak ditemukan hubungan bermakna antara usia dengan stadium. Tujuh puluh tujuh persen pasien dengan UDT ditemukan pada stadium lanjut, dibandingkan dengan 66,7 % pada pasien non-UDT, tetapi secara statistik tidak bermakna. Angka rata-rata LDH 1785,35 dan nilai LDH meningkat sesuai dengan stadium, tetapi tidak bermakna untuk memprediksi stadium. Angka rata-rata AFP pada pasien non-seminoma adalah 6421,13 dan mempunyai hubungan yang bermakna dengan stadium (p: 0,009).Jenis tumor yang paling banyak ditemukan di Jakarta adalah seminoma. Pada penelitian ini usia dan LDH tidak dapat memprediksi stadium tumor, tetapi AFP mempunyai hubungan yang bermakna dengan stadium pada penderita kanker testis non-seminoma. ABSTRACTThe aim of this research is to determine characteristics and predictive factors in adult testicular tumor in JakartaAnalyzed the data were collected from urology departemen at Cipto Mangunkusumo hospital and Dharmais hospital medical records. The collected data were age, AFP, LDH, HCG, type of operation, type of tumor, type of therapy and staging and reviewed retrospectively. Data were analyzed by using ANOVA in SPSS 11.5 for correlation between age and stage, undescended testicle (UDT) and stage, serum marker in all cases as well as and stage, and correlation between AFP in non-seminomatous patients with stage.During 10 years, 149 testicular tumors patients were registered, among these cases, 129 cases are adult patients and the range of age was 18-72 years old with mean of age was 33.03 years old. Type of tumors were seminoma 67 (51,9 %) patients, non-seminoma 50 (38.8 %) patients, non germinal 6(4,7 %) patients, unidentified 6 (4,7 %) patients. Eighty seven patients (67,4 %) underwent high ligation orchidectomy ivhile 23 (17,8 %) patients underwent transcrotal orchidectomy. Incidence of UDT patients was 13 (10,1%) patients. Most of the seminoma patients were diagnosed as stage 2c (36,9 %), and in non-seminoma was 3c (48.9 %). Chemotherapy was performed in 51 (39,5 %) patients, radiotherapy 24 (18,6 %) patients and 27 (20,9 %) patients refused to be treated, and 18 (14,0 %) patients were died before treatment due to poor condition. In this study, there was no significant correlation between age and stage.
