Abstract

Performance monitoring of primary health centers chief internal contracting system in JakartaPurpose: This study aimed to describe internet-based electronic monitoring control mechanisms and problems that occur in policy implementation at the regional public service agency in Jakarta.Methods: A qualitative research with exploratory technique was conducted using semi-structured interviews, observation and document research. The research subjects were 12 people. The informants were chosen by purposive sampling. Data analysis used content analysis methods.Results: Results showed that attendance and performance of staff recorded in the system helped in managerial duties, the items of work have been standardized on the application of e-kinerja together with the points for incentives. While the number of human resources were less, omissions in time limit entry performance which resulted in non-payment of incentives served to encourage officers to report the activities in proper time. There were problems with the community in the form of false complaints of moral hazard but there is no filter mechanism against the false complaints.Conclusion: The results showed it difficult for health centers to prevent the complaints of moral hazard in the presence of a multi-layered monitoring system both internally and externally.

Abstract

The intention of behavior to come to the coffee shop in Bengkalis societyPurpose: Smoking while drinking coffee at the coffee shop every morning are the behavior of society in Bengkalis. Those habits facilitate a person to interact with one another. Tar and nicotine in cigarette and caffeine in coffee conformity impair health, especially in the cardiovascular system. A qualitative study has to be done, to explore the intention of people to come to the coffee shop.Methods: Descriptive explorative research with a qualitative approach. Subjects were 5 visitors who smoke and drink coffee, 2 visitors who don’t smoke and don’t drink coffee, 2 coffee shop owners and a coffee shop worker in Bengkalis. Samples are taken by purposive sampling, the research did until getting saturated data. Data collection by participant observation and in-depth interview. Data analysis did with content analysis.Results: This research showed the intention of people to come to the coffee shops: having breakfast (drink, eat, smoke), meeting (chat, discuss, lobby, campaign), and looking for a chance (information, relation, promotion). That intention is influenced by attitude, subjective norm, and perceived control. Attitudes: come to the coffee shop are a habit, important, and necessity, visitors support this habit; subjective norm: perception about the importance of social interaction to get information and relations; and perceived control behavior: no wife support, advice from doctor, and prohibition of civil servant at coffee shop in working time.Conclusion: The intention is influenced by attitude, perception about social encouragement, and perceived control behavior to come to the coffee shop. The intervention of health promotion is needed with emphasizing factors that influence visitors’ intention to come to the coffee shop.

Abstract

Latar Belakang: Penyakit DBD masih merupakan masalah yang sangat penting dan menjadi salah satu tantangan utama bagi kesehatan masyarakat di seluruh dunia, kejadian DBD tidak hanya menyerang kelompok usia, jenis kelamin maupun status sosial tertentu. Penyakit DBD merupakan suatu infeksi arboviral yang ditularkan melalui vektor sehingga mudah menular dari satu orang ke orang lainnya. Kasus DBD di Kota Magelang terjadi di ketiga wilayah kecamatan yaitu Kecamatan Magelang Selatan yang terdiri dari enam kelurahan, Kecamatan Magelang Tengah terdiri dari enam kelurahan dan Kecamatan Magelang Utara terdiri dari lima kelurahan. Penelitian ini bertujuan untuk melihat gambaran spasial kejadian DBD dan status kerentanan Aedes aegypti terhadap malathion di Kota Magelang.Metode: Penelitian ini menggunakan metode penelitian deskriptif dengan pendekatan spasial temporal untuk melihat distribusi kasus DBD dari tahun 2014-2017 dan keberadaan telur nyamuk dengan menggunakan GPS untuk penentuan waypoint penderita dan lokasi pemasangan ovitrap serta lembar observasi. Status kerentanan dilakukan dengan uji susceptibility test dari nyamuk hasil rearing telur yang dipasang dengan ovitrap pada 195 rumah sampel terhadap malathion.Hasil: Kasus DBD Tahun 2014-2017 terjadi sebanyak 380 kasus dengan kasus tertinggi pada Tahun 2015 yaitu 158 kasus dan terendah Tahun 2017 dengan 66 kasus. Kematian DBD tertinggi Tahun 2016 yaitu 4 kasus kematian dan terendah Tahun 2014 dan 2017 dengan masing-masing 0 kematian. Seluruh kelurahan di Kota Magelang berstatus endemis kecuali Kelurahan Cacaban dan Kelurahan Panjang. Hasil NNA didapatkan Observed Mean Distance 60,380 meter, NNR sebesar 0,474, Z-score sebesar -19,309 dan p-value sebesar 0,000. Kasus DBD mengkluster menjadi kluster primer dan kluster sekunder. Setiap kelurahan terdapat daerah risiko tinggi dan rendah kasus DBD. Terjadi pergerakan kasus DBD sebesar 3,8o ke arah Timur Laut sejalan arah poros utama penghubung antar wilayah di Jawa Tengah dan DIY. Ovitrap Index didapatkan sebesar 61,03% positif telur dari 195 sampel yang dipasang. Uji kerentanan menunjukkan bahwa nyamuk Aedes aegypti telah resisten terhadap malathion di seluruh kelurahan Kota Magelang.Kesimpulan: Gambaran spasial kejadian DBD menunjukkan bahwa kasus DBD terjadi pada semua kelurahan di Kota Magelang, pergerakan kasus cenderung mengikuti arah mobilitas manusia, ketiga kecamatan merupakan daerah berisiko DBD. Insektisida malathion telah resisten di semua kelurahan di Kota Magelang.

Abstract

Support from spouse and hospital-based self help group members for hemodialysis patients in public hospitalPurpose: This study aims to document examples of social support given by hemodialysis friends and self-help group to people undergoing hemodialysis in the hospital.Method: This is a qualitative research with phenomenology research design. Subject was selected using purposive sampling based on criteria inclusion and maximum variation. Data were collected by in-depth interview, consisted of 11 informants (6 male and 5 female) with length of hemodialysis experience between 6 months to 2 years. In addition, family were also invited for in-depth interview. The validity of data were assessed through triangulation and peer debriefing.Results: This study documented examples of friendliness among people who undergoing hemodialysis in the hospital. Occasionally they schedule community meetings or just a joint meal between patients and families after the hemodialysis therapy schedule takes place. At such meetings, they can exchange experiences and transfer the information from the old members to the new one. The group involved activities outside the hospital, the group involved new members to work together at local and national events in support of chronic kidney diseases patients.Conclusion: Our findings show that depressed patient who undergoing the hemodialysis gained new perception of their diseases and got a support to continue their treatment through self-help group. This research emphasize the role of hospitals as entry points for sufferers to find groups of fellow sufferers whose social support programs.

Abstract

Purpose: To find out the relationship of capacity design toward effectiveness of Waste Water Treatment Plant (WWTP) Methods: This study was an observational study, using a cross-sectional design. Method of sampling of waster water is done by grab sampling method. The data analysis used a descriptive analysis. Results: The results showed that the highest level of effectivity of BOD and TSS are 91,09% and 97,50%. The level of MPN coliform and MPN E.coli are >1600x102 MPN/100ml, it means exceeded the quality standard of waste water. Suitability of capacity design with operational of WWTP including discharge, number of users, level of BOD and TSS, level of MPN Coliform and MPN E.coli are more appropriate, the effectiveness in processing both of BOD and TSS also increased. Conclusion: there is a trend between the design capacity of WWTP to the effetiveness of BOD and TSS levels. The more appropriate the design with operational, the higher the level effectiveness of BOD and TSS levels.

Abstract

Purpose: Implementation of national health coverage contributes to the increasing number of outpatient visits in Obstetrics and Gynecology Department, Dr. Cipto Mangunkusumo Hospital. It may be caused by improper referral system or number of government insurance patients called as BPJS patients. Therefore, this study aims to analyse the pattern and quality of referral system in the implementation of national health coverage.Method: The quasi experimental study was conducted using pre and post analysis of the cases pattern and referral quality. It included accuracy of referred case diagnosis, accuracy of referring health facility, and consistency of referred case diagnosis.Results: There was an increasing number of referral visits in the early implementation of  national health program; however, it declined overtime. There was reduction of general obstetrics and gynaecology cases and increase of sub-specialistic cases. It was in appropriate to the role of centre referral hospital in Indonesia. Around 98% referral diagnosis was correct to be referred, 82% cases came from correct health facility, and 98% referral diagnosis was consistent to Dr. Cipto Mangunkusumo hospital.Conclusion: The quality referral cases improves with the implementation of national health coverage program.
