Abstract

Contraceptives play an undeniable important role in woman?s lives. It is widely accepted that the availability of both hormonal and non-hormonal modern contraceptives have improved 0 women?s lives worldwide by allowing women to control their own fertility. Indirectly, contraceptives have been shown to reduce maternal mortality, unwanted pregnancy, and overpopulation. However, the contraceptives available today may not be suitable for all users. Furthermore, there is still need to expand available current contraceptives choices especially long-term contraceptives in order to improve acceptability. As a result, several novel products such as implants, contraceptive vaginal rings, and transdermal patches have recently been introduced in family planning programs. Additional issue of contraceptive needed to be addressed is the adverse effects of hormonal contraceptives. Therefore, new combinations with an improved metabolic profile is currently being researched. Generally, the introduction of new methods with additional health benefits and less side effects would help women with their unmet needs to obtain access to a wider range of contraceptives.

Abstract

This study is aimed to find out the effectiveness of oral 600 mcg single dose misoprostol for the evacuation of conception remnant in cases of incomplete abortion. This research is done by quasi experimental method with one group pretest – posttest design. The population of the study is all women who were diagnosed with incomplete abortion in less than 12 weeks of gestation in Cipto Mangunkusumo National general Hospital and affiliations hospital from June 2018 to June 2019. Women who met the study criteria, was treated with oral misoprostol 600 mcg single dose and then undergo conception evacuation evaluation and side effects evaluation. Effectiveness evaluation was seen after 7 days of single dose 600 mcg of misoprostol by measuring <15 mm endometrial thickness on transvaginal ultrasound. The effectiveness of using single dose oral misoprostol 600 mcg in the study participants was 93.55%, which were 29 out of a total of 31 participants. The side effects experienced after being given misoprostol were bleeding with less than the same amount as menstruation (71.0% and 9.0%), diarrhea (6.5%), and shivering (67.7%), and not found side effects of vomiting. Based on data analysis using paired T test before and 24 hours after administration of misoprostol, there was a significant difference between endometrial thickness (p value <0.001). Misoprostol 600 mcg per oral dose is effective for the evacuation of retained conception tissue of incomplete abortion at less than 12 weeks gestation, so that it can be an alternative non-operative method of evacuating retained conception tissue in incomplete abortion cases other than the operative method. Side effects found were bleeding, diarrhea, and chills. Side effects of vomiting were not found in this study.

Abstract

Purpose: Implementation of national health coverage contributes to the increasing number of outpatient visits in Obstetrics and Gynecology Department, Dr. Cipto Mangunkusumo Hospital. It may be caused by improper referral system or number of government insurance patients called as BPJS patients. Therefore, this study aims to analyse the pattern and quality of referral system in the implementation of national health coverage.Method: The quasi experimental study was conducted using pre and post analysis of the cases pattern and referral quality. It included accuracy of referred case diagnosis, accuracy of referring health facility, and consistency of referred case diagnosis.Results: There was an increasing number of referral visits in the early implementation of  national health program; however, it declined overtime. There was reduction of general obstetrics and gynaecology cases and increase of sub-specialistic cases. It was in appropriate to the role of centre referral hospital in Indonesia. Around 98% referral diagnosis was correct to be referred, 82% cases came from correct health facility, and 98% referral diagnosis was consistent to Dr. Cipto Mangunkusumo hospital.Conclusion: The quality referral cases improves with the implementation of national health coverage program.

Abstract

Background: Hysterectomy is the most common non-obstetric surgery in adult, reproductive-age women. Hysterectomy with or without ovarian conservation is known to increase the risk of cardiovascular disease. However, only a few studies regarding its immediate and short-term effects on hypertension are available. This study aimed to determine changes in blood pressure after a hysterectomy procedure.Methods: This study is a prospective cohort study of patients who underwent a total hysterectomy procedure with or without ovarian conservation at Cipto Mangunkusumo Hospital, Indonesia, from July 2018 to July 2020. Samples were grouped into patients with total hysterectomy only or hysterectomy with ovarian conservation (HT/HTSOU) and bilateral salpingo-oophorectomy hysterectomy (HTSOB). Statistical analysis was done using paired t-test and Wilcoxon test. Results: There were 80 patients included in this study (40 for each group). A significant increase in all blood pressure components occurred at 12 months after the HT/HTSOU and HTSOB procedures (p < 0.05). After six months of the HTSOB procedure, the increase in blood pressure components only occurred in systolic blood pressure and mean arterial pressure (MAP) (p < 0.05).Conclusions: There was a significant increase in all blood pressure components in the HT/HTSOU and HTSOB group at 12 months following hysterectomy while there was a significant increase in systolic blood pressure and MAP in the HTSOB group at 6 months following hysterectomy.

Abstract

Objective: To report a pregnancy that ensued following the successful recanalization of a previously sterilized fallopian tube. The novel aspect of this report is its demonstration of a successful intrauterine pregnancy post-recanalization, despite a prior ectopic pregnancy resulting from tubal sterilization. Methods: Case report Case: A 35-year-old woman successfully conceived after the recanalization of a previously sterilized fallopian tube. It is noteworthy that she had previously experienced an ectopic pregnancy due to the tubal sterilization procedure. Conclusion: This case study highlights the potential for successful pregnancy following fallopian tube recanalization, even after an ectopic pregnancy caused by a prior sterilization procedure. This report underscores the complexity of reproductive outcomes in these scenarios. Keywords: fallopian tube recanalization, pregnancy, tube sterilization.

Abstract

Introduction: Uterine Fibroids (UFs) are benign smooth-muscle neoplasms typically originating from the myometrium. Hysterectomy is the most common non-pregnancy-related surgical procedure performed on women. Laparoscopy Hysterectomy (LH) is currently accepted as a safe and efficient way to manage benign uterine disease with a lower risk of trauma and morbidity. A qualitative approach, which focuses on the Quality of Life of each patient, should help navigate which treatment is more beneficial.Case Illustration: A 56-year-old woman (G0P0) came for a health screening. It was known that about 10 years ago, the patient had a history of uterine myoma. Currently, the patient has no complaints, no abdominal pain, no history of bleeding, history of regular menstruation, and no dysmenorrhea. The patient underwent abdominal laparotomy bilateral salpingo-oophorectomy hysterectomy. Methods: We searched through 3 databases: Pubmed, Cochrane, and ProQuest with the keywords: “Abdominal Hysterectomy” OR “Laparoscopy Hysterectomy” AND “UFS-QOL” with critical review based on the criteria of the Oxford Centre for Evidence-Based Medicine for Qualitative-Studies. Conclusion: Studies show that laparoscopic hysterectomy increased patients’ QOL after the procedure, which revolves around symptom elimination and pain reduction. To ensure holistic and best-practice management, an individual assessment of each patient’s condition according to their symptoms and demographics is needed. Fibroid Uterus Asimptomatik: Penanganan Terbaik untuk Meningkatkan Kualitas Hidup Wanita dengan Membandingkan Histerektomi Laparoskopi dan Laparoskopi (Evidence-Based Medicine)AbstrakPendahuluan: Fibroid Uterus (UFs) merupakan neoplasma jinak otot polos yang biasanya berasal dari miometrium. Histerektomi adalah prosedur bedah non-kehamilan yang paling umum dilakukan pada wanita. Laparoskopi Histerektomi (LH) merupakan salah satu cara yang aman dan efisien untuk menangani penyakit uterus jinak dengan risiko trauma dan morbiditas yang lebih rendah. Pendekatan kualitatif yang berfokus pada kualitas hidup setiap pasien akan membantu menentukan pengobatan mana yang lebih bermanfaat.Ilustrasi Kasus: Seorang wanita berusia 56 tahun (G0P0) datang untuk pemeriksaan kesehatan. Diketahui sekitar 10 tahun yang lalu pasien memiliki riwayat mioma uteri. Saat ini pasien tidak memiliki keluhan apa pun, tidak ada nyeri perut, tidak ada riwayat perdarahan, riwayat menstruasi teratur, dan tidak ada dismenore. Pasien menjalani laparatomi abdomen histerektomi salpingo-ooforektomi bilateral. Metode: Peneliti mencari melalui 3 database: Pubmed, Cochrane, ProQuest dengan kata kunci: “Abdominal Histerektomi” ATAU “Histerektomi Laparoskopi” DAN “UFS-QOL” dengan tinjauan kritis berdasarkan kriteria Oxford Centre for Evidence Based Medicine for Qualitative-Studies. Kesimpulan: Penelitian menunjukkan bahwa histerektomi laparoskopi telah terbukti meningkatkan kualitas hidup pasien setelah prosedur dengan tujuan untuk menghilangkan gejala dan mengurangi rasa sakit. Untuk memastikan penatalaksanaan yang holistik dan praktik terbaik, diperlukan penilaian individual terhadap kondisi setiap pasien sesuai dengan gejala dan demografinya.Kata kunci: Fibroid Uterus, Kuesioner Gejala Fibroid Uterus, Laparoskopi Histerektomi, Abdominal Histerektomi

Abstract

Introduction: This case reports a pregnancy complicated by condyloma acuminata with co-infection of HIV and tuberculosis (TB). Such a combination is rarely documented and presents significant clinical and therapeutic challenges. This case aims to demonstrate the effectiveness of a simple surgical approach in managing extensive condyloma acuminata during high-risk pregnancy.Case Presentation: A 24-year-old pregnant woman was diagnosed with condyloma acuminata along with confirmed HIV and TB co-infection. Electrocauterization was performed successfully, leading to complete lesion removal and significant improvement in wound healing, with no early recurrence observed. The pregnancy was planned to be delivered by cesarean section.Conclusion: This case demonstrates that, even with multiple comorbidities and a high-risk pregnancy, simple surgical treatments like electrocauterization can be both safe and effective. Early multidisciplinary intervention is crucial for achieving the best maternal outcomes.Diagnosis dan Penatalaksanaan Kondiloma Genital dengan Komplikasi: Laporan KasusAbstrakPendahuluan: Laporan kasus ini membahas kehamilan yang disertai komplikasi berupa kondiloma akuminata dengan ko-infeksi HIV dan tuberkulosis (TB). Kombinasi tersebut jarang didokumentasikan dan menimbulkan tantangan klinis serta terapeutik yang signifikan. Kasus ini bertujuan untuk menunjukkan efektivitas pendekatan bedah sederhana dalam menangani kondiloma akuminata ekstensif pada kehamilan risiko tinggi.Presentasi Kasus: Seorang wanita hamil berusia 24 tahun didiagnosis menderita kondiloma akuminata dengan ko-infeksi HIV dan TB terkonfirmasi. Tindakan elektrokauter berhasil dilakukan dengan hasil berupa pengangkatan lesi secara menyeluruh dan perbaikan yang nyata pada penyembuhan luka, tanpa adanya kekambuhan dini yang teramati. Kehamilan direncanakan untuk dilahirkan dengan operasi sesar.Kesimpulan: Kasus ini menegaskan bahwa dalam konteks komorbiditas multipel dan kehamilan risiko tinggi, penatalaksanaan bedah sederhana seperti elektrokauter dapat aman dan efektif. Intervensi multidisiplin sejak dini sangat penting untuk mencapai luaran maternal yang optimal.Kata kunci: HIV: Kehamilan; kondiloma akuminata; tuberkulosis
