Abstract

Purpose : This study aimed to determine the diagnostic strength, optimal cut off point and diagnostic value of waist circumference / body height ratio in detecting the risk of cardiovascular disease in adults with Frammingham Risk Score (FRS).  Method: This study is a cross sectional research design using secondary data from the Fourth Wave of the Indonesian Family Life Survey 4 (IFLS 4). Subject selection uses the total sampling method, so that the entire study population that matches the inculution and exclusion criteria will be the subject of this study. The number of research subjects that fit the inclusion and exclusion criteria are 9,103 people. This research uses ROC analysis, with the gold standard is the risk of cardiovascular disease based on FRS and the independent variable is RLPTTB. Results: Based on sex, the risk of cardiovascular disease in the moderate category was higher in men (80.54%) compared to women (30.23%). AUC RLPTTB value in male subjects was 0.5817 (95% CI 0.5610-0.624), while the AUC value in female subjects was 0.5904 (CI 0.5727-0.6084). The optimal cut off point in male subjects is 0.5249 (sens = 0.3549; spe = 0.7626), while the cut point in female subjects is 0.5796 (sens = 0.4382; spe = 0.7024). Conclusion: The RLPTTB cut-off value based on this study is 0.5249 and 0.5796, so the health message that can be given to the public to prevent the risk of cardiovascular disease is keep your waistline less than half your height

Abstract

Abstract

Purpose: Program Keluarga Harapan (PKH) is a strategic government program that wants to help the poor meet their health needs. However, there are still PKH beneficiaries who do not understand the health mission of this program. This study intends to explore smoking behavior among PKH families beneficiaries and whether program providers pay attention to promoting healthy living for their beneficiaries. Method: This study uses primary data with cross-sectional design and multiple logistic regression. The number of samples analyzed was 379 households in the Kembangan Region of West Jakarta. Results: Eighty-two percent of PKH recipients were smokers. The four variables related to smoking are low education, low income, smoking psychological dependence, and socially motivated smoking. The psychological and social factors of smoking were among strong predictors and deserve attention in the PKH program. Conclusion: The long-term goal of PKH is to improve the health quality. Smoking reduces the health quality of PKH beneficiaries. The Ministry of Social Affairs needs to coordinate with the Ministry of Health to make this program an entry point for the movement of healthy living in PKH recipient families.

Abstract

Background: Since the MDA for LF started, a significant progress has been made in Indonesia. However, some challenges have been found in the implementation of MDA. South Bangka District failed post-MDA surveillance and re-implemented MDA in 2018 and 2019. If this failure continue to happen, the elimination goal might be difficult to attain. Therefore there is a need to investigate why there is a persistent transmission of LF despite eight rounds of MDA which has sufficient reported coverage. Objectives: This study aimed to assess implementation fidelity and its potential moderators of the MDA programme for LF in South Bangka District.Method: This was a qualitative study which used case study approach that took place in South Bangka District and Bedengung Village. Data were collected through IDI with six stakeholders from different level of health offices, one village secretary and fourteen community members. Meanwhile, FGD were conducted with eight CHWs and five community members. All participants should have involvement in the previous MDA programme to be recruited in this study. Results: MDA programme were implemented according to its guideline with some changes in terms of timeline, screening process of target population, implementation strategy, and monitoring process. Community leader engagement and DOT-MDA strategy have improved the community compliance. However, noncompliance and low participation in community-based health education were found as main challenges. Training, intersectoral socialization and advocacy, and health education were suboptimal. There was also potential bias on the screening process of eligible population. In addition, policy influenced the resources allocation during MDA implementation. Conclusion: The implementation of MDA can be improved by focusing on community mobilization. The screening process should also be done carefully in order to cover all eligible population. Besides, it is important to optimize the resource allocation by improving the coordination with all potential partners.
