Abstract

Background: The innovation of the cigarettes industry leads to evolved into an electric cigarette. US General Surgeon in 2016 reports that 13.5% of junior high school students, 37.7% of high school students and 35.8% of young adolescents in America have used electric cigarettes. This study aims to identify the use of electric cigarette among adolescent Methods: This study utilized systematic reviews using PRISMA (Preferred Reporting Items for Systematic Reviews & Meta-Analyzes) to identify all published literature using relevant keywords. The selection process is based on the literature of which the population is human, published in the last 5 years, and there is a complete text form, then selected back by title and abstract. The journal used is the selected journal based on the inclusion and exclusion criteria. Results: Electric cigarettes are used by adolescents influence through social media or commercial advertisement. One type of electric cigarette that is widely used is the Electronic Vapor Product (EVP) that can easily to obtain from retail stores, kiosks, malls, and the internet. Electric cigarette users were led to increasing the symptoms of bronchitis and asthma in adolescents. Conclusions: The use of electric cigarettes in adolescents has increased mainly among male adolescents. The current study revealed the use of electronic cigarette was influenced by the several factors: lifestyle, electric cigarette prices, and the teenager’s pocket money. Therefore, serious efforts needed to control the factors that affected the teenage users of electric cigarette. Campaign on ruthless effects of the electric cigarette among adolescents should be implemented in the school, furthermore in several places related to teenagers.

Abstract

Background:. The increasing number of hospitals built by both private and government parties to make patients can choose the hospital they want, so that will lead to increased competition between hospitals. It requires a hospital to be ready to compete and be more creative in developing its services. One of the most important elements in a hospital is the management department who play an active role. This study aims to explain the relationship between the five M’s of management with the performance improvement of an inpatient unit ”Orchid Pavilion”. So the other hospital with a low performance may be more concerned with the five management factors at the hospital and this study can be a good example which can be applied by other hospitals. Methods: This research was conducted at inpatient unit “Orchid Pavilion”Rumah Sakit Angkatan Laut (Navy Hospital) Dr. Mintohardjo. The research was conducted by qualitative method, that is to conduct in-depth interview with Head of Orchid Pavilion and Deputy Head of Orchid Pavilion, and study data obtained from Medical Administration Department Rumah Sakit Angkatan Laut (Navy Hospital) Dr. Mintohardjo. Results: The results show that there has been an improvement of performance in inpatient unit “Orchid Pavilion” and many improvement on the five M’s in management (Men, Money, Materials, Machines, and Method) resulting in significant increase of Bed Occupancy Ratio value in the room. Conclusions: Improvements in five M’s in management greatly affect the improvement of performance in inpatient unit “Orchid Pavilion” Rumah Sakit Angkatan Laut (Navy Hospital) Dr. Mintohardjo. The five M’s in management (Men, Money, Materials, Machines, and Method) have a strong role in improving the performance of inpatient unit “Orchid Pavilion”.

Abstract

Ketidakmerataan ketersediaan fasilitas kesehatan, tenaga kesehatan dan kondisi geografis yang sangat bervariasi, menimbulkan potensi melebarnya ketidakadilan pemanfaatan kesehatan pada masyarakat di beberapa wilayah di Indonesia. Tujuan tulisan ini adalah untuk mengetahui ketersediaan fasilitas dan tenaga kesehatan (khususnya dokter spesialis) dalam mendukung pelaksanaan JKN. Berdasarkan hasil penelitian terbukti bahwa ketersediaan faktor suplai (FKTL, TT, dan tenaga dokter spesialis) dalam mendukung kebijakan JKN secara umum jumlahnya masih belum mencukupi serta distribusinya belum merata di setiap wilayah kab/kota. Rasio FKTP dan FKTL per penduduk cenderung lebih tinggi di wilayah luar Jawa/ Bali. Rasio dokter spesialis per penduduk lebih tinggi di wilayah Jawa/Bali, sedangkan rasio TT di RS cenderung hampir sama range-nya di semua wilayah. Distribusi penyebarannya cenderung tidak merata hampir di semua wilayah. Rekomendasi bagi Pemerintah Daerah diharapkan mempunyai komitmen untuk memenuhi dari sisi suplai pelayanan kesehatan agar penduduk yang sakit dapat mengakses fasilitas kesehatan dengan mudah.Keywords: Fasilitas, tenaga, tempat tidur, rasio rumah sakit, suplai layanan kesehatan

Abstract

Background: Cervical cancer is the most common cancer among women. Cervical cancer mortality is also an indicator of health success, because 86% of all deaths due to cervical cancer are in developing, low and middle income countries. Cervical cancer is caused by the Human Papilloma Virus (HPV), one of the most common sexually transmitted infections in the world.Objective: This literature review aims to describe early detection of pap smear (Conventional Smear and Liquid Based Cytology) as prevention of cervical cancer.Method: This review used the literature review method to search all related and relevant articles in PubMed, ProQuest, Science Direct and Portal Garuda with publications between 2008-2018.Results and Discussion: Liquid Based Cytology sensitivity (88%) is much higher than that reported in Conventional Smear (20-30%), Liquid Based Cytology examination has the potential to increase sensitivity in the diagnosis of endometrial carcinoma.Conclusion: Although conventional smear sensitivity is lower than Liquid Based Cytology, the price of conventional smear is more affordable for the community, and the target of Liquid Based Cytology is adapted for the upper middle class. To improved conventional smear effectiveness can be accompanied by IVA examination so that it is not repeated, more practical, and higher validity.

Abstract

Background: Indonesia’s health development geared to achieve national health insurance scheme (JKN). However, funding of health in Indonesia is still dominated by domestic funding through out-of-pocket (OOP). Objective: Catastrophic health expenditure of households will disrupt household consumption and can lead to poverty. Using secondary data of the National Social Surveys 2 (Susenas) 2012, this study aims to prove health insurance ownership lowers catastrophic health expenditure of households in Indonesia in 2012. Methods: This study used a cross-sectional study design with models probit and bivariate probit. Results: The results found that the health insurance ownership lowers catastrophic health expenditure amounted 12.97% at the threshold 10% of total expenditure and amounted 18.42% at 20% of total non-food expenditure. Conclutions: Health insurance ownership provides protection for catastrophic health expenditure in Indonesia in 2012.ABSTRAKLatar Belakang: Pembangunan kesehatan Indonesia diarahkan untuk mencapai Jaminan kesehatan Nasional (JKN). Namun, sebagian besar pendanaan kesehatan masih didominasi oleh rumah tangga melalui out-of-pocket (OOP). Pengeluaran biaya kesehatan katastrofik rumah tangga akan mengganggu konsumsi rumah tangga dan dapat mengakibatkan kemiskinan. Tujuan: Menggunakan data sekunder Survei Sosial Nasional (Susenas) tahun 2012, penelitian ini bertujuan membuktikan bahwa kepemilikan jaminan kesehatan menurunkan belanja kesehatan katastrofik rumah tangga di Indonesia tahun 2012. Metode: Penelitian dengan desain studi potong lintang ini, menggunakan pendekatan ekonometrik dengan model probit dan bivariat probit. Hasil: Hasil penelitian menunjukkan bahwa kepemilikan jaminan kesehatan menurunkan belanja kesehatan katastrofik sebesar 12.97% pada ambang batas 10% dari total pengeluaran dan sebesar 18.42% pada ambang batas 20% total pengeluaran non-makanan. Kesimpulan: Kepemilikan jaminan kesehatan memberikan perlindungan terhadap belanja kesehatan katastrofik di Indonesia pada Tahun 2012.

Abstract

The incidence of injury increased from 7.5% to 9.2% (2007-2018). The proportion of injuries due to traffic accidents in Indonesia is 2.2% and the highest is at 15-24 years of age at 4.9%. Total per capita alcohol consumption is expected to increase in Indonesia by 2025. The behavior of consuming alcoholic beverages increases the risk of traffic accidents. This study aimed to determine the relationship and the magnitude of the risk of alcohol consumption behavior with the incidence of injuries caused by traffic accidents in people aged >10 years in Indonesia. This research was a quantitative study using univariate, bivariate, and multivariate analysis. The multivariate results of the study showed a significant relationship between alcohol consumption and injuries caused by traffic accidents with OR 2.436 (95% CI: 1.233 - 4.811) after being controlled by other variables, namely age, sex, education, occupation, area of residence and island of domicile. Male group had a significant relationship with OR 1.204 (95% CI: 1.131 - 1.282), higher education was significantly associated with OR 1.953 (95% CI: 1.783 - 2.138), work was significantly associated with OR 1.230 (95% CI: 1.138 - 1.329) ), living in urban areas was significantly associated with OR 1.176 (95% CI: 1.104 - 1.253) and domicile in Sulawesi Island was significantly associated with OR 1.811 (95% CI: 1.580 - 2.077). The age variable is not significant. Alcohol consumption is the variable that has the greatest influence on injuries caused by traffic accidents. Interventions need to be focused on population groups who are more at risk in formulating health policies and programs to reduce the incidence of injuries caused by traffic accidents. Abstrak Kejadian cedera meningkat dari 7,5% menjadi 9,2% (2007-2018). Proporsi cedera akibat kecelakaan lalu lintas di Indonesia sebesar 2,2% dan paling tinggi pada usia 15-24 tahun sebesar 4,9%. Total konsumsi alkohol per kapita diperkirakan meningkat di Indonesia tahun 2025. Perilaku mengonsumsi minuman beralkohol meningkatkan risiko kecelakaan lalu lintas. Penelitian ini bertujuan untuk mengetahui hubungan dan besaran risiko perilaku konsumsi minuman beralkohol dengan kejadian cedera disebabkan kecelakaan lalu lintas pada penduduk usia >10 tahun di Indonesia. Penelitian ini merupakan penelitian kuantitatif dengan menggunakan analisis univariat, bivariat, dan multivariat. Hasil multivariat penelitian menunjukkan hubungan yang signifikan antara konsumsi minuman beralkohol dengan cedera disebabkan kecelakaan lalu lintas dengan OR 2,436 (95% CI: 1,233 – 4,811) setelah dikontrol oleh variabel lain yaitu variabel umur, jenis kelamin, pendidikan, pekerjaan, wilayah tempat tinggal dan pulau domisili. Kelompok laki-laki berhubungan bermakna dengan OR 1,204 (95% CI:1,131 – 1,282), pendidikan tinggi berhubungan bermakna dengan OR 1,953 (95% CI: 1,783 – 2,138), bekerja berhubungan bermakna dengan OR 1,230 (95% CI: 1,138 – 1,329), tinggal di perkotaan berhubungan bermakna dengan OR 1,176 (95% CI: 1,104 – 1,253) dan domisili di Pulau Sulawesi berhubungan bermakna dengan OR 1,811 (95% CI: 1,580 – 2,077). Variabel umur tidak signifikan. Variabel konsumsi alkohol adalah variabel yang paling besar pengaruhnya terhadap cedera disebabkan kecelakaan lalu lintas. Perlu fokus intervensi terhadap kelompok penduduk yang lebih berisiko dalam merumuskan kebijakan dan program kesehatan untuk menurunkan kejadian cedera disebabkan kecelakaan lalu lintas.

Abstract

Tujuan: Penelitian ini untuk mengetahui pemanfaatan Cash Conditional Transfer (CCT) untuk Antenatal Care  (ANC) dalam upaya meningkatkan kesehatan ibu hamil. Metode: Penelitian ini merupakan Tinjauan Sistematik (Systematic Review). Penelitian ini dikumpulkan melalui pencarian dengan menggunakan strategi dan studi yang dikompilasi dari sumber basis data  yang meliputi google schoolar, Proquest dan PubMed dalam rentang waktu 5 (lima) tahun yaitu dari  tahun 2014 hingga  tahun 2018. Menggunakan kata kunci dasar dan kriteria inklusi dan eksklusi  menggunakan Teknik PICO-S (Population Intervention Comparison Outcome), menghasilkan beberapa kata kunci  (1) “Cash Conditional Transfer (CCT)”, (2) “kesehatan ibu hamil” (3) Antenatal Care (ANC).  Dari pencarian ditemukan 384 (tiga ratus delapan puluh empat) studi dan 5 (lima) studi masuk dalam penelitian ini. Hasil: Cash Conditional Transfer (CCT) secara significant memiliki dampak terhadap pemanfaatan ANC dalam meningkatkan kesehatan ibu hamil dengan perawatan bagi ibu hamil dan ibu hamil dengan komplikasi, perawatan ibu post partum, dan melahirkan di fasilitas kesehatan dengan tenaga kesehatan terampil. Program Conditional Cash Transfer (CCT) di Malawi memberikan manfaat bagi ibu hamil untuk mencari perawatan bagi ibu hamil yang komplikasi menurun 27,3%. Program CCT di India yang dikenal dengan nama program Janani Suraksha Yojana (JSY) memberikan peluang kepada ibu hamil dengan sosial ekonomi rendah untuk dapat meningkatkan kunjungan ke pelayanan di fasilitas kesehatan dari 39% menjadi 74% dan berdampak terhadap penurunan kematian neonatal. Selain itu CCT di India juga dengan program JSY dapat meningkatkan kunjungan post partum di fasilitas kesehatan sehingga dapat meningkatkan pemberian asi eksklusif. Di Nepal menunjukan bahwa untuk meningkatkan persalinan ibu hamil oleh tenaga kesehatan terampil  tidak hanya melalui program CCT saja tapi harus disertai dengan investasi infrastruktur untuk daerah-daerah dengan akses kondisi geografis yang tidak memadai. Simpulan: Dari hasil penelitian di beberapa negara, menunjukkan bahwa program CCT berdampak positif pada  pemanfaatan untuk ibu hamil dalam upaya meningkatan kesehatan ibu hamil.

Abstract

Purpose: Program Keluarga Harapan (PKH) is a strategic government program that wants to help the poor meet their health needs. However, there are still PKH beneficiaries who do not understand the health mission of this program. This study intends to explore smoking behavior among PKH families beneficiaries and whether program providers pay attention to promoting healthy living for their beneficiaries. Method: This study uses primary data with cross-sectional design and multiple logistic regression. The number of samples analyzed was 379 households in the Kembangan Region of West Jakarta. Results: Eighty-two percent of PKH recipients were smokers. The four variables related to smoking are low education, low income, smoking psychological dependence, and socially motivated smoking. The psychological and social factors of smoking were among strong predictors and deserve attention in the PKH program. Conclusion: The long-term goal of PKH is to improve the health quality. Smoking reduces the health quality of PKH beneficiaries. The Ministry of Social Affairs needs to coordinate with the Ministry of Health to make this program an entry point for the movement of healthy living in PKH recipient families.

Abstract

The presence of the VClaim application as a form of innovation in industry-based health services 4.0 seeks to create efficiency in submitting claims by the Hospital. However, in its implementation, especially at the Bakti Timah Karimun Hospital, Pending claims still occur which can disrupt the operation of the Hospital. Based on data obtained by researchers, in January-September 2022 there were pending claims with a total INACBGS of IDR 1,537,233,700, - with 1,353 total cases consisting of 7% inpatient care and 3% outpatient care. This study was conducted to assess the effectiveness of the VClaim application in capturing claims that still require confirmation back to the hospital. In addition to improving claim management as well as being used as material for evaluating V Claim applications in submitting claims for JKN participant patients. This research is a non-experimental observational study with qualitative data collection at the Bakti Timah Karimun Hospital. This research uses document review. The occurrence of Pending Claims at RSBT is caused by certain factors both inpatient and outpatient requiring specific improved data to make it easier for verifiers to verify Hospital claims. The VClaim application algorithm in verifying hospital claims against JKN patients is considered effective in measuring the feasibility of claims submitted by FKRTL or hospitals.

Abstract

Tujuan: untuk memberikan gambaran kinerja puskesmas yang menerapkan BLUD, baik kinerja keuangan maupun kinerja non keuangan. Metode: studi ini menggunakan metode scoping review dengan mengumpulkan berbagai jurnal dan artikel terkait kinerja puskesmas yang menerapkan BLUD dan dikelompokkan berdasarkan perspektif keuangan, perspektif pelanggan, perspektif proses bisnis internal, dan perspektif pertumbuhan dan pembelajaran. Pengumpulan artikel dilakukan pada mesin pencarian Google Scholar dari rentang waktu 2018-2023 menggunakan kata kunci “kinerja”, “puskesmas”, dan “BLUD”. Hasil: pencarian sistematis menghasilkan dua belas artikel yang mengulas kinerja puskesmas yang menerapkan BLUD dari perspektif keuangan, perspektif pelanggan, perspektif proses bisnis internal, dan perspektif pertumbuhan pembelajaran secara bersama-sama maupun dari masing-masing perspektif. Kesimpulan: kinerja puskesmas yang menerapkan BLUD masih bervariasi dilihat dari perspektif keuangan, perspektif proses bisnis internal, dan perspektif proses pembelajaran. Sedangkan dari perspektif pelanggan, kinerja puskesmas termasuk dalam kategori baik.
