Dr. Saiful Anwar Malang is a state hospital has done it is job and function, but in 3rd class of pavilion room, the number of patient decrease dramatically. It is concerned with quality of this hospital. To answer this problem, research was done using Quality Function Deployment (QFD). Quality Function Deployment is a tool which design some needs include customers represented as a voice of customer and including some competitions and also groups some activities that usually called affinity graphic ang getting a benchmarking for it is competition. From the result analysis can be showed that main attribute for patience is a accuracy. And from House Of Quality can be found that getting a periodic meeting to evaluate this hospital and also increase a service can be made 20 concept

Abstract

The A3 concept is one of the Toyota tools to propose problem-solving solutions. A3 provides status reports of ongoing activities. This research applies A3 concept to solve reject problem in plastic company assembling process. The reject rate in the assembly process is 15.6%. The implementation stage of A3 concept is identification of problem, targeting, root cause, goal setting, countermeasure, implementation plan, action and follow up. The A3 concept is in line with the Plan-Do-Check-Act (PDCA) process improvement cycle. The results show that using the concept of A3 can reduce the reject Assembly effectively and precisely.

Abstract

Production scheduling is an important activity in manufacturing. Optimal scheduling affects the time of completion of the work. PT. Interpack is a company that produces packaging machines. The job schedule of the product spare part still uses the random priority rules. Use of random rules makes a lot of time delay. Delay causes operator and machine idle. Delay also causes the total time of completion of the work (makespan) and delay the greater.  So it needs to be rescheduled to work on the spare part of product. Job scheduling using the Artificial Immune System (AIS) algorithm. AIS was developed by Farmer et al. The AIS algorithm refers to the human immune system. The stages of the AIS algorithm begin with random initialization, antibody representation and gene classification, clone breeding, selection of donor antibodies, germ-line construction, gene fragment redesign, and ending with diversification of antibodies. AIS algorithm scheduling resulted in a more optimal new schedule. AIS algorithm scheduling generates a future of 6483.91 minutes. Company scheduling generates makespan of 7059.99 minutes. AIS Schedule finishes algorithm 4 days of the due date. AIS algorithm schedule increase machine utility by 1%.

Abstract

Waste has a negative impact on the environment. Hospital medical waste is a hazardous waste for the environment. Environmental performance measurement systems are used to measure the performance of hospitals in handling waste. The measurement of its environmental performance in the hospital is expected to provide improved sewage treatment systems. AHP (Analytical Hierarchy Process) and OMAX (Objactive Matrix) methods are used to design environmental performance systems at hospitals. The data needed is medical and non medical treatment. Data were collected using pairwise comparison questionnaires. The questionnaire data shows the importance of key environmental performance indicators (KEPI). Results of questionnaires processed using Expert Choice. Expert Choice's output produces weight and consistency ratio. Weighted KEPI results are processed by OMAX method. The OMAX method is used to determine the value of environmental performance. There 41 KEPI aspects of the environment. Based on the results of the environmental performance assessment, liquid waste has the highest weight of 0.229.

Abstract

 CV. Wijaya Kusuma is a company producing sanitair. The production process is done manually. Workers work with work postures bent and squatting. Posture work bent and squat reduces the comfort. Posture work bend and squat and potential risks of musculoskeletal disorder. REBA method is suitable to solve the problem. REBA is a method used in job posture analysis. REBA method used to quickly assess the posture of the neck, back, arms, wrists, and feet of a worker. The initial stage of the research is to divide the Nordic Body Map (NBM) questionnaire. NBM aims to know complaints of body parts before and after work. The purpose study is to determine the cause of complaints of musculoskeletal disorders (MSDs) labor. This study provides a solution to the problem of labor MSDs complaints. The results show 6 work postures with "moderate" risk levels, 4 high-risk "work postures". All work postures in the sanitair production process require corrective action posture.

Abstract

Pembangkit Listrik Tenaga Uap (PLTU) PT Indo Pusaka Berau is a PLN work unit that provides electricity to consumers. The problem of the company is the frequent damage to boiler engine components. The company uses corrective maintenance system. On the problem, it is necessary to develop a machine maintenance system with Reliability Centered Maintenance (RCM) approach. The objective of the study was to select a boiler system maintenance action based on the RCM method and component replacement interval to minimize Total Minimum Downtime (TMD). Results of data processing RCM method, component in category Condition Directed (CD) is El Bow component. Components in Time Directed category (TD) are gland seal steam and check valve. Components in the Time Directed category are calculated for component replacement intervals on TMD criteria. The calculation result from component replacement interval was obtained 37 days for gland seal steam component and 58 days for check valve component. The proposed treatment of RCM method can reduce downtime by 11.33% of the company's maintenance methods.

Abstract

Marketing is an important activity of the company. The main purpose of company marketing is to increase profit. CV X is a trading company. The company’s problem is the stable product sales. Companies must determine the right marketing strategy. This study uses SWOT analysis to identify factors and formulate appropriate marketing strategies. The TOPSIS method is used to rank alternative marketing strategies. TOPSIS ranking show best and ideal strategy. Based on SWOT analysis are four marketing strategies alternative. the first priority marketing strategy is to expand the product marketing distribution network with a preference value of 0.497962. The second priority marketing strategy is to focus on one of the NPK variants with a preference value of 0.488088. The priority marketing strategy of three is to improve the quality service with a preference value of 0.465217. And four priority marketing strategy is the use of marketing strategy with the value of preference 0.462249.

Abstract

This article describes the development of an uncapacitated p-hub median problem model. The established model will be applied in determining the location of the hub. Generally in the median p-hub model problem, it is known that the number of hubs built with the total cost minimization function. In this developed model, there is no limit to the number of hubs and the function of the intended destination is how many hubs are built. The model also looks for where the hub location will be built and which nodes are allocated to each hub. By eliminating the limitation of the number of hubs built, the model adds a total total timeout limit. The numerical experiments are dealing with the model. The solution to solve the model using excel solver. So that it will be designed spread sheet excel appropriate model. In numerical experiments, dummy data representing real systems will be used with the aim of shortening computational time. Numerical experiments are performed in several conditions scenarios. Computational results generate the location of hubs and nodes allocated to each hub with various experimental scenarios.

Abstract

Koperasi BMT Syariah East Java is one of sharia-based cooperatives located in the area of Gresik, East Java. BMT cooperatives encountered many obstacles in society such as low awareness of the public in cooperatives, negative impression of the cooperative BMT, the level of customer confidence in the institution to be low, the limitations of market coverage and others. The matrices used in this study are IFE (Internal Factor Evaluation), EFE (External Factor Evaluation), CPM (Competitive Profile Matrix), SWOT, SPACE (Strategic Position and Action Evaluation), BCG (Boston Consulting), IE Matrix ( Internal-External), Grand Strategy Matrix, QSPM (Quantitative Strategic Planning Matrix) and Architecture Strategy. The results of this study are several alternative strategies such as Strategy 3 (S3), which is to improve the image of sharia cooperative BMT, Strategy 1 (S1) Increase participation, Strategy 6 (S6) establish partnership relationship with financial institution, Strategy 10 (S10) Product range and strategy 2 (S2) Expand the market. The next stage is the strategy architecture that is the stage toward the development stage, the stage of business development activities and the stage of maintaining the business. Strategy architecture stage is done by making short, medium and long term plan.
