Dr. Saiful Anwar Malang is a state hospital has done it is job and function, but in 3rd class of pavilion room, the number of patient decrease dramatically. It is concerned with quality of this hospital. To answer this problem, research was done using Quality Function Deployment (QFD). Quality Function Deployment is a tool which design some needs include customers represented as a voice of customer and including some competitions and also groups some activities that usually called affinity graphic ang getting a benchmarking for it is competition. From the result analysis can be showed that main attribute for patience is a accuracy. And from House Of Quality can be found that getting a periodic meeting to evaluate this hospital and also increase a service can be made 20 concept

Abstract

Evalry Konfeksi is a developing home industry in Tulungagung with woman's attire as it main product.Stitching section p lays important role here since most of work activities are centeredat this section. One facility that has a great impact is working chair, because with more ergonomic working chair the employees will be able to reduce boredom and fatigue at particular part of their body.This will eventually be able to minimize working time so that there will be an improvementon work output and productivity.From analysis on the research results upon 20 employees during 7 days of on-site observation, we get unimprovement on average work efliciency for each work element by22.05 %. The average working time of each element using the new designed working chair is in fact shorter if compared to those of using the old designed.This reduction is 5.72 minutes/product unit on average, thus there is an improvement on employees productivity.It can be concluded that using the new designed working chair will be capable in resulting in positive impact on company's output lmprovement.

Abstract

Intermetallic morphology influence on the fluidity of alloys Al-11% Si. From the experiments, fluidity in the alloy Al-11% Si were added 0.6, 0.8, and 1.0% Fe, it was found that the higher the Fe content is added to the values decreasing fluidity. On addition of 1% Fe provides the longest phase of intermetallic 50.2 mm, while the shortest intermetallic phase is given by the addition of 0.6% Fe 21.3 mm. In the alloy Al-11% Si with the addition of Fe are generally higher and higher, would produce intermetallic area fraction increased significantly. On addition of 0.6% Fe intermetallic area fraction of 1.01%, while the addition of 0.8% and 1% Fe Fe each area fraction 1.16% and 2.26%

Abstract

This research aims to find” Food Endurance Development Model Based on Banana with Revitalisation Local Wisdom Value Reinforcement” . This model serve the purpose of basis for formulate public policy and education efforts and advocation public in the field of food to push national food endurance. Approach research that used in this qualitative research with qualitative descriptive design. Subject research is bapeda, development, agriculture official, farmer, elite figure, farmer group at regency Lumajang, Malang, and Blitar. Technique sampling that used snowball sampling. Data collecting method that used documentation, indepth interview, observation participatory, and limited discussion. Research data that got to analyzed with qualitative analysis (content analysis, and domain analysis). Based on research result inferential: 1) found banana production profile unity, distribution, consumption, and local wisdom character at regency Lumajang, Malang, and Blitar, 2) local wisdom character can be made principal focus in the effort develop food endurance based on banana, and 3) several important components and strategic of food endurance development model based on banana: a) local wisdom (foodstuff use reinforcement based on local, woman character, society/religion figure character, food self-supporting village, environment friendly agriculture, agriculture multiculture, and planning based on society), b) local government character (wisdom development prima tani, pilot projecting, capitalization, assistance, and tool of productions-distributions-marketing-consumption), and c) and character BPTP, BBMP, DUDI (pilot development projecting, capitalization, assistance, and system reinforcement productions-distributions-marketing-consumption).

Abstract

In this era, companies to transfer some of their logistical functions to third party logistics (3PL). In this study, a risk evaluation, and mitigation system at the initiation of collaboration with third-party logistics companies for agricultural chemical industry companies were developed. The basic model used in system development is House of Risk combined with SOAR methodology and scenario analysis. This system is used for consideration in decision making by evaluating and giving suggestions on the extent to which the use of 3PL is used to resolve company problems due to risk. The risk evaluation and mitigation system at the initiation of cooperation with a third-party logistics company in the agrochemical industry were successfully developed. The system consists of four processes (risk identification, risk analysis, risk evaluation, and risk response) compiled based on the ISO 31000: 2009 framework

Abstract

This study aims to assess the quality of services by integrating Servqual and the Kano model. In this study, Servqual is used to determine the quality criteria that must be improved. At this method, the quality criteria are based on the gap between customer perceptions and expectations for the services they receive. Meanwhile, the Kano method is used to categorize service attributes that satisfy customer needs. Servqual integration and the Kano model were carried out by synchronizing the Servqual gap value and the Kano model grade. Assessment of service attributes obtained from both methods produces service attributes that will focus on management’s improvement to improve service quality. The results showed that the attribute criteria that must be prioritized were attributes that included attractive and one-dimensional categories, which had weak types at Servqual. Improvements in attributes in attractive and one-dimensional categories will improve service quality and increase customer satisfaction.

Abstract

This study aims to determine the machine maintenance schedule. We use the Simulated Annealing Algorithm. Fitness and reliability functions are functions that are used in the optimization process. Several weighting scenarios are done to see the unity of the function. The results of the scenario produce several alternative schedules. This algorithm is implemented on machines that have more than one sub-machine. This sub-machine is a smaller engine system part. This sub-machine also has one particular function. The results of the study show that the optimal engine maintenance period to use is six periods. There are five scheduling scenarios used in this problem. The resulting schedule can increase the value of reliability and can minimize costs.

Abstract

This article attempts to explore approaches and applications of the location-allocation problems in the supply chain perspective. A review has been done for articles which have relation with the approach of location-allocation problems that includes the drivers of the supply chain such as inventory, transportation, service level, and customer demand. Moreover, the application of location-allocation problems that consider supply chain drivers is also discussed. In this article, a discussion of location-allocation discussions previously investigated by researchers is done through a literature review published in several reputable journal databases. 48 articles successfully collected and reviewed related approaches and applications used by researchers in location-allocation problems in a supply chain perspective. Analysis of the findings of this article can be summarized that approaches of the location-allocation problem in supply chain perspective can be divided into 2 areas: the echelon of the supply chain and the drivers of the supply chain.

Abstract

Bread is food that many consumed in Indonesia. It usually consumed and a substitute for rice. PT. X is an Indonesian company that produces dry bread and wet bread. The company must implement a Good Manufacturing Process (GMP) and Halal Guarantee System (HGS) to ensure the food safety that the company produced. The company is also obliged to pay attention to Occupational Safety and Health in the work environment by applying WISE. This study consists of five staged. The stages include identification of the condition of the company, CPPB inspection, WISE examination, HGS inspection, and proposed improvements in the production process. The results of the study show some elements that have not been fulfilled. These elements include 6 CPPB elements, 9 WISE elements, and 8 HGS elements. The results of the study also provided several studies. Some of the recipients were improvements in the Standart Operational Procedure (SOP) for the receipt and receipt of materials, and the SOP for the receipt and receipt of industrial non-processed materials.

Abstract

As a non-profit organization in education, Senior High School has the responsibility to achieve educational goals. The school is required to have accountability to the public. Performance measurement is needed to determine organizational performance. Performance measurement can also find out the company's success in vision and mission. This study aims to measure performance in schools use an integrated performance measurement system (IPMS). This method becomes the starting point for setting goals and identifying Key Performance Indicators (KPI). The results of IPMS produced eight objectives and  33 KPIs. Objectives performance includes human resources, curriculum, finance, administration, teaching and learning activities, evaluation processes, work programs, and graduation. The results showed that 8 KPIs were red, 4 KPIs were yellow, and 11 KPIs were green. This performance measurement model can also be used in other organizations. This performance measurement model helps assess organizational performance.

Abstract

The raw material is an essential aspect of ensuring smooth production. The selection of raw material suppliers needs to be done to sustain the company's production and development. Wisarka company is an export-oriented furniture company. It uses water hyacinth (Eichhornia crassipes) as the primary raw material. The company is required to maintain and improve performance. The company needs to select the right supplier. This study uses the Multi-Criteria Decision Making (MCDM) the Fuzzy Analytical Hierarchy Process (FAHP) method. It was used for selecting suppliers with criteria like price, quality, flexibility, delivery, warranty, and service. The result of the study show FAHP effective in selecting suppliers.
