Dr. Saiful Anwar Malang is a state hospital has done it is job and function, but in 3rd class of pavilion room, the number of patient decrease dramatically. It is concerned with quality of this hospital. To answer this problem, research was done using Quality Function Deployment (QFD). Quality Function Deployment is a tool which design some needs include customers represented as a voice of customer and including some competitions and also groups some activities that usually called affinity graphic ang getting a benchmarking for it is competition. From the result analysis can be showed that main attribute for patience is a accuracy. And from House Of Quality can be found that getting a periodic meeting to evaluate this hospital and also increase a service can be made 20 concept

Abstract

The PT. ERATEX DJAJA. Tbk. Probolinggo, on the weaving department had often the matedal disturbance and retum flow of the moying material doe to the position of the department isn't suitable, so that needs arrangement the layout and production facilities that emcient and flexibility in the weaving deparhnent to avoid of the backtracking that can added the length of the moving distance ofmaterial, so that will drop down the productivity. Anangemenl and production facilities layout arrangem€nt using CRAFT algorithm method to know the activity of production process from part one to another part, include the moving distance of handling material frequency. Compare the suggestion layout with the fist layout, then can to determine the advantag€ layout by the base method. From the analysis we can know on the suggestion layout have long distance 10,82% conceming fist layout. Backtracking of the material haadling occur 6 time, therc are reducing 15,86% from the fist layout. The moving cost efficiency 2,05% occur fist layout.

Abstract

This article describes the development of an uncapacitated p-hub median problem model. The established model will be applied in determining the location of the hub. Generally in the median p-hub model problem, it is known that the number of hubs built with the total cost minimization function. In this developed model, there is no limit to the number of hubs and the function of the intended destination is how many hubs are built. The model also looks for where the hub location will be built and which nodes are allocated to each hub. By eliminating the limitation of the number of hubs built, the model adds a total total timeout limit. The numerical experiments are dealing with the model. The solution to solve the model using excel solver. So that it will be designed spread sheet excel appropriate model. In numerical experiments, dummy data representing real systems will be used with the aim of shortening computational time. Numerical experiments are performed in several conditions scenarios. Computational results generate the location of hubs and nodes allocated to each hub with various experimental scenarios.

Abstract

Performance measurement system is a critical component of a management system. Unfortunately, the success rate of performance measurement system implementation is relatively low. Even from the implementation categorized as success, only few of them are effective. Dysfunctional behavior is one of the factors that makes the implementaion of a performance measurement system is ineffective. This paper tries to elaborate the dysfunctional behavior of employees in performance measurement system implementation. Three examples of dysfunctional behavior cases are presented. Finally, the paper tries to propose some methods for reducingdysfunctional behavior. The three cases show that the alignment of incentive system to performance (performance related pay scheme) and the fear of unable to meet performance target have driven employees to behave dysfunctionally. The cases also show that gaming and biasing are two forms of dysfunctional behaviour practiced by employees. To reduce dysfunctional behavior the paper proposes three methods, namely: reducing the proportion of incentives tied up to performance, implementing the appropriate KPIs, and utilizing counter measures..

Abstract

Noise is a longitudinal wave which is known as pressure wave. Therefore, noise is expected to disturb the stability of fluid flow. The disturbance is expected to increase the heat transfer of fluid flow. In this research, noise was applied to condenser cooling of air in a refrigeration system. Effect generated is directly measured by amount of heat capable to be released into the air passing condenser, and indirectly by the compressor energy required and coefficient of performance (COP) of refrigeration system. Experimental results indicate that noise with intensity level of 115 dB with frequency around 100–500 Hz at air velocity of 5,3 m / s increases the amount of heat capable to be released into the air which is 10,1%, reduces compressor energy to 12,8% and increases coefficient of performance (COP) by 11,4% compared to flow without noise.

Abstract

PT. Bumi Buana Citra Malang found the existence of problems that is, company find difficulties in degrading the amount of defect product. This matter [is] caused [by] the existence of variation in determining usage of configuration of level at process parameter, like happened variation determination of temperature of furnace, temperature of molten metal, determination of molding time and determination of pressurizing sand. Existence of variation in determination level configuration of the process parameter result incidence of defect product which height affect of expense defect product which must be accounted by company. To overcome the problem, Six-Sigma method was selected. Six Sigma is a logical procedure by following cycle of DMAIC to execute to repair of process by [doing/conducting] improvement and operation of quality result of production. In cycle of DMAIC early by determining the target of and accurate object, conducting measurement to ability of process at company, conducting observation to cause factors incidence problems by using experiment method of Shainin Bhote as supporter method, executing planning of repair according to from result of [at] previous phase and terminated by documenting repair pickings which have successful executed by using process operation of statistic. From applying of method of Six Sigma, company can degrade the amount of defect product which initially 129 unit of per-months become 25 units during period of repair or experience of degradation equal to 80, 6%. To the expense of effect of defect product which initially equal to Rp.12.996.750,00 per-months become equal to Rp.2.518.750,00 or happened thrift equal to 80,6% during period of repair. Beside that by using system of DMAIC in method of Six Sigma will be obtained a determination of configuration of level [at] better process and can improve level of operation of company.

Abstract

The amount of hospital in a row with increasingly of the society level, 50 level hospital is demanded to diectly fix up and increase the servility system. The servility system very significant in competent to satisfy the patient. One of parameter to eveluate good or bad a servicing system is a patient wait time. This research take a sample at this emergency treatment Instalation RSUD Dr. Saiful Anwar Malang. Where the patient that come have to be handled correctly and quickly, because the patient that enter to the Emergency Treatment lnstallation in IRD must wait too long time will cause physical defect even death. There for this research is intended to analyze patient servicing system, determine the sum the health power, and the sum of maximal patient that can be handled y emergency treatment Installation, so got the wait time that is fit with patient aspiration level.

Abstract

Koperasi BMT Syariah East Java is one of sharia-based cooperatives located in the area of Gresik, East Java. BMT cooperatives encountered many obstacles in society such as low awareness of the public in cooperatives, negative impression of the cooperative BMT, the level of customer confidence in the institution to be low, the limitations of market coverage and others. The matrices used in this study are IFE (Internal Factor Evaluation), EFE (External Factor Evaluation), CPM (Competitive Profile Matrix), SWOT, SPACE (Strategic Position and Action Evaluation), BCG (Boston Consulting), IE Matrix ( Internal-External), Grand Strategy Matrix, QSPM (Quantitative Strategic Planning Matrix) and Architecture Strategy. The results of this study are several alternative strategies such as Strategy 3 (S3), which is to improve the image of sharia cooperative BMT, Strategy 1 (S1) Increase participation, Strategy 6 (S6) establish partnership relationship with financial institution, Strategy 10 (S10) Product range and strategy 2 (S2) Expand the market. The next stage is the strategy architecture that is the stage toward the development stage, the stage of business development activities and the stage of maintaining the business. Strategy architecture stage is done by making short, medium and long term plan.

Abstract

This research focuses on the development of Differential Evolution(DE) algorithmto solve m-machine flow shopscheduling problems with respect to both makespan and total flow time. Development of DE algorithm is done by modifyingthe adaptive parameter determination procedure in order to change the value of adaptive parameters in each generation,adding local search strategy to the algorithm in order to improve the quality of the resulting solutions, as ewell as modifyingthe crossover in order to reduce computation time. The result indicates that the proposed DE algorithm has proven to bebetter than the original DE algorithm, Genetic Algorithm (GA), and for certain cases it also out performs Multi-ObjectiveAnt Colony System Algorithm (MOCSA).

Abstract

Bread is food that many consumed in Indonesia. It usually consumed and a substitute for rice. PT. X is an Indonesian company that produces dry bread and wet bread. The company must implement a Good Manufacturing Process (GMP) and Halal Guarantee System (HGS) to ensure the food safety that the company produced. The company is also obliged to pay attention to Occupational Safety and Health in the work environment by applying WISE. This study consists of five staged. The stages include identification of the condition of the company, CPPB inspection, WISE examination, HGS inspection, and proposed improvements in the production process. The results of the study show some elements that have not been fulfilled. These elements include 6 CPPB elements, 9 WISE elements, and 8 HGS elements. The results of the study also provided several studies. Some of the recipients were improvements in the Standart Operational Procedure (SOP) for the receipt and receipt of materials, and the SOP for the receipt and receipt of industrial non-processed materials.

Abstract

The measurement of productivity employer was crucial for the development of a organization and company. This was influenced for the company's working. One of the electricity's source company wanted to know how the achievements of employer's productivity are. This research is focused to maintaining department which concerned with three level division, they were specialist, supervisor, staff or technician crew. This research have done by using Will, can, May approach as the way to measure of the productivity's measurement. To conduct this research, it must be known the criterion of each elements, it used brainstorming with the company, and also the literature's reference. Another method which was used in this research is the priority each Will, Can, and May elements used Analytical Hierarchy Process (AHP) and fuzzy method to found single value which can form the productivity each employer as the achievement every elements of Will, can, and May. The result of this research showed that the productivity's achievement in middle and high scale from 43 employers who have measured their productivity. From the result of productivity's approach which have done was to know how are incentives which have got as the result of productivity each employer, and the result of incentive giving which have formulated gave the higher incentive's value than before. The result of giving more motivate the productivity of employer and the incentive's measurement didn't decrease the company from the benefit of allocation which is old incentive's giving and it is not look the stagnancy's nominal.
