Abstract

This study aims to analyze occupational accident risks in the production process at Barokah Furniture Pasuruan using the Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA) methods. The furniture industry involves various potential hazards in its production process, such as the use of cutting tools, dust exposure, and high-risk manual work techniques, necessitating a systematic evaluation to minimize workplace accidents. The FMEA method is used to identify and prioritize risks based on the Risk Priority Number (RPN), calculated from a combination of severity, occurrence, and detection. The analysis results identified 20 failure modes, with the three primary risks having the highest RPN: cuts from saw blades (RPN=250), respiratory issues due to excessive dust (RPN=196), and hand injuries from the use of manual planers and machine knives (RPN=180). Furthermore, a Fault Tree Analysis (FTA) was conducted to map the root causes of each priority failure mode. Based on the analysis, it was found that the main causes of workplace accidents at Barokah Furniture include the absence of safe working SOPs, lack of personal protective equipment (PPE), risky manual work techniques, and minimal control over dust hazards. Recommended improvements include the creation of written SOPs, provision of auxiliary tools and PPE, installation of dust extraction systems, and safety training for all employees. This research is expected to serve as a reference for the company to continuously improve its occupational safety and health systems.

Abstract

This study aims to analyze the organizational strategy, business model, and risk management implementation at PT Totalindo Eka Persada Tbk, a national construction services company. The research is conducted conceptually using a qualitative, descriptive, and analytical approach, employing PESTEL, IFAS, EFAS, SFAS, and the Business Model Canvas (BMC) to map internal–external factors and formulate priority strategies integrated with a risk management system based on ISO 31000:2018. The analysis shows that political and economic factors are the primary determinants of the continuity of construction projects. Internally, the company has strengths in reputation, human resource competence, and experience in large-scale projects, but faces weaknesses in terms of project diversification and liquidity. The SFAS results indicate that the company’s main strategies include expansion of project portfolios, technological innovation, operational digitalization, and strengthening of corporate governance (GCG). Totalindo’s project-based EPC business model emphasizes efficiency, quality, innovation, and sustainability, and also serves as the basis for performance measurement and managerial compensation through a Key Performance Indicator (KPI) system. The integration of organizational strategy and risk management has been proven to strengthen the company’s competitiveness and resilience against external changes, economic fluctuations, and operational and reputational risks. Strategic recommendations include project diversification, digital transformation, human resource development, and the implementation of sustainable, green building–based projects.

Abstract

This study aims to analyze the effectiveness of implementing Enterprise Risk Management (ERM) and operational risk mitigation strategies in securities companies in the digital era. Amid the high dependence on technology, disruptions to trading systems have become a serious threat to business stability. The research method uses a descriptive qualitative approach with primary data obtained through interviews and observations, as well as secondary data in the form of financial reports and capital market statistics. Risk analysis is carried out using a likelihood–impact matrix based on Financial Services Authority regulations. The results show that operational risks in the form of technical disruptions (glitches) in online trading applications and reputation risk occupy the highest risk level (high risk). System reliability, including the Sharia Online Trading System (SOTS), is proven to be a fundamental factor in maintaining investor confidence. In addition, disciplined risk management implementation contributes positively to net profit growth and long-term business sustainability. The study concludes that mitigation priorities must be focused on strengthening information technology infrastructure and enforcing credit risk control procedures in order to minimize financial losses.

Abstract

This study aims to analyze the implementation of risk management in rice milling businesses in Karangwuluh Village. The research focuses on three main aspects: risk identification, risk evaluation and measurement, and risk management. A quantitative descriptive method was employed using descriptive statistical analysis based on questionnaire data from four active rice mill owners. The findings reveal that the overall implementation of risk management is categorized as high to very high. The average score for risk identification (4.50) and risk evaluation and measurement (4.475) indicates that business owners have strong capabilities in recognizing and assessing potential risks. The average score for risk management (4.00) demonstrates that respondents have applied mitigation strategies such as regular machine maintenance, supplier diversification, and post-incident evaluation. However, aspects related to risk documentation and financial management need improvement, particularly in establishing reserve funds and utilizing business insurance. The results of this study are expected to enhance the awareness and capability of rice mill entrepreneurs in managing risks effectively and contribute to the academic development of risk management in rural agricultural sectors.
