Abstract

Introduction: Cognitive function, which is also known as the higher cortical function, is the most important aspect in determining the independence of patient. In patients with intracranial tumors, the presence of a tumor can directly affect cognitive function which affects quality of life. Quality of life is an important component in clinical neurooncology, especially intracranial tumors in determining the independence and choice of therapeutic approach. However, research related to the relationship between cognitive function and quality of life is still minimal. Method: This study used a cross-sectional method using an observational analytic approach in patients with intracranial tumors. The study subjects were 30 intracranial tumor patients and the MoCA-INA and Barthel Index were assessed. Results: Of the 30 research subjects, the majority were female (60.0%), average age 49.8 years, university education (50.0%), and Javanese ethnicity (53.3%). Headache is the most common manifestation (83.3%). Most of the patients had tumor involvement of one lobe (63.3%), and located in the parietal lobe (63.3%). There is a significant relationship between cognitive function and quality of life of intracranial tumor patients (p = 0.012). Conclusion: There is a relationship between cognitive function and quality of life of intracranial tumor patients.

Abstract

Introduction: Chronic kidney disease (CKD) is one of the most serious health problems still faced globally. Cardiovascular complications are a major cause of morbidity and mortality in patients with chronic kidney disease. In general, cardiomegaly is found in patients with uremia which causes dysregulation of calcium and phosphate levels and increases the development of cardiovascular disease in hemodialysis patients. The aim of this study was to analyze the relationship between stages of chronic kidney disease with cardiomegaly imaging in posteroanterior  chest x-ray at H. Adam Malik Hospital Medan. Method: This study was an observational analytic study with a cross-sectional design on 78 CKD patients who underwent posteroanterior chest x-ray at H. Adam Malik Medan General Hospital from January to December 2022. Results: Of the 78 patients studied, the subjects were female (56.4%) with a mean age of 50.94 ± 12.80 years. The mean weight and BMI of the subjects were 70.49 ± 9.01 and 24.80 ± 2.82kg/m2, respectively. The mean creatinine and GFR were 7.91 ± 4.22 mg/dl and 16.08 ± 15.22 mL/min/1.73m2, respectively. The majority of subjects had creatinine >2.5 mg/dl in as many as 72 subjects (92.3%) and GFR <60 ml/min/1.73m2 as many as 62 subjects (79.4%). Most of the subjects were in CKD stage 5, as many as 59 people (75.6%). There was a positive and significant correlation between the stages of chronic kidney disease and cardiomegaly imaging in posteroanterior chest x ray with p-value <0.001. Subjects with GFR < 60 mL/min/1.73m2 and overweight BMI status (25.0 - 29.9 kg/m2) had a greater risk factor for cardiomegaly. Conclusion: There is a positive and significant correlation between the stages of chronic kidney disease with cardiomegaly imaging in posteroanterior chest x-ray at H.Adam Malik General Hospital Medan with p value <0.001.

Abstract

Introduction: Indonesia still has a high burden for stroke, and stroke became the number one cause of mortality, in which the condition caused 328,5 thousand deaths (21,2% of death from all cause) in 2012 according to WHO. Until now, there are multiple parameters that can be used to determine the severity of an acute ischemic stroke event, one of the laboratory parameters being serum fibrinogen level. The aim of this study was to find the relationship between serum fibrinogen level and the severity of acute ischemic stroke. Method: The study used a cross-sectional design from acute ischemic stroke patients in the inpatient wards of H. Adam Malik Medan Hospital and network hospital. We used contingency coefficient correlation analysis test to determine the relationship between serum fibrinogen level and the severity of acute ischemic stroke. This study involved 40 acute ischemic stroke patients. Results: The results of statistical analysis found a significant positive correlation between serum fibrinogen level and acute ischemic stroke severity with weak correlation strength (p=0.028; r= 0.328) with the median NIHSS score of the subject 6,5 and median serum fibrinogen level being 320,9mg/dL (87 – 881mg/dL). Conclusion: There was a significant relation between serum fibrinogen level and acute ischemic stroke severity. The result of this study proved that a higher serum fibrinogen level is associated with increased stroke severity.

Abstract

Introduction: The PURSUIT score is a practical risk score for patients with acute coronary syndromes to assess prognosis and risk stratification. Prognosis and revascularization options in patients with multivessel disease can be determined based on the severity of coronary lesions based on angiography. However, only a few studies have investigated the association of this score with coronary lesion severity. This research is aim to evaluate correlation between PURSUIT score and severity of coronary artery lesions assessed by SYNTAX score in patients with NSTEMI at H.Adam Malik General Hospital Medan. Method: This research was a cross-sectional study in which the sample was taken by consecutive sampling at H. Adam Malik General Hospital in Medan on 70 patients diagnosed with NSTEMI. The hypothesis was evaluated using Pearson's correlation test for normally distributed numerical data or Spearman's correlation test for abnormally distributed data. The p value of 0.05 indicates statistical significance. Results: Mean PURSUIT score in this research was 11.0 + 3.31, while mean SYNTAX score was 25.6 + 10.8. A positive correlation was found between the PURSUIT score and the SYNTAX score with a low strength level (r = 0.258). Conclusion: The PURSUIT score has a weak correlation in predicting the complexity of coronary angiography among NSTEMI patients

Abstract

Introduction: Diabetes Mellitus Type 2 (DM T2) is a major health problem, especially in people in developing countries. DM T2 is the most frequently reported comorbid disease in patients with COVID-19. The relationship between DM T2 and poorer outcome in COVID-19 infection is thought to be because hyperglycemia and chronic inflammation impair control of viremia and inflammation by weakening the immune response. This research aim to determine the effect of glycemic parameters on the mortality of DM T2 patients with COVID-19. Method: This research is a descriptive analytic study with a retrospective design which was carried out at the H. Adam Malik General Hospital in Medan. The data obtained was analyzed statistically and said to be statistically significant if p <0.05. Results: Based on the results of the study, there were significant differences in the values of inflammatory markers such as leukocytes, neutrophils, NLR, PCT, LDH, D-dimer, ferritin, SGOT, urea and creatinine in the severity of COVID-19 patients and there were significant differences in the mean fasting blood sugar, blood sugar 2PP, in moderate, severe and critical COVID-19 patients. Conclusion: Glycemic parameters in patients who died were worse than patients who recovered, although the statistical results were not significantly different.
