Abstract

Pregnancy, particularly during the third trimester, is commonly associated with lower-limb edema due to increased pressure on the pelvic veins and inferior vena cava from the enlarging uterus. Foot edema in late pregnancy results from this venous compression, leading to elevated hydrostatic pressure and fluid accumulation in the interstitial spaces of the feet and ankles. Management of foot edema can be performed non-pharmacologically, one approach being a combination of foot massage and leg elevation. The objective of this study was to determine the effect of the combination of foot massage and leg elevation on foot edema in third-trimester pregnant women. This research employed a quasi-experimental study design with a two-group pretest-posttest design and a control group. The sample was 30 respondents. The sampling technique used was purposive sampling. The instruments used were the SOP and the pitting scale observation sheet. Data analysis using the Wilcoxon test showed a p-value < 0.001, indicating a statistically significant reduction in foot edema before and after the intervention. Additionally, the Mann-Whitney test yielded a p-value of 0.003, confirming a statistically significant difference in posttest results between the intervention and control groups. The combination of foot massage and leg elevation significantly reduced foot edema in pregnant women in the third trimester. Clinically, this combination can be applied as a simple and effective non-pharmacological intervention to reduce physiological foot edema during pregnancy.

Abstract

Hebephrenic schizophrenia presents with symptoms such as disorganized thinking, emotional disturbances, and behavioral issues that often lead to social isolation. Clients experiencing social isolation demonstrate impaired interpersonal relationships and a tendency to withdraw from social settings. Acquaintance therapy is a psychiatric nursing intervention conducted gradually to enhance the client's social interaction capabilities. This study aims to determine the effectiveness of acquaintance therapy in improving interaction skills in a client with social isolation at Garuda Ward, Radjiman Wediodiningrat Hospital, Lawang, Malang. Using a case study design, this research involved a 30-year-old male client diagnosed with hebephrenic schizophrenia and social isolation. The intervention was conducted over six consecutive days, lasting 15 to 30 minutes each. Data collection included observation, interviews, and nursing documentation. The study revealed a significant improvement in the client interaction skills after the intervention, with symptoms of social isolation reducing from 13 to 2. The client could introduce himself, converse with others, and participate in group activities. These findings indicate that acquaintance therapy is effective in enhancing social interaction in clients with social isolation and can be considered a recommended non-pharmacological approach in psychiatric nursing practice.

Abstract

School-age children are susceptible to disease and need health supervision. Children who are sick have a high dependence on others in terms of self-care. Children who are sick and unable to fulfill their self-care will experience self-care deficit problems. One component of personal hygiene often forgotten when self-care during illness oral hygiene is. This study aims to determine how health education using animated video media on oral hygiene can improve family and patient knowledge of self-care, especially oral hygiene. This research is a descriptive, quantitative study that uses a case study approach. The research design was a one-group pre-test-post-test design, in which a knowledge questionnaire was administered before and after health education. There are differences in knowledge scores before and after health education is provided to both families and patients, with increased knowledge of oral hygiene. Evaluation of oral hygiene using the OHI-S score shows a decrease in score, indicating improvement from poor to moderate, and is performed 4 times every 24 hours until discharge. Providing oral hygiene education can increase parents' and patients' knowledge and interest in self-care.

Abstract

Lower extremity edema is a common complication in patients with stage 5 chronic kidney disease (CKD) due to fluid imbalance. Non-pharmacological therapies such as ankle pump exercises and 30°-foot elevation can be effective alternative interventions to reduce edema. This study aims to determine the effectiveness of combining ankle pump exercises and 30°-foot elevation in reducing the degree of edema in CKD patients. This research employed a quantitative descriptive case study method involving one patient with stage 5 CKD. The intervention consisted of ankle pump exercises and 30°-foot elevation conducted over three consecutive days. Each ankle pump session lasted 1 minute per position, and foot elevation was maintained for 5–10 minutes per session. Edema assessment was conducted pre- and post-intervention using the Grading Pitting Edema scale. The study showed no reduction in the edema grade from day one to day three. However, a decrease in edema depth was observed on the third day of the intervention, indicating that no significant reduction in the overall edema grade occurred. The application of ankle pump exercises and 30°-foot elevation over three days did not show a clinically significant reduction in edema. However, a positive effect began to emerge on the third day, with a 1 mm decrease in edema depth. The combination therapy has the potential to reduce lower extremity edema in CKD patients, but it requires consistent application and optimal intensity to achieve significant outcomes. This intervention may serve as a safe, non-invasive, and practical non-pharmacological option in evidence-based nursing care.

Abstract

A disaster-response software system has been developed to minimize the risk of missing patients/injured individuals, infants, and children, and to track them during disasters. This study investigates the software's contributions to field use and healthcare personnel's triage experiences. Software has been developed based on contributions of 10 expert senior managers and assessed via Lawshe's technique; the Content Validity Index (CVI) is 0.988. Subsequently, 25 disaster-experienced healthcare workers from Istanbul's 112 Emergency Health Services station evaluated the system through semi-structured, in-depth interviews. Collected qualitative data were coded and analyzed using content analysis in MAXQDA 24. The research is organized into two themes—disaster software system and triage experiences—with subcodes. Findings show that triage depends not only on clinical knowledge but also on environmental conditions, social response, intra-team communication, and psychological factors. The disaster software supports triage by facilitating identification, patient tracking, acceleration of interventions, and data security. Integrated modules—such as QR codes, fingerprint, photo, and location—reduce healthcare personnel's workload and improve data accuracy. However, challenges include inadequate infrastructure, manual data entry burdens, mobile device incompatibility, and a lack of legal regulation. Suggested improvements by participants include integration with MERNİS (Turkey’s central population registry), voice command capability, barcode technology, and DNA-based modules. Overall, digital solutions can critically support triage in disaster settings operating within multidimensional dynamics.

Abstract

Chronic Kidney Disease (CKD) is a significant health problem with increasing prevalence, characterized by a progressive decline in kidney function resulting in fluid accumulation in the body. One common clinical manifestation is edema in the lower extremities. Ankle pump exercises and foot massage are non-pharmacological interventions that can help reduce edema by improving venous and lymphatic circulation. This study aims to analyze the implementation of nursing care in relation to the effect of ankle pump exercise and foot massage on reducing leg edema in patients with chronic kidney disease (CKD). This study used a case study design with a nursing care approach in a CKD patient with edema in the Anturium Ward of Dr. Soebandi Regional General Hospital, Jember. Ankle pump exercises were performed for 10 minutes, followed by a 15-minute foot massage, which was repeated for three consecutive days of treatment. The degree of edema was evaluated using the edema pitting scale before and after the intervention. The results showed a decrease in edema from +2 to +1 (1 mm in the left leg) and from +1 to 0 (2 mm in the right leg). There was also a decrease in ankle circumference of 20 cm and instep of 25.5 cm on the right leg, and 19 cm at the ankle and 23.7 cm at the left leg. Furthermore, muscle strength increased from 2 cm to 5 cm in the left leg and from 1 cm to 4 cm in the right leg. Ankle pump exercises and foot massage effectively reduce edema in CKD patients. This therapy can be used as an independent nursing intervention to enhance the quality of life for patients with CKD who experience edema.

Abstract

Anxiety during pregnancy is often intensified by the anticipation of labor and delivery, which can be overwhelming due to a lack of prior experience. The consequences of untreated maternal anxiety extend beyond the mother, potentially affecting fetal growth and long-term child development. This study aims to determine the effect of classical Lullaby music therapy on anxiety in primigravida pregnant women facing childbirth. The research design employed a quasi-experimental pretest-posttest control group design. The total sample in the experimental group was n = 30, and in the control group, n = 30, using a purposive sampling technique. The population in this study consisted of all primigravida pregnant women who visited the antenatal care clinic at the Sananwetan Health Center in March and April 2025. Based on the T-test, the mean data on the pretest is 39.40, and the posttest is 32.43. There is a decrease in the score of 6.967. The significance value (Sig. 2-tailed) is 0.000, which is <0.05, so it can be concluded that there is a significant influence. The test results show that H0 is rejected and H1 is accepted, which means that classical lullaby music therapy is more effective in reducing anxiety levels in primigravida mothers compared to the control group. Therefore, it can be concluded that classical lullaby music has a positive effect on primigravida mothers in facing childbirth. This type of music can be used as a therapeutic option for pregnant women experiencing anxiety.

Abstract

Anxiety is a psychological problem often experienced by preoperative patients, especially in cases of breast cancer requiring major surgery such as Modified Radical Mastectomy (MRM). This condition can increase physiological stress, disrupt hemodynamic stability, slow down the healing process, and even affect prognosis. Non-pharmacological spiritual interventions, such as dzikir, are believed to help patients manage anxiety. This study aims to analyze the effect of dzikir on anxiety levels in preoperative MRM patients at Sultan Agung Islamic Hospital in Semarang. A quantitative study with a one-group pre-test post-test design was conducted on 30 patients who met the inclusion criteria. Anxiety levels were measured using the Depression Anxiety Stress Scale (DASS-42) questionnaire before and after the intervention. Data analysis used a paired t-test with a significance level of 0.05. Patients' anxiety levels before the intervention were in the moderate to severe category, with an average score of 26.4. After receiving dzikir istighfar therapy twice daily over an 8-hour observation period, the average anxiety score decreased to 15.2. The paired t-test showed a significant difference (p = 0.002). Dzikir therapy had a significant effect in reducing the anxiety levels of patients prior to MRM surgery. Dzikir can be used as a complementary intervention in perioperative nursing care, especially in hospitals based on Islamic values.

Abstract

Burnout syndrome is a psychological condition characterized by emotional exhaustion, depersonalization, and reduced personal accomplishment, which can affect the quality of nursing care. Nurses in surgical inpatient wards are at high risk of experiencing burnout due to the high demands of patient care and heavy workload. This study aimed to analyze the relationship between workload level and the incidence of burnout syndrome among nurses in the surgical inpatient ward of Sultan Agung Islamic Hospital, Semarang. This research employed a quantitative method with a cross-sectional design. The study sample consisted of 118 nurses selected using a total sampling technique. The instruments used were the Nursalam workload questionnaire and the Maslach Burnout Inventory (MBI) to assess burnout syndrome. Data analysis was conducted using the Chi-Square test at a significance level of p < 0.000. The results showed that most respondents had a moderate workload (87.3%) and experienced a high level of burnout (66.9%). Statistical analysis indicated a significant relationship between workload level and the incidence of burnout syndrome (p = 0.000). Nurses with higher workloads tended to experience more severe burnout than those with lighter workloads. These findings emphasize that workload is an important factor influencing burnout among nurses. Therefore, hospital management is advised to implement workload management strategies, increase the number of nursing staff, and provide psychological support programs to reduce the risk of burnout and improve nurses’ well-being and performance.

Abstract

Low Back Pain (LBP) affected 619 million people globally in 2020, and the number is projected to increase to 843 million by 2050. Among nurses, the prevalence of LBP reaches 72%, with significant risk factors including long working hours, poor posture, and heavy workloads. This condition negatively impacts nurses' well-being, productivity, and the quality of healthcare services. Physiotherapy interventions, particularly core stability exercises, have been proposed as a practical approach to reduce pain and improve functional mobility. This study aimed to examine the effect of core stability exercise on pain intensity and flexibility in nurses with non-specific LBP. A proper experimental study with a pre–post test control group design was conducted. A total of 47 nurses who met the inclusion criteria were recruited using simple random sampling. Pain intensity and flexibility were measured before and after the intervention. Data were analyzed using the paired sample t-test. Core stability exercise significantly reduced pain intensity (p = 0.000, <0.05) and significantly improved flexibility (p = 0.000, <0.05). These findings indicate that the intervention positively impacted musculoskeletal health among nurses with non-specific LBP. Core stability exercise is an effective intervention for reducing pain and enhancing flexibility in nurses with non-specific LBP. Activating core muscles through exercise promotes vasodilation, improves blood circulation, and increases oxygen and nutrient delivery to myofascial tissue, producing analgesic effects. In addition, coordinated muscle contractions enhance lumbar stability, reduce intervertebral disc pressure, and minimize muscle tension, improving functional flexibility.
