Abstract

Introduction: Non-arteritic Ischemic Optic Neuropathy is the most common type of ischemic optic neuropathy. Nearly half of NAION patients presented with 20/30 or better visual acuity. Central vision is often preserved relative to the visual field loss. Macular edema in NAION patient is rare, and its occurrence may confuse to other disease and lead to unnecessary treatment. Case Report: We identified three NAION patients presented with macular edema. The patients' age was above 40 and had diabetes mellitus (DM). Our patients had clinical findings consistent with NAION with poor visual acuity at the early presentation. Optical coherence tomography examination was performed, and all patients had prominent subretinal fluid. Intravitreal anti-VEGF injection was done on our first patient and no treatment was given to our second and third patient. All patients showed a significant improvement of macular edema. Discussion: Macular edema is a rare clinical manifestation that may contribute a visual deterioration in NAION. The incidence of macular edema in NAION patients is difficult to measure since OCT of macula is not routinely performed. In our report, we encountered three cases of NAION complicated by macular edema that underwent a different approach in therapy. Spontaneous regression of subretinal fluid was seen during observation without any treatment given. Our patients achieved a stable visual acuity and visual field defect. This finding is under the literature that spontaneous recovery will be observed during the natural course of acute NAION. Conclusion: Macular edema is an unusual but self-limiting presentation of NAION. Therefore, a comprehensive examination is needed to prevent any unnecessary treatment.

Abstract

Introduction: Diabetic Macular Edema is a diffuse thickening in macula which can be found in various severity of Diabetic Retinopathy. There are issue about relationship between Diabetic Macular Edema and albuminuria caused by chronic renal failure. The aim of this study is to know and compare urine albumin creatinine ratio among Diabetic Retinopathy patients with and without Diabetic Macular Edema in Moh. Hoesin Hospital. Methods: Cross sectional study with 25 sample was conducted. Diabetic Retinopathy and Diabetic Macular Edema was classified base on Early Treatment Of Diabetic Retinopathy Study (ETDRS) criteria. T-test, odd ratio and multiple logistic regretion analysis was used to analysed sociodemography characteristic (age and gender), clinical characteristic (duration of DM, hipertension, treatment, body mass indeks and antioksidan consumption), ophtalmology characteristic (visus, anterior segment anomaly and posterior segment/ severity of Diabetic Retinopathy), laboratory characteristic (HbA1c, ureum, creatinine, urine albumin creatinine ratio, and lipid profile). Result: Urine albumin creatinine ratio mean (2146.77 ± 3796.19) in Diabetic Macular Edema and (49.0 ± 45.35) in non-Diabetic Macular Edema; cutoff point 62.00 mg/dL. Odd ratio adjusted for urine albumin creatinine ratio = 18,8. In this research, risk factors which has significantly were urine albumin creatinine ratio (p=0.047) and High-Density Lipoprotein/HDL (p=0.028) with odd ratio 8.571 and 6.67 respectively. Urine albumin creatinine ratio showed significantly high Mann whitney analysis 0.02 (p<0.005). Conclusion: Urine albumin creatinine ratio in Diabetic Retinopathy with Diabetic Macular Edema was higher than without Diabetic Macular Edema. Urine albumin creatinine ratio and High Density Lipoprotein (HDL) are the two important risk factors associated with Diabetic Macular Edema.

Abstract

Introduction : Macular hole (MH) can cause severe visual disturbance, but remarkable progress has been achieved in surgical treatment for eyes with this condition. Vitrectomy with internal limiting membrane (ILM) peeling allows a very high success rate for MH closure (approaching 90%). To compare characteristics patients with closed and unclosed macular hole after pars plana vitrectomy (PPV) surgery and internal limiting membran (ILM) peeling. Method : This was retrospective study which data was obtain from patient’s medical records who underwent PPV and ILM peeling since July 1st to December 31th 2018. Results : 27 eyes from 25 patients had MH surgery. 20 eyes (74%) had closed MH and 7 (26%) eyes unclosed MH after surgery. HFF value before surgery was 1,34 ± 0,90 and MHI was 1,09 ± 0,81 in closed MH. Meanwhile in unclosed MH, HFF value before surgery 0,53 ± 0,12 and MHI was 0,75 ±0,10. MHI ≥0,5 and HFF value ≥ 0,9 had a good prognostic factor. Conclusion : Despite of good prognostic factor from OCT measurement and achieve anatomical success, foveal microstructure also important in visual recovery after MH surgery.

Abstract

Introduction: Central retinal vein occlusion (CRVO) and branch retinal vein occlusion (BRVO) caused sudden visual decrease that most often treatable. This paper aims to describe clinical characteristic and outcome using bevacizumab for macular edema caused by retinal vein occlusion in real-life practice. Methods: This retrospective study included 91 treatment-naïve eyes with macular edema due to CRVO (55 eyes) and BRVO (36 eyes), who were treated with intravitreal bevacizumab (IVB) in pro re nata (PRN) regimen. Best corrected visual acuity (BCVA) and central macular thickness (CMT) before and after treatment were evaluated. Result: The mean age of patients was 60.3 + 11.2 years for CRVO and 55.7 + 8.2 years for BRVO. The mean baseline BCVA in the CRVO group was 1.41 + 0.55 logMAR. There was statistically significant improvement in BCVA after intravitreal bevacizumab compared to baseline (p < 0.001) in CRVO and BRVO group. Twenty six (47.3%) eyes with CRVO had BCVA > 1.0 logMAR (Snellen 20/200) at the last follow-up. In the BRVO group, the mean baseline BCVA was 0.93 + 0.48 logMAR. At the end of the follow up, 19 eyes (52.8%) with BRVO had BCVA > 0.3 logMAR (Snellen 20/40). There was also statistically significant improvement in CMT between all time points and baseline (p < 0.001) in both groups. At the end of the follow up, 26 (47.3%) eyes with CRVO and 25 eyes (69.4%) in BRVO group presented resolution of macular edema (CMT < 300). Conclusion: Intravitreal bevacizumab resulted in significant anatomical and functional improvement in macular edema associated with CRVO and BRVO, although outcome in CRVO group was sub-optimal.

Abstract

Introduction: The prevalence of Diabetic Retinopathy (DR) and Vision Threatening Diabetic Retinopathy (VTDR) in Greater Bandung on 2017 was 24,7% and 9% respectively. It mean almost 30% DR fall into VTDR, although it could be prevented. Fundus examination in primary level of prevention is important to prevent VTDR, and potential barriers on it included knowledge, attitude, practice and related factors should be overcome. The objective of this research was to assess knowledge, attitude, practice (KAP) and related factors on diabetic fundus examination among GPs in Puskesmas of Bandung City. Method: This was a population based cross sectional study conducted on April-June 2020, involving 115 GPs at 62 Puskesmas of Bandung City who fulfilled the eligibility criteria by purposive sampling. It used self administered KAP questionnaire on diabetic fundus examination. The data were presented in descriptive and analytical explanation using Chi-square with p value of 0,05 as statistical significant result . Result: The mean of knowledge and attitude on fundus examination were 70,67 (95% CI ± 3,26), 72,87 (95% CI ± 2,61) respectly, included in good criteria, but Practice was poor for 95,65% participants. Poor practice was associated significantly with no availability of direct ophthalmoscope (p= 0.00038) that can not be accessed by 98 of participants (85%).Other GPs with ophthalmoscope access still did not conduct fundus examination because of no mydriasis to dilate pupil. Conclusion: Knowledge and attitude on fundus examination among GPs was good, but poor for practice. It was associated with direct ophthalmoscope and mydriasis availability. It need advocation approach to overcome.

Abstract

Introduction: Current study aimed to evaluate efficacy of intravitreal Bevacizumab in Diabetic macular edema, and to identify their ocular and systemic complications if any. Method: It is a prospective, interventional study with 68 patients in 82 eyes with Diabetic macular edema. All treated by 3 injection of intravitreal bevacizumab with 1 month interval. Visual acuity, macular edema, and complications evaluated at every month upto 6th months. Ranibizumab was offered if the patient is a non-respondent to Bevacizumab. Results: Majority age group of 61-70 years with a mean age of 59 ± 6.72 years. The mean duration of diabetes was 11.68 ± 7.2 years. The mean baseline BCVA and CRT are 0.64 ± 0.28 Log MAR units and 436.99 ± 135.10 μm. After 3 injections, BCVA values are 0.48±0.27 (p< 0.01)), 0.36±0.24 (p<0.01)), and 0.27±0.24 (p<0.01), 0.23±0.27 Log MAR (p<0.01)) at 1month, 2 months, and 3 months, and 6 months follow-up respectively. CMT levels are 315.79±124.60 μ at 1 month, after 3rd IVA and this significant change (p<0.01) followed with subsequent follow-up with mean CRT of 296.04±122.97 μ (p< 0.01) at 6th month. The BCVA improved ≥ 2 Snellen lines in 69.5% and 78% cases, resolution of CMT in 56% and 69.5% cases at 3 months and 6 months respectively. At the end of the 3rd month, 46 eyes macular edema completely resolved with 3 injections of IVA, 23 eyes persistent macular edema present, 13 eyes refractory to bevacizumab injections. Persistent & Refractory macular edema eyes (36) switched to Ranibizumab injections.In persistent macular edema, significant resolution (p≤0.01) of macular thickness (370.52 ± 71.43 µm vs. 341.08 ± 122.75 µm) without (p=0.09) improvement in visual acuity (0.45 ± 0.20 vs. 0.34 ± 0.23) was observed post Ranibizumab injections. In Refractory macular edema, thickness of cases no significant (497.76 ± 161.07µm vs 407.84 ± 169.64 µm) improvement seen after Ranibizumab injections(p=0.1). Recurrence of macular edema seen in 10.9%. Subconjunctival haemorrhage seen in 10.9%-12.1% cases, raised IOP in 2.4%-3.6% cases at post injection day 1. No other ocular and systemic complications were observed during follow-up. Conclusions: Intravitreal bevacizumab is effective in treatment of diabetic macular edema but therapeutic effect is temporary and repeat treatment is needed. It does not show any potential drug related ocular and systemic side effects, hence it is safe and economical therapeutic agent

Abstract

Introduction: Multifocal electroretinography (ERG) is an examination which measures retinal electrical activity as a response to lighting stimulus and allows simultaneous recording in many topographic locations. Various electrodes have been developed to balance examination accuracy and also patients’ comfort. The objective of this study is to establish normal values of multifocal ERG and to compare the values and the comfort level using Jet, Dencott and DTL electrode in Indonesian Adult. Methods: Through convenient sampling 49 normal Indonesian subjects between 19 and 49 years old were selected. Multifocal ERG amplitudes and implicit time values were measured according to recommendation by the ISCEV. Evaluation consisted of N1 and P1 wave in ring 1 to 5. after the examination, all subjects filled in a questionnaire about comfort level, adopted from the visual analog scale. Result: We observed a statistically significant difference in multifocal ERG normal values between electrode with higher wave amplitudes and longer implicit time in Jet and Dencott electrodes compared to DTL electrodes. Jet and DTL electrodes are more comfortable than Dencott electrodes for Indonesian adults in multifocal ERG. Conclusion: DTL electrodes give the lowest wave amplitude and the shortest implicit time and are the most comfortable electrode compared to Jet and Dencott electrodes, in multifocal ERG in Indonesian adults.

Abstract

Introduction: Retinal detachment is divided into three categories. The most common type is the regmatogen retinal detachment (rhegmatogenous retinal detachment), which is the result of the tearing of the retinal lining. Actions can be Buckle Sclera, vitrectomy pars plana and pneumatic retinopexy. The purpose of this case report to reported Management of Rhegmatogenous Retinal detachment with Buckle Sclera and Cryoretinopexcy. Method: A 58 years old man with chief complaint The left eye blurred like a curtain covered since 5 days,floaters (+), photopsia (+),headache (+), Patients never complain of lost vision suddenly before, eyeball pain is not there.History of trauma (+),On examination, the visual acuity 6/9 RE and 1/60 LE, anterior segment lens cloudy (+)nuclaer gr III, fundus photograph we found retinal contours of the blood vessels well, tear (+) is directed at 2-3 hours of superior-temporal and fovea reflex (-), B-scan ultrasound of the posterior segment the retina is not intact, Membran like lession detachment which attach to optic nerve. Results: The Sclera buckles and cryoretinopexy were performed under general antesthesia. Visual acuity post-operative on left eye 2/60. subconjunctival bleeding (+), the fundus photographs retinal attach but the macular reflex is still negative. post-operative theraphy with topical steroid and antibiotic eye drops, oral antibiotic, and oral analgetic. follow up 1-month post-operative visual acuity 5/60 with fundus photograph obtained retina attach. Conclusion: a diagnosis of retinal detachment can be found, with the discovery of fullthickness breaks or defects occurring from the retinal neurosensory, this break will allow the vitreous to enter the defect gap between the retinal neurosensory and RPE. Based on the clinical features of the posterior segment, the detachment area was found with the location of the Linchoff Rule 2 break based on the American Academy of Ophthalmology. The objective of operative therapy was to reattach the retinal portion of which one of them was buckle sclera and cryoretinopexcy.

Abstract

Introduction: To report a patient with acute lymphoblastic leukemia (ALL) who developed proliferative leukemic retinopathy with bilateral exudative retinal detachments and optic disc swelling. Case Report: A 11-year-old girl diagnosed with ALL-L2 on maintenance chemotherapy presented with severe blurred vision in both eyes for the past 1 year. Her visual acuity was 0,5/60 for both eyes. Slit-lamp examination of the anterior segment was found normal. Dilated fundus examination revealed tortuous retinal vessels, retinal haemorrhage, retinal and optic disc neovascularizations, optic disc with indistinct margins, and total retinal detachments in both eyes, as well as vitreous haemorrhage in the left eye. The ultrasonography examination revealed bilateral subretinal fluid. Discussion: The patient was diagnosed with bilateral proliferative leukemic retinopathy with exudative retinal detachments and infiltrative optic neuropathy, and left eye vitreous haemorrhage due to ALL. Conservative treatment was given. She continued the maintenance chemotherapy from pediatric department. After 2 months follow up, the visual acuity did not improve although retinal detachments were reduced. There were atrophic changes at the retina of her both eyes. Conclusion: It was reported a rare case of proliferative leukemic retinopathy with bilateral exudative retinal detachments and optic disc swelling. Early recognition and treatment is crucial to improve prognosis.

Abstract

Introduction: Endophthalmitis is an inflammation of the intraocular tissues and fluid, and an emergency in ophthalmology. One of the most common causes of endophthalmitis is post-traumatic endophthalmitis. Post-traumatic endophthalmitis has a poorer prognosis than post-cataract surgery endophthalmitis. The immediate pars plana vitrectomy (PPV) surgery in post-traumatic endophthalmitis is beneficial for optimizing of final visual. Methods: A retrospective descriptive study of post-traumatic endophthalmitis patients underwent pars plana vitrectomy in the Ciptomangunkusumo Hospital, Indonesia from January 2017 – June 2017. Result: There were twelve cases of post-traumatic endophthalmitis undergoes pars plana vitrectomy within the period of January 2017-June 2017. The most frequently identified causative organism was Staphylococcus (33,3%). There was an improvement of final visual acuity after the procedure of pars plana vitrectomy in 58.3% post-traumatic endophthalmitis patients. Conclusion: Early PPV leads to vision improvement and maintains visual acuity in post-traumatic endophthalmitis. Keywords: Pars Plana Vitrectomy, Endophthalmitis, Post-traumatic
