Abstract

This study aims to analyze the factors that influence the conversion of paddy fields to non-rice fields on rice production in Mataram City. In particular, the purpose of this study is to look at the factors that influence the conversion of paddy fields to non-rice fields. This study looks at the effect of annual household growth, GRDP, development of standard rice fields, the ratio of rice and non-rice fields prices, the ratio of agricultural and non-agricultural production values. To see what affects rice production in particular, namely land conversion, area of harvested land, the ratio of rice and non-rice fields prices and the ratio of agricultural and non-agricultural production values. This study uses secondary data obtained from the BPS (Central Bureau of Statistics) Mataram City office. The data is then analyzed using the multiple linear regression analysis method. The results showed that there were only 3 factors that were influential or significant, namely 1) the number of households with a coefficient value of 100.301 with a sig value of 0.022 percent, 2) GRDP coefficient value of 7.396E-8 with a sig value of 0.006 percent, 3) Standard rice field area with a coefficient value of - 1.527 with a sig value of 0.030 percent, although of the three factors there is one factor that has a negative or inversely effect. As for the factors that affect the conversion of paddy fields to non-rice fields on rice production, there is only one factor that has a real or significant effect on rice production, namely the area of rice harvested land (HA) by obtaining a coefficient value of 7.958 (sig 0.038 percent) which This means that the area of rice harvested land (HA) greatly affects the yield of rice production in the city of Mataram.

Abstract

The incident that occurred some time ago in Bima Bay, West Nusa Tenggara Province, which began to be seen on April 25/26 2022 is a phenomenon that is quite horrendous for residents of the City/Regency of Bima and outside the Bima area. According to several preliminary studies based on laboratory tests and local inspections, it is estimated that there are three possible causes, namely: (1) Sea Snot, (2) Algae Explosion and Metabolism (Algae Blooms) and (3) Oil Spill (Algae Blooms). oil spill). This study aims to examine the assessment of the sea on human health and welfare, in particular to examine the causes of seawater phenomena that occur in Bima Bay by considering many aspects. This study identifies and makes estimates based on data and facts that relate the phenomena that occur/pollutants to health effects on marine biota and on humans. It was carried out by observing the quality of sea water in Bima Bay based on the results of several laboratory tests on specimens taken on 27-29 April 2022. The test results of sea water samples taken on 27 April 2022 showed a high nitrate content. levels of 9.75 mg/l to 34.75 mg/l (water quality standard for nitrate content = 0.008 mg/l) and accompanied by an increase in ammonia level of 0.41 mg/l (water quality standard for ammonia content = 0, 3 mg/l) l). l) and phosphate content of 0.06-0.08 mg/l (Phosphate quality standard = 0.015 mg/l). Total ammonia, nitrate and phosphate are environmental parameters that contain nutrients which if present in high concentrations and even continue to increase in marine waters will cause eutrophication (bloom) which can be very dangerous for other marine biota by causing a decrease in dissolved oxygen, plankton growth which can cause decrease in fish population, bad smell, and bad taste and can cause the formation of biomass in the form of jelly. The biomass can be in the form of polysaccharides and proteins with an estimated containing bacteria, viral pathogens known as sea snot.

Abstract

Introduction: Multifocal electroretinography (ERG) is an examination which measures retinal electrical activity as a response to lighting stimulus and allows simultaneous recording in many topographic locations. Various electrodes have been developed to balance examination accuracy and also patients’ comfort. The objective of this study is to establish normal values of multifocal ERG and to compare the values and the comfort level using Jet, Dencott and DTL electrode in Indonesian Adult. Methods: Through convenient sampling 49 normal Indonesian subjects between 19 and 49 years old were selected. Multifocal ERG amplitudes and implicit time values were measured according to recommendation by the ISCEV. Evaluation consisted of N1 and P1 wave in ring 1 to 5. after the examination, all subjects filled in a questionnaire about comfort level, adopted from the visual analog scale. Result: We observed a statistically significant difference in multifocal ERG normal values between electrode with higher wave amplitudes and longer implicit time in Jet and Dencott electrodes compared to DTL electrodes. Jet and DTL electrodes are more comfortable than Dencott electrodes for Indonesian adults in multifocal ERG. Conclusion: DTL electrodes give the lowest wave amplitude and the shortest implicit time and are the most comfortable electrode compared to Jet and Dencott electrodes, in multifocal ERG in Indonesian adults.

Abstract

Introduction and Objection: Traumatic Optic Neuropathy is a rare and potentially vision-threatening condition caused by ocular or head trauma. The optic nerve axons may be damaged either directly or indirectly. The management of traumatic optic neuropathy could be observed, giving steroids or decompression. At center which optic nerve decompression is not routinely performed, high dose steroid is preferred for traumatic optic neuropathy patients. The study aims to analyze characteristics (mean age and gender, onset trauma, most symptom, visual acuity, consensual reflex as a sign), radiology feature, and onset corticosteroid therapy on patient traumatic optic neuropathy. Methods: This study was a descriptive observational case series which data retrospectively analysis from medical record. Result: From 13 cases most common gender affected were males (76,9%). The mean age group 32.85±14.67 years old with blurry vision and bleeding are the most common symptom, the onset of trauma below 24 hours (76.9%), the initial visual acuity is no light perception and hand movement with a negative consensual reflex (76.9%). Visual acuity improves with initial corticosteroid therapy below 24 hours are 7 cases and more than 24 hours are 2 cases. From 9 cases with visual acuity improvement, there are 4 cases with orbital fracture and 5 cases without orbital fracture. Conclusion: Visual improvement in our cases happened more on initial corticosteroid therapy more than 24 hours and no orbital fracture.

Abstract

This study aims to assess 1000 mg citicoline each day given for 60 days on chronic phase NAION (non–arteritic anterior ischemic optic neuropathy) patients. Double masked randomized clinical trial divided the patients into 2 groups: citicoline group (C–NAION) and placebo group (P–NAION). Analysis was done in 17 patients in each group. There were increament of ? amplitude P50 30 days in C–NAION group (0,775±2,6 µV) and ? amplitude N95 60 days in C–NAION group (–0,356±2,992 µV) but statistically insignificant compared to P–NAION. Age has a good correlation with ? amplitude P50 (r = –0.517; p = 0.002). Subjects with 1 risk factor experienced a higher ? amplitude of P50 than the subjects with more than 1 risk factor (1.856 ± 2.121 ?V vs –0.26 ± 2.45; p = 0.025). Citicoline did not show any changes in Retinal Ganglion Cell Thickness. There were improvement of ? mean deviation 60 days in C–NAION group (3,13±6,467 dB), but statistically insignificant compared to P–NAION. Citicoline tends to increase the ? amplitude of P50, N95 and mean deviation in chronic phase NAION. Age and the number of risk factors affects the increase of ? amplitude P50.

Abstract

Abstract Introduction : Breast cancer can metastasize to many sites, but the orbit is an infrequent location. Orbital metastases represent 1%–13% of all orbital tumors, and the prognosis is rather poor. We reported a case of orbital metastases of breast cancer that led to findings of other sites involvement. Case Illustration : A 50-year-old female presented with an inward squint and double vision for 9 months before admission. Esotropia, abduction, and elevation deficits in both eyes were observed. The visual acuity of both eyes was 6/6. Orbital MRI revealed an intraconal irregular lesion near the medial rectus muscle, which was hyperintense with contrast and thickened medial rectus muscles of both eyes. She underwent lumpectomy of breast mass two years before admission and no follow-up after surgery. Histopathology showed invasive lobular carcinoma (ILC). We referred her to hematologist- oncologist to have workup on metastases and treatment. Lung and extensive bone metastases were discovered. She was diagnosed with metastatic breast cancer (stage IV) and treated with hormonal therapy and bisphosphonates. On 3-month follow-up, diplopia and gaze limitations persist. Discussion : Diplopia and motility disturbances are the most common symptoms and signs of orbital metastases, including extraocular muscle involvement. The horizontal rectus muscles are more commonly involved. ILC is more frequent histological subtype involving orbital as the orbits are rich in fat and may attract disseminated ILC cells. Conclusion : Ocular symptoms in patients with a history of breast cancer should always be investigated for metastatic disease. A multidisciplinary approach is mandatory to achieve the best therapeutic management and long-term surveillance.

Abstract

Abstract Introduction : Idiopathic orbital myositis, an inflammation localized at the extraocular muscles, is rarely reported in pediatrics. Until now, the prevalence and incidence of orbital myositis remain unknown. This paper aims to report a case of idiopathic orbital myositis in a young girl with headache. Case Illustration : A-16-year-old presented with a droopy eyelid of the left eye (LE) and double vision since 1 week before admission. She also experienced swollen eyelid and cluster headache for 5 months with no neurological abnormalities. Ophthalmic examinations showed exotropia with limited eye movement of LE. Ptosis of LE was noted with margin reflex distance 1 (MRD-1) +2 mm, while MRD-1 +6 mm for the right eye. Other examinations were unremarkable with normal funduscopy and negative RAPD. Non-contrast orbital sinus CT-Scan revealed an enlargement of the superior rectus and lateral rectus muscle. The laboratory result showed slight leucocytosis and normal ESR. High-dose intravenous corticosteroid was administered and tapered with oral steroid. The orbital clinical findings showed improvements, better ocular movements, and diminished headache complaints. Discussion : As a rare case, the diagnosis of orbital myositis in adolescence can be challenging and requires detailed examination, and proper imaging to exclude other diagnosis such as ophthalmoplegic migraine, infections, malignancies, or congenital lesions (hemangioma). The first-line therapy is a systemic corticosteroid, which well responded in this patient. Conclusion : In this patient, intravenous corticosteroid represent an effective treatment. Close monitoring of the disease progression is required after switching to oral therapy to prevent relapse.

Abstract

Abstract Introduction : Perfusion deficiency of the optic nerve head is presumed to have an immense role in the pathogenesis of non-arteritic ischaemic optic neuropathy (NAION). Optical coherence tomography angiography (OCTA) is a non-invasive method that can assess posterior pole vasculature at multiple levels. Thus, these case series want to compare the OCTA in the acute (before four weeks) and chronic phases (after six weeks) of NAION with their fellow unaffected eyes to give an overview of vasculature changes in NAION. Case Illustration : This case series consists of four patients with NAION. Half of them came with a sudden painless blurry vision, and the remaining presented with an abrupt visual field defect. All of them have diabetes mellitus and hypertension. None of them have superimposed vascular pathology. The first OCTA was taken within seven days of the first symptoms, and the second OCTA was taken more than six weeks after. Discussion : The high percentage of systemic vascular risk factors and the acute onset supported the vascular occlusive hypothesis in the pathogenesis of NAION. Meanwhile, the OCTA provides an actual overview of the vasculature changes. Microvasculature is more likely to expand in the acute phase, and a more significant expansion was found in greater optic disc edema. On the other hand, microvascular tend to attenuate in the chronic phase, representing the pruning of capillaries. A swept-source OCTA with deeper penetration might give a better peripapillary choriocapillaris image in the acute phase. Conclusion : OCTA is a promising adjunctive tool for diagnosing and monitoring progression in patients with NAION.
