Abstract

In the past decades, indications for PPV in paediatric patients has been rising with large heterogeneity outcomes among studies. Our objective is to present a pilot descriptive study in Indonesia, regarding pars plana vitrectomy (PPV) success rate in paediatric patients with rhegmatogenous retinal detachment (RRD) in Dr. Cipto Mangunkusumo Hospital (CMH). A retrospective study was conducted on 16 paediatric eyes diagnosed for RRD and treated in CMH during the period of January 2017 to December 2017. A total of 16 RRD eyes were identified in 15 individuals. Majority of the patients were male and were in the age group of 12-18 years. There were 13 patients undergone PPV with or without SB. Functional anatomical success rate was determined from the macula status while functional success rate on final BCVA. Among 16 patients, 14 had constant or increased final BCVA and 11 had reattached macula. The success rate of PPV in terms of anatomical outcome was 11 out of 16 eyes and in functional outcome was 8 out of 16 eyes. Keywords: pars plana vitrectomy, paediatric ophthalmology, rhegmatogenous retinal detachment.   Karakteristik dan Hasil Vitrektomi Pars Plana pada Pasien Pediatrik di Divisi Vitreoretina Rumah Sakit Cipto Mangunkusumo Tahun 2017 Abstrak Indikasi tindakan vitrektomi pars plana (PPV) pada pasien pediatrik bertambah secara signifikan dalam beberapa dekade terakhir. Penelitian ini dilakukan sebagai studi awal di Indonesia, terkait tingkat keberhasilan PPV pada pasien anak dengan rhegmatogenous retinal detachment (RRD) khususnya di RS Cipto Mangunkusumo (RSCM). Studi retrospektif dilakukan terhadap 16 mata RRD pada 15 pasien anak di RSCM pada periode Januari 2017 – Desember 2017. Mayoritas pasien merupakan laki-laki dalam kelompok umur 12-18 tahun. Terdapat 13 pasien yang menjalani PPV dengan atau tanpa scleral buckle (SB). Tingkat keberhasilan anatomi dilihat dari status penempelan makula, sementara tingkat keberhasilan fungsional dinilai dari visus akhir. Tingkat keberhasilan anatomi PPV dalam studi ini adalah 11/16 mata, dan tingkat keberhasilan fungsional mencapai 8/16 mata. Kata kunci: vitrektomi pars plana, oftalmologi pediatrik, rhegmatogenous retinal detachment.

Abstract

Central serous chorioretinopathy (CSC) is one of the leading causes of blindness in middle-aged population. This case report describes case of CSC associated with amlodipine consumption. A 62-years old woman came with blurry vision on the right eye (RE) for 25 days, prior to admission. Patient was hypertensive and on routine calcium channel blocker (CCB, amlodipine) for the last two months. Initial best corrected visual acuity (BCVA) was 0.3 for her RE and 0.8 on her left eye (0.8). Anterior segment examinations of both eyes were within normal limits. Funduscopy revealed serous retinal detachment on the macula of RE. Optical coherence tomography (OCT) of RE showed subretinal and below choroid fluid accumulation and central macular thickness of 352 µm. Patient was treated with non-steroidal anti-inflammatory drug (NSAID) and vitamins but the complaint worsened. Amlodipine was ceased on the third month of treatment. CSC resolved and final BCVA was 1.0. There might be an association between CCB consumption to incidence of CSC. Our study was the first to describe such occurrence. Further study is required to confirm this association Keywords: Central serous chorioretinopathy, calcium channel blocker, amlodipine, calcium ion, pathogenesis.   Korioretinopati Serosa Sentral Terkait Penggunaan Penghambat Kanal Kalsium: Sebuah Laporan Kasus Abstrak Central serous chorioretinopathy (CSC) merupakan salah satu penyebab utama kebutaan pada populasi usia paruh baya. Laporan kasus ini mendeskripsikan sebuah kasus CSC yang berkaitan dengan konsumsi Amlodipine. Seorang wanita berusia 62 tahun datang dengan keluhan pandangan buram pada mata kanan sejak 25 hari sebelum datang ke Rumah Sakit. Pasien didiagnosis dengan hipertensi dan rutin mengkonsumsi calcium channel blocker (CCB, amlodipine) dalam 2 bulan terakhir. Pemeriksaan tajam visual terbaik / best corrected visual acuity (BCVA) di awal menunjukkan hasil 0.3 pada mata kanan dan 0.8 pada mata kiri. Pemeriksaan anterior segmen pada kedua mata dalam batas normal. Pemeriksaan funduskopi menunjukan ablasio retina serosa pada makula mata kanan. Pemeriksaan optical coherence tomography (OCT) pada mata kanan menunjukkan hasil akumulasi cairan pada subretina dan dibawah koroid, serta ketebalan makula sentral yaitu 352µm. Pasien diberikan tatalaksana berupa non-steroidal anti-inflammatory drug (NSAID) dan vitamin, namun keluhan dirasa memburuk. Konsumsi amlodipine dihentikan pada bulan ke 3. CSC dinyatakan sembuh dengan BCVA akhir 1.0. Terdapat kemungkinan hubungan antara konsumsi CCB dengan insidensi CSC. Studi kami merupakan studi pertama yang mendeskripsikan kejadian tersebut. Abstract

Introduction: Central serous chorioretinopathy (CSC) is a condition characterized by the accumulation of transparent fluid in the posterior pole of the fundus. Currently, there are several therapeutic options that can be used to treat chronic CSC. This study aimed to describe the clinical characteristics and results of the management of chronic CSC. Methods: The design of this study was a descriptive study. This research was conducted at the division of vitreoretina, department of ophthalmology, Faculty of Medicine, Universitas Indonesia, Dr. Cipto Mangunkusumo National General Hospital (RSCM), Jakarta, Indonesia, from April to June 2022. The inclusion criteria were newly diagnosed patients with chronic CSC in the specified period. Data analysis in this study was carried out using descriptive statistics. Snellen vision in patients is converted into logMar vision with the formula-log (decimal vision). Results: There were 17 patients who were included in this study. Among them, 76.4% were men, 52.94% were in the left eye, and there were risk factors for hypertension, as much as 29.41%. Five patients received non-steroidal anti-inflammatory drugs (NSAID) therapy, with 2 of them experiencing increased visual acuity and 3 of them complete resolution. Five patients received anti-VEGF therapy, with 3 of them experiencing increased visual acuity and also complete resolution. Seven patients received subthreshold laser therapy, with all experiencing increased visual acuity, and 4 of them complete resolution. There were 15 patients with subretinal fluid and 2 patients with subretinal fluid combined with retinal pigment epithelial detachment. Conclusion: Most of the patients were male, with an average age of 45.8 years. Most CSC patients affect one eye and still have fairly good vision. The treatments received by patients consisted of non-steroidal anti-inflammatory drugs (NSAID) therapy, anti-VEGF therapy, and subthreshold laser therapy.

Abstract

Globaly, the prevalence of refractive errors, especially myopia in children, has increased significantly. The cause of myopia is multifactorial. Near-work activities and family history are some suggested factors associated with myopia. However, there has not been enough data on these factors and their association with myopia in Indonesia. This study aims to determine the prevalence and factors associated with myopia in primary and middle schoolaged children in Pamijahan Village, Bogor District, West Java, Indonesia. Data were taken in a cross-sectional design based on the visual acuity, anterior and posterior eye examination. A 14-item questionnaire regarding myopia-associated factors including family history, playing outdoors, and near-work activities, was distributed to the subjects. The data were taken from June to September 2018. Analysis was conducted using SPSS version 20. The prevalence of myopia in Pamijahan Village is 4,9% out of 474 subjects. Family history, including parental (p=0.001, OR 4.49) and sibling history (p=0.001, OR 5.58), was shown to be associated with the increasing risk of myopia in children while near-work activities and playing outdoors were not associated with myopia in children of Pamijahan Village.

Abstract

Purpose: to assess the levels of Hypoxia-inducible factor-1α (HIF-1α) and intercellular adhesion molecule-1 (ICAM-1) in vitreous of proliferative diabetic retinopathy patients which were given intravitreal bevacizumab (IVB), as well as its relation to the central macular thickness (CMT) measured prior to vitrectomy. Methods: thirty-two eyes were randomized into two groups, one that received an IVB injection at 1-2 weeks previtrectomy and the control group which did not receive any injection. Measurement of HIF-1α and ICAM-1 was conducted using enzyme-linked immunosorbent assay (ELISA). The CMT were measured at the initial visit, prior to vitrectomy, and at follow up time (2, 4, and 12 weeks postoperatively) using Stratus OCT. Results: The mean levels of HIF-1α vitreous (ng/mg protein) in the control group and IVB respectively 0.020 (0.006; 0.077) and 0.029 (0.016; 0.21). Vitreous levels of ICAM-1 (ng /mL) in control group and IVB group were 20.10 (3.41; 40.16) and 23.33 (0.63; 68.5). The mean levels of HIF-1α and ICAM-1 vitreous obtained did not differ significantly between the two groups. Conclusion: The levels of HIF-1α and ICAM-1 in PDR patients do not decrease after one injection of intravitreal Bevacizumab 1-2 weeks prior to vitrectomy. The concentration of vitreous HIF-1α and ICAM-1 are not directly related to the CMT.

Abstract

Abstract Background: The incidence of nucleus drop or intraocular lens (IOL) drop as the complication of phacoemulsification increases due to the increased frequency of phacoemulsification. Pars plana vitrectomy (PPV) followed by endofragmentation and secondary IOL implantation is the choice of procedure for management. This study aims to determine the frequency, outcomes, and complication of PPV in the case of nucleus drop or IOL drop in the Department of Ophthalmology, Fakultas Kedokteran Universitas Indonesia – Rumah Sakit Cipto Mangunkusumo (FKUI-RSCM) Methods: This study is a retrospective descriptive study conducted in the Vitreoretinal Division of the Department of Ophthalmology, FKUI - RSCM. Research data was taken from the medical records of all nucleus drop or IOL drop patients underwent PPV in January 2017-December 2017. Results: There were 19 cases studied. The incidence of nucleus drop occurred in phacoemulsification surgery techniques (94.7%) and ECCE techniques (5.3%). Vitrectomy surgery was performed ≤2 weeks in 31.6% and >2 weeks in 68.4% after the patient first arrived at the vitreoretinal clinic. Most pre-PPV visual acuity was 1/60-6/60 (47.1%). In the final follow-up, visual acuity improved from 6/45 to 6/6 occurred in 42.2% of cases. Complication after PPV and secondary IOL implantation include elevated IOP (10.5%), IOL decentration (5.3%), corneal decompensation (5.3%), macular edema (5.3%), and retinal detachment (5.3%). Conclusion: Nucleus drop or IOL drop generally occurs in phacoemulsification cataract surgery techniques. Improved visual acuity was achieved after PPV and secondary IOL implantation at the end of the follow-up period. Most common post-PPV complication is elevated IOP.

Abstract

Background: Silicone oil (SO) removal following vitrectomy and injection of silicone oil may cause retinal redetachments (re-RD), with duration of tamponade being considered as the risk factor. So, we wanted to evaluate the duration of silicone oil tamponade as a risk factor of retinal redetachments after silicone oil removal. Result: Ten studies comprising of 2120 eyes were included in this review. Pooled analysis showed no statistically significant difference between the re-RD group and the non-re-RD group in terms of SO duration (mean difference -1.87 months; 95% CI, -3.87 to 0.13; p=0.07; I2=53.1%). The re-RD rate did not differ between the shorter and longer duration of SO tamponade (OR=1.44; 95% CI, 0.54 to 3.82; p=0.47; I2: 63.3%). Conclusion: The duration of SO tamponade was not associated with re-RD after SO removal.

Abstract

Background: Injection of Silicon oil is a standard procedure for vitreous replacement in vitrectomy procedure for retinal detachment cases. It acts as a great tamponading agent for reattachment of retinal breaks or retinal detachment. Despite its minor side effect, silicon oil could cause several complications such as cataract, endothelia decompensation, increased intraocular pressure, and secondary glaucoma. Thus needed to be evacuated after the retinal reattachment is stabilized. Following evacuation procedure, visual acuity are known to be significantly improved. However, some cases shows decreased of visual acuity due to retinal redetachment, optic nerve damage due to secondary glaucoma, hypotonym vitreous hemorrhage, expulsive hemorrhage, and cornea abnormality. Methods: A retrospective descriptive study of retinal detachment patients underwent silicon oil evacuation procedure in Cipto Mangunkusumo Hospital, Indonesia from September 2017 until Januari 2018. Results: There were seventy seven cases of retinal detachment undergoes silicon oil evacuation within period of September 2017-Januari 2018. There were improvement of visual acuity (greater than 6/60) after one month of silicon oil evacuation. Anatomical retinal reattachment was successfully observed in 91% patient. The most occuring complication after silicon oil evacuation includes secondary glaucoma and retinal redetachment. Conclusion: Silicon oil evacuation is a standard procedure following a vitrectomy in retinal detachment cases. The evacuation procedure yields in positive benefit for patient in term of visual acuity and anatomical structure.

Abstract

Introduction: This study aims to determine the anatomical and functional outcomes in patients with macular hole (MH) underwent vitrectomy with internal limiting membrane (ILM) peeling. Method: A descriptive retrospective study at Vitreoretinal Division of Ophthalmology Department, Faculty of Medicine Universitas Indonesia – Cipto Mangunkusumo National General Hospital (FKUI-RSCM). Secondary data obtained from medical records of patient with MH within January – December 2017. The anatomical outcome was observed from the closure of MH. Functional outcome was observed from post-operative visual acuity at day-1, month-1, month-3, and month-6. Results: 16 patients who met the criteria were included in this study. MH closure was observed in 43.8% of cases and failed closure in 56.2%. Improvement of visual acuity was observed on closure cases in 3 months and 6 months, occurred in 71.43% and 100% of cases, respectively. Conclusion: MH closure rate was 43.8%. Satisfying result of improvement in visual acuity achieved after vitrectomy with ILM peeling.

Abstract

Introduction: Endophthalmitis is an inflammation of the intraocular tissues and fluid, and an emergency in ophthalmology. One of the most common causes of endophthalmitis is post-traumatic endophthalmitis. Post-traumatic endophthalmitis has a poorer prognosis than post-cataract surgery endophthalmitis. The immediate pars plana vitrectomy (PPV) surgery in post-traumatic endophthalmitis is beneficial for optimizing of final visual. Methods: A retrospective descriptive study of post-traumatic endophthalmitis patients underwent pars plana vitrectomy in the Ciptomangunkusumo Hospital, Indonesia from January 2017 – June 2017. Result: There were twelve cases of post-traumatic endophthalmitis undergoes pars plana vitrectomy within the period of January 2017-June 2017. The most frequently identified causative organism was Staphylococcus (33,3%). There was an improvement of final visual acuity after the procedure of pars plana vitrectomy in 58.3% post-traumatic endophthalmitis patients. Conclusion: Early PPV leads to vision improvement and maintains visual acuity in post-traumatic endophthalmitis. Keywords: Pars Plana Vitrectomy, Endophthalmitis, Post-traumatic
