Abstract

This study aims to determine the effect of the completeness of outpatient medical resumes on the quality of medical records at the Kimia Farma 43 Buah Batu Clinic. The research method used is quantitative with a descriptive approach. Data collection techniques used are by way of questionnaires, observation, and literature review. The sampling technique used saturated sampling technique with a sample of 30 clinical officers. The results of calculations carried out using a questionnaire with a simple linear test model, the number of samples N = 30 and a significant α = 5% obtained tcount = 5.582 and ttable = 1.701 the result is tcount > ttable then H0 is rejected and H1 is accepted, meaning that the completeness of the outpatient medical resume form effect on the quality of medical records at the Kimia Farma Clinic 43 Buah Batu. Abstract

The purpose of this study is to get an overview of the completeness of the claims file using the 5M element. The research was carried out using descriptive quantitative methods. By describing the results of interviews and observations that have been made. The sample in this study was taken from the claims of inpatient Covid-19 patients at the Hazardngkara TK.II Sartika Asih Hospital in Bandung. From research conducted at Bhayangkara Sartika Asih Hospital, Bandung, there are still problems found, namely: (1) Filling out an incomplete resume, (2) There is a written diagnosis filled in by the DPJP that is not clear and unreadable and medical support that does not include lab and radiology results, (3) Lack of discipline and health personnel to fill out complete medical records in accordance with hospital standard operating procedures (SOP), (4) hardware and software that often experience interference. The suggestions given are: (1) conducting socialization and evaluation of filling out resumes, (2) other medical and health record officers must continue to remind each other in filling out the completeness of medical records and other medical supporting results, (3) Giving warnings and sanctions if mistakes are made. repeated, (4) perform maintenance on hardware and software. Abstract

The aim of the study was to find out the need for medical record document file shelves to support the effectiveness of services at the Dr. Central General Hospital. Hasan Sadikin Bandung.The research method used is descriptive method, namely a research conducted to describe or describe a phenomenon that occurs.Based on the results of research and observations the authors found that: 1). data on the number of patients increases each year, 2) The need for shelves according to the anthropometric data filling is 272 shelves, 3) There has been a shortage of medical record storage shelves since 2019 so that files have piled up and many DRMs have been damaged, 4) The service has not been effective this is because the unit medical records have not been maximized in carrying out DRM storage because the available shelves are not sufficient, the shelf design is too high and not suitable for filling anthropometric data so that it can cause work accidents.The suggestions that the author conveys include the Central General Hospital Dr. Keywords: Workload Analysis, Effectiveness

Abstract

Electronic medical records are an important achievement of information technology in the world of health and have considerable benefits, one of which is to increase the effectiveness of officer work. The use of electronic medical records at Hermina Pasteur Hospital is still not perfect and there are still several obstacles that can hinder the effectiveness of officers' work. The purpose of this study was to determine the analysis of electronic medical records in supporting work effectiveness in the medical records unit at Hermina Pasteur Hospital Bandung. This research uses descriptive qualitative methods with data collection techniques with observation, interviews, literature studies and questionnaires. The results of this study showed security aspects (91.7%), integrity aspects (82.8%), availability aspects (87.1%), work quality aspects (81.7%), work quantity aspects (79.6%), and work time (86.1%). The suggestions given in this study are: It is necessary to evaluate the use of electronic medical records so that appropriate training can be carried out, carry out routine system maintenance, coordinate with related electronic medical record vendors to update the electronic medical record system, legalize electronic signatures and provide electronic signature tools, hospitals should provide automatic generators for better electricity availability, it is necessary to hold SPO on electronic medical records so that electronic medical records in hospitals can be ensured that they meet the established security, privacy and quality standards.

Abstract

The guidelines for writing major diagnoses have an impact on how they are coded; each diagnostic statement must be clear and informative in order to place the disease in the ICD-10 category with the highest level of specificity. The accuracy of the diagnosis, the readability of the doctor's writing, the professionalism of the doctor and the coding, and other factors all influence the quality of the coding findings. The literature review was used as the research design. The codification analysis has not been carried out ideally so that there is still inaccurate code and makes the reports that have been made less accurate and seen from the preparation and delivery of reports that have been carried out on time so that the level of relevance of the report is 92%. Research analysis on the accuracy of codification of diagnoses on the quality of outpatient morbidity reports (RL 4b), based on a sample of 100 medical records examined, 100 medical records that were accurate 60 or 60% and 40 medical records that were inaccurate or 40%, Reliability test results with using the Cronbachs Alpha method with a significance level of 10% (0.1), namely 0.923, was then taken into consideration. This value is greater than the r table value of 0.4409 which indicates that the reliability test was successful and the questionnaire results are reliable. .Keywords: Codification; diagnosis; Medical records

Abstract

This research was conducted with the aim of knowing the effect of PMIK (Medical Recorder and Health Information) professional competence on the factors causing damage to medical record files at Hospital X. The research method used is a quantitative method with a descriptive approach. Data collection techniques used are by means of observation, interviews, questionnaires and literature study. The sampling technique used a total saturation sampling technique of 19 people. Based on the results of hypothesis testing, it was obtained that the influence between variable X and variable Y was 16,048. The problems that arise are: 1) The officers are not careful and not thorough in carrying out the work of storing medical record files. 2) There is still a lack of knowledge of officers regarding the maintenance and protection of medical record files. 3) The medical record file storage rack is still made of wood so it will affect the medical record file because it is eaten by termites or rats. 4) There is no SPO (Standard Operating Procedure) for the maintenance and protection of medical record files.The suggestions given to the Hospital X include: 1) It is advisable for medical record officers at Hospital X to carry out the work of storing medical record files in a professional manner according to established standards. 2) It is advisable for the leadership of Hospital X to provide direction to the medical records manager of Hospital X to provide RMIK training in order to increase the knowledge and skills of medical record officers in medical record services. 3) We recommend that the medical records lead at Hospital X carry out Control and Monitoring (supervision and monitoring) on ​​facilities and infrastructure in medical record services in order to increase the quality and quantity of medical record service management. 4) It is recommended that the medical records manager at Hospital X make and carry out a controlling checklist (supervision checklist) for medical record officers so that officers are supervised and do their job correctly, precisely and consistently. Abstract

This study aims to determine the effect of codification accuracy on reporting 5.3 (a list of the top 10 inpatient diseases) at Al-Ihsan Hospital Bandung. The research method used is a quantitative research methodology with a descriptive approach. Data collection techniques used are by means of observation, questionnaires, interviews, and literature studies. The sampling technique used the Proportional Stratified Random Sampling technique with 6 medical record officers as respondents. The results of the study found that the accuracy of codification of inpatients had an effect of 97% on the reporting of RL 5.3 (list of the top 10 inpatient diseases) at Al-Ihsan General Hospital Bandung. While the remaining 3% is influenced by other variables not examined by the author, and produces a very strong correlation between the two variables, which is equal to 0.963. The problems found in this study were: 1) The doctor's writings were illegible, 2) incomplete medical records of discharged patients kept in the nursing room. The suggestions given are: 1) It is better for the DPJP to write down the diagnosis in a complete and legible way, 2) it is better to carry out outreach, monitoring and evaluation regarding SOP. Abstract

Background: Osteoarthritis is a chronic joint disease characterized by abnormalities in cartilage (cartilage), where abnormalities in the cartilage cause the bones to rub against each other resulting in stiffness, pain, limited movement, and disruption of daily activities. Objective: To determine the implementation of Physiotherapy in reducing pain, increasing muscle strength, increasing the scope of joint motion, and increasing functional abilities in cases of Osteoarthritihis Genu Sinistra by using tens modalities, ultrasound and exercise therapy. Results: After 6 times of therapy, the results of pain assessment in the left knee from silent pain T1 = 7 to T6 = 4 motion pain T1 = 8 to T6 = 4 tenderness T1 = 4 to T6 = 2. Increased muscle strength of knee extensors T1 = 3 to T6 = 4 and knee flexors T1 = 3 to T6 = 4. As well as an increase in the scope of motion of the passive left knee joint, namely T1 = S 00-00-1200 to T6 = S 00-00-1300 and the scope of motion of the active left knee joint, namely T1 = S 00-00-1000 to T6 = S 00-00-1200. Conclusion: Ultrasound can help reduce pain, Tens can relax the muscles, and Exercise therapy can increase muscle strength, increase the joint motion, and can improve functional abilities. Abstract

This study aims to determine the influence of environmental sanitation on employee work productivity ini Klinik Pratama Medika Antapani. The research method used is a quantitative method with a descriptive approach, data collection techniques by distributing questionnaires, observation, and literature review. The sampling technique used saturation sampling with a total of 40 employees as respondents. From the results of the study it can be concluded that the influence of environmental sanitation on work productivity using the spearmen rank correlation test is 77.5%, which means that it has a close relationship and is in a positive direction. The results of the determination test of 67.1% environmental sanitation have a strong relationship to affect employee productivity by 67.1%, the remaining 32.9% is influenced by other factors not examined. The problems that arise include: 1) the cleanliness of the building as a whole is still not maintained in several workspaces, 2) there are several rooms with hot temperatures, 3) there are mosquitoes in the workspace, 4) training during working hours. As for suggestions for fixing the problem: 1) regular discussions with related departments, 2) monitring room temperature supporting facilities, 3) carry out fogging and close the water tank, 4) have replacement employees when there is training. Abstract

Human conduct is divided into three domains: knowledge, attitudes, and acts. Compliance is a type of behavior that results from interactions between health professionals and patients. When the patient's conduct is needed to comply with carrying a Medical Identity Card (MIC) when registering at the registration unit, patients carrying a MIC make it simpler for registration officers to find the patient's identification. Based on the findings of an initial survey of 10 outpatients conducted through interviews, patients were not yet compliant in carrying a MIC, because they did not understand the benefits and purpose of carrying a MIC while seeking treatment at health centre. The study aimed to examine the association between patient behavior and adherence to carrying MNH at Medan Baru Eye Center in 2022. The study employed was quantitative using a cross-sectional approach, and conducted at Medan Baru Eye Center, located in Abdullah Lubis District. The population was 3881 patients who had gotten treatment in the previous year at Medan Baru Eye Center. The sample was done by chance. The research instrument was a questionnaire that has been validated and reliable. Finally, with a p value of 0.05, knowledge, attitude, and action factors associated with adherence to carrying identity cards for treatment. The Medan Baru Eye Center were advised to educate the population about the need of having a Medical Identity Card (MIC) while seeking treatment at further time.
