Abstract

Neonates are newborns up to 28 days old, where there is a very big change from life inside the womb to outside the womb. Lactation counseling is most effective for increasing exclusive breastfeeding in infants. The success of exclusive breastfeeding cannot be separated from being influenced by several factors including age, level of education, experience and husband's support. Mothers who give birth for the first time do not have health information about breastfeeding, so midwives need to conduct lactation counseling. Lactation counseling about exclusive breastfeeding, how to care for breasts and proper breastfeeding techniques. This case study describes the management of midwifery care for Mrs. A, a 20-year-old primipara, who was confused about caring for a baby after giving birth. Handling efforts are carried out with a lactation counseling and guidance approach. Case studies use assistive devices in the form of a midwifery care format for normal newborns and status sheets or patient documentation and a list of interview questions. Data analysis used is data reduction, data presentation and conclusion. The result of the case study was that Mrs. A's baby's weight increased by 550 grams for 14 days after being given lactation counseling and breastfeeding guidance. Comprehensive midwifery care provided increases mothers' understanding of newborn care.

Abstract

Failure in the breastfeeding process is often caused by the emergence of several problems, both problems for the mother and the baby. Problems from mothers that arise during breastfeeding can start before delivery (antenatal period), in the early postpartum period, and after late labor. Problems with breastfeeding can also be caused by special circumstances, such as delayed onset of lactation on early postpartum period, the mother complaining that her baby often cries or refuses to breastfeed, which then means that the milk is not enough or not good enough to make the decision to stop breastfeeding. Anxiety caused by fear of not being able to produce milk and not having enough milk is the most common reason women fail to start breastfeeding, stop breastfeeding too soon, or start complementary feeding before it is needed. One alternative to help facilitate breast milk is to do oxytocin massage. In this case study, after full breast oxytocin massage before breastfeeding, the milk output was good, the baby showed a good sucking and swallowing response, after the oxytocin massage technique was carried out independently and assisted by the husband, the milk came out smoothly. Mother says that the baby is calm and sleeps soundly not fussy.

Abstract

Various problems that arise in the third trimester of pregnancy are problems that are often complained of by pregnant women, such as anxiety and pain. Among these complaints, low back pain is the most commonly reported, occurring in 60% -90% of pregnant women, and is one of the causes of the incidence of cesarean delivery. Back pain is caused by pain in the lumbosacral area. The increase in intensity as the gestational age increases is caused by a shift in the center of gravity and changes in body posture. Back pain from sacroiliac/lumbar can be a long term back problem if not treated immediately. One of the complementary therapies that can reduce or relieve pain, reduce or prevent muscle spasms, provide a sense of comfort, namely with warm compresses and prenatal massage. This case study describes the management of complementary therapy consisting of warm compresses and prenatal massage in third trimester pregnant women with complaints of back pain. After midwifery care was implemented, Mrs. F experienced a decrease in back pain so that the problem could be resolved.

Abstract

Exclusive breastfeeding is recognized as the single most effective preventive intervention to reduce child mortality in the world. Unfortunately, the success of this exclusive breastfeeding has still a lot of obstacles in daily practice. Breast pain due to nipple crack is one of the most common reasons for premature weaning of breastfeeding. This case study describes the management of midwifery care for Mrs. A, a 29-years-old primipara, who experienced nipple crack on the 19th day of postpartum. She was confused because her baby refused to be given milk formula while she had difficulty giving breastmilk because her nipple soreness. Expressed Breastmilk (EBM) was suggested to be applied topically on the nipple to heal the crack. Meanwhile correction of position and attachment as well as breast care were suggested to prevent repeated nipple trauma. The midwifery care provided in this case made Mrs. A was able to breastfeed her baby comfortably and the sores on her nipple had completely healed within 8 days. Single intervention is not enough to treat nipple crack for breastfeeding mothers. It is necessary to provide combination of continuous midwifery interventions so that the results are more effective. Interventions to prevent nipple trauma in breastfeeding mothers which include correct breastfeeding positions and techniques, breast care and EBM for topical use should be given as standard information provided by health workers in antenatal and postnatal counseling to ensure continuity of breastfeeding and the success of exclusive breastfeeding.

Abstract

Perineal rupture is a tear that occurs in the perineum during labour. Perineal rupture occurs in almost all first deliveries and is not uncommon in subsequent deliveries. The occurrence of perineal rupture is caused by maternal factors (parity, birth spacing and baby's weight), improper delivery leadership, history of delivery. Forceps extraction, vacuum extraction, device trauma and episiotomy. Perineal rupture occurs in almost all first deliveries and is not uncommon in subsequent deliveries. This case study describes how perineal rupture occurs and its healing process in Mrs. M, a 21 year old primipara. Efforts to handle or treat perineal wounds in this case are carried out by teaching how to do proper wound care, maintaining personal hygiene and providing education to increase client knowledge. Midwifery care provided can help clients accelerate the healing of perineal wounds. Comprehensive midwifery care regarding postpartum care needs to be carried out from the time of pregnancy until later in the puerperium. Thus, the client can prevent and deal with problems that occur independently so that there are no infections or complications during the puerperium.
