Abstract

Introduction: Platelet rich plasma (PRP) contains not only several growth factors but also proinflammatory cytokines that promote the healing process of tendon ruptures. Therefore, this study aimed to determine the effect of PRP administration on TGF- β1 expression during the healing process of Achilles tendon rupture in white rats (Rattus norvegicus). Method: This study was an experimental research with a Posttest-Only Control Group Design approach. This study enrolled 30 male white rats with a ruptured Achilles tendon, aged 3-4 months and weighing 200-250 grams.  The rats were randomly divided into 2 groups. The first (I) group as the control group included rats with Achilles tendon rupture without PRP administration, and group II as the experimental group included rats with Achilles tendon rupture with PRP 100 ul administration. Equal number of white rats from both groups were observed in 3 different times (on the day 7th, 14th, and 21st) after intervention. Part of the substance of the right Achilles tendon of each white rat was taken for examination of TGF-β1 expression by immunohistochemical examination with anti-TGF- β1 primary antibody. The data were analyzed by using SPSS 25.0. The descriptive data were provided in table, and the analytic data were analyzed by using unpaired t-test. The significant p value was <0.05. Results: We established that the intensity of TGF- β1 expression of Achilles tendon in the experimental group (with PRP administration) was statistically significant (P<0.05) compared to the control group. TGF- β1 expression in the experimental group established stronger and more extensive staining at weeks 1 and 2 (P < 0.05), while the control group established more extensive staining at week 3 (P < 0,05). Conclusion: PRP administration can increase TGF-β1 expression during the healing process of Achilles tendon rupture in white rats (Rattus norvegicus) and its statistically significant.

Abstract

Introduction: Herpes zoster (HZ) is caused by reactivation of latent varicella-zoster virus (VZV) infection due to disturbance in T-cell mediated immunity. Risk factors include malignancy and therapy related to malignancy. Bladder cancer has a moderate association with HZ, with a risk increment of 10-50%. The rate of HZ in bladder cancer patients is 8/1000. Case description: A 57-years-old man with bladder cancer T4bN3M0 complained of a painful erythematous vesicular rash on the left front abdomen extended to the back 1 week prior, accompanied by fever and burning sensation. He has had regular chemotherapy using Gemcitabine and Cisplatin, and radiotherapy with 2 Gray doses each session. Dermatological examination showed multiple well-defined erythematous-based vesicles, clustered on the left abdominal, flank, and posterior thorax region at T7-T9 dermatome. Tzanck-smear examination showed multinucleated giant cells. The patient was given acyclovir, paracetamol, and wet dressing on vesicles. His chemoradiotherapy was also halted. On the 7th day of evaluation, there were no new lesions and his complaints improved. In cancer patients, CD4+ and CD8+ levels decreased, accompanied by lymphocyte proliferation impairment. Cytotoxic chemotherapy agents work by attacking proliferation cells, affecting immune-related cells. Ion radiation in radiotherapy interferes with a regional cellular immune response that inhibits viral reactivation. Conclusion: Principal HZ management in cancer patients includes antiviral and analgesics. In cancer patients, the occurrence of HZ can disturb and postpone management related to malignancy. The postponement of chemoradiotherapy is based on the patient's conditions and the severity of the disease.

Abstract

Introduction:  Rib fractures can cause patient morbidity that causes long hospitalization if conservative treatment is carried out. Current international guidelines recommend a minimally incisional surgical-based treatment approach to reduce pain and provide optimal clinical benefit. Numerous studies have demonstrated that surgical fixation in conjunction with multidisciplinary integrated care pathways offers the highest therapeutic benefit for developing countries, which are typically more accountable for a higher share of unfavorable global outcomes. This case report describes the selection of appropriate decision-making in the management of multiple rib fractures. Case description: We report that a 51-year-old, still actively working young female came to our emergency department after a motorcycle accident, left chest hitting the road boundary, 2 days earlier. He presented with severe upper chest pain, chest deformity, mild dyspnea, tachycardia, subcutaneous emphysema, and hematoma. The chest radiograph shows a hypo ventilated lung field and a minor pleural effusion. A computed tomography scan showed right lateral rib fractures 3–4 posterior and 5–6 posterolateral, with areas of hyperdensity of simple bone fragments, and pulmonary contusion. Treatment consists of minimal incision with surgical fixation of the 5th and 6th posterolateral ribs which causes pain. For rib fractures 3-4, because it is stable and protected by the scapula, we only perform conservative measures. Rib fixation uses titanium reconstruction plates and cortical fixing screws. The patient's clinical condition improved rapidly postoperatively with a length of stay of 2 days after surgery. Follow-up at 6 weeks, the patient confirmed a full return to preoperative daily activities with the return to normal work activities as well as high quality of life. Conclusion: In this case report, reported that in rib fractures in a developing country, thoracic trauma can be treated through medical treatment with minimal incisions which allows rapids recovery of the patient.

Abstract

Introduction: Currently, the use of Cardiac Implantable Electronic Devices (CIED) is widely used, but in line with the increase in the use of CIEDs, there has also been an increase in the rate of complications. Infection accounts for about 1% of them. All of these were seen in chronically ill patients with multiple comorbid conditions within a few years of implantation. It has not become a standard treatment for infection in the pacemaker bag until now. This study aims to report an immunocompetent patient who developed a pacemaker pocket infection with Escherichia coli bacteria after more than 10 years post-implantation who was successfully treated by moving the bag more medial to sterilization on a pacemaker battery. Case Report: We report an 89 year-old, elderly patient with sick sinus syndrome with a pacemaker pocket infection. The method we used was to explant the battery from the pacemaker and sterilize it with gas, the next day it was reassembled by moving the bag more medially. Loading of the vancomycin antibiotic was carried out for 14 days while still controlling the renal function Conclusion: Staphylococcus aureus causes the majority of CIED infections, especially in immunocompromised elderly patients. Proper treatment of infected pacemaker pouches is necessary with minimal improvement on antibiotics. If left untreated, it can develop into endocarditis which has a high mortality rate.

Abstract

Introduction: Appendicitis is the inflammation of appendix vermiformis and is designated a national health priority issue by the Ministry of Health. Obstruction of the appendicitis lumen is the main etiology of acute appendicitis, and one of the risk factors is eating habits. This study aimed to determine how eating habits are at risk of causing appendicitis in children. Eating habits that were becoming the focus of this study are fiber intake, water consumption, and fast food. Methods: This research is a literature study with the scoping review method, compiled based on PRISMA-ScR. Results: The results obtained are four works of literature stating that low fiber intake and inadequate water consumption in children have a relationship with acute appendicitis, and one piece of literature states that there is no correlation. However, the consumption of fast food has no direct relationship to the occurrence of appendicitis. Conclusion: In this review, a low-fiber diet was significantly correlated with an increased incidence of appendicitis in children.

Abstract

Introduction: Previous studies have shown the risky behavior that causes women’s vulnerability in markets to STI transmission is sexual intercourse with more than one partner. The present study reveals in depth risky sexual behavior and STI prevention among female merchants based on the behavioral theory of the Health Belief Model.  This study aims to determine the risky sexual behavior and prevention of STIs in female merchants based on the behavioral theory of health belief model. Methods: This study uses mix-methods (quantitative and qualitative methods). Using the quantitative method, this study first found a descriptive picture with a cross-sectional design. Then with the qualitative method, it extracted more in depth the experiences and social contexts experienced by the participants. The research sample was taken from 100 female merchants and in-depth interviews were conducted with 20 of those who had had sexual intercourse with an age range of 18-45 years in Denpasar City. Respondents were selected by purposive sampling. Results: Risky sexual behavior by most of the respondents is by having premarital sexual intercourse. Most of the respondents, which is 60%, in the market perceive such action would not risk getting an STI because of loyalty to their partner. In terms of the seriousness of STIs, they assume it is indeed a serious disease. Nevertheless, to prevent STIs, the respondents assure us that avoiding risky sexual behavior is not truly what matters. The respondents believe they can manage to afford not to engage in risky sexual behavior for the prevention of STIs. Conclusion: Most female merchants believe having risky sexual intercourse will not risk causing them to contract an STI. To overcome this, it is necessary to establish special programs to reduce the number of STIs in the community, especially in low-risk groups such as female merchants.

Abstract

Introduction: Arthrogryposis multiplex congenita (AMC) is a complex abnormal condition characterized by multiple congenital contractures in the body's joints. Although this condition is not progressive, if not treated properly, it will increase the degree of disability in children. There are two types of AMC. The most common type of AMC is amyoplasia. The second type is distal arthrogryposis (DA), a genetically inherited type characterized by congenital contractures in multiple joints and no neurologic or muscular abnormalities. The incidence of AMC 1:3000 births with clubfeet and congenital hip dislocation is the most common. In this retrospective study, the authors described the profile of AMC patients who underwent surgery at our center with the approach and management. Method: The study design was a descriptive retrospective. The population of this study is all AMC patients who underwent surgery at the orthopedic hospital of Prof. Soeharso Surakarta, Indonesia. We used the total sampling technique who underwent surgery in the 2018-2021 period. Result: In this study, we got eleven AMC patients who underwent surgery. Distribution based on sex found six boys and five girls. Half of the patients underwent surgery under the age of three years, while the other half underwent surgery after the age of five. The presence of patients who are consulted at an older age indicates a lack of public literacy about the abnormal condition of AMC or a lack of concern for the condition. Almost all AMC patients who undergo surgery are in the lower extremities. Conclusion: Rehabilitation plays a very important role in improving the quality of life for AMC patients. A multidisciplinary approach is needed in the treatment of AMC patients. Surgery is performed if stretching and external support splinting/casting cannot achieve functional ROM and repair the deformity.

Abstract

Introduction: One of the most common diseases in men is benign prostatic hyperplasia (BPH). In this condition, the prostate gland becomes larger, thus it can clamp the urinary tract area and cause obstruction. Obstruction in the lower urinary tract increases the high risk of urinary tract infections (UTI). When the prostate volume becomes larger, the levels of prostate-specific antigen increase. This research aims to determine the correlation between PSA levels and prostate volume in BPH patients with urinary tract infections (UTI). Methods: This study used an analytic observational method with a cross-sectional design. Data were taken from the medical records of BPH patients suffering from UTIs at Bethesda Hospital Yogyakarta from January 2018 to December 2020. Data collection was carried out from January 17 2022 to March 17 2022. The data used in this study were selected using inclusion and exclusion criteria so that the obtained 33 medical record data were analyzed. The data used in this study were analyzed using SPSS software. The univariate analysis describes the distribution of data. Bivariate analysis used the chi-square test so that it could show the relationship between the independent variable (prostate volume) and the dependent variable (PSA level). Result: The data show moderate prostate volume (30-80cc) in 17 (51.5%), high prostate volume (>80ml) in as many as 9 patients (27.3%) and normal prostate volume (<30 ml) in as many as 7 patients (21.2%). High PSA levels are 26 (78.8%) and normal PSA levels are 7 patients (21.2%) that noted. Bivariate analysis data with a chi-square test shows that the significant score is 0.019 (p<0.05).   Conclusion: The research shows an association of PSA levels and prostate volume in BPH patients with UTI.

Abstract

Introduction :Inverted papilloma is a benign tumor that generally arises from the lateral wall of the nasal cavity and mostly grows on the middle meatus.The incidence is rarely found in the ear, throat, nose, throat, head and neck, around 0.5% -4% of all nasal and paranasal sinus tumors.The etiology of inverted papilloma remains unknown.Clinical features of inverted papilloma are unilateral nasal obstruction, rhinorrhea, epistaxis, anosmia, tinnitus and other symptoms.The diagnostic examination are histopathological examinations, CT scans and MRI.Histopathology is the gold standard in diagnosing of inverted papilloma.Management of inverted papilloma is carried out with surgical approach including lateral rhinotomy, maxillectomy, degloving, and non-invasive surgery via endoscopy depend to the krouse classification. Case description : A 44 year-old female patient diagnosed with inverted papilloma was reported.The patient complained of progressiveright nasal obstructionappeared two years ago that turn into total obstruction and rhinorrhea in the last 6 months.A biopsy was done with the result of inverted papilloma. CT scan of the paranasal sinuses showed a mass filling the right nasal cavity, right maxillary sinus and right ethmoid sinus. Patientwas diagnosed with sinonasalinverted papilloma with krouse 2 classificationbased on clinical examination, radiological finding and preoperative endoscopy. Patient was treated with sinonasal inverted papiloma extirpation via endoscopic approach. Conclusion: Inverted papilloma is a benign tumor of the nasal and paranasal sinuses, and biopsy is the gold standard for diagnostic tests. At the same time, the main treatment is done by surgical approachdepend on the krouse classification.
