Abstract

Background: Vulvovaginalis Candidiasis (VVC) is a vulva and vaginal infection of Candida, which one of predisposition factors is systemic antibiotics. Humoral immunity does not play a role on VVC, thus VVC in HIV/AIDS patients is as much as VVC in immunocompetent patient. Purpose: Identify the profile of Candida sp. of VVC in HIV/AIDS patients who got systemic antibiotics. Methods: Descriptional study to identify the pattern of Candida sp. of VVC in HIV/AIDS patient who got systemic antibiotic with cultures of Saboroud Dextrose Agar, Cornmeal Tween80 with Carbohydrat Test and Candida CHROM agar. Results: Broad-spectrum antibiotics were given to HIV/AIDS patient with VVC at Dr. Soetomo general hospital Surabaya two times higher than narrow-spectrum antibiotics (66,7% vs. 33,3%), they were ciprofloxacin (42,8%), ceftazidime (19,1%) and ceftriaxone (4,8%); the  narrow-spectrum antibiotics were cotrimoxazole (19,0%) and rifampicin (14,3%). Conclusion: C.albicans (57,1%) and C. glabrata (9,6%) were found in VVC of HIV/AIDS patients that have been given broad-spectrum antibiotics, while C. albicans (28,6%) & C. glabrata (4,7%) also found in VVC of HIV/AIDS patients that have been given narrow-spectrum antibiotics. Abstract

Leprosy reaction contributes to disability due to peripheral nerve damage. Nerve conduction study (NCS) provides a profound physiological description of peripheral nerves. This study aims to report a case of polyneuropathy in leprosy with type 2 reactions (T2R) which is evaluated using NCS. A 33-year-old woman complain of painful bumps in her arms and legs, fever, swollen feet since 2 days ago, and history of leprosy. Dermatologic examination on the right superior palpebra, right and left arms and legs revealed multiple tenderness erythematous nodules; right claw hand; and both legs oedema. Slit skin smear revealed positive result. Histopathologic examination supported T2R description. The NCS examination concluded severe axonal demyelinating motoric sensoric polyneurophaty, with left worse. She was treated with MDT-MB, bed rest, orally methylprednisolone, vitamin B, paracetamol, ferrous sulfas, and topical olive oil. Clinical improvement was achieved after 2 weeks. The NCS is used to assess the nerve impuls conduction along the peripheral nerves. In this case, it was found that NCS could showed early neuropathy in nerves that were clinically undetectable. It can be concluded that the NCS examination is an important diagnostic modalities for early detection of neuropathy and confirmed the diagnosis of clinical neuropathy in leprosy.

Abstract

Response to attack Acute Coronary Syndrome (ACS) significantly contributes to the incidence of complications and death in ACS patients. This response is concerned with making decisions regarding emergency care. The family is one of the decision makers regarding emergency care for ACS patients. Economic problems and the ownership of health insurance can cause a poor response. Analytic observational design, cross sectional was used in 104 patient families (parents / husband / wife / children) who are in charge of care for ACS patients at the Ende Regional General Hospital using a modified Acute Coronary Syndrome Response Index questionnaire. Less response was found in 42.3% of respondents. There was a significant relationship between response and family income (P = 0.000, OR = 0.599) but not with health insurance ownership (P = 0.359; OR = 0.090). Response is less closely related to economic problems experienced even though the patient has been insured. Therefore, it is important to develop strategies to increase the response (knowledge, attitudes and beliefs) of families related to SKA, especially in low-income groups. This will help them speed up decision making when SKA's re-attack occurs.

Abstract

Introduction: The evidence of transplancental transmission of Morbus Hansen (MH) in animal models is a challenge for MH treatment in pregnancy because it brings anxiety for patients and their fetuses. Case: A 31-year-old pregnant woman, first trimester, came with complaints of reddish numbness rash on her hands since 2 years ago. Dermatological examination showed erythematous plaque, multiple, well-defined, irregular borders with hypoaesthesia. There were right and left ulnar nerve enlargement. Acid Fast Bacteria (AFB) from ear lobes and lesion revealed Bacterial Index +2 and Morphological Index 10%. Histopathological examination showed gren zone which indicate MH type BL. Patient was diagnosed with MH Multi Basiler (MB) on G3P2002Ab000 and given MDT MHMB. Postpartum IgM and IgG anti-PGL-1 examination of cord blood and infant blood, also PCR examination of cord blood and amnion showed negative results. Histopathological examination found no AFB on the umbilical cord and placenta. Conclusion: MDT is effective in preventing transplacental transmission of MH from mother to fetus.

Abstract

Osteoarthritis is a chronic joint disease that causes pain, disability, and loss of function. More than 80% of cases of osteoarthritis affect the knee joint. The exact cause of knee osteoarthritis is still unknown, but it is generally associated with aging and obesity. This study aimed to determine the relationship between age and the severity of knee osteoarthritis. This research used a cross-sectional study involving patients who suffered from knee osteoarthritis. Data on gender and the severity of osteoarthritis was collected from medical records. The severity of knee osteoarthritis was assessed using the Kellgren-Lawrence system. The data was analyzed using the Spearman correlation. There were 3 patients (8.1%) in the category of late adulthood, 7 patients (18.9%) in the middle-aged category, 13 patients (35.1%) in the early-onset elderly category, and 14 patients (37.8%) in the elderly. Seven patients (23.7%) suffered from osteoarthritis grade 1, 13 patients (35.1%) had grade 2, 16 patients (43.2%) had grade 3, and one patient had grade 4. The Spearman test showed that there was no significant relationship (p>0.05) between age and the severity of knee osteoarthritis. According to the study's findings, there was no correlation between age and the severity of knee osteoarthritis in women and men patients at Baptis Hospital, Batu City.

Abstract

As one of the countries with the highest prevalence of leprosy sufferers, this problem must be seriously addressed. So far, most of the treatment is still done on the medical aspect, but not much has been done to shape the psychology of the patients or OYPMK. Therefore, it is necessary to do real interventions that help from the psychological side of the patient or OYPMK. This service method uses psychological-based training, which includes several activities, namely opening, initial assessment, introduction, recognizing self-potential, forming positive perceptions, introduction to time management, increasing fighting power, training evaluation, and closing. As a result, this training has succeeded in achieving the goals set for each activity. In addition, the participants also gave testimonies of positive changes in their self-acceptance. Thus, this training activity can be a structured guideline for partners who want to implement it.

Abstract

Osteoarthritis is a chronic disease characterized by the destruction of cartilage in the joints, causing stiffness, pain, and impaired movement. The condition most commonly affects the joints of the knees, hands, feet, spine, and often the shoulders and hips. Knee osteoarthritis is a major public health problem that causes chronic pain and reduces physical function and quality of life. This study determined the relationship between the degree of Osteoarthritis and the pain level in patients at Baptist Hospital, Batu City. This research used a cross-sectional study with 31 respondents. Data collection was conducted directly using the WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index) questionnaire, modified in Indonesian. To determine the grade of Osteoarthritis using the Kellgren-Lawrence system. The results showed that out of 31 respondents, there were nine patients (29%) who had grade 1 osteoarthritis, 12 patients (38.7%) had grade 2, and 10 patients (32.3%) had grade 3. There were 22 patients (71%) who had a mild pain level, four patients (13%) had a moderate level, and five patients (16%) had a severe pain level. The Spearman correlation test showed no significant relationship (p>0.05) between the degree of Osteoarthritis and the level of pain in patients. This study concludes that there was no relationship between the increasing degree of Osteoarthritis and the level of pain.
