Abstract

Introduction: Lung cancer is one of the types of malignancies that is the leading cause of cancer deaths worldwide. In Indonesia, the incidence of lung cancer is quite high, with pulmonary TB as a risk factor that can increase the risk of lung cancer in the future. This study aims to describe the histopathological profile and radiological picture of lung cancer patients in patients with and without a history of pulmonary TB at dr. Zainoel Abidin Hospital Banda Aceh. Methods: This study uses retrospective methods and univariate analysis. The subjects of this study are lung cancer patients at dr. Zainoel Abidin Hospital from January 2022 to December 2023. Result: The results of this study showed that of the 184 patients, 6.5% of them had lung cancer with a history of pulmonary TB, while the other 93.5% had lung cancer without a history of pulmonary TB. Of the two groups of lung cancer, the majority of patients were aged 51-60 years (32.6%), male (83.7%), still working (81%), from Aceh (99.5%), and Muslim (99.5%). Most lung cancer patients were diagnosed at stage IV (86.4%), with Squamous Cell Carcinoma being the most common type of histopathology (57.1%). In lung cancer patients with a history of pulmonary TB, post-TB lesions are often found, such as fibrosis (83.3%), calcification (50%), cavitation (33.3%), pleural thickening (16.7%), and atelectasis (8.3%). Conclusion: Efforts to prevent lung cancer, especially in patients with a history of pulmonary TB, need to be continued so that the risk of developing lung cancer can be reduced.

Abstract

Introduction: As the fourth leading of cancer death among worldwide for women, cancer of ovary remains a serious public health issue. Late-stage diagnosis is common and significantly reduces the 5-year survival rate—from 92% in early stages to just 17%–28% in advanced cases.  Unspecific symptoms of ovarian cancer cause most patients to present at an advance stage. Histopathology plays a role in diagnosis, prognosis, and decision-making for patient management. Recognizing histopathology early on helps direct therapy and improve patient outcomes. To date, however, no researches have been established on the relationship between ovarian cancer stage and histopathology type at Dr. Zainoel Abidin General Hospital Banda Aceh, Indonesia. Methods: This study utilized a retrospective cross-sectional design, gathering data from medical records from patients with cancer of ovary treated between January and December 2024. Statistical analyses, including univariate and bivariate tests, were conducted using the Kruskal-Wallis method and SPSS version 26.0 Result: Analysis of the relationship between surgical stage and histopathological features revealed a statistically significant correlation, with a p-value of 0.000. Conclusion: Surgical staging can be considered when developing appropriate therapeutic strategies while awaiting histopathology results. This allows patients to receive treatment as soon as possible.

Abstract

Introduction: An arteriovenous fistula (AVF) is the preferred method of vascular access for patients with end-stage chronic kidney disease (CKD) undergoing hemodialysis. This study aimed to investigate whether there is a difference in the post-operative maturation of brachiocephalic AVFs at 6 weeks compared to 8 weeks in CKD patients, utilizing Doppler ultrasonography at RSUDZA Hospital. Methods: The study involved a sample of 16 patients who underwent brachiocephalic arteriovenous fistula (AVF) surgery at Zainoel Abidin General Hospital (RSUDZA) in Banda Aceh, between November 13 and December 14, 2023. This observational study utilized a prospective cohort design, and data analysis was performed using the Wilcoxon test, with a significance level set at p < 0.05. Results: The study found that most patients who underwent brachiocephalic arteriovenous fistula (AVF) procedures were women, particularly those aged between 56 and 65 years. Doppler ultrasonography was used to evaluate the patients at 6 and 8 weeks after the surgery. The results showed that 40% to 50% of patients had not met the maturation criteria set by KDOQI by the 6th week, and 30% to 40% still had not met these criteria by the 8th week. Conclusion: There is no significant difference in the post-operative maturation of brachiocephalic AVF for 6 weeks and 8 weeks in CKD patients using Doppler ultrasonography at RSUDZA hospital.

Abstract

Introduction: Invasive papillary carcinoma represents a rare subtype of breast cancer, constituting about 0.5% of all invasive breast cancers and mainly occurring in postmenopausal women. Because most information comes from limited case reports and small retrospective analyses, awareness of this tumor remains low, posing challenges for clinical management. There are distinct pathological characteristics and biological behaviors seen in invasive papillary carcinoma, according to the research.  One reason for its generally good prognosis is because it is seldom involved with lymph nodes and is classified as a luminal subtype. [1, 2]. Case Presentation: We report a rare case of Invasive Papillary Breast Carcinoma in a 48-year-old premenopausal woman who presented with a gradually enlarging, painless retroareolar mass in her right breast, persisting for four months. The patient previously underwent an incisional biopsy at another hospital, and the histopathology results showed invasive papillary breast carcinoma. The patient has undergone a chest  CT-scan for diagnostic confirmation and surgical guidance. The patient underwent a right-sided modified radical mastectomy, and histopathological examination of the specimen confirmed the diagnosis of Invasive Papillary Carcinoma. Immunohistochemical analysis further validated this diagnosis. Conclusion: Although the treatment of Invasive Papillary Breast Carcinoma generally aligns with that of more common breast cancers, its protocol remains unclear due to the condition's rarity. We chose to present this Invasive Papillary Breast Carcinoma case because of the disease's uncommon nature, the severity of the symptoms, and the need for urgent intervention.

Abstract

Introduction: A diaphragmatic hernia is a rare condition. A Morgagni hernia is one of four type of diaphragmatic hernia and it’s located in anterior and retrosternal location. This condition can be caused by trauma or congenital condition. The defect of Morgagni originally is small. Incidental finding during X-ray examinations performed for symptoms not related to the hernia or when symptoms develop as a result of incarceration, strangulation and visceral rupture inside the chest cavity. Case Report: In this case, we present a 35-year-old man with a 5-year history of stomach discomfort and episodes of severe dyspnea who arrived to the emergency room, during his medical work-up we incidentally found an intrathoracic gastric bubble. A laparoscopy surgery was performed but the operator did not find any holes.  We decided to perform thoracotomy; the findings were in the form of intestinal entry into the thoracic cavity without any sign of incarcerated or necrotic tissue. Then, a laparotomy was performed by digestive surgeon to returns the intestine to abdominal cavity.  Thoracic surgeon was involved to close the defect, which is a large defect, for that reason we choose to use prolene mesh. Discussion: This pathology is infrequent in adults, among this age group, there are two different clinical presentations: asymptomatic patients who are diagnosed incidentally when abdominal organs are found in the thorax in a chest CT scan, and symptomatic patients due to side effects of incarceration, strangulation, hemorrhage and visceral perforation in the chest cavity. Morgagni’s defect was found to be large so it was decided to use a mesh. Conclusion: Diaphragmatic hernias are rare among adult population, and they are usually asymptomatic, in this case we presented a symptomatic patient, diagnosed with a chest radiograph and treated surgically. The surgical approach for the resolution of this pathology is variable and it depends on the presence and severity of visceral complications. Abstract

Introduction: Traumatic brain injury (TBI) from trauma like gunshot wounds occurs when a sharp object penetrates the skull and brain tissue. Intracerebral Hemorrhage (ICH) and Subdural Hemorrhage (SDH) are common head trauma-related bleedings. ICH results from torn brain blood vessels, causing bleeding into brain tissue, while SDH occurs when bridging veins between the brain and dura mater rupture. Case description: A 4-year-old child presenting with decreased consciousness. Eight hours before admission, the patient was playing with his friend. While playing, the patient was hit by a gunshot wound to the head. After the incident, the patient complained of head pain that continued to worsen. On physical examination, there was tenderness and a subgaleal hematoma in the left parietal region. MSCT examination of the head without contrast was then performed on the patient. The patient underwent craniotomy to evacuate ICH, SDH, and extraction of bullet corpus alienum in the temporal region with C Arm guiding, with a total duration of 3 hours. The patient received treatment for 5 days after surgery. Conclusion: Intracranial pellet injuries from air gun shots can be fatal if they affect critical brain areas like the hypothalamus, thalamus, or brainstem. If accessible, prompt removal via craniotomy can improve outcomes by evacuating bleeding and relieving symptoms like headaches.

Abstract

Background: Endometriosis is a common gynecological condition with a prevalence of 2–10% among women of reproductive age. Secondary infection of endometriotic cysts constitutes an uncommon yet potentially severe complication that may arise through multiple mechanisms, including trauma-mediated cyst rupture. Case Presentation: The present case describes a-36-year_old multiparous woman with fever after abdominal massage for chronic pelvic discomfort.. The patient had previously undergone surgery for endometriosis. in 2021 and chronic dysmenorrhea. Physical examination revealed a fixed abdominal mass, and ultrasonography showed an 11.5×9.5×7.9 cm cystic mass with ground glass appearance. Laboratory findings showed elevated leukocytes (29,500 cells/mm³) and CA125 (261 μg/dL). After antibiotic treatment, unilateral salpingo-oophorectomy was performed. Intraoperatively, infected endometriotic cyst with purulent material was found. Conclusion: This case highlights the risk of endometriotic cyst rupture and subsequent infection following abdominal trauma, even from traditional massage therapy. Early detection and appropriate surgical care are critical for good results.

Abstract

Introduction: Coronavirus disease 2019 or COVID-19 infection has showed an immunologic disregulation that could affect the clinical outcomes. Neutrophil lymphocyte ratio or NLR is one of inflammation marker that rapid and easily available at the initial moment of hospitalization. This study aimed to determine the correlation between NLR and length of stay of COVID-19 patients at Dr. Zainoel Abidin General Public Hospital Banda Aceh. Methods: This was an analitic retrospective study that observed medical records and complete blood count data of 577 COVID-19 hospitalized patients on January – March 2022 .The sample of this study was elected by purposive sampling and the sample size was 83 that obtained by lemeshow formula (1990). Result: The results showed that COVID-19 was dominated by male (53%) and 26 – 35 age group (19.3%). The value of NLR was dominated by ≥ 3.13 group (60.2%). The length of stay was dominated by ≤ 11 days group (69.9%). Bivariate analysis of this study was Spearman Rank test. The results of correlation test obtained p-value was 0.003 and r was 0.319. Conclusion: This study obtained that was a significant correlation between NLR and length of stay of COVID-19 patients at Dr. Zainoel Abidin General Public Hospital Banda Aceh.

Abstract

Introduction: Thrombosis is a complication that may arise in End-Stage Renal Disease (ESRD) patients with an arteriovenous fistula (AVF), usually caused by stenosis (narrowing of the vein more than 50% of its diameter) and changes in blood flow rate (BFR). The aim of this study is to explore the relationship between blood flow rate and the incidence of AVF thrombosis in patients with ESRD at Dr. Zainoel Abidin General Hospital Banda Aceh. Methods: In this study, a cross-sectional design was employed to investigate ESRD patients who had undergone the AVF procedure at Dr. Zainoel Abidin General Hospital Banda Aceh from October to November 2023. The study examined Blood Flow Rate values obtained via Doppler ultrasound. The data were analyzed using SPSS version 26 and the contingency coefficient (Cramer's V) to assess the relationship between the variables. Result: 51 patients were sampled. The average BFR value six weeks after surgery in the event of thrombosis and non-thrombosis was 274 ml/min and 786,8 ml/min. There was a significant correlation between BFR values with the incidence of AVF thrombosis (p = 0.000 and Cramer’s V = 0,658) Conclusion: Inadequate blood flow through the fistula may contribute to clot formation, leading to fistula failure in patients undergoing hemodialysis. Optimizing monitoring and early intervention for blood flow in AVF may help reduce the incidence of thrombosis and improve patient outcomes.

Abstract

Introduction : Among the gravest complications occurring during childbirth are fourth-degree perineal lacerations, characterized by complete disruption of the anal sphincter mechanism with potential for persistent sequelae including fecal incontinence and impaired sexual health. These traumatic injuries profoundly compromise women's wellbeing, with sexual health disturbances documented in 30-60% of affected women three months following delivery. The Female Sexual Function Index (FSFI) provides validated assessment across six critical dimensions: libido, arousal capacity, vaginal lubrication, orgasmic function, overall satisfaction, and dyspareunia. Case presentation: We present the clinical course of a 33 year old multiparous woman who sought consultation nine months postpartum with chronic fourth-degree perineal disruption accompanied by anal incontinence and sexual dysfunction. Initial FSFI assessment revealed a score of 16.2, demonstrating substantial sexual impairment. Surgical intervention employed a comprehensive ten-stage reconstruction protocol involving systematic restoration of anal mucosa, internal and external sphincter components, vaginal wall integrity, and perineal tissue architecture. At five-month postoperative evaluation, the patient exhibited complete continence restoration and remarkable sexual function recovery, with FSFI scoring advancing to 28.0, indicating normalization of sexual health parameters. Quality of life indicators demonstrated meaningful enhancement across all assessed dimensions. Conclusion: Secondary reconstruction of chronic fourth-degree perineal disruptions can successfully restore both continence mechanisms and sexual function when executed using meticulous surgical methodology. Prompt recognition and appropriate timing of intervention remain paramount for achieving optimal patient outcomes and restoring quality of life.
