Abstract

Background: Extensive literature has established a strong correlation between breastfeeding and a decreased susceptibility to breast cancer. The objective of this study was to examine the breastfeeding status of moms diagnosed with breast cancer. Methods: This study employs a descriptive-analytical approach with a cross-sectional design. The study comprised adult female patients (aged 20-60) who were diagnosed with Breast Cancer, irrespective of the specific kind of cancer confirmed by anatomical pathological investigation. Only non-smokers were considered for analysis. The study excluded patients who had a history of malignancy and those with a family history of cancer. A bivariate analysis was performed to evaluate the study hypothesis. The analysis was conducted utilizing the Chi-Square and unpaired T-test statistical methods using SPSS software. A p-value less than 0.05 is deemed statistically significant. Result: This research encompassed a total of 45 individuals diagnosed with breast cancer. The majority of individuals diagnosed with breast cancer (55.6% of the total) did not engage in exclusive breastfeeding. Similarly, individuals without a breast cancer diagnosis also tended to refrain from exclusive nursing (24.4%). The association between the two variables is statistically significant, as indicated by a p-value of 0.000. The average length of nursing in individuals diagnosed with breast cancer was 2.04 ± 1.81, but in those without breast cancer, it was 10.10 ± 9.01. The statistical tests indicate a substantial association between the two variables, as evidenced by a p-value of 0.000. Conclusion: A notable correlation exists between breastfeeding and a diminished likelihood of developing breast cancer.

Abstract

Background: Upper eyelid contracture is a regular occurrence after severe facial burns. Numerous problems, such as corneal ulcers and exposure keratitis, may result from it. It is difficult for plastic surgeons to operate in this situation to preserve worst-case scenarios and enhance palpebral function. Methods: In this piece, we showcase two instances: (1) Eight months after suffering a chemical burn injury, a 26-year-old male was diagnosed with bilateral superior palpebral contracture with iris prolapse. Both eyes' visual acuity was 6/6 - 1/¥. A 75-year-old female patient was identified as having a corneal ulcer and a contracture on her left upper eyelid. Both eyes' visual acuity was 20/6 - 1/¥. In order to restore the top eyelid in both patients, we undertook a contracture release and a full-thickness skin graft. Results: Evaluation five days after surgery revealed no evidence of graft lysis and that the graft had taken nicely. On the patient graft from the second, we discovered shifting, although new epithelization will take place. Both of the patients had good eyelid looks following restoration. Conclusion: Plastic surgeons should be aware of the seriousness of significant burn injuries to the face. Patients must receive appropriate instruction on the significance of avoiding consequences from eyelid contractures. In this instance, creating the top eyelid via a skin graft and contracture release technique made sense. We acknowledge that one of the limitations of our study is the brief observational period of the patient's conditions.

Abstract

Introduction: Because SLE patients receive immunosuppressive and glucocorticoid medication, they are more susceptible to opportunistic infections. Multiple brain abscesses are an extremely uncommon side effect of systemic lupus erythematosus (SLE). Case description: The purpose of this case study is to raise awareness of the 14-year-old girl who reported experiencing weakness in her limbs. A year ago, she received a diagnosis of systemic lupus erythematosus. Upon physical examination, nervus VII central paresis was discovered; the lower extremities' muscular strength was 3333/2222 and the upper extremities' 3333/2222. A head CT scan revealed findings that were consistent with lupus cerebritis. Multiple ring lesions with extensive edoema were detected by magnetic resonance imaging, raising the possibility of multiple brain abscesses. For six weeks, the patient had intravenous metronidazole and ceftriaxone. The patient was allowed to go home with instructions to keep taking corticosteroids and mycophenolic acid as well as to schedule routine outpatient appointments. This case study aims to raise awareness about the 14-year-old girl who reported experiencing weakness in her limbs. Her condition was identified by specialists as systemic lupus erythematosus a year ago. Conclusion: It is critical to being aware that individuals with impaired immune systems should always be suspected of having an opportunistic central nervous system infection when they experience new neurologic symptoms. A brain abscess is a chronic infection of the central nervous system that can arise from a number of risk factors and infection sources. A brain abscess is one of the uncommon side effects of SLE.

Abstract

Background: The Wide Awake Local Anesthesia No Tourniquet (WALANT) has been reported to offer simpler pre-surgical instruction and assessment, faster operating time, better surgical visualization, shorter hospital stay, fewer complications, and side effects, lower cost, faster postoperative recovery time, better patient's satisfaction, and less pain and discomfort. This study aims to review the pain score in hand surgery using the WALANT technique compared to local anesthesia or general anesthesia with a tourniquet. Methods: A comprehensive literature search was conducted in PubMed, Cochrane Library, and Google Scholar in March 2021. Eligible randomized-controlled trials (RCTs) and cohort studies comparing injection time, intra-operative, or postoperative pain using Visual Analog Score (VAS) in WALANT and local anesthesia or general anesthesia with tourniquet were included. Results: Five studies (3 RCTs and 2 cohorts) were included in this article, including 645 hand surgeries (376 carpal tunnel syndrome cases, 181 trigger finger cases, 42 cubital tunnel syndrome cases, 44 de Quervain's disease cases, and 2 ganglion cyst cases). Three studies reported a significantly lower VAS in the WALANT group, while the other two studies reported a lower VAS in the WALANT group, but it was not statistically significant. Conclusion: WALANT is still a technique worth considering in hand surgery because of its superiority in minimizing intra-operative and postoperative pain.

Abstract

Background: A rare congenital abnormality known as "mirror image polydactyly" of the feet is typified by symmetrical duplication of the foot's fingers. According to Fukuzawa et al., only 28 cases—seven of which include treatment records—have been described in the English literature. Mirror foot can arise as a single congenital defect or a component of a hereditary syndrome involving several congenital defects. Methods: Our outpatient department received a referral for a 3-month-old infant who was born with unilateral mirror-image polydactyly of the foot, along with congenital talipes equinovarus (CTEV) and congenital genu recurvatum. A radiographic examination verified that the left foot had nine metatarsals and ten fingers. Five digits were ablation after preaxial duplicates of the first through fourth metatarsals were disarticulated. Results: Rebuilding a functional and aesthetically beautiful foot was the goal of the surgical care of this case, which will help orthopaedic surgeons manage CTEV for serial cast correction. The foot appeared excellent six months following the surgical procedure. The youngster was referred to the pediatric orthopaedic facility in Jakarta after the local orthopaedic department managed the CTEV without incident. Conclusion: Mirror feet are uncommon, and each patient needs treatment specifically designed for them. Coordination across various departments should be pursued to provide patients with the best care possible.

Abstract

Introduction: As much as 40% of low back pain patients are caused by degenerative disc disease. The current treatments are fusion stabilization and total disc replacement, which have a risk for adjacent segment disease. These suboptimal results have become the beginning of the development of regenerative therapy. The success of this therapy will increase if the scaffold meets the ideal conditions. Biological scaffolds provide a cell growth environment that resembles the original tissue. Bovine intervertebral nucleus pulposus can become promising scaffolds. Objective: This research describes the proper freeze-drying approach for decellularizing bovine nucleus pulposus to create a biological scaffold. Methods: We conducted an experimental post-test control design study using bovine coccygeal nucleus pulposus material. All treatment groups underwent freeze-drying with Buchi Lyovapor™ L-200/L-200 Pro. All groups were evaluated for the level of decellularization using the Quick-DNA™ Miniprep Plus Kit (Zymo Research®) and remaining glycosaminoglycan levels by Alcian Blue staining. Results: Comparison of DNA concentrations obtained p-values respectively <0.0001 (p <0.05), which means that all the treatments showed a decrease in DNA concentration compared to the control group. The comparison of the glycosaminoglycan percentage between the P1 vs. Control group obtained a value of p=0.381 (p>0.05), which means that the glycosaminoglycan percentage results for the P1 group were not significantly different from the control group. Conclusion: This study of group P1 showed that decellularization of the bovine nucleus pulposus by freeze-drying technique can form an excellent biological scaffold.

Abstract

Introduction: Tuberculosis (TB) is an infectious disease caused by the bacterium Mycobacterium tuberculosis and is one of the leading causes of death worldwide. Indonesia ranks second in global TB case counts, with a high transmission rate, particularly in high-risk environments such as Islamic boarding schools. This study aims to evaluate the knowledge, attitudes, and behaviors related to TB among students at Dayah Madrasah Aliyah Swasta Babun Najah in Banda Aceh. Additionally, it will analyze the relationship between their knowledge, attitudes, and behaviors in the context of TB prevention. Methods: This study employed an observational analytic method with a cross-sectional design and utilized cluster sampling. Primary data were gathered through questionnaires. The research was conducted at Dayah Madrasah Aliyah Swasta Babun Najah, located in the Ulee Kareng District of Banda Aceh, on August 29, 2024. Bivariate analysis was performed using the non-parametric Spearman's rank correlation coefficient test. Results: A total of 90 students participated in the study. The findings revealed that 67.8% of respondents had good knowledge, 78.9% demonstrated positive attitudes, and 47.8% exhibited good behaviors regarding tuberculosis (TB) prevention. Spearman Rank analysis indicated a moderately strong positive correlation between knowledge and attitudes (r = 0.527) and between knowledge and behaviors (r = 0.535), with a p-value of less than 0.025. Conclusion: Enhancing knowledge among students is crucial for fostering better attitudes and behaviors related to TB prevention, particularly in high-risk environments.

Abstract

Trigeminal neuralgia (tic douloureux) is a rare and debilitating facial pain condition affecting one or more branches of the trigeminal nerve. It is frequently misdiagnosed as a dental issue or temporomandibular disorder due to overlapping symptoms, delaying appropriate treatment. Diagnosis is primarily clinical, often requiring the expertise of a neurologist to distinguish it from other causes of facial pain. Imaging studies such as MRI or CT scans are not diagnostic but play a crucial role in ruling out secondary causes and identifying pathological changes in the affected nerve root. Although not life-threatening, trigeminal neuralgia significantly impacts the quality of life, causing severe, recurrent pain episodes that can be challenging to manage. Timely and accurate diagnosis, along with the implementation of effective therapeutic strategies, is essential to alleviate symptoms and improve patient outcomes. This review highlights the clinical presentation, diagnostic approach, and current treatment modalities for trigeminal neuralgia, emphasizing the importance of a multidisciplinary approach to care.

Abstract

Introduction Craniotomy and hematoma evacuation is the gold standard of treatment for epidural hematoma (EDH). However, minimal invasive surgery for EDH might come in handy in conjunction or as a standalone therapy for EDH, as some of them might be easier to perform in a dire situation. In this literature review, we aim to review the available literatures regarding minimally invasive surgery for EDH. Method This literature review described the available studies about minimal invasive surgery for EDH. Result Thirteen studies were identified. The procedures consisted of burrhole-based approach, minicraniotomy, endovascular, and image-guided evacuation. Several of the procedures were performed on hypodense-appearing EDH, some on post-operative EDH, and one on venous-sinus-involving EDH. The majority of studies reported favourable outcome after minimal invasive surgery. Conclusion Available literatures showed that most minimal invasive surgery can be performed quickly and is thus suitable for the emergency nature of EDH. Liquified-appearing EDH, post-operative EDH, or EDH situated near the dural venous sinus were among the reasons to pursue minimal invasive EDH evacuation. Minimal invasive surgery for EDH generally resulted in favourable outcome. Larger studies are required to validate the benefit of minimal invasive surgery as a standalone therapy or in combination with standard craniotomy for EDH.

Abstract

Introduction: Intraventricular meningiomas account for approximately 1.5% of meningiomas, 80% of these were localized in the lateral ventricles, 15% in the third ventricle (mostly in the ventricle trigone), and the remaining 5% in the fourth ventricle. This case report aims to highlight clinical findings and difficulty on removal of the tumor intraoperatively. Case description: We presented 2 cases of Intraventricular Meningioma. Both patient have a chief complaint of chronic headache since 2 years ago. Through the magnetic resonance imaging examination, we found a  solid mass at right ventricle trigone with enhance homogenously 7.3 x 5.5 x 6.5 cm  on the first patient and 6.8 x 8.4 x 3.2 cm on the second patient. Both patients went through craniotomy tumor removal. The pathology showed a Transitional Meningioma who grade I and KI-67 immunohistochemical staining result: 1-2 % on the first patien and angiomatous meningioma (WHO grade 1) on the second patient. Both patients showed improvement post-surgery Conclusion: We  have  successfully  performed  a  total  craniotomy  tumor  removal  on  the  right  trigone  intraventricular  meningioma.  However, the primary challenge lies in excising intraventricular meningiomas located in the trigone without causing injury to the geniculocalcarine tracts. Therefore, the surgical approach must be carefully planned based on the tumor's location and intraoperative monitoring as well as surgical strategy to avoid the neurological deficits.
