Abstract

Ovarian cancer is a major malignancy in the female reproductive system. Treatment for this cancer can involve surgical procedures or the use of chemotherapy drugs, either orally or intravenously. This study was conducted to evaluate the use of chemotherapy drugs in ovarian cancer patients at RSUP Fatmawati. Data collection was carried out through a retrospective observation by gathering information on drug usage from the patients' medical records, which included data from 31 patients receiving treatment at RSUP Fatmawati in September 2024. Based on the obtained data, it was found that the most commonly used chemotherapy drug for cancer patients at RSUP Fatmawati is TAC (Paclitaxel + Carboplatin), accounting for 80.65%. Additionally, based on the chemotherapy regimens used at RSUP Fatmawati, the recovery percentage from several regimens ranged from 54.90% to 131.93%.

Abstract

Systemic Lupus Erythematosus (SLE) merupakan penyakit autoimun kompleks yang menyerang berbagai sistem tubuh. Penyakit ini ditandai dengan pembentukan autoantibodi patogenik terhadap asam nukleat dan protein pengikatnya akibat adanya intoleransi terhadap komponen tubuh. Pasien SLE membutuhkan penekan imun untuk mengurangi inflamasi akibat autoantibodi tersebut serta digunakan berkepanjangan untuk mencegah perburukan kondisi dan mengurangi resiko kekambuhan. Salah satu obat penekan imun yang menjadi tatalaksana penyakit SLE ialah kortikosteroid seperti metilprednisolon (MP). Sayangnya, walaupun terdapat penggunaan regimen obat MP, namun penggunaan MP jangka panjang dapat meningkatkan resiko munculnya berbagai Efek Samping Obat (ESO). Efek samping MP yang dialami salah satu pasien di Rumah Sakit (RS) Fatmawati meliputi diabetes, hipertensi, dan infeksi. Dari hasil studi kasus tersebut, mekanisme ESO yang terjadi meliputi efek mineralokortikoid dan juga glukokortikoid dari kortikosteroid. Kerusakan organ dan perubahan fisiologi tubuh merupakan mekanisme utama dari efek tersebut. Upaya meminimalisasi ESO pada penggunaan MP jangka panjang meliputi penggunaan dosis minimal yang efektif, prinsip tappering off, dan penggunaan steroid-sparing-agent perlu diupayakan untuk meminimalisasi efek samping tersebut.

Abstract

Proper management of drug storage in hospitals is essential for ensuring quality and patient safety, particularly for high-alert drugs, look-alike sound-alike (LASA) drugs, and emergency drugs. This study evaluates the storage system for these drugs in the Fatmawati Hospital Pharmacy Warehouse according to the PKPO and SKP standards established by the Indonesian Ministry of Health. The evaluation was conducted through direct observation, a document study, and a comparison to Permenkes and Kepmenkes standards. The results show that, while most of the storage system is appropriate, there are still non-conformities, such as high-alert drugs being placed outside the red zone, suboptimal LASA marking, and limited warehouse space. The study recommended improvements to the marking zone, staff training, and layout arrangements. The results showed that 94% of high-alert drug storage was in the excellent category, 70% of LASA drug storage was in the good category, 80% of trolley emergency drug storage was in the good category, and 85.14% of general drug storage was in the good category. Statistical analysis using a t-test revealed that the storage of high-alert, LASA, emergency, and general drugs had a significance value of p > 0.05. These results illustrate that drug storage in the Fatmawati Hospital Pharmacy warehouse meets the standards. Abstract

Diabetes mellitus is a non-communicable disease that has a large prevalence and is increasing every year in Indonesia. Evaluation of the use of antidiabetic drugs aims to ensure that patients with diabetes mellitus use antidiabetic drugs appropriately. This study is an observational study with retrospective descriptive analysis using patient medical record data. In this study, an evaluation was carried out on outpatient type 2 diabetes mellitus geriatric patients with a total of 200 patients based on PERKENI in 2021 with an accuracy standard of 100%. Evaluation of drug use is divided into 3 parameters, namely the right indication, the right drug, and the right dose. Based on the analysis of demographic data, most patients are at the age of 60-69 years and are female. Based on clinical data analysis, patients with the highest level of diabetes are type 2 diabetes mellitus without comorbidities or complications. Based on the results of the evaluation, the most common pattern of drug use is monotherapy. Based on the evaluation results, the right indication was obtained for 200 patients (100%), the right drug for 178 patients (89%) and the right dose for 199 patients (99.5%). Based on Hemoglobin A1c, FBS, PGB and RBGS, 188 patients (94%) had controlled diabetes. In this study, it can be concluded that there are still discrepancies in the use of antidiabetic drugs in outpatient type 2 diabetes mellitus geriatric patients at Fatmawati General Hospital in 2023.

Abstract

Prescribing errors often serve as an initial gateway to broader medication errors in hospital patient care. These errors must be minimized, particularly through digitalization efforts such as e-prescribing. This study aimed to identify prescribing errors in outpatient prescription services at Fatmawati Central General Hospital following the implementation of a hospital-wide e-prescribing system. A prospective direct observation method was applied to all prescriptions received at the Outpatient Pharmacy Depot, supported by clarification with the responsible pharmacists to ensure contextual accuracy and data validity. In November 2025, a total of 10,842 outpatient prescriptions were recorded. Several prescribing errors were still found during the observation period, including input errors such as mistyping and incorrect drug selection. High-risk drug interactions, especially among patients using warfarin, required closer monitoring. Patient education on the use of medications requiring special devices was also not consistently delivered. Meanwhile, prescriptions for narcotic and psychotropic medications complied with indication requirements and prescribing limits set by the National Formulary. These findings indicate that e-prescribing effectively reduces the frequency of errors compared with manual prescriptions. The results also underscore the need for enhanced prescription verification, improved patient counseling, and further optimization of the e-prescribing system.

Abstract

The management of high-alert medications requires special attention due to their high risk of causing patient harm when errors occur. This study aims to identify deviations in the storage and distribution of high-alert medications at the Central Pharmacy Warehouse of Fatmawati Central General Hospital and to assess their compliance with the standards of the Ministry of Health (2019) and ISMP (2018). The method used was direct observation during November 2025. A total of 50 deviations were found, consisting of 26 cases related to labeling (76.5%) and 8 cases related to storage (23.5%). The most common deviations included the absence of “High Alert” labels, discrepancies in stock cards, and improper arrangement of medications according to risk categories. Based on the NCC MERP adaptation, all findings were classified as potential errors without immediate clinical impact but with the possibility of leading to medication errors. These results indicate that the labeling and monitoring system for high-alert medications at Fatmawati Central General Hospital has not been optimal, necessitating improvements through consistent labeling, separation of storage areas, and strengthened stock monitoring.

Abstract

Medication errors involving high-alert medications in the emergency department (ED) have the potential to cause serious harm, making early identification during the dispensing stage essential. This study aims to describe potential medication errors related to the storage, distribution, and use of high-alert medications in the ED Pharmacy Depot and emergency trolleys at Fatmawati Central General Hospital. A prospective observational study was conducted in November 2025 through direct observation of the dispensing process, evaluation of emergency trolleys, and interviews with nurses who administer high-alert medications. Incidents were classified using an adapted NCC MERP Index. Seven discrepancies were identified, all of which involved the absence of “high-alert” labels on medication packaging and shelving, as well as limited sticker availability; these discrepancies were categorized as level 1 (potential error without harm). Nurses demonstrated good knowledge of high-alert medications, but were not sufficiently attentive to missing labels. These findings highlight the need for standardized labeling, adequate sticker supply, and routine audits to strengthen patient safety in the ED.
