Abstract

Schizophrenia is a severe mental disorder characterized by positive symptoms such as hallucinations, delusions, and a risk of violent behavior. One non-pharmacological approach to managing this risk is the implementation of Strategy Implementation (SP 1–4), supported by classical music therapy. This study aims to describe the application of SP 1–4 combined with classical music therapy in reducing the risk of violent behavior in patients with schizophrenia. The research used a case study design with a descriptive qualitative approach. The intervention was conducted on one patient diagnosed with schizophrenia who was hospitalized in the Gili Trawangan ward at RSSA Malang, with a nursing diagnosis of risk for violent behavior. Data were collected through observation, interviews, and documentation over a four-day period. The results showed gradual improvement in the patient, including decreased emotional tension, increased relaxation, enhanced assertive communication skills, and better medication adherence. Classical music therapy also had a calming effect. The combination of SP 1–4 and classical music therapy proved effective in reducing the risk of violent behavior, helping regulate emotions, and improving compliance with treatment. Future studies should include more subjects and longer interventions to better assess long-term effects.

Abstract

A 64-year-old male patient diagnosed with residual schizophrenia was admitted to the Bhakti Luhur Pasuruan Hospital for chronic low self-esteem. He exhibited withdrawn behavior, appeared depressed, lacked self-confidence, described himself as worthless, and was unable to care for himself. He also had a history of traumatic experiences and discriminatory treatment from his environment, which led to a decline in self-concept and motivation for activities. The intervention provided consisted of five days of positive skills training, which included building trusting relationships, recognizing remaining abilities, expressing positive feelings, and strengthening adaptive behavior through praise and support. After the intervention, the patient showed positive changes in the form of improved communication, appeared more confident, began to be active in simple activities, was able to articulate his abilities, and performed self-care independently. These results indicate that positive skills training is effective in improving self-esteem and adaptive functioning in patients with residual schizophrenia, so it can be used as a sustainable nursing intervention in psychosocial rehabilitation.

Abstract

The First 1000 Days of Life constitutes a critical window for optimal child growth and development, during which nutritional deficiencies may result in long-term consequences such as stunting and cognitive impairment. Globally, 22% of children under five are stunted and 7.5% experience wasting, while in Indonesia, the prevalence of undernutrition reached 17.1% and wasting increased to 7.7% in 2022. One of the primary determinants of child nutritional status is maternal nutrition knowledge, which remains insufficient in many communities. Audio-visual media has been recognized as an effective tool to enhance maternal understanding and support appropriate nutritional practices for young children. This study aimed to assess the effectiveness of audio-visual media in improving maternal knowledge regarding toddler nutrition in Penyaguan Village, Indragiri Hulu Regency, in 2025. A pre-experimental one-group pretest-posttest design was employed, involving 39 mothers of toddlers selected through total sampling. Maternal knowledge was measured using a structured questionnaire, and the intervention consisted of nutrition education delivered through audio-visual media. Data were analyzed using the paired sample t-test. Findings demonstrated an increase in mean knowledge scores from 10.33 prior to the intervention (range 7–18) to 13.26 following the intervention (range 9–17). Statistical testing yielded a p-value of 0.001 (<0.05), indicating a significant improvement in maternal knowledge after the intervention. In conclusion, audio-visual-based education effectively enhances maternal knowledge regarding toddler nutrition and may be recommended as a strategic educational approach to support early childhood nutrition programs. The findings of this study will be disseminated through publication in a nationally accredited scientific journal.

Abstract

Type 2 diabetes mellitus (DM) is a metabolic disorder caused by pancreatic dysfunction, characterized by hyperglycemia and associated with macrovascular and microvascular complications such as heart disease, stroke, retinopathy, diabetic foot, and neuropathy affecting sensory, motor, and autonomic functions. Effective management of DM requires a comprehensive approach, including non-pharmacological interventions such as diabetic foot exercises and health education through Diabetes Self-Management Education (DSME). To evaluate the effect of combining DSME and diabetic foot exercises on blood glucose levels in patients with type 2 DM. This case study was conducted as part of a community service program in Dusun Cokro, Malang, from April 23 to May 1, 2025, involving one patient with type 2 DM (Mrs. A, 62 years old) who was not undergoing routine treatment. The intervention consisted of 10–20 minutes of education and 20–30 minutes of daily foot exercises for 6 days (3 days supervised and 3 days independent practice). Data were collected through interviews, observation, documentation, and the DKQ-24 questionnaire. Evaluation was performed by measuring blood glucose levels and assessing knowledge before and after the intervention. There was an improvement in the patient’s knowledge and a reduction in blood glucose levels following the intervention. The combination of DSME and diabetic foot exercises is effective in increasing knowledge and lowering blood glucose levels in patients with type 2 DM, and it can be applied within community nursing practice.

Abstract

Hypertension is a major non-communicable disease often referred to as a silent killer due to its asymptomatic nature and potential to cause serious complications. Walking exercise is one of the recommended non-pharmacological interventions proven to help reduce blood pressure. This case study, using a nursing care approach, aims to examine the effect of walking therapy on lowering blood pressure and reducing headache intensity in a 62-year-old female patient living in Pakis Village, Malang Regency. The intervention consisted of a 30-minute daily walking session for seven consecutive days. The results showed a decrease in blood pressure from 160/95 mmHg to 140/85 mmHg, along with a reduction in headache intensity from a score of 6 (moderate pain) to 2 (mild pain). Physiologically, walking improves vascular elasticity, reduces stress hormones, and stimulates the release of endorphins that act as natural analgesics. These findings indicate that walking therapy is an effective, safe, and practical non-pharmacological nursing intervention to support hypertension management and improve the quality of life in older adults.

Abstract

Type 2 diabetes mellitus is a chronic disease characterized by hyperglycemia due to metabolic disorders related to insulin resistance and decreased pancreatic ?-cell secretion function. The high prevalence of DM, which continues to increase every year, indicates the need for comprehensive treatment to prevent complications. One non-pharmacological approach that can be used to help control blood glucose levels is autogenic relaxation therapy. This study used a case study design with a qualitative descriptive approach to explore in depth the phenomenon of changes in blood glucose levels in a patient with Type 2 DM. The research subject was one patient residing in Cokro Hamlet, Sukoanyar Village, with inclusion criteria including Type 2 DM patients, GDS >200, not consuming DM drugs or certain drugs, able to communicate, and willing to be a respondent. Primary data were obtained through interviews, observation of physical and psychological conditions, and blood glucose tests using a glucometer. Secondary data were obtained through patient health documentation, daily treatment records, and family statements. The research process was conducted in three stages, namely orientation, the working phase with the implementation of autogenic relaxation therapy in several sessions, and termination through subjective and objective evaluations. The results showed a decrease in blood glucose levels from 345 mg/dL to 216 mg/dL (a difference of 129 mg/dL) after the intervention, accompanied by improvements in physical and emotional complaints in the form of improved sleep quality, reduced fatigue, and decreased anxiety. These findings indicate that autogenic relaxation therapy can be an effective non-pharmacological intervention in helping to control blood glucose levels in patients with Type 2 DM.

Abstract

Chronic Kidney Disease (CKD) is a progressive and irreversible illness that requires patients to undergo lifelong hemodialysis therapy. This condition not only affects physical health but also triggers various psychological problems, including anxiety, stress, depression, and loss of self-worth. This study aims to evaluate the effectiveness of spiritual interventions in improving the mental health of hemodialysis patients. The research employed a case study design involving a 36-year-old male patient receiving routine hemodialysis at Universitas Muhammadiyah Malang Hospital. Data were collected through interviews, observation, and documentation, and analyzed qualitatively using a descriptive approach. The spiritual intervention was implemented over seven days and included building a trusting relationship, practicing gratitude, identifying personal strengths, spiritual affirmation, self-reflection, and routine prayer based on the patient’s beliefs. Results indicated significant improvements in the patient’s emotional, spiritual, and behavioral responses. The patient became more relaxed, accepted his condition, displayed positive expressions, and showed improved sleep patterns. Spiritual intervention was found effective in reducing anxiety, enhancing self-worth, and restoring the patient’s sense of meaning in life. In conclusion, spiritual intervention is an effective strategy that should be integrated into holistic nursing care to support the mental well-being of hemodialysis patients.
