Abstract

Health care facilities are places used for promotive, preventive, curative, and rehabilitative health services carried out by the government and the community. Calculating the germ count on door handles of healthcare facilities is very important, because patients are not aware that the door handles contain and transmit germs that cause infections. This study aims to determine the percentage of germ contamination on door handles of healthcare facilities at X Hospital and X Community Health Center. The type of research conducted is descriptive quantitative with an observational case report approach. This study is a frequency distribution in tabular form using a percentage formula. The number of samples used by the researchers was 8 samples, using 4 sampling points at the Hospital and 4 sampling points at the Community Health Center (Puskesmas): the Emergency Room, Main Door, Pharmacy Room, and Laboratory Room. Data collection by conducting laboratory tests on door handle swab samples was then examined using the ALT method. The results showed that the percentage of door handles that did not meet ALT standards was 3 samples (75%) at X Hospital. Meanwhile, on door handles at health facilities Puskesmas. The percentage of door handles at Puskesmas was obtained 4 samples (100%) at X Puskesmas. Door handles at health facilities do not meet the standards of the Republic of Indonesia's Minister of Health Decree No. 1204 / MENKES / SK / X / 2004, the decision Ho was accepted H1 was rejected.

Abstract

Open fractures are serious orthopedic injuries with a high risk of infection and soft tissue complications. A 20-year-old male patient presented to the Emergency Department with severe pain in his left leg and wrist, accompanied by a laceration on his face, following a traffic accident. Physical examination revealed deformity and an open wound on the left leg, along with severe swelling and tenderness. Radiographic results showed an open fracture of the proximal third of the left tibia and fibula, an open fracture of the left pedal metatarsal, a closed fracture of the distal phalanx of the third digit of the left foot, and a closed fracture of the distal third of the left radius with distal radioulnar joint dislocation. Based on the Gustilo–Anderson classification, the case was categorized as type IIIB. The patient received initial management including debridement, broad-spectrum antibiotics, analgesics, and splinting. Surgical intervention was performed in stages, comprising Open Reduction External Fixation (OREF) for the tibia–fibula and left foot, followed by Open Reduction Internal Fixation (ORIF) for the distal left radius. Postoperative evaluation showed clinical improvement with reduced pain, decreased edema, and improved extremity function. This case emphasizes the importance of rapid and appropriate management of multiple open fractures to prevent infection, maintain bone stability, and accelerate the healing process.

Abstract

Appendicitis is one of the most common causes of acute abdominal pain in children and is considered a surgical emergency requiring immediate diagnosis and management to prevent serious complications. Diagnosing appendicitis in children is often challenging because the symptoms are non-specific, and the child's ability to articulate complaints is limited. This case report discusses a 6-year-and-9-month-old boy who presented with a chief complaint of lower right abdominal pain accompanied by nausea, vomiting, and fever. Based on the results of the anamnesis, physical examination, and supporting investigations in the form of laboratory tests and abdominal ultrasound (USG), a diagnosis of acute appendicitis was established. The patient then underwent an appendectomy with good recovery and no complications. This case emphasizes the importance of comprehensive clinical evaluation and the use of non-invasive supporting examinations such as ultrasound in establishing the diagnosis of appendicitis in children. It highlights the critical role of timely diagnosis and intervention, which can lead to favorable outcomes and prevent the risk of perforation or other serious complications. Early recognition and appropriate surgical decision-making are key to improving patient prognosis.

Abstract

Pediculus humanus capitis, according to the WHO, is classified as a skin disease and ectoparasite in the list of neglected tropical diseases. The cause can infect the scalp, leading to many children suffering from iron deficiency and anemia. Its spread occurs through shared items. Therefore, traditional insecticides such as srikaya seeds and kemiri seeds, which contain saponins and flavonoids capable of killing head lice, are required. The obective of this study is to determine the most effective concentration, compare their efficacy, and determine the killing time of both extracts. The experimental research method involved treatments at concentrations of 20%, 40%, and 60% with four repetitions. The results of this study indicate that there is an effect of srikaya seed extract with kemiri seeds in terms of concentration and time on lice mortality, as shown by a two-way ANOVA test with a p-value of 0.000 (P<0.05). In the Tukey test, the average mortality rate for srikaya seeds was 4, and the average mortality rate for kemiri seeds was 3. The concentration of srikaya seed extract killed 50% of head lice LC50 at a dose of 0,14%, while the concentration of kemiri seed extract killed 50% of head lice at a dose of 0,28% with contact times of 30 and 60 minutes. This demonstrates that srikaya seed extract at a dose of 0,14% is effective in causing mortality in head lice (Pediculus humanus capitis)

Abstract

Elevated blood pressure above the normal range is a hallmark of a medical condition known as hypertension. It is estimated that as the population ages, the number of hypertension cases will also increase. If this condition is properly managed early, it can be controlled. Controlling daily eating habits is one of the most effective non-pharmacological methods for lowering blood pressure. Therefore, finding alternative food sources that can naturally lower blood pressure is crucial. Red dragon fruit (Hylocereus polyrhizus), which is high in fiber, potassium, and antioxidants such as vitamins B, C, and E, is one possible option. These nutrients are important for maintaining blood vessel flexibility and controlling hypertension. The purpose of this study was to determine the effect of red dragon fruit juice consumption on blood pressure in the Paku Village Community Health Center (Puskesmas Pembantu) operational area. A single-group pre-experimental design was used in this study. A paired-sample t-test was used to assess the data. Based on the findings, the average systolic blood pressure was 184 mmHg before consuming red dragon fruit juice and decreased to 164 mmHg after the intervention. With a t-value of 23.4, a correlation of 0.96, and a p-value of 0.000 (p < 0.05), this decrease was statistically significant. Meanwhile, the average diastolic blood pressure dropped from 101 mmHg to 84 mmHg, with a correlation of 0.961, a t-value of 18.65, and a p-value of 0.000 (p < 0.05). These results indicate that consuming red dragon fruit juice significantly lowers blood pressure, with an average decrease in systolic pressure of 20 mmHg and diastolic pressure of 17 mmHg.

Abstract

Background : The Maternal Mortality Rate (MMR) worldwide reaches 183,000 per 100,000 live births (WHO, 2024). In Indonesia, there were 4,305 reported cases (Ministry of Health RI, 2024), while Central Java Province recorded 315 cases (Central Java Health Office, 2024). In Brebes Regency, there were 54 maternal deaths in 2024 (Brebes Health Office, 2024), and in the Bumiayu Public Health Center area, 2 maternal deaths were recorded (Bumiayu PHC Data, 2024). Objective : To analyze, implement, and provide comprehensive midwifery care for Mrs. A, 25 years old, with anemia in the Bumiayu Public Health Center area in 2025, using the Varney midwifery management approach and the SOAP method. Research Methods : This study used a qualitative descriptive design in the form of a case study. Results : Midwifery care during pregnancy for Mrs.A with anemia was carried out bassed on the mother’s needs. During labor, newborn care, postpartum period, and family planning services, no complications were found. Conclusion: The comprehensive midwifery care provided to Mrs. A with anemia was carried out in accordance with the Standard Operating Procedures (SOP), with both the mother and baby in good condition.

Abstract

Hypotension following spinal anesthesia is a frequent perioperative complication, particularly among pregnant patients, with an incidence ranging from 60% to 80%. This condition may compromise uteroplacental perfusion and pose serious risks to both mother and fetus. Leg elevation represents a simple, safe, inexpensive, and effective non-pharmacological intervention that functions through gravitational autotransfusion, enhancing venous return and improving cardiac output. This scoping review was conducted using the Joanna Briggs Institute (JBI) methodology and the PRISMA-ScR framework, guided by the PICO approach. The population included adult patients undergoing spinal anesthesia, with the intervention consisting of elevating the legs at an angle of 20–30° or approximately 30 cm for 10–15 minutes, compared with the standard supine position. Literature searches were performed through PubMed  and Google Scholar, focusing on primary studies published within the last five years. Of the 130 articles identified, 10 met the inclusion criteria and were analyzed descriptively. The findings demonstrate that leg elevation significantly increases systolic and diastolic blood pressure as well as mean arterial pressure (MAP), while reducing the incidence of post-spinal hypotension. The technique also decreases vasopressor requirements without notable adverse effects, particularly in obstetric populations. Based on the available evidence, leg elevation is recommended as an effective and feasible initial non-pharmacological strategy for preventing hypotension after spinal anesthesia, especially in resource-limited clinical settings, with an optimal elevation of 20–30° for 10–15 minutes.

Abstract

Cases of sexual violence are rife in educational environments and generally happen to women. Ironically, 55% of sexual violence perpetrators are committed by teachers, in fact many cases are still unreported. Preventing sexual violence is a shared responsibility. This study aims to determine the conditions, impacts and strategies that can be used to prevent sexual violence. A qualitative analytical approach with secondary data in the form of interviews broadcast via the Narasi TV and YouTube websites (Crime Lab, COKRO TV, KAWANUATV MANADO, Tribun Health, UNPAR OFFICIAL) was used in this research. The impacts felt by victims of sexual violence include feeling afraid, embarrassed, depressed, traumatized and low self-esteem. Strategies that can be implemented include promoting social norms, implementing social emotional learning related to sexual violence, forming a task force, mentoring support in the form of counseling and medical services as well as parental supervision and assistance. Mutual cooperation and synergy need to be carried out together to create a school environment that is safe and comfortable for students and free from sexual violence.

Abstract

Nosocomial infections, such as Surgical Site Infections (SSI), phlebitis, and Urinary Tract Infections (UTI), continue to challenge hospital service quality, including at Dharmais Cancer Hospital. The performance of Infection Prevention and Control Link Nurses (IPCLNs), key in executing the Infection Prevention and Control (IPC) program, remains suboptimal, with delays in reporting, inaccurate surveillance documentation, and weak coordination with the IPCN team. This study aims to analyze the effect of training and work motivation on IPCLN performance, with supervision as a moderating variable that strengthens the relationship. Using a quantitative approach, the study employed multiple linear regression and moderated regression analysis (MRA) on data from 43 IPCLNs selected through total sampling. A structured, valid, and reliable questionnaire was used for data collection. The results showed that training, motivation, and supervision had a significant effect on IPCLN performance. Training and motivation had a positive impact, while supervision not only had a direct effect but also moderated the relationship between training, motivation, and performance. These findings emphasize the importance of structured training, motivation enhancement, and sustainable supervision to improve the IPC program's effectiveness. The practical implication is the need for integrated managerial strategies to strengthen the role of IPCLNs in reducing infection risks and improving hospital service quality.

Abstract

Introduction:All-zirconia crowns are the strongest ceramic artificial crowns, made from zirconium, a silicate compound (ZrSiO4) and resistant to high temperatures. The advantages of zirconia include good mechanical properties, biocompatibility, and high corrosion resistance. Zirconia has disadvantages, such as a lack of aesthetic similarity to natural teeth. Methods and Results: Search for published articles on Elsevier, PubMed and Science Direct using predetermined keywords, namely crown,fracture resistance, and zirconiaThis literature review utilizes literature published between 2020 and 2025, accessible in full-text PDF format. Based on data searches using predetermined keywords, data was searched for the last five years, resulting in four selected articles. Discussion: Fracture and loss of retention are problems that can occur in zirconia crowns. Several factors influence the fracture resistance of clinical crowns, such as the load received, the preparation results, material thickness, cement used, marginal adaptation and the zirconia content used.
