Abstract

Introduction: Intra-arterial chemotherapy for retinoblastoma has been widely implemented as a primary or secondary treatment choice, yet its efficacy alone is limited in cases with extensive vitreous seed. The effect of intravitreal chemotherapy combined with intra-arterial chemotherapy on tumor control rates and globe salvage is still uncertain. The objective of this review was to explore the effectiveness and safety of intra-arterial plus intravitreal chemotherapy in retinoblastoma. Methods: A comprehensive search strategy was implemented across three electronic databases (PubMed, Scopus, and Cochrane) for relevant studies from inception until June 2024. Results: Six studies involving 471 eyes were included in the analysis,. Only three studies were comparative. The level of heterogeneity varied across each reported outcomes. The pooled globe salvage rate was 66% (311/471 eyes, 0.78 [95% CI: 0.55, 0.91]). The overall tumor control rate was 83.5% (304/364 eyes, 0.86 [95% CI: 0.74, 0.93]). Metastatic disease occurred in 3 of 471 eyes (0.63%, 0.015 [95% CI: 0.006, 0.036]). Conclusion: The pooled evidence from this meta-analysis supports the efficacy and favorable safety profile of combined intra-arterial and intravitreal chemotherapy in treating retinoblastoma. Further research with extended follow-up period are needed to validate these findings.

Abstract

Pendahuluan: Perkembangan teknik fakoemulsifikasi dan lensa intraokular (LIO) modern telah meningkatkan harapan pasien terhadap kualitas penglihatan pasca operasi yang optimal. Biometri ultrasonik dan optikal merupakan dua metode untuk menghitung panjang aksial dan parameter mata lainnya, yang berperan penting dalam menentukan ukuran LIO. Tujuan penelitian ini untuk mengetahui perbandingan refraksi pasca fakoemulsifikasi menggunakan kedua biometri di BLUD UPT Klinik Mata Ambon Vlissingen. Metode: Desain penelitian adalah eksperimen murni, melibatkan 72 pasien katarak yang dibagi secara acak ke kelompok biometri ultrasonik (n=36) dan optikal (n=36). Pemeriksaan refraksi dilakukan satu minggu pasca fakoemulsifikasi untuk menganalisis Absolute Prediction Error (APE) dengan uji statistik Independent T-test. Hasil: Hasil penelitian menunjukkan rata-rata APE kelompok ultrasonik 0,57±0,33 D (range 0,08–1,23 D), diketahui 31 orang (86,1%) memiliki APE ≤1,00 D. Pada kelompok optikal, rata-rata APE 0,53±0,28 D (range 0,05–1,12 D), dengan 34 orang (94,4%) memiliki APE ≤1,00 D. Uji Independent T-test menunjukkan tidak terdapat perbedaan bermakna antara kedua kelompok (p=0,620). Kesimpulan: Biometri optikal memberikan rata-rata APE lebih kecil dibandingkan ultrasonik, namun perbedaan tidak signifikan secara statistik. Kedua metode dapat digunakan dalam perhitungan daya LIO untuk mengoptimalkan refraksi pasca fakoemulsifikasi.

Abstract

Introduction: Glaucoma is one of the leading causes of irreversible blindness worldwide, affecting more than 76 million people globally, with numbers projected to increase. In Indonesia, glaucoma accounts for approximately 13–15% of blindness cases, with many patients presenting at an advanced stage. End-stage glaucoma may progress to phthisis bulbi and is often associated with ocular surface impairment. Conjunctivitis in this setting presents a diagnostic challenge as it may be caused by infectious or non-infectious mechanisms. Case Report: A patient presented with bilateral red eyes for one week, accompanied by epiphora and foreign body sensation without purulent discharge. Examination revealed advanced glaucoma in the right eye with elevated intraocular pressure, and absolute glaucoma with phthisis bulbi in the left eye with no light perception. Both eyes showed conjunctival hyperemia without signs of acute infection. Discussion: Conjunctivitis in end-stage glaucoma is often associated with ocular surface disease due to chronic inflammation, tear film instability, and long-term use of topical medications. This condition may mimic infection, requiring careful clinical evaluation to avoid misdiagnosis and inappropriate treatment. Conclusion: Conjunctivitis in end-stage glaucoma is not always infectious but is frequently part of ocular surface disease. Management should be individualized, focusing on patient comfort and quality of life.

Abstract

Introduction: A serious intraocular infection called endophthalmitis can cause permanent blindness and severe vision impairment. Family preparedness as the primary caregiver is essential to successful home care because this disease necessitates sophisticated continuous care following hospital release. The purpose of this study was to investigate family preparedness for endophthalmitis patients' post-hospital care. Methods: Study design is a qualitative descriptive with a case study approach. Participants consisted of six families of endophthalmitis patients in the inpatient ward of Cicendo Eye Hospital, selected through purposive sampling. Data were collected through in-depth semi-structured interviews and analyzed using verbatim transcription, data reduction, coding, categorization, and theme development until data saturation was achieved. Results: The study identified four main themes: family knowledge and skills in patient care, family perceptions and emotional readiness in facing discharge, support from healthcare providers and resources in discharge planning, and barriers and challenges in providing home care. Family readiness was influenced by health literacy, prior caregiving experience, healthcare support, and caregiver burden. Conclusion: Family readiness is a dynamic process influenced by cognitive, emotional, and healthcare system factors. Family centered care approach plays a key role in strengthening family readiness through active involvement, effective communication, and practice-based education during hospitalization.
