The pathogenesis theory of COVID-19 is still developing. While there are lot of theory coming up, the spotlight mostly reveal on inflamation, coagulopaty, and thrombo-infalamation. We reported an 53 years-old man arrived at the hospital with confirmed COVID-19, severe pneumonia and hypercoagulability state. Patient was referred to Hasan Sadikin Hospital and treated at COVID-19 isolation room, with the used of ventilator for 21 days. Further laboratory test found thromboelastrograph hypercoagulation from elevating level of fibrinogen and thrombocyte. SARS-CoV-2 enter host cells and cause a disruption of both epithelial and endothelial cells together with an alveolar inflammatory cell infiltrate leading to high levels of early response-proinflammatory cytokines. In severe COVID-19 patients, this immune response is excessive and thus described as a systemic “cytokine storm”. It is responsible for endotheliopathy and hypercoagulability state, leading to both systemic and macrothrombosis and microthrombosis. In this case we found hypercoagulation in thromboelastograph with increased D-dimer and fibrinogen, so that the patient was diagnosed with hypercoagulopaty Patient was given therapy of heparin. After heparin therapy patient had an improvement of clinical symptoms and laboratory result.

Insulinoma is a rare benign functional neuroendocrine in pancreas. Inappropriate secretion of insulin in patient with insulinoma results in hypoglycaemic episode with clinical symptoms. Meanwhile, epilepsy is a manifestation of brain disorder with a variety of clinical symptoms, cause by the release of electrical charges from brain neuron. We reported a 50 years old man with insulinoma which was planned for laparoscopic tumor resection. Patient also have epilepsy as a comorbid. The choice of anesthesia technique is intubation general anesthesia. Hypoglycemia is the main problem that can happen in preoperative and intraoperative period, so blood glucose was monitored every 30 minutes. Anesthesia consideration in this patient was avoiding hypoglycemia in preoperative and intraoperative period and avoiding proconvulsan anesthetic agent because the patient also have epilepsy as comorbid.

Thyroid disease is a disorder that affects the thyroid gland. Thyroid hormones regulate metabolism and affect nearly every organ in the body. The management of anesthesia on the hypothyroid is crucial and consist of preoperative, intraoperative, and postoperative management. We reported a 36-year-old female patient with multigravida and hypothyroid on therapy, which would performed sectio caesarea. Patient had hypothyroid and consumed levothyroxine since a year earlier. Spinal regional anesthesia with low doses was administered and sedation was neither prescribed before nor during surgery to maintain hemodinamic stability. A pregnant women with hypothyroidism need special management of anesthesia. Hypothyroid patients are very sensitive to anesthetic drugs so that preoperative sedation should be avoided. During intraoperation, surveillance of haemodynamics must be strict because hypothyroidism can trigger refractory hypotension. Sedation drugs should not use both in preoperative and intraoperative period.

Acute pulmonary edema is a condition when there is accumulation of fluid in the lungs (insterstitial space and alveoli). This fluid fills the alveoli in the lungs which makes it difficult for someone to breathe. The most common cause of pulmonary edema is caused by heart problems. However, the accumulation of fluid in the lungs can be caused by several reasons including pneumonia, some poisons, or drugs. The number of deaths for one year for patients hospitalized with acute pulmonary edema reaches 40%. The most common causes of acute pulmonary edema include myocardial ischemia, cardiac arrhythmias (eg atrial fibrillation), cardiac valve dysfunction and excess fluid volume. It can also be caused by other causes including pulmonary embolism, renal artery stenosis, non-compliance with previous disease treatments and side effects from medications can also trigger pulmonary edema. Management in patients with pulmonary edema we first look for diseases that underlie the occurrence of edema. Because it is a very important factor in treatment, so it needs to know the cause immediately. Because specific therapy cannot always be given until the cause is known, supportive therapy is very important. The general goal is to maintain basic physiological and cellular functions. The selection of anesthesia techniques for caesarian section is determined by various factors, namely the indication of the operation, the choice of the patient and surgeon, and the technical ability (skill) of the surgeon. Clinical evaluation of patients with coagulopathy is very important to determine the anesthetic technique. Clinical evaluation must be individualized, taking into account the risks and benefits of anesthesia techniques. The risk of spinal hematoma or epidural in patients has not been completely eliminated, but it remains a factor that must be considered individually in determining whether neuraxial anesthesia is appropriate for that patient.

Opioids have become the flagship agents of modern perioperative care and pain management. The word opium comes from the Greek opion (“juice poppy”) or opium poppy (Papaver somniferum) is the source of 20 different alkaloids. Recent studies have shown that the role of opioid receptors on immune function is complicated, but they work through a variety of different mechanisms. Administration of various opioids shows different effects on the immune system: immunosuppressive, immunostimulatory, or both. Previously it was believed that most opioids suppress the immune system, but recent research suggests they may have a dual effect. However, the mechanism by which opioids and opioid receptors work in the immune system is still not clearly understood.

Nutrition is important in critical patient care. Malnutrition may lead to poor outcome of the patient in ICU, including increases morbidity, mortality, and length of stay. Patient in ICU also experienced increased metabolism and catabolism which can lead to malnutrition. Therefore, the provision of nutrition for patients in the ICU needs a special care. The nutritional requirement of critical ill patients depend on the severity of the disease and previous nutritional status. To achieve the demand of nutritional demand can be given enterally, parenterally or both.

Bleeding is one of the most common cases that will be encountered by an anesthesiologist. Patients with internal bleeding have a high mortality rate and tend to detoriate quickly. The patient should get immediate and proper treatment. Preoperative management from an anesthesiologist has important role to the outcome of the patients. Management of patients with internal bleeding should be based on recent studies. Damage Controlled Resuscitation (DCR) and permissive hypotension is now the recommended concept of managing bleeding patients. Further studies should be conducted for the best management of patients.

World population’s access to safe blood is limited. Only 30% of countries in the world have comprehensive transfusion services in their countries. Meanwhile, massive transfusion therapy has poor survival rate with mortality rate of about 45-67%. The patient’s age, duration and severity of shock, Disseminated Intravascular Coagulation (DIC), and the amount of blood transfused affect the fnal outcome. Coagulopathy has a high mortality rate within 4-6 days after hospitalization. Many obstetrics and gynecology patients have the potential to require blood transfusions. Cesarean section (SC) and hysterectomy are two of the surgical procedures that are frequent and potentially bleeding, requiring blood transfusion. Other conditions are postpartum hemorrhage, placenta previa, and ruptured ectopic pregnancy. Bleeding in the obstetric feld is still a highest cause of maternal death in Indonesia.

Renal transplantation is a principal therapy for end stage renal disease. Renal transplantation can decrease mortality and improve recipient quality of life substantially. However, immunosuppressant therapy and comorbid disease in those patients make perioperative management challenging. A good urine output is a usual sign of good allograft function. Postoperatively, recipient should have a slight positive fuid balance and higher blood pressures to maintenance adequate posttransplanted organ perfusion. Fluid replacement should consider urine output, insensible water loss dan volume status of the recipient. Goal of the management is to reduce fuid replacement gradually while maintain hemodynamic stability
