Abstract

We reported a 54-year-old male patient with leptospirosis, septic shock, and MODS. Patient is referred from RSUD Prambanan with chief complaint of high fever accompanied by yellow eyes and body, headache, calf pain, body aches rheumatic pain, nausea, vomiting, shortness of breath, and tea colored urine. From history taking, it was found that there was a possibility of a history of contact with animals that were the source of transmission of leptospirosis, namely rats. The patient was treated in the ICU for several days. During treatment, hemodialysis was performed several times due to an increase in blood urea. After being treated for several days, the patient’s condition improved and treatment was carried out on the ward by internist.

Abstract

Acute kidney injury was a complication recently occurred in critical ill patients of ICU. A 64-years-old man came to emergency department with chief complain of lump originated at colli dextra accompanying with diffcult of breathing symptom. Patient was diagnosed with septic shock due to deep neck abcess complicated with AKI and uremic encephalopathy, diabetes mellitus type 2 with hyperosmolar hyperglycemia state. Patient was treated to manage dehydration, high blood glucose, and source control of infection, then patients was transferred to ICU with support of mechanical ventilator. The major problem of this patient were decrease of consciousness and diffculty in treating AKI stage because there were increase in serum ureum and creatinine level even though urine production was excessive. Patients condition was improved and could be extubated after performing renal replacement therapy on the sixth day. On patient with sepsis and AKI, renal replacement therapy was one of the modalities considered to support management in critically ill patients in ICU.

Abstract

A 32-year-old woman, G3P2A0, at 34 weeks of gestation came to Sardjito hospital emergency department with complaints of not being able to speak and right limb weakness since 8 hours before entering the hospital. Patient have hypertension since before pregnancy. CT-scan result obtain bleeding in the parietal lobes sinistra and basal nuclei with a bleeding volume 60 cc accompanied by midline shifting. Patient was diagnosed with superimposed severe preeklamsia, coma cum dekstra lateralization, and spontaneous intracerebral hemmorhage. Section cesarean and craniectomy hematoma evacuation then was performed. Postoperatively, patient was treated in the ICU and then transferred to the stroke unit.

Abstract

Background: Mortality rate in emergency department (ED) of Dr. Sardjito General Hospital did not achieve national target. Result of evaluation and analysis of this condition was established of High Care Unit for critical care management of patient in ED with the goal to decrease morbidity and mortality rate. Objective: To compare outcome such as mortality rate, MSOFA exit score, Length of Stay(LOS) in IMC, HCU, hospital and ventilator day at patients 3rd level of care (LOC) using ventilator before and after the implementation of HCU at the Dr. Sardjito General Hospital. Method: This research was an observational study using retrospective cohort analysis. Subjects had been collected from medical record included 198 meet the inclusion criteria; aged ≥18 years who were treated with critical illness with 3rd level of care (LOC) who used a ventilator in the HCU or IMC of RSUP dr.Sardjito. Exclusion criteria were patients with end-stage malignancies, patients with HIV / AIDS infections, patients with end-stage chronic disease and incomplete medical record. Result: Mortality rate in HCU (40.4%) and IMC (73.3%) with p = 0.000. MSOFA exit score in HCU (5.5) and IMC (7.4) with p= 0.001. HCU and IMC length of stay (LOS) showed no statistically signifcant difference. Hospital LOS in HCU (314.7) hours and IMC (210.6) hours with p = 0.000. Ventilator day in HCU (5.1) days and IMC (4.8) days and showed no statistically signifcant difference. Conclusion: Mortality rate and MSOFA exit score in HCU was lower than IMC. There was no significant difference between LOS in HCU and IMC. Hospital LOS in HCU group 314.7 hours and IMC group was 192.6 hours. Ventilator day in HCU group was 5.1 days and IMC group 4.8 days.

Abstract

Background: High dose opioid has been clinically proven to provide effective blockage of stimuli during head incision but it has several unwanted side effects. Scalp block is an alternative modality which has been used as an adjunct to general anesthesia aimed to blunt stress response throughout craniotomy while maintaining more stable condition in terms of less hemodynamic fuctuation. Objective: The study is aimed to compare fentanyl comsumption in general anesthesia combined with scalp block using saline and general anesthesia combined with scalp block using levobupivacaine 0.5% in patient which underwent craniotomy. Method: Twenty patients suffered from intracranial tumor with age ranging from 18 - 65 years who underwent craniotomy tumor removal at RSUP Dr. Sardjito were divided into 2 treatment groups in which the Levobupivacaine 0.5% block group received general anesthesia with a combination of scalp blocks using Levobupivacaine 0.5% while the control group received general anesthesia with a combination of scalp blocks using saline. Patients with local anesthetic allergy and uncontrolled bleeding were excluded from data collection. Result: The mean of fentanyl addition in patients with Levobupivacaine block was 513.0 mcg with a standard deviation of 164.2 mcg. Whereas in patients with Levobupivacaine block the average fentanyl addition was 1149.5 mcg with a standard deviation of 318.4 mcg. The mean difference in the addition of fentanyl between the two groups of 636.5 mcg showed a signifcant difference p = 0.001 (p <0.05). Conclusion: Combination of scalp blocks using Levobupivacaine 0,5% with general anesthesia showed reduction of fentanyl consumption alongside with statistically insignifcant hemodynamic fuctuation compared to general anesthesia and saline block in supratentorial brain tumor patient undergoing craniotomy. Hemodynamic profle in Levobupivacaine 0,5% group was maintained better than control group.

Abstract

Adrenal gland surgery needs a multidisciplinary team including endocrinologist, radiologist, anesthesiologist, and surgeon. The indications for adrenal gland surgery include hormonal secreting and non-hormonal secreting tumors. Adrenal hormonal secreting tumors present to the anesthesiologist unique challenges requiring good preoperative evaluation, perioperative hemodynamic control, corrections of all electrolytes and metabolic abnormalities, a detailed and careful anesthetic strategy, overall knowledge about the specific diseases, control and maintaining of postoperative adrenal function, and finally a good collaboration with other involved colleagues.

Abstract

Any technique that employs a fresh gas flow that is less than the alveolarventilation can be classified as low-flow anesthesia. The complexities involvedin the calculation of uptake of anesthetic agents during the closed-circuitanesthesia made this technique less popular. However, the awareness of thedangers of operating theatre pollution with trace amounts of the anestheticagents and the prohibitively high cost of the new inhalational agents, havehelped in the rediscovery of low-flow anesthesia.

Abstract

Patients in intensive care are generally exposed to catabolic stress with complications that increase morbidity and mortality, and affect the length of stay in the intensive care unit. Optimal nutritional management can help enhance the metabolic response to stress, prevent oxidative cellular injury, and modulate the immune response. We reported a 30 years old male underwent an esophagectomy procedure due to esophageal tumor. After the third day of surgery the patient was given enteral nutrition, there was a brownish residue in the nasogastric tube, in exchange of this condition, parenteral nutrition was given until the tenth day of intensive care. Enteral nutrition was continued on the tenth day of care until the patient transferred to the ward on the thirteenth day in the intensive care unit.

Abstract

Background: Geriatrics is defined as someone who reaches the age of 60 years and over. With the increase in the level of the economy and health, the geriatric population will increase. So that surgery and anesthesia in geriatric patients also increased. Geriatric patients tend to have more comorbidities accompanied by a decrease in physiological function and cognitive function so that it has an impact on mortality and morbidity. Objective: To identify the factors related to mortality and morbidity in geriatric patients undergoing anesthesia procedures at Dr. Sardjito General Hospital. Method: This study was conducted in a prospective analytical observational to identify risk factors in geriatric patients undergoing anesthesia at Dr. Sardjito General Hospital. The affordable population was all geriatric patients aged 60 years and over, who underwent anesthesia in the operating room and the time of the study was carried out for 3 months. In this study, variables that are thought to have a relationship with mortality and morbidity of geriatric patients will be tested using a univariate test using the Student's t-test if the data is numerical, while if the data is categorical using the Chi Square test. If the results of p<0.25 will be continued with the logistic regression test of each variable. And if p<0.05 then the variable is included in the multivariate analysis and if p<0.05 is considered significant. Results: The factors identified as factors affecting mortality of geriatric patients undergoing anesthesia at Dr. Sardjito General Hospital are age ≥75 years, dementia, and digestive surgery with p <0.05 in both univariate and multivariate tests. In the multivariate logistic regression test, factors affecting morbidity of geriatric is digestive surgery with p < 0,05. Conclusion: Patients’ age≥75 years, dementia and digestive surgery are the identified factors affecting mortality and morbidity of geriatric patients undergoing anesthesia at Dr. Sardjito General Hospital.
