Journal of Indonesian Health Policy and Administration is a journal that presents scientific articles mainly in the field of health policy, programs, and administration. This journal is intended to disseminate research from students, lecturers, or researchers in general who are concerned about improving health efforts through program evaluation and preparing policy recommendations. The articles or manuscripts contained in the Journal of Indonesian Health Policy and Administration include the realm of research, case studies, or conceptual, and limited to literature review. It is an online open-access, blindly peer-reviewed journal that is published every 4 (four) months or 3 (three) times a year, usually in January, May, and September. This journal is published by the Department of Health Administration and Policy, Faculty of Public Health, University Indonesia . Journal of Indonesian Health Policy and Administration is also supported by the Indonesian Public Health Association / Ikatan Ahli Kesehatan Masyarakat Indonesia (IAKMI).

Abstract

Policies regarding utilizing and rationalizing Personal Protective Equipment (PPE) during the COVID-19 pandemic challenge every healthcare service provider. Within the maternal health sector, it is suspected that the negative perception of patients and the lack of implementation and compliance with infection prevention and control policies disrupt the quality of care provided. This research is a narrative review highlighting all proper considerations for using and rationalizing PPE within the maternal health sector. Articles are gathered and filtered from well-known scientific publishing sites, and ten articles became the main discussion of this research. The utilization of PPE impacts all levels of maternal health service, from prenatal care to postnatal and neonatal care. Standardized PPE utilization and rationalization are needed to maintain the quality of care and reduce any negative impacts that PPE use might bring. Policies regarding the use of PPE must be well thought out to ensure that it will benefit its user without the risks of its side effects.

Abstract

The use of a contact tracing application is one of the efforts made to overcome the global COVID-19 pandemic with an increasing number of cases. The contact tracing application used in Indonesia is PeduliLindungi which is one of the requirements for accessing public areas, which was initially applied to the Java-Bali region. However, in its use, there are still challenges that affect the quality of electronic services. This study aims to analyze user perceptions of the service quality of the PeduliLindungi application based on the E-Servqual dimension. The method used is observational with a quantitative approach using a cross-sectional design. The results showed that overall, the user's perception of the quality of the PeduliLindungi application was good with positive ratings on the service quality dimensions of the PeduliLindungi application. The dimensions that have a significant relationship to the user's perception of the quality of the PeduliLindungi (ESQ) application are the dimension of user experience (EXP) and user trust (TRU). The dimension of reliability (REL), responsiveness (RES), ease of use (EOU), privacy (PRI), system availability (SA), application design (WD), and information quality (IQ) do not have a significant relationship to user perceptions of the quality of the PeduliLindungi (ESQ) application. Suggestions that can be given are to improve and improve the system and technical functions and improve security for user data.

Abstract

Hypertension is a major cardiovascular risk factor that causes disability throughout the world and among those who are not aware of their health condition, it is often referred to as the silent killer . The prevalence rate of hypertension in Indonesia is quite high. Based on Riskesdas (2018), South Kalimantan Province shows the highest prevalence of hypertension, but the percentage of hypertension sufferers who receive health services is still low. Banjarbaru City has the lowest percentage, namely 11.3% of the target of 100%. Health services for hypertension sufferers are included in the Minimum Service Standards (SPM) for the Health Sector which are regulated in statutory regulations and government policies. The aim of this research is to analyze the implementation of the SPM policy in the health sector for hypertension sufferers in Banjarbaru City. Qualitative research with a Rapid Assessment Procedure (RAP) research design was conducted from May to June 2023 in Banjarbaru City, South Kalimantan Province. Data collection was carried out through in-depth interviews, observation and document review. The results of the research show that the implementation of the hypertension SPM policy has not been optimal, as can be seen from the SPM achievement report in the health sector in Banjarbaru City which is in the lowest position among 12 types of basic health services. In conclusion, several obstacles in the implementation of SPM for hypertension are recording and reporting hypertension sufferers, low public awareness of carrying out examinations and treatment at community health centers so that the data is not recorded, there is no special budget allocation for SPM for hypertension at community health centers, and the dual duties of health workers so that they are not optimal in achieve a program. Cross-sector collaborative efforts are needed for integrated recording and reporting of hypertensive patient data, submission of hypertension SPM budgets, efforts to invite the community, and fair distribution of tasks for community health center officers.

Abstract

Age is an independent risk factor for COVID-19. Thus, the government of Indonesia prioritizes the elderly and adults with significant comorbidities for COVID-19 vaccination. To discover whether this policy is still appropriate, we evaluated the effects of vaccination against the COVID-19 incidence and infection risk.It was a retrospective cohort study of the NAR COVID-19 and Dashboard KPCPEN COVID-19 of the Depok City Health Department. The tests used to analyze the data were the chi-square and logistic regression tests. We discovered that there was a 92.7% total reduction in the COVID-19 infection rate with vaccination. The infection risk for COVID-19 in vaccinated people aged ≥ 60 years was far lower than those who were unvaccinated (RR = 0.10 vs. 1.72). In conclusion, vaccination reduces the transmission of COVID-19 significantly, especially in the ≥ 60 years age group. It affirms that the age priority policy in the vaccination policy is still applicable and does not require any amendments.

Abstract

The National Health System in Indonesia is developing and being discussed more seriously after entering the COVID-19 pandemic. With a national health system and several existing tools, Indonesia needed more time to be ready to face the challenges of that time. Upstream health policies should be optimized to mobilize resources and increase the nation's efforts to build a national resilience system. This paper was built using selected policy documents up-to-date and reliable literature. By reviewing empirical evidence, the author believes and shows the role of strengthening health policies in building a robust national health resilience system. Indonesia has several policies that have the potential to become a sub-system for national health resilience. The health resilience system is formed from the health system and health resilience components, which impact creating a Healthy Indonesia. Health system resilience can be seen from stable funding, good governance, flowing and integrated information systems, risk adaptation, adequate health workforce capacity, logistics, and robust health efforts. The interaction between the health system, health resilience, and the health resilience system formed is within the umbrella of health policy, the completeness of resilience policy, and the highest public health institutions. Strengthening the role of health policy in the formation of a health resilience system in a country is one proof of health policy reform. Health policy is no longer seen as the upstream side of health programs or efforts but as an essential side in functioning a system to create a healthy country. Reforming the national health system should followed by institutional reform of The Ministry of Health to become the Ministry of Public Health, and health policy reform by strengthening health resilience.

Abstract

The study focused on the feminization of Sri Lankan generalist doctors in all major and selected minor specialties between 2000 and 2020. The study analyzed all relevant documents by exploring Sri Lanka's male-to-female doctor and specialist ratio over 20 years. This was determined by analyzing state medical faculty records from 1990 to 2020. For better clarity, the researchers analyzed the gender ratio changes in every five-year block from 1990 to 2020. Focus groups filled PGIM's gender data fields. Five-year medical students surged from 43% to 63% between 1990 and 2020. Linear trend analysis predicts 68% of female medical students by 2025 and 72% by 2030. This study simulated generalist doctors with medical students. Until recently, men dominated medicine, surgery, obstetrics, and pediatrics. Female specialists have increased in all fields over the past 20 years. The feminization of medical doctors challenges national human resource policies and health sector reforms. They emphasize gender-sensitive health workforce planning that considers a country's economic development and healthcare system. Sri Lankan health policymakers should consider gender dynamics in national health HR planning for the next decade.

Abstract

This study aims to determine the marketing strategy 7P carried out by cardiac surgery services in Indonesia and to identify actionable policy recommendations. This study is observational descriptive research. Data were collected from Cardiothoracic and Vascular surgeons and hospital websites with cardiac surgery services. Through the marketing analysis approach of 7P, this study identifies problems and policy recommendations that can be alternative solutions. The price of the service is high, but most of the patients used BPJS, and it is found that the hospital rate is higher than BPJS claims. The actionable recommendation to respond to it is that the hospital should provide the costing and financial analysis that generate the hospital rate to BPJS revise the BPJS's claims. It can only be done if hospitals from many regions produce similar rates or slight variations. Jabodetabek still dominates as the region (place) with the most cardiovascular surgery services in Indonesia, with 29 of 35 hospitals. Actionable recommendations to respond to this issue include ensuring data on demand for cardiac surgery services from various regions and encouraging the government through specialist expert associations and legislation to provide cardiovascular surgery services in various regions - outside Jabodetabek. Regarding the specialist (people), the government must provide scholarships for doctors to take BTKV specialists, especially those who can perform pediatric ones.

Abstract

The Lapor Vaksin COVID-19 Application was developed to support a vaccination coverage surveillance database sourced from community self-report. This application provides data on vaccination coverage and allows users to view updates to educational content about vaccines and visualization of COVID-19 vaccine data in real-time. This study aims to analyze the perception among health worker about Lapor Vaksin COVID-19 application in the community. This study uses descriptive research with a case study design. This study was conducted in September 2021 through a virtual zoom meeting, in Ogan Ilir Regency, South Sumatra. The research sample was 42 health workers consisting of vaccine officers at the Health Office, RSUD and Puskesmas in Ogan Ilir Regency who were selected by purposive sampling. Data is presented descriptively. Based on the evaluation results of the application's use, more than half of the respondents were quite satisfied (52.4%) with the Lapor Vaksin COVID-19 application. This application can be used and developed as an alternative to the COVID-19 surveillance database.

Abstract

Ambulance is one of the hospital infrastructures that must meet service standards, security, and occupational safety and health in hospital operations. Therefore, it is the obligation of every hospital to ensure the standard of ambulance service. In the DKI Jakarta area, every ambulance, including hospital ambulances, must meet the standards in accordance with regulations and have an ambulance operating license. Based on data until the end of 2021, there were 82 (34.9%) hospital ambulances in Jakarta that carried out the recommendation process for an ambulance operation license from a total of 235 hospital ambulances, meaning that most hospital ambulances in Jakarta did not meet service standards. DKI Jakarta Governor Regulation number 120 of 2016 concerning Ambulance Services and Hearse is a reference for hospitals in Jakarta in order to meet ambulance service standards. In conclusion, there are 3 (three) standards that must be met in standardizing hospital ambulance services in Jakarta, namely standardization of vehicle units, standardization of equipment and standardization of human resources.

Abstract

The Cardiovascular disease (CVD) prevalence continues to increase. This study aims to identify the CVD spending and utilization in the Indonesia National Health Insurance (JKN) scheme 2017-2021 using the JKN claims classified by Disease Accounts analyzed descriptively. The CVD spending increased by $125,4 million in 2019 compared to 2017 and mostly for the hospital (91% in 2019) with the highest spending occurring in the age above 60 years. Moreover, the hospital inpatient spending was dominated during these five years. The highest portion of spending by severity level was at mild level, followed by moderate and severe levels. Health facilities’ capacities across the regions remain the issue in providing health services, especially for the CVD that require further treatment using advanced technology. The CVD cost continues to increase annually; fortunately, the JKN plays a key role as financial protection for participants. But, there was a decrease in the number of visits and spending on CVD during the COVID-19 pandemic because of lockdown policies and health systems shifting to provide capacity to overcome COVID-19. The decrease in the number of visits to health facilities raises the potential of being diagnosed later with more severe. As policy recommendations, strengthening primary care screening and hospital capacity across regions through implementing the Health System Transformation are expected to reduce the impact of these problems.
