Abstract

Background: Peripartum cardiomyopathy (PPCM) is a rare and potentially life-threatening condition affecting 1 in 1300 to 1 in 5,000 births, particularly in underdeveloped countries. It is characterized by difficulty breathing, lightheadedness, and fatigue. PPCM is more common in women with pre-eclampsia, chronic hypertension, smoking, alcohol abuse, advanced maternal age, malnutrition, long-term tocolysis, or a history of tuberculosis. Untreated PPCM increases the risk of complications like cardiogenic shock, arrhythmias, heart failure, cardiopulmonary arrest, and mortality. Methods: Following PRISMA 2020 guidelines, this systematic review concentrated on full-text English literature published between 2007 and 2024. Editorials and review articles that appeared in the same journal as the submission were not accepted without a DOI. A number of websites, including ScienceDirect, PubMed, and SagePub, were utilized to gather the literature. Result: The study looked at more than 1800 publications using reputable sources including Science Direct, SagePub, and PubMed. After it was decided that eight publications needed greater investigation, a more extensive review of the entire literature was carried out. Conclusion: Pregnancy can lead to the development of Peripartum cardiomyopathy, a potentially fatal disorder, often diagnosed after delivery due to symptoms mimicking normal physiological findings. Tuberculosis, a common cause of maternal and fetal morbidity, often occurs during pregnancy. Diagnosis is often delayed due to overlapping symptoms and clinician reluctance, necessitating coordination between healthcare providers and community services.

Abstract

Background: Oral isotretinoin (13-cis-retinoic acid) is a retinoid, derivedfrom vitamin A. Warts constitute the most frequently observed dermatological manifestations of human papillomavirus (HPV). The aim: The aim of this study to show about oral isotretinoin for treatment of mucocutaneous human papillomavirus infection. Methods: By the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020, this study was able to show that it met all of the requirements. This search approach, publications that came out between 2014 and 2024 were taken into account. Several different online reference sources, like Pubmed, SagePub, and Sciencedirect were used to do this. Result: Eight publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Subsequently, a comprehensive analysis of the complete text was conducted, and additional scrutiny was given to these articles. Conclusion: low dose oral isotretinoin when combined with topical tretinoin 0.1% can be superior to low dose oral isotretinoin alone in terms of faster resolution of lesions in addition to complete clearance with minimal side effect profile.

Abstract

Introduction: Hypokalemic periodic paralysis (HypoPP) is a rare skeletal muscle channelopathy characterized by episodic attacks of moderate to severe flaccid muscle tetraparesis accompanied by hypokalemia. This case report aims to discuss the case and management of hypokalemic periodic paralysis with potassium therapy at RSUD Kabupaten Empat Lawang. Case Presentation: A 29-year-old female presented with sudden bilateral limb weakness, vomiting, and nausea. She had no breathing, swallowing issues, or pain. Physical examination showed reduced muscle strength and epigastric tenderness. Laboratory tests indicated elevated leukocytes and decreased kalium. Diagnosed with hypokalemic periodic paralysis and a UTI, she received IV fluids, potassium chloride, and other supportive treatments, leading to immediate condition resolution. Conclusion: Hypokalemic Periodic Paralysis (HypoPP) is a rare neuromuscular disorder characterized by periodic muscle weakness ranging from mild to complete paralysis.. The etiology in this case was secondary to hyperemesis. Failure to diagnose and manage periodic paralysis can be fatalIt can lead to respiratory failure and death. Treatment involves potassium supplementation and monitoring for cardiac complications.

Abstract

Background: Diabetes mellitus triples the risk of infection, leading to poor clinical outcomes. Hyperglycemia suppresses immune responses, affecting TB and diabetes patients. Modulation of host cell responses and targeting pathogens could improve treatment outcomes and reduce drug resistance. Methods: Following PRISMA 2020 guidelines, this systematic review focused exclusively on full-text articles published in English between 2014 and 2024. Result: The study conducted a comprehensive review of over 600 publications sourced from reputable databases, including ScienceDirect, SagePub, and PubMed. Following an initial screening, eight publications were identified as warranting more in-depth analysis. Consequently, a thorough review of these selected studies was performed to ensure a detailed and rigorous evaluation. Conclusion: Metformin, an oral antidiabetic (OAD) agent, is associated with reduced incidence and mortality from active tuberculosis in patients with diabetes. In contrast, saxagliptin, another oral antidiabetic drug, is linked to an increased risk of active tuberculosis. The continuing examination of the association between metformin and tuberculosis risk highlights the need for further research to better elucidate and validate these associations.

Abstract

Background: Diabetes, a chronic metabolic disorder, has a high prevalence worldwide due to changing lifestyles and urbanization. It is associated with life-threatening hazards and disabling complications, decreasing life expectancy. Gastroparesis, a well-known manifestation, is more prevalent in women. Understanding the prevalence of gastrointestinal complications in type 2 diabetes mellitus patients is crucial for creating effective treatment regimens. Methods: Adhering to the PRISMA 2020 principles, this systematic study focused on analyzing complete English literature texts published from 2011 to 2024. Several websites, such as ScienceDirect, PubMed, and SagePub, were used to collect the literature. Result: The study examined over 4000 publications sourced from recognized platforms such as Science Direct, SagePub, and PubMed. Following the decision to subject eight publications to further scrutiny, a comprehensive examination of the complete body of literature was conducted. Conclusion: Diabetes often leads to gastroparesis, a condition affecting the upper gastrointestinal tract, associated with metabolic imbalance, high blood sugar levels, low insulin, hormone release, and H. pylori infection. This condition is linked to the contractile activity of the duodenum, and is associated with various liver conditions, including diabetic fibrosis and cirrhosis.

Abstract

Background: Asthma, a prevalent and potentially debilitating chronic disease, is traditionally managed using reliever medications such as short-acting β2-agonists (SABA) or low-dose inhaled corticosteroids (ICS). Recent evidence, however, indicates that a combination of as-needed ICS and a rapid-acting long-acting β2-agonist (LABA) provides superior outcomes compared to SABA alone. Furthermore, this combination therapy is at least as effective as regular ICS in mitigating the risk of exacerbations. This emerging evidence highlights a significant shift in treatment strategies for mild asthma, underscoring the potential for enhanced management and improved patient outcomes with the new therapeutic approach. Methods: Following PRISMA 2020 guidelines, this systematic review focused exclusively on full-text articles published in English between 2014 and 2024. Result: The study conducted a comprehensive review of over 400 publications sourced from reputable databases, including ScienceDirect, SagePub, and PubMed. Folowing an initial screening, eight publications were identified as warranting more in-depth analysis. Conclusion: The study found that as-needed ICS/LABA is effective in preventing exacerbations in mild-to-moderate asthma patients, but less effective than regular ICS in improving FEV1 and ACQ scores. Combining ICS/LABA with SMART regimen was highest for severe exacerbations prevention.

Abstract

Background: Cataract remains a major cause of visual impairment worldwide. However, in many countries cataract surgery is easily available. Cataract surgery is a kind of effective way to deal with cataract, but it is acompanied with many surgical complications and expensive costs that present enormous social and economic problems to a society, especially in the remote and poor areas of developing countries. The aim: The aim of this study to show about prevalence, diagnosis, and management of cataract in adult eye. Methods: By the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) 2020, this study was able to show that it met all of the requirements. Result: Eight publications were found to be directly related to our ongoing systematic examination after a rigorous three-level screening approach. Subsequently, a comprehensive analysis of the complete text was conducted, and additional scrutiny was given to these articles. Conclusion: Cataracts to increase in the near future due to an increase in the aging population and pre-senile cataracts. The current study also provides clinical determinants for cataract and cataract surgery. Since cataract has emerged as a public concern and the cost burden for cataract surgery has increased with an aging population, it is expected that the present study will form a cornerstone for further investigations and the establishment of a public health plan.

Abstract

Introduction : This systematic review explores the histopathologic features of GERD and its differential diagnoses, emphasizing the role of endoscopic and histologic assessments in clinical practice. Methods : A systematic review was conducted following PRISMA 2020 guidelines to evaluate the histopathologic features of GERD and its differential diagnoses. Results : From an initial pool of 6006 publications, eight studies were identified as directly relevant after a thorough screening process. The findings highlight the importance of biopsy sampling and histologic evaluation in differentiating GERD from other esophageal inflammatory conditions. Key diagnostic features included basal cell hyperplasia, elongation of lamina propria papillae, and dilatation of intercellular spaces, which were critical for distinguishing GERD from other conditions. Conclusion : This systematic review emphasizes the importance of histopathologic evaluation in the accurate diagnosis of GERD and its differential diagnoses. Understanding the distinct histopathologic features of GERD, as well as technical aspects of biopsy sampling, is essential for improving diagnostic accuracy and preventing progression to severe complications.

Abstract

Introduction: Preterm infants, particularly those with very low birth weight (VLBW), are at heightened risk for life-threatening complications such as necrotizing enterocolitis (NEC), late-onset sepsis, feeding intolerance, and other gastrointestinal and infectious conditions. In recent years, there has been growing interest in exploring non-pharmacological interventions, such as probiotics, prebiotics, and lactoferrin, to mitigate these risks and improve outcomes in preterm infants. Methods: This systematic review adhered to the PRISMA 2020 guidelines to ensure transparency and consistency. Results: Eight relevant studies were identified and assessed in detail. Findings indicate that probiotic, prebiotic, and lactoferrin interventions have potential benefits in reducing NEC incidence and improving clinical outcomes in VLBW infants. For instance, a study by Chowdhury et al. (2016) found a significantly lower incidence of NEC in the probiotic group (1.9%) compared to the control group (11.5%) with a p-value of 0.044. Additionally, the duration of hospital stay was shorter for the probiotic group, averaging 15.82 days versus 19.57 days for controls (p < 0.001). These results support the use of probiotics in enhancing recovery in VLBW infants. This review highlights the potential of probiotics, prebiotics, and lactoferrin in reducing the incidence of NEC and sepsis, as well as promoting faster achievement of full enteral feeding and shorter hospital stays in VLBW infants. However, the heterogeneity among studies regarding probiotic strains, dosages, and study designs complicates definitive conclusions. Further research is needed to establish standardized treatment protocols and ensure the long-term safety of these interventions in clinical practice.

Abstract

Introduction: In Indonesia, colorectal cancer ranks fourth in cancer incidence, with an estimated over 30,000 new cases each year. This study aims to investigate the association between colorectal cancer recurrence rates and CEA levels, histopathology, tumor-infiltrating lymphocytes, and lymphatic invasion at H. Adam Malik Central General Hospital, Medan. Methods: This cross-sectional study investigates the correlation between colorectal cancer recurrence and clinical parameters such as CEA levels, histopathology, TIL, and lymphatic invasion using retrospective data from patients at H. Adam Malik Central General Hospital in North Sumatra. Results:  Out of 75 patients identified, 45 met the inclusion criteria. The study found significant relationships between recurrence and CEA levels (p=0.007), histopathological grade (p<0.001), and lymphovascular invasion (p=0.026), while tumor-infiltrating lymphocytes showed no significant correlation (p=0.358). Conclusion: Colorectal cancer recurrence are primarily shown in male patients with rectal and sigmoid tumors, predominantly adenocarcinoma at stage IIb. Significant CEA levels, moderate histological differentiation, tumor-infiltrating lymphocytes, and lymphovascular invasion were identified.
